A Bausch + Lomb Zywave II Wavefront Aberrometer (Rochester, NY, USA) was used to measure the lower and higher order aberrations of each eye. Data analysis was conducted using linear regression analysis to determine the relationship between total spherical aberration, ametropia, age, corneal curvature, and image quality.Results: Linear regression analysis showed no correlation (r = 0.0207, P = 0.4874) between degree of ametropia and the amount of spherical aberration. There was also no correlation when the population was stratified into myopic and hyperopic refractive groups (rm = 0.0529, Pm = 0.0804 and rh = 0.1572, Ph = 0.2754). There was a statistically significant and weak positive correlation (r = 0.1962, P < 0.001) between age and the amount of spherical aberration measured in the eye; spherical aberration became more positive with increasing age. Also, there was a statistically significant and moderately positive correlation (r = 0.3611, P < 0.001) with steepness of corneal curvature; spherical aberration became more positive with increasing power of the anterior corneal surface. Assessment of image quality using optical design software (Zemax™, Bellevue, WA, USA) showed that there was an overall benefit in correcting the average spherical aberration of this population.Conclusion: Analysis of this dataset provides insights into the inherent spherical aberration of a typical phakic, pre-presbyopic, population and provides the ability to determine what drives the spherical aberration of the eye, as well as what potential benefit a person could gain by compensating for that average spherical aberration.Keywords: ocular aberrations, contact lenses, wavefront sensor

Purpose: To develop a predictive optical modeling process that utilizes individual computer eye models along with a novel through-focus image quality metric., Methods: Individual eye models were implemented in optical design software (Zemax, Bellevue, WA) based on evaluation of ocular aberrations, pupil diameter, visual acuity, and accommodative response of 90 subjects (180 eyes; 24-63 years of age). Monocular high-contrast minimum angle of resolution (logMAR) acuity was assessed at 6 m, 2 m, 1 m, 67 cm, 50 cm, 40 cm, 33 cm, 28 cm, and 25 cm. While the subject fixated on the lowest readable line of acuity, total ocular aberrations and pupil diameter were measured three times each using the Complete Ophthalmic Analysis System (COAS HD VR) at each distance. A subset of 64 mature presbyopic eyes was used to predict the clinical logMAR acuity performance of five novel multifocal contact lens designs. To validate predictability of the design process, designs were manufactured and tested clinically on a population of 24 mature presbyopes (having at least +1.50 D spectacle add at 40 cm). Seven object distances were used in the validation study (6 m, 2 m, 1 m, 67 cm, 50 cm, 40 cm, and 25 cm) to measure monocular high-contrast logMAR acuity., Results: Baseline clinical through-focus logMAR was shown to correlate highly (R² = 0.85) with predicted logMAR from individual eye models. At all object distances, each of the five multifocal lenses showed less than one line difference, on average, between predicted and clinical normalized logMAR acuity. Correlation showed R² between 0.90 and 0.97 for all multifocal designs., Conclusions: Computer-based models that account for patient's aberrations, pupil diameter changes, and accommodative amplitude can be used to predict the performance of contact lens designs. With this high correlation (R² ≥ 0.90) and high level of predictability, more design options can be explored in the computer to optimize performance before a lens is manufactured and tested clinically.

Higher-order aberration correction in abnormal eyes can result in significant vision improvement, especially in eyes with abnormal corneas. Customized optics such as phase plates and customized contact lenses are one of the most practical, nonsurgical ways to correct these ocular higher-order aberrations. We demonstrate the feasibility of correcting higher-order aberrations and improving visual performance with customized soft contact lenses in keratoconic eyes while compensating for the static decentration and rotation of the lens. A reduction of higher-order aberrations by a factor of 3 on average was obtained in these eyes. The higher-order aberration correction resulted in an average improvement of 2.1 lines in visual acuity over the conventional correction of defocus and astigmatism alone.

Purpose: To determine the aberrations induced in wavefront-guided laser refractive surgery due to shifts in pupil center location from when aberrations are measured preoperatively (over a dilated pupil) to when they are corrected surgically (over a natural pupil)., Setting: Center for Visual Science and Department of Ophthalmology, University of Rochester, Rochester, New York, USA., Methods: Shifts in pupil center were measured between dilated phenylephrine hydrochloride (Neo-Synephrine [2.5%]) and nonpharmacological mesopic conditions in 65 myopic eyes treated with wavefront-guided laser in situ keratomileusis (Technolas 217z, Bausch & Lomb). Each patient's preoperative and 6-month postoperative wave aberrations were measured over the dilated pupil. Aberrations theoretically induced by decentration of a wavefront-guided ablation were calculated and compared with those measured 6 months postoperatively (6.0 mm pupil)., Results: The mean magnitude of pupil center shift was 0.29 mm +/- 0.141 (SD) and usually occurred in the inferonasal direction as the pupil dilated. Depending on the magnitude of shift, the fraction of the higher-order postoperative root-mean-square wavefront error that could be due theoretically to pupil center decentrations was highly variable (mean 0.26 +/- 0.20 mm). There was little correlation between the calculated and 6-month postoperative wavefronts, most likely because pupil center decentrations are only 1 of several potential sources of postoperative aberrations., Conclusions: Measuring aberrations over a Neo-Synephrine-dilated pupil and treating them over an undilated pupil typically resulted in a shift of the wavefront-guided ablation in the superotemporal direction and an induction of higher-order aberrations. Methods referencing the aberration measurement and treatment with respect to a fixed feature on the eye will reduce the potential for inducing aberrations due to shifts in pupil center.

Purpose: To determine the amount of static and dynamic pupil decentrations that occur during laser refractive surgery., Setting: The Center of Visual Science and the Department of Ophthalmology, University of Rochester, Rochester, New York, USA., Methods: The surgeon's accuracy in aligning the pupil center with the laser center axis was measured when engaging the eye-tracker in 17 eyes receiving conventional laser in situ keratomileusis (LASIK) procedures (Technolas 217z; Bausch & Lomb). Eye movements were measured subsequently during the treatment in 10 eyes using a pupil camera operating at 50 Hz. Temporal power spectra were calculated from the eye movement measurements., Results: The mean pupil misalignment by the surgeon at the beginning of the procedure was 206.1 microm +/- 80.99 (SD) (with respect to the laser center). The laser center was typically misaligned below (inferiorly) and to the left (nasally and temporally in left and right eyes, respectively) of the pupil [corrected] center. Small amounts of cyclotorsion were observed during the ablation (<2 degrees). The mean magnitude of dynamic pupil decentration from the laser center during treatment was 227.0 +/- 44.07 microm. The mean standard deviation of eye movements was 65.7 +/- 25.64 microm. Temporal power spectra calculated from the horizontal and vertical changes in eye position during the ablation were similar. Ninety-five percent of the total power of the eye movements was contained in temporal frequencies up to 1 Hz, on average, in both directions., Conclusions: Most eye movements during LASIK are slow drifts in fixation. An eye-tracker with a 1.4 Hz closed-loop bandwidth could compensate for most eye movements in conventional or customized ablations.

Purpose: To investigate the distribution of the eye's higher-order aberrations in postoperative laser refractive surgery patients with visual complaints and highly irregular corneal shapes., Design: Retrospective case-control study., Participants: Thirty-three symptomatic postoperative LASIK and/or photorefractive keratectomy eyes with subjective visual complaints not corrected by spectacles more than 6 months after surgery are compared with 46 normal preoperative and 46 asymptomatic successful postoperative conventional LASIK eyes., Methods: Postoperative wave aberrations were measured for each patient using a Shack-Hartmann wavefront sensor (Zywave, Bausch & Lomb, Rochester, NY) over a 6-mm pupil. These measurements were averaged across patients with similar corneal topographic diagnoses (central islands, decentered ablations, a new group termed baby bowties, and irregularly irregular corneas)., Main Outcome Measures: Higher-order aberrations and corneal topography., Results: The average (+/-1 standard deviation) higher-order root-mean-square (rms) wavefront error values (third, fourth, and fifth orders) for the symptomatic patients was 1.31+/-0.58 microm. This was an average of 3.46 times greater than the average magnitude of normal preoperative eyes (mean rms, 0.38+/-0.14 microm), and an average of 2.3 times greater than the average magnitude of asymptomatic successful postoperative conventional LASIK eyes (mean rms, 0.58+/-0.21 microm) over a 6-mm pupil. Higher-order rms wavefront error increased with pupil size, roughly doubling for every millimeter of increasing pupil diameter. On average, eyes with central islands (n = 6) had the most vertical coma (Z3(-1); mean, -1.35+/-0.43 microm). Eyes with central islands and decentered ablations (n = 2) also had elevated amounts of spherical aberration (Z4(0); means of 0.83+/-0.11 microm and 0.69+/-0.29 microm, respectively) compared with successful postoperative LASIK eyes (mean of 0.42+/-0.20 microm). Eyes with a topographic central baby bowtie demonstrated the most secondary astigmatism (Z4(2) and Z4(-2); mean rms, 0.56+/-0.17 microm; n = 3), despite the lowest average higher-order rms (mean, 0.84+/-0.05 microm) among symptomatic topographic subgroups. Eyes with irregularly irregular corneas had a mean higher-order rms of 1.10+/-0.39 mum., Conclusions: Symptomatic postoperative laser refractive surgery patients with irregular corneas have higher-order aberrations that are 2.3 to 3.5 times greater than asymptomatic postoperative LASIK and normal preoperative eyes, respectively. The higher-order aberrations seem to correlate with corneal topography.

Purpose: To develop a corneal model to better explain how refractive surgery procedures induce spherical aberration., Setting: Department of Ophthalmology and Center for Visual Science, University of Rochester, Rochester, New York, USA., Methods: The preoperative cornea was modeled as a rotationally symmetric surface with various radii of curvature and asphericities. The postoperative cornea was defined as the difference between the preoperative cornea and an ablation thickness profile computed based on the Munnerlyn equation. A ray-tracing program and Zernike polynomial fitting were used to calculate the induced amount of spherical aberration assuming a fixed ablation depth per pulse or a variable ablation depth depending on the incidence angle of each pulse on the cornea. A biological eye model of the corneal surface change after laser refractive surgery was also developed to explain the induced spherical aberrations after myopic and hyperopic treatments., Results: The clinical data showed that positive spherical aberration was induced after myopic correction and negative spherical aberration increased after hyperopic correction. In contrast, assuming a fixed ablation depth per pulse, the theoretical prediction was that negative spherical aberration with myopic treatment and positive spherical aberration with hyperopic treatment would increase. However, when assuming a variable ablation depth per pulse caused by non-normal incidence of laser spot on the cornea, the theoretically predicted induction of spherical aberration tends to fit better with the myopic and hyperopic clinical data. The effect of a variable ablation depth accounted for approximately half the clinically observed amount of spherical aberration. The biological model of the corneal surface change used to explain this remaining discrepancy showed the magnitude of the biological response in myopic correction is 3 times smaller than in hyperopic correction and that the direction of the biological response in hyperopic treatment is opposite that in myopic treatment., Conclusions: This nontoric eye model, which separates the effects of differences in ablation efficiency and biological corneal surface change quantitatively, explains how spherical aberration is induced after myopic and hyperopic laser refractive surgery. With the corneal topographic data, this model can be incorporated into the ablation algorithm to decrease induced spherical aberrations, improving the outcomes of conventional and customized treatments.

Purpose: To find predictors of the induced biomechanical and optical effects of lamellar flap creation on the cornea., Setting: Optimed Eye and Laser Clinic, Pretoria, South Africa, and the Department of Ophthalmology and Biomedical Engineering Center, The Ohio State University, Columbus, Ohio, and Bausch & Lomb Vision Research Laboratory, Rochester, New York, USA., Methods: This prospective study monitored the refractive, wavefront aberration, and corneal topographic changes in 29 eyes of 15 patients for 3 months after the creation of a corneal lamellar flap. The main outcome measures for statistical analysis were refraction, total corneal thickness, residual corneal bed thickness, horizontal white-to-white corneal diameter, horizontal flap diameter, topography data, and wavefront data., Results: Statistically significant changes were seen in the autorefraction mode. Wavefront data showed significant change in 4 Zernike modes-90/180-degree astigmatism, vertical coma, horizontal coma, and spherical aberration. The topography data indicated the corneal biomechanical response was significantly predicted by stromal bed thickness in the early follow-up period and by total corneal pachymetry and flap diameter in a 2-parameter statistical model in the late follow-up period., Conclusions: Uncomplicated lamellar flap creation is responsible for systematic changes in corneal topography and induction of higher-order optical aberrations. Predictors of this response include stromal bed thickness, flap diameter, and total corneal pachymetry.

Purpose: To psychophysically demonstrate vision improvement when correcting higher-order aberrations with phase plates in normal eyes., Methods: The wavefront aberrations of three nonsurgical normal eyes were measured with a Shack-Hartmann wavefront sensor. With these measured aberrations, phase plates were fabricated using a lathing technique. Theoretical improvement in retinal image quality was estimated by calculating the optical modulation transfer functions under the white light condition. Visual acuity measurements were also conducted to demonstrate improvement in visual performance after correcting higher-order aberrations with the phase plate. In this visual acuity measurement, a tumbling "E" with high (100%) and low (10%) contrast was used., Results: The phase plate reduced the higher-order root mean square (RMS) wavefront error from 0.39 +/- 0.09 to 0.15 +/- 0.02 microm (mean +/- standard deviation from three eyes) for a 6-mm pupil. With the phase plate, retinal image quality based on the volume of modulation transfer function under 60 cycles per degree (c/deg) was improved by a factor of 1.8 +/- 0.4 over that of the eyes with spherocylindrical correction only. Average improvement in visual acuity achieved by correcting the higher-order aberration was 0.23 lines with high-contrast letters and 1.12 lines with low-contrast letters. All subjects reported subjective improvement in image quality of the letter with the phase plate., Conclusion: The phase plate effectively corrected the higher-order aberrations in normal eyes. As a result, both retinal image quality and visual acuity especially with the low-contrast letters were improved. This study demonstrated the feasibility of correcting higher-order aberrations and improving vision with customized optics.

Purpose: To study the optical changes induced by the microkeratome cut, the subsequent laser ablation, and the biomechanical healing response of the cornea in normal laser in situ keratomileusis (LASIK) eyes., Design: Prospective randomized clinical trial., Methods: A Hansatome microkeratome was used to cut a corneal flap in one eye (study eye) of 17 normal myopic patients and a subsequent laser ablation was performed 2 months after this initial microkeratome incision. Control eyes received conventional LASIK treatments at the latter time point. The wave aberration of both the study and contralateral control eyes were measured over a 6-mm pupil with a Shack-Hartmann wavefront sensor for all preoperative, postflap cut, and postablation visits., Results: The eye's higher order aberrations had a small, but significant increase (P =.03) of approximately 30% 2 months after cutting a flap. No systematic changes were observed in nearly all Zernike coefficients from their preoperative levels at 2 months postflap cut. A significant difference between the study and control eyes was observed for one trefoil mode, Z(3)(3) (P =.04)., Conclusions: There was a wide variation in the response of individual Zernike modes across patients after cutting a flap. The majority of spherical aberration induced by the LASIK procedure seems to be due to the laser ablation and not the microkeratome cut. In addition, the total and higher order root mean square of wavefront errors were nearly identical for both the study and control eyes 3-months after the laser ablation, indicating that a procedure in which the incision and the ablation are separated in time to better control aberrations does not compromise the outcome of a conventional LASIK treatment.

We calculated the impact of higher-order aberrations on retinal image quality and the magnitude of the visual benefit expected from their correction in a large population of human eyes. Wave aberrations for both eyes of 109 normal subjects and 4 keratoconic patients were measured for 3-, 4-, and 5.7-mm pupils with a Shack-Hartmann sensor. Retinal image quality was estimated by means of the modulation transfer function (MTF) in white light. The visual benefit was calculated as the ratio of the MTF when the monochromatic higher-order aberrations are corrected to the MTF corresponding to the best correction of defocus and astigmatism. On average, the impact of the higher-order aberrations for a 5.7-mm pupil in normal eyes is similar to an equivalent defocus of approximately 0.3 D. The average visual benefit for normal eyes at 16 c/deg is approximately 2.5 for a 5.7-mm pupil and is negligible for small pupils (1.25 for a 3-mm pupil). The benefit varies greatly among eyes, with some normal eyes showing almost no benefit and others a benefit higher than 4 at 16 c/deg across a 5.7-mm pupil. The benefit for keratoconic eyes is much larger. The benefit at 16 c/deg is 12 and 3 for 5.7- and 3-mm pupils, respectively, averaged across four keratoconics. These theoretical benefits could be realized in normal viewing conditions but only under specific conditions.

The use of a correcting element to compensate for higher-order aberrations in an optical system often requires accurate alignment of the correcting element. This is not always possible, as in the case of a contact lens on the eye. We propose a method consisting of partial correction of every aberration term to minimize the average variance of the residual wave-front aberration produced by Gaussian decentrations (translations and rotations). Analytical expressions to estimate the fraction of every aberration term that should be corrected for a given amount of decentration are derived. To demonstrate the application of this method, three examples are used to compare performance with total and with partial correction. The partial correction is more robust and always yields some benefit regardless of the amount of decentration.

From both a fundamental and a clinical point of view, it is necessary to know the distribution of the eye's aberrations in the normal population and to be able to describe them as efficiently as possible. We used a modified Hartmann-Shack wave-front sensor to measure the monochromatic wave aberration of both eyes for 109 normal human subjects across a 5.7-mm pupil. We analyzed the distribution of the eye's aberrations in the population and found that most Zernike modes are relatively uncorrelated with each other across the population. A principal components analysis was applied to our wave-aberration measurements with the resulting principal components providing only a slightly more compact description of the population data than Zernike modes. This indicates that Zernike modes are efficient basis functions for describing the eye's wave aberration. Even though there appears to be a random variation in the eye's aberrations from subject to subject, many aberrations in the left eye were found to be significantly correlated with their counterparts in the right eye.

An ideal correcting method, such as a customized contact lens, laser refractive surgery, or adaptive optics, that corrects higher-order aberrations as well as defocus and astigmatism could improve vision. The benefit achieved with this ideal method will be limited by decentration. To estimate the significance of this potential limitation we studied the effect on image quality expected when an ideal correcting method translates or rotates with respect to the eye's pupil. Actual wave aberrations were obtained from ten human eyes for a 7.3-mm pupil with a Shack-Hartmann sensor. We computed the residual aberrations that appear as a result of translation or rotation of an otherwise ideal correction. The model is valid for adaptive optics, contact lenses, and phase plates, but it constitutes only a first approximation to the laser refractive surgery case where tissue removal occurs. Calculations suggest that the typical decentrations will reduce only slightly the optical benefits expected from an ideal correcting method. For typical decentrations the ideal correcting method offers a benefit in modulation 2-4 times higher (1.5-2 times in white light) than with a standard correction of defocus and astigmatism. We obtained analytical expressions that show the impact of translation and rotation on individual Zernike terms. These calculations also reveal which aberrations are most beneficial to correct. We provided practical rules to implement a selective correction depending on the amount of decentration. An experimental study was performed with an aberrated artificial eye corrected with an adaptive optics system, validating the theoretical predictions. The results in a keratoconic subject, also corrected with adaptive optics, showed that important benefits are obtained despite decentrations in highly aberrated eyes.

We investigated the relationship between ocular dominance and monovision visual performance in 15 presbyopic subjects. Ocular dominance was determined using sighting (hole-in-the-card and mirror tests) and sensory (anisometropic blur suppression test) methods. Correcting the dominant sighting eye for a given viewing distance was found to be an unreliable method of optimizing blur suppression or binocular high/low contrast visual acuity at that distance. If there is any advantage to a particular strategy for selecting the distance monovision eye, it must be realized in vision performance areas other than visual acuity.

Attitudes of Asian patients to the delivery of primary health care in two Birmingham general practices were investigated by questionnaires administered by an Asian ethnic minorities worker who spoke dialects appropriate to the population under investigation. One practice was staffed by Asian doctors the other by British doctors. The responses to the questionnaires were analysed with reference to religion - Sikh, Hindu and Muslim and to the two practices. Choice of doctor appears to be determined more by the proximity of the patient's home to the practice premises than by ethnic considerations. Reported failures to meet the special needs of Asian patients were those inherent in the difficulties of British general practice and were not peculiar to Asian patients. The need for help from an interpreter did not seem to be important.
