In response to different viral infections, including SARS-CoV-2 infection, pro-inflammatory, anti-inflammatory cytokines, and bioactive lipids are released from infected and immune cells. One of the most critical bioactive lipids is prostaglandins (PGs) which favor perseverance of inflammation leading to chronic inflammation as PGs act as cytokine amplifiers. PGs trigger the release of pro-inflammatory cytokines, activate Th cells, recruit immune cells, and increase the expression of pro-inflammatory genes. Therefore, PGs may induce acute and chronic inflammations in various inflammatory disorders and viral infections like SARS-CoV-2. PGs are mainly inhibited by non-steroidal anti-inflammatory drugs (NSAIDs) by blocking cyclooxygenase enzymes (COXs), which involve PG synthesis. NSAIDs reduce inflammation by selective or non-selective blocking activity of COX2 or COX1/2, respectively. In the Covid-19 era, there is a tremendous controversy regarding the use of NSAIDs in the management of SARS-CoV-2 infection. As well, the possible role of PGs in the pathogenesis of SARS-CoV-2 infection is not well-defined. The polymorphism of HLA class II genes was studied in COVID-19 survivors and vaccinated people. Six allelic variants of DQA1, 10 of DQB1, and 11 of DRB1 were identified. The DQA1*05:01 allele predominated in vaccinated volunteers and the DQA1*02:01, *01:03, and DQB1*06:02-8 alleles predominated in convalescents. In the groups with DQA1*01:01, *05:01 and *06:01 alleles, a high proportion of individuals seropositive for SARS-CoV-2 S protein was found. In the group with DQA1*02:01 allele, 40% of volunteers were found to have IgM and a significantly lower quantity of IgG to N protein of coronavirus. Comparative analysis showed a statistically significant increase in IL-10 levels in carriers of the DQA1*01:01 allele, which may indicate the influence of this allele on cytokine production. Further study of the immune response indices and their association with HLA class II gene polymorphism will provide better understanding of the mechanisms of COVID-19 immunogenesis. In a project to preserve photographic archives in Southeast Asia, the authors sought to develop sustainable solutions with low environmental impact, avoiding air conditioning and using locally available materials. Dry cabinets were tested and implemented to store a glass plate negative collection that was rehoused in envelopes and boxes made locally. The project took just over a year to complete, and two years of monitoring environmental conditions have shown that relative humidity can be stabilized at low levels. However, the dry cabinets are airtight to limit the introduction of moisture and dust, preventing air exchange. In order to avoid the build-up of degradation by-products released by the stored materials, and in particular off-gassing of acetic acid from the cellulose acetate-based photographs, the use of an adsorbent is necessary. To achieve this, a cellulose-based composite with particles of a porous hybrid inorganic/ organic solid, denoted as a metal–organic framework (MOF), at a very high loading over 70% by weight and shaped as a sheet of paper, has been prepared and tested. This special paper maintains acetic acid-free air in the cabinets and also helps to reduce the energy consumption by reducing the need for cool storage. This MOF paper composite traps small carboxylic acids and was found to be an appropriate solution for maintaining clean air in the dry cabinets or any other confined space or enclosure. This study investigates corner solidification in a closed cavity in which the left and bottom walls are kept at a temperature lower than its initial temperature. The liquid material in the cavity initially lies at its phase transition temperature and, due to cold boundary conditions at the left–bottom walls, solidification starts. The simulation of corner solidification was performed using a kinetic-based lattice Boltzmann method (LBM), and the tracking of the solid–liquid interface was captured through the evaluation of time. The present investigation addresses the effect of natural convection over conduction across a wide range of higher Rayleigh numbers, from 106 to 108. The total-enthalpy-based lattice Boltzmann method (ELBM) was used to observe the thermal profiles in the entire cavity with a two-phase interface. The isotherms reveal the relative dominance of natural convection over conduction, and the pattern of interface reveals the effective growth of the solidified layer in the cavity. To quantify the uniformity of cooling, a coefficient of variation (COV) for the thermal field was calculated in the effective solidified zone at a wide range of Ra. The results show that the value of COV increases with Ra and reduces with time. The thermal instability in the flow field is also quantified through FFT analyses. [ABSTRACT FROM AUTHOR]

Objectives: The study involving 20 SARS CoV patients found that elevated microRNA levels, specifically miR-23a overexpression and miR-195-F1gene downregulation, could serve as potential diagnostic factors for COVID-19 infections, potentially aiding in early detection. Materials and Methods: 20 patients suffered from coronavirus (SARS CoV) (Patient group) and ten samples of healthy persons (control group). The blood sample was collected from the patients who attended Al-Zafaraniyah General Hospital, Fatima-Alzahra Hospital, and Al-Yarmook Teaching Hospital in Baghdad City. Nasopharyngeal and oropharyngeal swabs for each patient were taken simultaneously and added immediately into viral transport media (VTM). The microRNAs in the serum of coronavirus (SARS CoV patients were miRNA concentration upon conversion to cDNA is necessary to analyze and compute regulatory element levels of one or more genes—all procedures, including data analysis, qPCR amplification, and total RNA purification. Results: The miR-23a-3p levels were markedly elevated, and serum levels of the miR-195-F1 gene were shown to be substantially downregulated in COVID-19 individuals. Conclusions: The study reveals that COVID-19 patients exhibit increased expression levels of miR-23a-3p and miR-195-F1gene, suggesting it as a potential diagnostic indicator for COVID-19 diagnosis and treatment monitoring.

Coronaviruses (CoVs), the largest positive-sense RNA viruses, have caused infections in both humans and animals. The cross-species transmission of CoVs poses a serious threat to public health. Rodents and bats, the two largest orders of mammals, serve as significant natural reservoirs for CoVs. It is important to monitor the CoVs carried by bats and rodents. In this study, we collected 410 fecal samples from bats and 74 intestinal samples from rats in Yunnan Province, China. Using RT-PCR, we identified one positive sample for alphacoronavirus (TC-14) from Rhinolophus sinicus (Chinese rufous horseshoe bat) and two positive samples for betacoronavirus (GS-53, GS-56) from Apodemus ilex (Rodentia: Muridae). We successfully characterized the complete genomes of TC-14 and GS-56. Phylogenetic analysis revealed that TC-14 clustered with bat CoV HKU2 and SADS-CoV, while GS-56 was closely related to rat CoV HKU24. The identification of positive selection sites and estimation of divergence dates further helped characterize the genetic evolution of TC-14 and GS-56. In summary, this research reveals the genetic evolution characteristics of TC-14 and GS-56, providing valuable references for the study of CoVs carried by bats and rodents in Yunnan Province. To address the growing scarcity and rising costs of fossil fuels, researchers are exploring alternative energy sources that can mimic the conventional fuels. This study focused on ternary fuel blends made from waste fried oil (WFO) biodiesel, methanol, and diesel. To enhance their stability, 10, 20, and 30 ml of n-butanol per litre of ternary blends are added. The blends were tested in a variable compression ratio (VCR) engine under peak load conditions. The experiments are carried out by varying the compression ratio (CR) of the engine from 16:1 to 18:1. The results showed that increasing the CR improved combustion for all fuel blends. Among the ternary blends, B30M20D50 outperformed pure diesel in terms of combustion characteristics. When compared to diesel, B30M20D50 yielded 5.08 %, 5.31 %, and 4.59 % higher Pmax at CR 16:1, 17:1, and 18:1, respectively. Under the same conditions, NHRR outperformed diesel by 1.46 %, 3.12 %, and 2.44 %, respectively. While ternary blends with up to 20 % methanol exhibited stable combustion, higher methanol concentrations led to erratic rise in the coefficient of variation. The interdependency analysis revealed a strong correlation between the combustion parameters for the B20M30D50 blend across different compression ratios.

Objective Health-care systems are facing a brand new difficulty as a result of post-COVID-19 syndrome. The relationship between long COVID and vitamin d.(25OHD levels was not extensively studied. Also, to our knowledge, no study evaluated vit d.(25OHD) levels and their relation to long COVID in the long-term period [3–6 months) after COVID infection. This study aims to evaluate vit d.(25OHD) levels post-COVID-19 infection and its relation to persistent symptoms in long-term COVID. Patients and methods A cross-sectional, single-center study involving 84 patients who were recruited during the period from December 2021 to May 2022, during the previous 6 months after the COVID-19 sickness. BackgroundAlthough severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection has been associated with increased risk of adverse perinatal health outcomes, few large-scale, community-based epidemiological studies have been conducted.MethodsWe conducted a national cohort study using deidentified administrative claims data for 78 283 pregnancies with estimated conception before 30 April 2020 and pregnancy end after 11 March 2020. We identified SARS-CoV-2 infections using diagnostic and laboratory testing data, and compared the risk of pregnancy outcomes using Cox proportional hazard models treating coronavirus disease 2019 (COVID-19) as a time-varying exposure and adjusting for baseline covariates.ResultsOf the pregnancies, 2655 (3.4%) had a documented SARS-CoV-2 infection. COVID-19 during pregnancy was not associated with risk of miscarriage, antepartum hemorrhage, or stillbirth, but was associated with 2-3 fold higher risk of induced abortion (adjusted hazard ratio [aHR], 2.60; 95% confidence interval [CI], 1.17-5.78), cesarean delivery (aHR, 1.99; 95% CI, 1.71-2.31), clinician-initiated preterm birth (aHR, 2.88; 95% CI, 1.93-4.30), spontaneous preterm birth (aHR, 1.79; 95% CI, 1.37-2.34), and fetal growth restriction (aHR, 2.04; 95% CI, 1.72-2.43).ConclusionsPrenatal SARS-CoV-2 infection was associated with increased risk of adverse pregnancy outcomes. Prevention could have fetal health benefits.

In this paper, an investigation of the use of gasoline-ethanol-methanol on the spark ignition engine is presented, it is not common practice on public roads to use three fuels simultaneously in a spark-ignition engine. Using methanol reduces the ignition delay during combustion, especially at lean air-fuel ratios, and reduces knocking potential in small amounts. The best result ignition delay with value λ= 1,3 obtained in the E5M15 mixture with SoC occurred at 325 CAo, while the value λ= 1,0 also obtained on the same mixture with SoC occurred at 321,5 CAo. The CCV results indicate a more sloping increase in the COV (coefficient of variation) value when using GEM fuel, particularly with the addition of more methanol. The addition of methanol enhances combustion progression and improves the ability of the fuel blend to sustain combustion under lean conditions. Regarding the torque and power values, at λ= 1,0; 1,1; 1,2 are not significantly different, while the value λ= 1,3 is below the other λ values.

Introduction: The main aim of the study was to find the most common CT feature in the lungs of patients associated with Delta variant and characterize thoracic computed tomography and report mortality. Material and Methods: A total of 156 patients with suspected or confirmed COVID-19 infection were randomized in this study which their chest CTs performed at the initial positive and negative RT-PCR. There were two groups of patients involved. Results: The patients had typical imaging features such as; coagulation (33 [21.2%]), ground glass opacities (GGO) (140 [89.7%]), vascular enlargement of the lesion (41 [26.3%]). Lesions that appear on CT images are more likely to have peripheral distribution (79 [50.6%]), peripheral and central distribution (61 [39.1%]) and architectural distortion (14 [9%]. Other CT features include pleural effusion (15 [9.6 %]) and crazy-paving pattern (25 [16 %]). Only two patients had tree-in-bud and traction bronchiectasis (2 [1,3%]). In contrast, the overall mortality rate was (19 [12.2%]). Conclusion: The most common CT feature was peripheral GGO (140 [89.7%]) in the lungs of patients with COVID-19. The initial positive RT-PCR group had a higher peripheral distribution, GGO, and frenzy pattern during their CT scan than patients in the group with a negative initial RT-PCR result. Mortality rates were nearly identical between groups with a positive and negative RT-PCR baseline at 10.7% and 13%, respectively. It has been shown that most patients with negative RT-PCR should be considered suspected of having COVID-19.

The infectiaon causes perturbed the cellular metabolism that favours viral replication leading to mitochondrial dysfunction and chronic inflammation. By localizing to the mitochondria, SARS CoV proteins increase reactive oxygen species (ROS) levels, perturbation of Ca2+ signaling, changes in mtDNA copy number, mitochondrial membrane potential (MMP), mitochondrial mass, and induction of mitophagy. These proteins also influence the fusion and fission kinetics, size, structure, and distribution of mitochondria in the infected host cells. This results in compromised bioenergetics, altered metabolism, and innate immune signaling, and hence can be a key player in determining the outcome of SARS-CoV infection. SARS-CoV infection contributes to stress and activates apoptotic pathways. This review summarizes how mitochondrial function and dynamics are affected by SARS-CoV and how the mitochondria-SARS-CoV interaction benefits viral survival and growth by evading innate host immunity. We also highlight how the SARS-CoV-mediated mitochondrial dysfunction contributes to post-COVID complications. Objective The outbreak of COVID-19 caused by SARS-CoV-2 has led to a serious worldwide pandemic. Quantitative reverse transcription–polymerase chain reaction (qRT-PCR)–based methods were recommended for routine detection of SARS-CoV-2 RNA. Because the reaction time and analytical sensitivity of qRT-PCR limits the diagnosis of SARS-CoV-2, development of a quick process of SARS-CoV-2 detection technology with high analytical sensitivity remains urgent. Methods We combined isothermal amplification and fluorescence detection technology to develop a new auto-recombinase polymerase amplification (RPA)-fluorescence platform that could be used in the diagnosis of SARS-CoV-2. Results By optimization of primers and probes, the RPA platform could detect SARS-CoV-2 nucleotides within 15 min. The limits of detection and specificity of the auto-RPA-fluorescence platform were 5 copies/µL and 100%, respectively. The accuracy of detection of the auto-RPA-fluorescence platform in the 16 positive samples was 100%. Conclusion The RPA platform is a potential technology for the diagnosis of SARS-CoV-2 infection. SARS-CoV and MERS-CoV originated in bats, and there is a possibility that this could be the case for SARS-CoV-2 as well. There is already evidence that a probable intermediary host is responsible for the emergence of viruses in humans as was the case for SARS-CoVs and MERS-CoV. As the SARS-CoV-2 originated from a live animal market, there is always the question if domestic animals are susceptible to these viruses and the possible risk of zoonotic transmission with mammals, including humans. This uncertainty of the transmission of the COVID-19 virus between humans and animals is of great significance worldwide. Background & objectives: COVID-19 has been a global pandemic since early 2020. It has diverse clinical manifestations, but consistent immunological and metabolic correlates of disease severity and protection are not clear. This study was undertaken to compare seropositivity rate, antibody levels against nucleocapsid and spike proteins, virus neutralization and metabolites between adult and child COVID-19 patients. Methods: Plasma samples from naïve control (n=14) and reverse transcription (RT)-PCR positive COVID-19 participants (n=132) were tested for reactivity with nucleocapsid and spike proteins by ELISA, neutralization of SARS-CoV-2 infectivity in Vero cells and metabolites by 1H nuclear magnetic resonance (NMR) spectroscopy. Results: An ELISA platform was developed using nucleocapsid and spike proteins for COVID-19 serosurvey. The participants showed greater seropositivity for nucleocapsid (72%) than spike (55.3%), and males showed higher seropositivity than females for both the proteins. Antibody levels to both the proteins were higher in intensive care unit (ICU) than ward patients. Children showed lower seropositivity and antibody levels than adults. In contrast to ICU adults (81.3%), ICU children (33.3%) showed lower seropositivity for spike. Notably, the neutralization efficiency correlated with levels of antinucleocapsid antibodies. The levels of plasma metabolites were perturbed differentially in COVID-19 patients as compared with the naive controls. Interpretation & conclusions: Our results reflect the complexity of human immune response and metabolome to SARS-CoV-2 infection. While innate and cellular immune responses are likely to be a major determinant of disease severity and protection, antibodies to multiple viral proteins likely affect COVID-19 pathogenesis. Biodiesel has great potential as an alternative fuel for diesel engines. However, poor low-temperature flow property and high viscosity of biodiesel limit its use in engines. Therefore, alcohol comes into researchers’ eyes as a promising additive for diesel–biodiesel (DB) blends. In this study, the effects of lower and higher alcohol contents with different speeds on combustion and emissions characteristics were investigated in a modified single cylinder diesel engine. The results indicated that the maximum pressure rise rate (MPRR) maintained a consistent drop ratio as speed increased from 1500 rpm to 1800 rpm for the tested fuels. However, B10E10 (10% biodiesel and %10 ethanol by volume) had a big drop, which meant that engine’s power ability had a big loss at high speed. The declining rate with speed decreased for indicated thermal efficiency (ITE) when adding alcohol to diesel–biodiesel blends, especially n-pentanol. Moreover, compared with other tested fuels, B10P10 (10% biodiesel and 10% n-pentanol) had smaller cycle variation and more stable combustion state as speed changed, despite it had the highest coefficient of variation (COV) at 1000 rpm. It can be found that has advantages in combustion stability, and can well reduce the total hydrocarbon (THC) and carbon monoxide (CO) emissions under the condition of medium speed commonly used by vehicle. Overall consideration, using diesel/biodiesel/alcohol ternary fuels may be a better choice for diesel engines.

Metal–organic frameworks (MOF) are a new type of porous materials that have great potential for adsorption of voltaic organic compounds (VOCs). These types of materials composed of metal ions and organic ligands are easy to synthesize, have high surface areas, their surface chemistry can be adjusted to the desired application, and they can also have good chemical and thermal stability. Therefore, this work focuses on the synthesis of a highly hydrophobic MOF material called CIM-80, a porous material that is made up of the Al3+ cation and the mesaconate linker. This MOF has a B.E.T. of approximately 800 m2/g and has potential applications for the adsorption of hydrophobic organic compounds. However, its synthesis is expensive and very dirty. Therefore, we have studied the synthesis conditions necessary to achieve high synthesis yields (85%) and materials with high crystallinity and accessible porosity. To achieve these results, we have used urea as a mild deprotonation reagent and modulator as an alternative to NaOH, which is traditionally used for the synthesis of this MOF. Once the synthesis of this material was controlled, its adsorption/desorption behavior of water and organic compounds such as toluene, cyclohexane and m-xylene was studied by means of vapor adsorption isotherms. The results show the hydrophobic character of the material and the greater affinity the material has toward aliphatic compounds than toward aromatic ones, with toluene being the most adsorbed compound, followed by cyclohexane and m-xylene. We observed an increased frequency of massive perivillous fibrin deposition (MPFD) during the second coronavirus disease 2019 (COVID-19) pandemic wave dominated by the Alpha variant of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2). MPFD associated with 100% reverse transcription polymerase chain reaction (RT-PCR) positivity for SARS-CoV-2 and detection by immunohistochemistry. Background Severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) can infect human and other mammals, including hamsters. Syrian (Mesocricetus auratus) and dwarf (Phodopus sp.) hamsters are susceptible to SARS-CoV-2 infection in the laboratory setting. However, pet shop-related Coronavirus Disease 2019 (COVID-19) outbreaks have not been reported. Methods We conducted an investigation of a pet shop-related COVID-19 outbreak due to Delta variant AY.127 involving at least 3 patients in Hong Kong. We tested samples collected from the patients, environment, and hamsters linked to this outbreak and performed whole genome sequencing analysis of the reverse transcription polymerase chain reaction (RT-PCR)-positive samples. Results The patients included a pet shop keeper (Patient 1), a female customer of the pet shop (Patient 2), and the husband of Patient 2 (Patient 3). Investigation showed that 17.2% (5/29) and 25.5% (13/51) environmental specimens collected from the pet shop and its related warehouse, respectively, tested positive for SARS-CoV-2 RNA by RT-PCR. Among euthanized hamsters randomly collected from the storehouse, 3% (3/100) tested positive for SARS-CoV-2 RNA by RT-PCR and seropositive for anti-SARS-CoV-2 antibody by enzyme immunoassay. Whole genome analysis showed that although all genomes from the outbreak belonged to the Delta variant AY.127, there were at least 3 nucleotide differences among the genomes from different patients and the hamster cages. Genomic analysis suggests that multiple strains have emerged within the hamster population, and these different strains have likely transmitted to human either via direct contact or via the environment. Conclusions Our study demonstrated probable hamster-to-human transmission of SARS-CoV-2. Background Data on SARS-CoV-2 vaccine immunogenicity in PLWH are currently limited. Aim of the study was to investigate immunogenicity according to current CD4 T-cell count Methods PLWH on ART attending a SARS-CoV-2 vaccination program, were included in a prospective immunogenicity evaluation after receiving BNT162b2 or mRNA-1273. Participants were stratified by current CD4 T-cell count (poor CD4 recovery, PCDR: <200/mm3; intermediate CD4 recovery, ICDR: 200–500/mm3; high CD4 recovery, HCDR: >500/mm3). RBD-binding IgG, SARS-CoV-2 neutralizing antibodies (nAbs) and IFN-γ release were measured. As control group, HIV-negative healthcare workers (HCWs) were used Findings Among 166 PLWH, after 1 month from the booster dose, detectable RBD-binding IgG were elicited in 86.7% of PCDR, 100% of ICDR, 98.7% of HCDR, and a neutralizing titre ≥1:10 elicited in 70.0%, 88.2%, and 93.1%, respectively. Compared to HCDR, all immune response parameters were significantly lower in PCDR. After adjusting for confounders, current CD4 T-cell <200/mm3 significantly predicted a poor magnitude of anti-RDB, nAbs and IFN-γ response. As compared with HCWs, PCDR elicited a consistently reduced immunogenicity for all parameters, ICDR only a reduced RBD-binding antibody response, whereas HCDR elicited a comparable immune response for all parameters Conclusion Humoral and cell-mediated immune response against SARS-CoV-2 were elicited in most of PLWH, albeit significantly poorer in those with CD4 T-cell <200/mm3 versus those with >500 cell/mm3 and HIV-negative controls. Background We aimed to quantify the unknown losses in health-related quality of life of coronavirus disease 2019 (COVID-19) cases using quality-adjusted lifedays (QALDs) and the recommended EQ-5D instrument in England. Methods Prospective cohort study of nonhospitalized, polymerase chain reaction (PCR)-confirmed severe acute respiratory syndrome coronavirus 2–positive (SARS-CoV-2–positive) cases aged 12–85 years and followed up for 6 months from 1 December 2020, with cross-sectional comparison to SARS-CoV-2–negative controls. Main outcomes were QALD losses; physical symptoms; and COVID-19-related private expenditures. We analyzed results using multivariable regressions with post hoc weighting by age and sex, and conditional logistic regressions for the association of each symptom and EQ-5D limitation on cases and controls. Results Of 548 cases (mean age 41.1 years; 61.5% female), 16.8% reported physical symptoms at month 6 (most frequently extreme tiredness, headache, loss of taste and/or smell, and shortness of breath). Cases reported more limitations with doing usual activities than controls. Almost half of cases spent a mean of £18.1 on nonprescription drugs (median: £10.0), and 52.7% missed work or school for a mean of 12 days (median: 10). On average, all cases lost 13.7 (95% confidence interval [CI]: 9.7, 17.7) QALDs, whereas those reporting symptoms at month 6 lost 32.9 (95% CI: 24.5, 37.6) QALDs. Losses also increased with older age. Cumulatively, the health loss from morbidity contributes at least 18% of the total COVID-19-related disease burden in the England. Conclusions One in 6 cases report ongoing symptoms at 6 months, and 10% report prolonged loss of function compared to pre-COVID-19 baselines. Background Coronavirus disease 2019 (COVID-19) vaccines are implemented worldwide in efforts to curb the pandemic. This study investigates the risk of a positive severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) reverse transcriptase polymerase chain reaction (RT-PCR) test following BNT162b2 vaccination in a large real-life population in Denmark. Methods Vaccination status and positive SARS-CoV-2 RT-PCR results from adults in the Capital Region of Denmark (n = 1 549 488) were obtained from national registries. PCR testing was free and widely available. The number of positive PCR tests per individual at risk was calculated as weekly rates. Time to positive PCR test was modelled using Kaplan-Meier methods and hazard ratios (HRs) were calculated using Cox regression. Results A total of 1 119 574 individuals received the first dose of BNT162b2 and 1 088 879 received a second dose of BNT162b2. Individuals were followed up to 8.7 months after first dose (median: 5.5 months; interquartile ratio: 4.1–8.7). Rates of PCR-confirmed SARS-CoV-2 infection 2–4 months after the second dose were 0.21, 0.33, and 0.36 per 1000 individuals per week at risk for July, August, and September, respectively. Four or more months after the second dose, the rates were 0.56, 0.76, and 0.53 per 1000 individuals per week at risk for July, August, and September, respectively. HR of SARS-CoV-2 infection after the second dose was 0.2 (95% confidence interval,.05–.48; P  = .001) for individuals with 8 months' follow-up. Conclusions Individuals who received 2 doses of the BNT162b2 COVID-19 vaccine had a low risk of breakthrough infection after up to 8 months of follow-up. Background Although BNT162b2 vaccine-efficacy analyses have been published, the effectiveness of the vaccine in preventing coronavirus disease 2019 given confirmed exposure has not been previously demonstrated, even though it has policy implications, such as the need for self-quarantine when exposure has occurred. Methods In a retrospective cohort study, we used data collected between 20 December 2020 and 17 March 2021 from the second largest healthcare provider in Israel to analyze the probability of an additional household infection occurring within 10 days after an index infection. In model 1, vaccine effectiveness was described for Fully Vaccinated individuals (7 or more days from second dose) vs either Unvaccinated individuals or those Recently Vaccinated Once (0–7 days from the first dose, presumably still unprotected). Secondary analyses included correction for differing testing rates. In model 2, we conducted a separate analysis of households comprised of only adults with the same vaccination status. Results A total of 173 569 households were included, of which 6351 had an index infection (mean [standard deviation] age, 58.9 [13.5] years); 50% were women. Adjusted vaccine effectiveness of Fully Vaccinated compared with Unvaccinated participants was 80.3% (95% confidence interval [CI], 73.5–85.4) and 82.0% (95% CI, 75.6–86.8) compared with those Recently Vaccinated Once. Conclusions The BNT162b2 vaccine is effective in high-risk real-life exposure scenarios, but the protection afforded in these settings is lower than that previously described. Background There are few data on the full spectrum of disease caused by severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection across the lifespan from community-based or nonclinical settings. Methods We followed 2338 people in Managua, Nicaragua, aged <94 years from March 2020 through March 2021. SARS-CoV-2 infection was identified through real-time reverse transcription polymerase chain reaction (RT-PCR) or through enzyme-linked immunosorbent assay. Disease presentation was assessed at the time of infection or retrospectively by survey at the time of blood collection. Results There was a large epidemic that peaked between March and August 2020. In total, 129 RT-PCR–positive infections were detected, for an overall incidence rate of 5.3 infections per 100 person-years (95% confidence interval [CI], 4.4–6.3). Seroprevalence was 56.7% (95% CI, 53.5%–60.1%) and was consistent from age 11 through adulthood but was lower in children aged ≤10 years. Overall, 31.0% of the infections were symptomatic, with 54.7% mild, 41.6% moderate, and 3.7% severe. There were 2 deaths that were likely due to SARS-CoV-2 infection, yielding an infection fatality rate of 0.2%. Antibody titers exhibited a J-shaped curve with respect to age, with the lowest titers observed among older children and young adults and the highest among older adults. When compared to SARS-CoV-2–seronegative individuals, SARS-CoV-2 seropositivity at the midyear sample was associated with 93.6% protection from symptomatic reinfection (95% CI, 51.1%–99.2%). Conclusions This population exhibited a very high SARS-CoV-2 seropositivity with lower-than-expected severity, and immunity from natural infection was protective against symptomatic reinfection. Background Data on the development of neutralizing antibodies (nAbs) against SARS-CoV-2 after SARS-CoV-2 infection and after vaccination with mRNA COVID-19 vaccines are limited. Methods From a prospective cohort of 3975 adult essential and frontline workers tested weekly from August 2020 to March 2021 for SARS-CoV-2 infection by reverse transcription–polymerase chain reaction assay irrespective of symptoms, 497 participants had sera drawn after infection (170), vaccination (327), and after both infection and vaccination (50 from the infection population). Serum was collected after infection and each vaccine dose. Serum-neutralizing antibody titers against USA-WA1/2020-spike pseudotype virus were determined by the 50% inhibitory dilution. Geometric mean titers (GMTs) and corresponding fold increases were calculated using t tests and linear mixed-effects models. Results Among 170 unvaccinated participants with SARS-CoV-2 infection, 158 (93%) developed nAbs with a GMT of 1003 (95% confidence interval, 766–1315). Among 139 previously uninfected participants, 138 (99%) developed nAbs after mRNA vaccine dose 2 with a GMT of 3257 (2596–4052). GMT was higher among those receiving mRNA-1273 vaccine (GMT, 4698; 3186–6926) compared with BNT162b2 vaccine (GMT, 2309; 1825–2919). Among 32 participants with prior SARS-CoV-2 infection, GMT was 21 655 (14 766–31 756) after mRNA vaccine dose 1, without further increase after dose 2. Conclusions A single dose of mRNA vaccine after SARS-CoV-2 infection resulted in the highest observed nAb response. Two doses of mRNA vaccine in previously uninfected participants resulted in higher nAbs to SARS-CoV-2 than after 1 dose of vaccine or SARS-CoV-2 infection alone. nAb response also differed by mRNA vaccine product. Background There is an urgent need to understand the real-world effectiveness of remdesivir in the treatment of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2). Methods This was a retrospective comparative effectiveness study. Individuals hospitalized in a large private healthcare network in the United States from 23 February 2020 through 11 February 2021 with a positive test for SARS-CoV-2 and ICD-10 diagnosis codes consistent with symptomatic coronavirus disease 2019 (COVID-19) were included. Remdesivir recipients were matched to controls using time-dependent propensity scores. The primary outcome was time to improvement with a secondary outcome of time to death. Results Of 96 859 COVID-19 patients, 42 473 (43.9%) received at least 1 remdesivir dose. The median age of remdesivir recipients was 65 years, 23 701 (55.8%) were male, and 22 819 (53.7%) were non-White. Matches were found for 18 328 patients (43.2%). Remdesivir recipients were significantly more likely to achieve clinical improvement by 28 days (adjusted hazard ratio [aHR] 1.19, 95% confidence interval [CI], 1.16–1.22). Remdesivir patients on no oxygen (aHR 1.30, 95% CI, 1.22–1.38) or low-flow oxygen (aHR 1.23, 95% CI, 1.19–1.27) were significantly more likely to achieve clinical improvement by 28 days. There was no significant impact on the likelihood of mortality overall (aHR 1.02, 95% CI,.97–1.08). Remdesivir recipients on low-flow oxygen were significantly less likely to die than controls (aHR 0.85, 95% CI,.77–.92; 28-day mortality 8.4% [865 deaths] for remdesivir patients, 12.5% [1334 deaths] for controls). Conclusions These results support the use of remdesivir for hospitalized COVID-19 patients on no or low-flow oxygen. Background We evaluated a standardized interferon-γ (IFN-γ) release assay (IGRA) for detection of T-cell immune response after severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection or vaccination. Methods This prospective study included patients with coronavirus disease 2019 (COVID-19) with different severity of illness and follow-up (FU), vaccinated subjects, and healthy unvaccinated persons. SARS-CoV-2 T-cell response was measured using a specific quantitative IGRA in whole blood (Euroimmun, Germany) and TrimericS-IgG and neutralizing antibodies with validated serological platforms. Positivity of reverse transcription–polymerase chain reaction or vaccination was considered as the reference standard. Results A total of 239 individuals were included (152 convalescent, 54 vaccinated, and 33 uninfected unvaccinated). Overall sensitivity, specificity, and positive- and negative-predictive values (95% confidence interval) of the IGRA were 81.1% (74.9–86%), 90.9% (74.5–97.6%), 98.2% (94.5–99.5%), and 43.5% (31.8–55.9%), respectively. All vaccinated SARS-CoV-2-naive subjects had positive IGRA at 3 months. In convalescent subjects the magnitude of IFN-γ responses and IGRA accuracy varied according to disease severity and duration of FU, with the best performance in patients with severe COVID-19 at 3 months and the worst in those with mild disease at 12 months. The greatest contribution of IGRA to serological tests was observed in patients with mild disease and long-term FU (incremental difference, 30.4%). Conclusions The IGRA was a reliable method of quantifying T-cell response after SARS-COV-2 infection or vaccination. In convalescent patients, the sensitivity is largely dependent on disease severity and time since primary infection. Severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) has rapidly become a major global health problem, and public health surveillance is crucial to monitor and prevent virus spread. Wastewater-based epidemiology has been proposed as an addition to disease-based surveillance because virus is shed in the feces of ≈40% of infected persons. We used next-generation sequencing of sewage samples to evaluate the diversity of SARS-CoV-2 at the community level in the Netherlands and Belgium. Phylogenetic analysis revealed the presence of the most prevalent clades (19A, 20A, and 20B) and clustering of sewage samples with clinical samples from the same region. We distinguished multiple clades within a single sewage sample by using low-frequency variant analysis. In addition, several novel mutations in the SARS-CoV-2 genome were detected. Our results illustrate how wastewater can be used to investigate the diversity of SARS-CoV-2 viruses circulating in a community and identify new outbreaks.

Genomic studies have shown that approximately 2% of the human genome encodes protein, and the rest consists of non-coding RNAs (ncRNA). ncRNAs are the modifications that can alter gene expression at different levels without making any changes on the DNA sequence. One of the most important function of non-coding RNAs is to modulate host immunity and inflammatory response by regulating chromatin structure and gene expression. The World Health Organization has declared Severe Acute Respiratory Syndrome Coronavirus 2 (SARS-CoV-2) as Coronavirus Disease 2019 (COVID-19). COVID-19 shows symptoms such as fever, cough, shortness of breath and it can lead to severe symptomatic acute respiratory distress syndrome (ARDS), bring along many problems such as cardiovascular complications, kidney damage, stroke and death. Virus host interactions associated with the immune system is also important beside the viral replication mechanism in the development of a successful therapeutic target for SARS-CoV-2. Epigenetic mechanisms that regulate virus-host interactions affect the extent and adequacy of immune and inflammatory responses that are important for clinical outcomes. Thus, identifying ncRNAs involved in SARS-CoV-2 infection will help to understand the epigenetic regulation underlying the immune response that occurs and to develop new specific strategies to prevent and treat the infection. In this review, the importance of ncRNA mediated gene regulation in SARS-CoV-2 infection and its role in the ongoing COVID-19 pandemic were discussed.

Recently, coronavirus disease 2019 (COVID-19) has been considered as a major health problem around the globe. This severe acute respiratory syndrome has a bunch of features, such as high transmission rate, which are adding to its importance. Overcoming this disease relies on a complete understanding of the viral structure, receptors, at-risk cells or tissues, and pathogenesis. Currently, researches have shown that besides the lack of a proper anti-viral therapeutic method, complications provided by this virus are also standing in the way of decreasing its mortality rate. One of these complications is believed to be a hematologic manifestation. Commonly, three kinds of coagulopathies are detected in COVID-19 patients: disseminated intravascular coagulation (DIC), pulmonary embolism (PE), and deep vein thrombosis (DVT). In this paper, we have reviewed the relation between these conditions and coronavirus-related diseases pathogenesis, severity, and mortality rate.

En diciembre de 2019 se identificó por primera vez en Wuhan China el SARS-CoV-2, un nuevo tipo de coronavirus de la familia Coronaviridae del género β-CoV. El 11 de marzo del 2020 la OMS lo declara pandemia y hasta el 19 de diciembre de 2021 han sido afectados 192 países con 235 547 850 casos confirmados y 4 811 951 personas fallecidas. El SARS-CoV-2 afecta a los humanos, sin distinción de género, raza o edad, pero las personas con comorbilidades como hipertensión arterial, enfermedades cardiovasculares o diabetes Mellitus tienen peor pronóstico. Entre las rutas de transmisión de persona a persona se describen el contacto directo con mucosas y por inhalación de aerosoles o saliva. Por lo cual solo con guantes, mascarilla quirúrgica y visor como protección, los profesionales con más alto riesgo de contagio son los odontólogos, al estar en contacto directo con el paciente, con instrumental, materiales contaminados de fluidos del paciente y con los aerosoles que se generan en algunos de los procedimientos que realizan.Se realizó una búsqueda bibliográfica en PubMed, JOMOS, NIH y CDC, analizando los datos encontrados para proponer los cambios más adaptables a nuestro entorno.El objetivo de este trabajo de revisión de información reciente y relevante acerca de la atención odontológica en tiempos de SARS-CoV-2, es proponer cambios estructurales en la atención, para garantizar la bioseguridad de pacientes, personal odontológico y talento humano presente en la consulta odontológica.Teniendo en cuenta la situación ocasionada por la pandemia de SARS-CoV-2, es recomendable adaptar e implementar medidas en la atención, pertinentes al comportamiento de este virus.

Las vacunas son productos biológicos que contienen antígenos que buscan generar protección contra a la exposición real de un agente patógeno. En cuanto a su importancia, hacen parte de las intervenciones más costo-efectivas en salud pública, siendo superadas únicamente por el agua potable. A grandes rasgos, podemos dividir las vacunas en vivas atenuadas e inactivadas; no obstante, el nuevo coronavirus ha producido la emergencia de plataformas innovadoras que utilizan mecanismos intracelulares y moleculares con el mismo objetivo de generar inmunidad.Se realizó una búsqueda sistemática de la literatura utilizando las bases de datos electrónicas Pubmed, Scopus y Web of Science. Todos los tipos de diseño de estudio fueron considerados, sin embargo, se prefierieron aquellos redactados en idioma inglés o español. Se hace una revisión de la literatura presente sobre las plataformas existentes para generar inmunidad frente al coronavirus SARS-CoV-2 y se desarrolla cada una según su ruta y forma de acción en aquellas basadas en subunidades proteicas, vector viral recombinante, ácidos nucleicos, virus inactivados, partículas virales y virus vivos atenuados.Los mecanismos por los cuales dichas vacunas generan inmunogenicidad son diferentes, no obstante, la constante inserción de mutaciones por parte del virus sigue siendo un objeto de interés y preocupación por los investigadores.

Background One- and two-dose mRNA vaccine effectiveness (VE) estimates against severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection by dosing interval and time since vaccination were assessed among healthcare workers (HCWs) in publicly funded acute and community (nonresidential) healthcare facilities in British Columbia, Canada. Methods A test-negative design was used with controls matched to cases (6:1) on epidemiological week of SARS-CoV-2 test date. mRNA vaccination was defined by receipt of the first dose ≥21 days or second dose ≥14 days before the test date. HCWs ≥18 years old tested for SARS-CoV-2 between epi-weeks 3 and 39 (January 17–October 2, 2021) were included, when varying dosing intervals and a mix of circulating variants of concern contributed, including Delta dominance provincially from epi-week 31 (August 1). Results Single- and two-dose analyses included 1265 and 1246 cases, respectively. The median follow-up period (interquartile range) was 49 (34–69) days for single-dose and 89 (61–123) days for two-dose recipients, with 12%, 31%, and 58% of second doses given 3–5, 6, or ≥7 weeks after the first. Adjusted mRNA VE against SARS-CoV-2 was 71% (95% CI, 66%–76%) for one dose and 90% (95% CI, 88%–92%) for two doses, similar to two heterologous mRNA doses (92%; 95% CI, 86%–95%). Two-dose VE remained >80% at ≥28 weeks post–second dose. Two-dose VE was consistently 5%–7% higher with a ≥7-week vs 3–5-week interval between doses, but with overlapping confidence intervals. Conclusions Among HCWs, we report substantial single-dose and strong and sustained two-dose mRNA vaccine protection, with the latter maintained for at least 7 months. Background Severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) viral RNA (vRNA) is detected in the bloodstream of some patients with coronavirus disease 2019 (COVID-19), but it is not clear whether this RNAemia reflects viremia (ie, virus particles) and how it relates to host immune responses and outcomes. Methods SARS-CoV-2 vRNA was quantified in plasma samples from observational cohorts of 51 COVID-19 patients including 9 outpatients, 19 hospitalized (non–intensive care unit [ICU]), and 23 ICU patients. vRNA levels were compared with cross-sectional indices of COVID-19 severity and prospective clinical outcomes. We used multiple imaging methods to visualize virions in plasma. Results SARS-CoV-2 vRNA was detected in plasma of 100%, 52.6%, and 11.1% of ICU, non-ICU, and outpatients, respectively. Virions were detected in plasma pellets using electron tomography and immunostaining. Plasma vRNA levels were significantly higher in ICU > non-ICU > outpatients (P  < .0001); for inpatients, plasma vRNA levels were strongly associated with higher World Health Organization (WHO) score at admission (P  = .01), maximum WHO score (P  = .002), and discharge disposition (P  = .004). A plasma vRNA level >6000 copies/mL was strongly associated with mortality (hazard ratio, 10.7). Levels of vRNA were significantly associated with several inflammatory biomarkers (P  < .01) but not with plasma neutralizing antibody titers (P  = .8). Conclusions Visualization of virus particles in plasma indicates that SARS-CoV-2 RNAemia is due, at least in part, to viremia. Data visualization and use of statistics resulting into reliable databases could minimize uncertainty and optimize decision making in terms of both time and resources. Accordingly, this study presents a comprehensive geotechnical database of soil types found in Lahore division, which consists of four important districts of Punjab province, namely, Lahore, Sheikhupura, Kasur, and Nankana-Sahib. Data from 584 geotechnical investigation sites have been collected and analyzed that comprised of 433 sites for Lahore, 57 sites for Sheikhupura, 53 sites for Nankana Sahib, and 41 sites for Kasur District. The data is analyzed digitally and classification of areas into various zones based on standard penetration test results, which have been transformed into digitized geotechnical maps using ArcGIS package. To ensure repeatability and enhanced rigor, the data have been properly evaluated using methods of descriptive and inferential statistics to give the geotechnical designer a confidence to select appropriate SPT-N value while evaluating soil strength in a specific zone. The outcome of this study validated using tools of statistical analysis shall help practicing engineers to carry out prompt preliminary estimation of bearing capacity and other geotechnical parameters with reasonable reliability for foundation design considerations and planning the scope of geotechnical investigations for a site. Individuals on immunosuppressive (IS) therapy have increased mortality from severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection, and delayed viral clearance may lead to new viral variants. IS therapy reduces antibody responses following coronavirus disease 2019 (COVID-19) messenger RNA (mRNA) vaccination; however, a comprehensive assessment of vaccine immunogenicity is lacking. Here we show that IS therapy reduced neutralizing, binding, and nonneutralizing antibody functions in addition to CD4 and CD8 T-cell interferon-γ responses following COVID-19 mRNA vaccination compared to immunocompetent individuals. Moreover, IS therapy reduced cross-reactivity against SARS-CoV-2 variants. Background We aimed to describe the clinical presentation of individuals presenting with prolonged recovery from coronavirus disease 2019 (COVID-19), known as long COVID. Methods This was an analysis within a multicenter, prospective cohort study of individuals with a confirmed diagnosis of COVID-19 and persistent symptoms >4 weeks from onset of acute symptoms. We performed a multiple correspondence analysis (MCA) on the most common self-reported symptoms and hierarchical clustering on the results of the MCA to identify symptom clusters. Results Two hundred thirty-three individuals were included in the analysis; the median age of the cohort was 43 (interquartile range [IQR], 36–54) years, 74% were women, and 77.3% reported a mild initial illness. MCA and hierarchical clustering revealed 3 clusters. Cluster 1 had predominantly pain symptoms with a higher proportion of joint pain, myalgia, and headache; cluster 2 had a preponderance of cardiovascular symptoms with prominent chest pain, shortness of breath, and palpitations; and cluster 3 had significantly fewer symptoms than the other clusters (2 [IQR, 2–3] symptoms per individual in cluster 3 vs 6 [IQR, 5–7] and 4 [IQR, 3–5] in clusters 1 and 2, respectively; P  < .001). Clusters 1 and 2 had greater functional impairment, demonstrated by significantly longer work absence, higher dyspnea scores, and lower scores in SF-36 domains of general health, physical functioning, and role limitation due to physical functioning and social functioning. Conclusions Clusters of symptoms are evident in long COVID patients that are associated with functional impairments and may point to distinct underlying pathophysiologic mechanisms of disease. This analysis describes clinical characteristics and outcomes of 128 fully vaccinated patients hospitalized with coronavirus disease 2019 (COVID-19) prior to booster vaccines. We found that 27% were asymptomatic, 52% required intensive care, and 15% died from COVID-19.
