1. Information Trust Falls: The Role of Social Networks and Information During the COVID-19 Pandemic Among Suburbanites

Abstract: Public cooperation with health experts and authorities plays a critical role in curbing the spread of disease during outbreaks such as the COVID-19 pandemic. Through data collected using mixed methods in May and June of 2020, we investigate the influence of information from horizontal and vertical ties on the likelihood that individuals would practice safe and healthy behavior. We look at actions such as staying home, wearing personal protective equipment, and increased handwashing in two northeastern U.S. metro areas. Controlling for factors thought critical in previous studies, our analysis of more than eight hundred survey responses and more than sixty interviews finds that reliance on information from horizontal and vertical ties correlates significantly with behaviors designed to curb the spread of the virus.

2. Social capital's impact on COVID-19 outcomes at local levels

Abstract: Abstract Over the past thirty years, disaster scholars have highlighted that communities with stronger social infrastructure—including social ties that enable trust, mutual aid, and collective action—tend to respond to and recover better from crises. However, comprehensive measurements of social capital across communities have been rare. This study adapts Kyne and Aldrich’s (Risk Hazards Crisis Public Policy 11, 61–86, 2020) county-level social capital index to the census-tract level, generating social capital indices from 2011 to 2018 at the census-tract, zipcode, and county subdivision levels. To demonstrate their usefulness to disaster planners, public health experts, and local officials, we paired these with the CDC’s Social Vulnerability Index to predict the incidence of COVID-19 in case studies in Massachusetts, Wisconsin, Illinois, and New York City. We found that social capital predicted 41–49% of the variation in COVID-19 outbreaks, and up to 90% with controls in specific cases, highlighting its power as diagnostic and predictive tools for combating the spread of COVID.

3. Cardiovascular Events and Long‐Term Risk of Sudden Death Among Stabilized Patients After Acute Coronary Syndrome: Insights From IMPROVE‐IT

Abstract: Background Unlike patients with low ejection fraction after an acute coronary syndrome (ACS), little is known about the long‐term incidence and influence of cardiovascular events before sudden death among stabilized patients after ACS. Methods and Results A total of 18 144 patients stabilized within 10 days after ACS in IMPROVE‐IT (Improved Reduction of Outcomes: Vytorin Efficacy International Trial) were studied. Cumulative incidence rates (IRs) and IRs per 100 patient‐years of sudden death were calculated. Using Cox proportional hazards, the association of ≥1 additional postrandomization cardiovascular events (myocardial infarction, stroke, and hospitalization for unstable angina or heart failure) with sudden death was examined. Early (≤1 year after ACS) and late sudden deaths (>1 year) were compared. Of 2446 total deaths, 402 (16%) were sudden. The median time to sudden death was 2.7 years, with 109 early and 293 late sudden deaths. The cumulative IR was 2.47% (95% CI, 2.23%–2.73%) at 7 years of follow‐up. The risk of sudden death following a postrandomization cardiovascular event (150/402 [37%] sudden deaths; median 1.4 years) was greater (IR/100 patient‐years, 1.45 [95% CI, 1.23–1.69]) than the risk with no postrandomization cardiovascular event (IR/100 patient‐years, 0.27 [95% CI, 0.24–0.30]). Postrandomization myocardial infarction (hazard ratio [HR], 3.64 [95% CI, 2.85–4.66]) and heart failure (HR, 4.55 [95% CI, 3.33–6.22]) significantly increased future risk of sudden death. Conclusions Patients stabilized within 10 days of an ACS remain at long‐term risk of sudden death with the greatest risk in those with an additional cardiovascular event. These results refine the long‐term risk and risk effectors of sudden death, which may help clinicians identify opportunities to improve care. Registration: URL: http://www.clinicaltrials.gov. 4. The importance of weight stabilization amongst those with overweight or obesity: Results from a large health care system

Abstract: Data on patterns of weight change among adults with overweight or obesity are minimal. We aimed to examine patterns of weight change and associated hospitalizations in a large health system, and to develop a model to predict 2-year significant weight gain.Data from the Duke University Health System was abstracted from 1/1/13 to 12/31/16 on patients with BMI ≥ 25 kg/m2 in 2014. A regression model was developed to predict patients that would increase their weight by 10% within 2 years. We estimated the association between weight change category and all-cause hospitalization using Cox proportional hazards models.Of the 37,253 patients in our cohort, 59% had stable weight over 2 years, while 24% gained ≥ 5% weight and 17% lost ≥ 5% weight. Our predictive model had reasonable discriminatory capacity to predict which individuals would gain ≥ 10% weight over 2 years (AUC 0.73). Compared with stable weight, the risk of hospitalization was increased by 37% for individuals with > 10% weight loss [adj. HR (95% CI): 1.37 (1.25,1.5)], by 30% for those with > 10% weight gain [adj. HR (95% CI): 1.3 (1.19,1.42)], by 18% for those with 5–10% weight loss [adj. HR (95% CI): 1.18 (1.09,1.28)], and by 10% for those with 5–10% weight gain [adj. HR (95% CI): 1.1 (1.02,1.19)].In this examination of a large health system, significant weight gain or loss of > 10% was associated with increased all-cause hospitalization over 2 years compared with stable weight. This analysis adds to the increasing observational evidence that weight stability may be a key health driver.

5. Gut-Brain Endocannabinoid Control of Obesity and Anxiety

Abstract: The endocannabinoid system (ECS) exerts control over energy homeostasis via interactions between lipid messengers called endocannabinoids (eCBs) and cannabinoid receptors. ECS components are abundant in the CNS and the gastrointestinal (GI) tract. Bi-directional communication between the CNS and GI tract occurs via the vagus nerve. Dysregulation of vagal signaling is associated with adverse physiological and psychological outcomes, such as obesity and anxiety disorders, respectively. We previously showed that eCBs in the GI tract of obese rodents are elevated and drive feeding through peripheral cannabinoid receptor activation. Here, we examined the effects of diet-induced obesity (DIO) on efferent vagus nerve signaling and intestinal eCB formation. We tested the hypothesis that elevated parasympathetic signaling by the efferent vagus is the source of the elevated eCB content and hyperphagia observed in obese mice. We first measured cFos immunoreactivity in the dorsal motor nucleus (DMV) of the efferent vagus in DIO mice. Next, we tested the effects of treatment with muscarinic acetylcholine receptor (mAChR) antagonists on intestinal eCB formation, eCB synthetic enzyme activity, and food intake. Finally, we utilized our conditional intestinal epithelium-specific cannabinoid receptor subtype-1 (CB1R) knockout model (IntCB1-/-) to elucidate the role of intestinal CB1Rs in this process. DMV neuronal activation was significantly elevated in DIO mice compared to lean controls. Treatment with mAChR antagonists reduced intestinal eCB levels, eCB synthetic enzyme activity, and caloric intake in DIO animals. Furthermore, we showed that intestinal CB1Rs are required for mAChR antagonist-induced attenuation of food intake. To evaluate the contribution of intestinal ECS components to the expression of anxious behaviors, we subjected IntCB1-/- male and female mice to a battery of behavioral tests. We quantified circulating corticosterone (CORT) levels at baseline and immediately following behavioral testing. IntCB1-/- male mice exhibited an anxiolytic phenotype that was absent in females. These sex differences were associated with a significant increase in plasma CORT levels for female mice at both time points, regardless of genotype. This body of work reveals a previously unidentified role for the vagus nerve in the context of DIO and behavioral anxiety and highlights critical contributions of the ECS to gut-brain signaling.

9. Use of Sodium-Glucose Cotransporter 2 Inhibitors and Glucagonlike Peptide-1 Receptor Agonists in Patients With Diabetes and Cardiovascular Disease in Community Practice

Abstract: ImportanceRecent national guidelines recommend sodium-glucose cotransporter 2 inhibitors (SGLT2i) and glucagonlike peptide-1 receptor agonists (GLP-1 RA) in patients with type 2 diabetes (T2D) and atherosclerotic cardiovascular disease (ASCVD); yet, there are limited data on the use of these agents in contemporary community practice.ObjectiveTo evaluate the use of SGLT2i and GLP-1 RA in adults with T2D and ASCVD across a diverse sample of health care systems in the US.Design, Setting, and ParticipantsThis multicenter, retrospective cohort study used electronic health record data from 88 US health care systems participating in Cerner Real World Data between January 2018 to March 2021. Adults with ASCVD and T2D taking at least 1 glucose-lowering medication, had end-stage kidney disease, or had stage 5 chronic kidney disease were excluded.Main Outcomes and MeasuresTreatment with SGLT2i or GLP-1 RA.ResultsA total of 321 304 patients were identified with T2D and ASCVD ASCVD (130 280 female [40.5%]; median [IQR] age, 70.9 [62.9-78.0] years) who were potentially eligible for SGLT2i and/or GLP-1 RA, including 37 754 Black individuals (11.8%), 51 522 Hispanic individuals (16.0%), and 256 008 White individuals (11.8%). From January 2018 to March 2021, the use of SGLT2i increased from 5.8% (11 285 of 194 264) to 12.9% (11 058 of 85 956), GLP-1 RA increased from 6.9% (13 402 of 194 264) to 13.8% (11 901 of 85 956), and use of either agent increased from 11.4% (22 069 of 194 264) to 23.2% (19 909 of 85 956). Those taking an SGLT2i or GLP-1 RA were younger, less frequently hospitalized in the year prior, and more likely to be taking additional secondary prevention medications. Treated and nontreated populations were similar in terms of race, ethnicity, and outpatient health care utilization. Sulfonylureas and dipeptidyl peptidase 4 inhibitors remained more commonly used than SGLT2i or GLP-1 RA through 2021.Conclusions and RelevanceIn this study, uptake of SGLT2i and GLP-1 RA in adults with T2D and ASCVD increased modestly after guideline recommendations, although less than a quarter of persons with ASCVD and T2D receiving medical therapy were taking either. Further efforts are necessary to maximize the potential population benefit of these therapies in this high-risk population.

10. Predictive Utility of a Validated Polygenic Risk Score for Long-Term Risk of Coronary Heart Disease in Young and Middle-Aged Adults

Abstract: Background: Understanding the predictive utility of previously derived polygenic risk scores (PRSs) for long-term risk of coronary heart disease (CHD) and its additive value beyond traditional risk factors can inform prevention strategies. Methods: Data from adults 20 to 59 years of age who were free of CHD from the FOS (Framingham Offspring Study) and the ARIC (Atherosclerosis Risk in Communities) study were analyzed. Because the PRS was derived from samples of predominantly European ancestry, individuals who self-reported White race were included. The sample was stratified by age and cohort: young (FOS, 20–39 years [median, 30 years] of age), early midlife (FOS, 40–59 years [median, 43] years of age), and late midlife (ARIC, 45–59 years [median, 52 years] of age). Two previously derived and validated prediction tools were applied: (1) a 30-year traditional risk factor score and (2) a genome-wide PRS comprising >6 million genetic variants. Hazard ratios for the association between each risk estimate and incident CHD were calculated. Predicted and observed rates of CHD were compared to assess discrimination for each model individually and together with the optimism-corrected C index (95% CI). Results: Among 9757 participants, both the traditional risk factor score (hazard ratio per 1 SD, 2.60 [95% CI, 2.08–3.27], 2.09 [95% CI, 1.83–2.40], and 2.11 [95% CI, 1.96–2.28]) and the PRS (hazard ratio, 1.98 [95% CI, 1.70–2.30], 1.64 [95% CI, 1.47–1.84], and 1.22 [95% CI, 1.15–1.30]) were significantly associated with incident CHD in young, early midlife, and late midlife, respectively. Discrimination was similar or better for the traditional risk factor score (C index, 0.74 [95% CI, 0.70–0.78], 0.70 [95% CI, 0.67–0.72], and 0.72 [95% CI, 0.70–0.73]) compared with an age- and sex-adjusted PRS (0.73 [95% CI, 0.69–0.78], 0.66 [95% CI, 0.62–0.69], and 0.66 [95% CI, 0.64–0.67]) in young, early-midlife, and late-midlife participants, respectively. The ΔC index when PRS was added to the traditional risk factor score was 0.03 (95% CI, 0.001–0.05), 0.02 (95% CI, −0.002 to 0.037), and 0.002 (95% CI, −0.002 to 0.006) in young, early-midlife, and late-midlife participants, respectively. Conclusions: Despite a statistically significant association between PRS and 30-year risk of CHD, the C statistic improved only marginally with the addition of PRS to the traditional risk factor model among young adults and did not improve among midlife adults. PRS, an immutable factor that cannot be directly intervened on, has minimal clinical utility for long-term CHD prediction when added to a traditional risk factor model.

Abstract: BACKGROUND: Among patients with stable coronary disease and moderate or severe ischemia, whether clinical outcomes are better in those who receive an invasive intervention plus medical therapy than in those who receive medical therapy alone is uncertain. METHODS: We randomly assigned 5179 patients with moderate or severe ischemia to an initial invasive strategy (angiography and revascularization when feasible) and medical therapy or to an initial conservative strategy of medical therapy alone and angiography if medical therapy failed. The primary outcome was a composite of death from cardiovascular causes, myocardial infarction, or hospitalization for unstable angina, heart failure, or resuscitated cardiac arrest. A key secondary outcome was death from cardiovascular causes or myocardial infarction. RESULTS: Over a median of 3.2 years, 318 primary outcome events occurred in the invasive-strategy group and 352 occurred in the conservative-strategy group. At 6 months, the cumulative event rate was 5.3% in the invasive-strategy group and 3.4% in the conservative-strategy group (difference, 1.9 percentage points; 95% confidence interval [CI], 0.8 to 3.0); at 5 years, the cumulative event rate was 16.4% and 18.2%, respectively (difference, -1.8 percentage points; 95% CI, -4.7 to 1.0). Results were similar with respect to the key secondary outcome. The incidence of the primary outcome was sensitive to the definition of myocardial infarction; a secondary analysis yielded more procedural myocardial infarctions of uncertain clinical importance. There were 145 deaths in the invasive-strategy group and 144 deaths in the conservative-strategy group (hazard ratio, 1.05; 95% CI, 0.83 to 1.32). CONCLUSIONS: Among patients with stable coronary disease and moderate or severe ischemia, we did not find evidence that an initial invasive strategy, as compared with an initial conservative strategy, reduced the risk of ischemic cardiovascular events or death from any cause over a median of 3.2 years. The trial findings were sensitive to the definition of myocardial infarction that was used. Abstract: International audience; Background: Prior analyses suggest an association between formula-based plasma volume (PV) estimates and outcomes in heart failure (HF). We assessed the association between estimated PV status by the Duarte-ePV and Kaplan Hakim (KH-ePVS) formulas, and in-hospital and postdischarge clinical outcomes, in the ASCEND-HF trial.Methods and results: The KH-ePVS and Duarte-ePV were calculated on admission. We assessed associations with in-hospital worsening HF, 30-day composite cardiovascular mortality or HF rehospitalization and 180-day all-cause mortality. There were 6373 (89.2%), and 6354 (89.0%) patients who had necessary characteristics to calculate KH-ePVS and Duarte-ePV, respectively. There was no association between PV by either formula with in-hospital worsening HF. KH-ePVS showed a weak correlation with N-terminal prohormone BNP, and with measures of decongestion such as body weight change and urine output (r < 0.3 for all). Duarte-ePV was trending toward an association with worse 30-day (adjusted odds ratio 1.07, 95% confidence interval [CI] 1.00-1.15, P = .058), but not 180-day outcomes (adjusted hazard ratio 1.03, 95% CI 0.97-1.09, P = .289). A continuous KH-ePVS of >0 (per 10-unit increase) was associated with improved 30-day outcomes (adjusted odds ratio 0.75, 95% CI 0.62-0.91, P = .004). The continuous KH-ePVS was not associated with 180-day outcomes (adjusted hazard ratio 1.05, 95% CI 0.98-1.12, P = .139).Conclusions: Baseline PV estimates had a weak association with in-hospital measures of decongestion. The Duarte-ePV trended toward an association with early clinical outcomes in decompensated HF, and may improve risk stratification in HF.

Abstract: Guidelines recommend aggressive low-density lipoprotein cholesterol (LDL-C) lowering in patients with atherosclerotic cardiovascular disease (ASCVD). However, the recommended threshold of LDL-C ≤70 mg/dL is often not achieved. We used data from the Duke University Health System electronic health record to characterize patterns of lipid levels and lipid management in patients with ASCVD to estimate the number of clinical events that could be prevented by achieving LDL-C ≤70 mg/dL . A multivariable logistic regression model was developed to predict the 1-year composite of all-cause mortality, myocardial infarction, stroke, or coronary revascularization and was validated through bootstrapping. The number needed to treat to prevent an event was then determined. Among 56,230 patients with ASCVD, the median (quartile 1, quartile 3) age was 68.6 years (59.9, 76.2), 47% were women, and 27% were non-Hispanic Black. LDL-C was70 mg/dL in 39,566 of patients (70%); these patients were more frequently female (51% vs 36%), non-Hispanic Black (28% vs 23%), and less frequently on statin therapy (67% vs 91%) than those with LDL-C ≤70 mg/dL . A predictive model with reasonable discrimination (c-index 0.77, 95% confidence interval 0.760 to 0.77) and calibration (slope 0.99) determined that if the overall population achieved an LDL-C ≤70 mg/dL, 734 clinical events (455 myocardial infarctions, 186 strokes, and 93 coronary revascularizations) could be prevented in a year. Achieving LDL-C ≤70 mg/dL in patients with ASCVD across a health system could prevent significant clinical events within a single year. In conclusion, this study quantifies the potential benefit of a system-wide effort to achieve guideline-based LDL-C goals.

Abstract: Social media is changing the narrative during crisis events. It has facilitated citizen-led emergency dispatch and rescue, information sharing and communication between loved ones in the moments before, during and after a disaster. Researchers of social capital have found that bonding, bridging, and linking social capital can lead to resilient outcomes. With increased use of social media on a day-to-day basis and during a crisis, we still know little about the association between social capital and online social media use. Controlling for demographic characteristics and earthquake intensity, I investigate the association bonding, bridging and linking social capital and hyperlocal social media use following the earthquake and its aftershocks. Using a quantitative cross-sectional longitudinal study across 121 Nextdoor online neighborhoods in California’s Napa Valley region across 42 days in August and September of 2014 (N = 3570), I find that bridging and linking social capital led to more online communication via Nextdoor. This finding comes with an important implication, namely, that social media can serve as a primary source of recourse for individuals and communities following a disaster. Social media platforms provide a conduit for accessing and activating bridging and linking ties that can expedite collective action in a time of need. Communities should consider policies that increase levels of social capital, well as social media platforms that can activate social ties when they are needed most.

Abstract: Hurricane Harvey was social media's first real stress test as a disaster response and recovery mechanism. A confluence of conditions makes it an ideal case study of social media's role in disaster recovery: the lack of a government-issued evacuation order, a call from government leadership for willing and able volunteers with a boat or high-water vehicle to perform life-saving rescues, and wide-spread adoption of social media platforms in the Houston area. While research on online social networks and disasters continues to grow, social scientists know little about how these online networks transform during a crisis and, further, how they drive disaster outcomes. With two original datasets, this study investigates how Houston's online social network transformed during Hurricane Harvey (2017), and the relationship between social media activity and post-Harvey recovery. The findings of a social network analysis (N= 2,387,610) and subsequent statistical analyses reveal the Houston-area online social network grew denser, clustered, and more efficient during the disaster. A spatial analysis and three separate regression models of activity before, during, and after Hurricane Harvey reveal that among 333 Nextdoor Neighborhoods, hyperlocal social media activity was a statistically significant predictor of the rate of rebuilding in these geographically based online communities. These findings suggest that policy and decision-makers should invest into online and offline hyperlocal social networks well before a disaster strikes, and leverage resources and legislation to maintain and strengthen the telecommunications and energy infrastructure that supports access to social media and telecommunications infrastructure during a time of crisis.

Abstract: BACKGROUND: Unanticipated respiratory compromise that lead to unplanned intubations is a known phenomenon in hospitalized patients. Most events occur in patients at high risk in well-monitored units; less is known about the incidence, risk factors, and trajectory of patients thought at low risk on lightly monitored general care wards. The aims of our study were to quantify demographic and clinical characteristics associated with unplanned intubations on general care floors and to analyze the medications administered, monitoring strategies, and vital-sign trajectories before the event. METHODS: We performed a multicenter retrospective cohort study of hospitalized subjects on the general floor who had unanticipated, unplanned intubations on general care floors from August 2014 to February 2018. RESULTS: We identified 448 unplanned intubations. The incidence rate was 0.420 per 1,000 bed-days (95% CI 0.374–0.470) in the academic hospital and was 0.430 (95% CI 0.352–0.520) and 0.394 per 1,000 bed-days (95% CI 0.301–0.506) at our community hospitals. Extrapolating these rates to total hospital admissions in the United States, we estimate 64,000 events annually. The mortality rate was 49.1%. Within 12 h preceding the event, 35.3% of the subjects received opiates. All received vital-sign assessments. Most were monitored with pulse oximetry. In contrast, 2.5% were on cardiac telemetry, and only 4 subjects used capnography; 53.7% showed significant vital-sign changes in the 24 h before the event. However, 46.3% had no significant change in any vital signs. CONCLUSIONS: Our study showed unanticipated respiratory compromise that required an unplanned intubation of subjects on the general care floor, although not common, carried a high mortality. Besides pulse oximetry and routine vital-sign assessments, very little monitoring was in use. A significant portion of the subjects had no vital-sign abnormalities leading up to the event. Further research is needed to determine the phenotype of the different etiologies of unexpected acute respiratory failure to identify better risk stratification and monitoring strategies.

Abstract: Protective policies have been unequally and inconsistently applied in the United States throughout the Covid-19 pandemic. This study investigates the relationship between state and local policies and Covid-19 deaths, combining three datasets: the Centers for Disease Control and Prevention's Social Vulnerability Index; local laws and regulations from the COVID Analysis and Mapping of Policies (AMP) database; and Covid-19 deaths by county reported by The New York Times. It examines, using propensity score matching, local policies and regulations as treatments during the crisis, and assesses how, inter alia, face mask requirements, gathering restrictions, stay-at-home orders, and social distancing mandates enacted at the county level altered Covid-19 deaths. The results indicate that the first three variables reduced average Covid-19 deaths in high-vulnerability communities. Despite clear gaps in federal policy guidance and coordinated policies, some efforts led by local and state governments promoted safer behaviour and lessened the impact of Covid-19 in communities, especially those with higher social vulnerability rates.

Abstract: Background: The ISCHEMIA trial (International Study of Comparative Health Effectiveness With Medical and Invasive Approaches) postulated that patients with stable coronary artery disease (CAD) and moderate or severe ischemia would benefit from revascularization. We investigated the relationship between severity of CAD and ischemia and trial outcomes, overall and by management strategy. Methods: In total, 5179 patients with moderate or severe ischemia were randomized to an initial invasive or conservative management strategy. Blinded, core laboratory–interpreted coronary computed tomographic angiography was used to assess anatomic eligibility for randomization. Extent and severity of CAD were classified with the modified Duke Prognostic Index (n=2475, 48%). Ischemia severity was interpreted by independent core laboratories (nuclear, echocardiography, magnetic resonance imaging, exercise tolerance testing, n=5105, 99%). We compared 4-year event rates across subgroups defined by severity of ischemia and CAD. The primary end point for this analysis was all-cause mortality. Secondary end points were myocardial infarction (MI), cardiovascular death or MI, and the trial primary end point (cardiovascular death, MI, or hospitalization for unstable angina, heart failure, or resuscitated cardiac arrest). Results: Relative to mild/no ischemia, neither moderate ischemia nor severe ischemia was associated with increased mortality (moderate ischemia hazard ratio [HR], 0.89 [95% CI, 0.61–1.30]; severe ischemia HR, 0.83 [95% CI, 0.57–1.21]; P =0.33). Nonfatal MI rates increased with worsening ischemia severity (HR for moderate ischemia, 1.20 [95% CI, 0.86–1.69] versus mild/no ischemia; HR for severe ischemia, 1.37 [95% CI, 0.98–1.91]; P =0.04 for trend, P =NS after adjustment for CAD). Increasing CAD severity was associated with death (HR, 2.72 [95% CI, 1.06–6.98]) and MI (HR, 3.78 [95% CI, 1.63–8.78]) for the most versus least severe CAD subgroup. Ischemia severity did not identify a subgroup with treatment benefit on mortality, MI, the trial primary end point, or cardiovascular death or MI. In the most severe CAD subgroup (n=659), the 4-year rate of cardiovascular death or MI was lower in the invasive strategy group (difference, 6.3% [95% CI, 0.2%–12.4%]), but 4-year all-cause mortality was similar. Conclusions: Ischemia severity was not associated with increased risk after adjustment for CAD severity. More severe CAD was associated with increased risk. 22. Social capital's impact on COVID-19 outcomes at local levels

Abstract: Over the past thirty years, disaster scholars have highlighted that communities with stronger social infrastructure—including social ties that enable trust, mutual aid, and collective action—tend to respond to and recover better from crises. However, comprehensive measurements of social capital across communities have been rare. This study adapts Kyne and Aldrich’s (Risk Hazards Crisis Public Policy11, 61–86, 2020) county-level social capital index to the census-tract level, generating social capital indices from 2011 to 2018 at the census-tract, zipcode, and county subdivision levels. To demonstrate their usefulness to disaster planners, public health experts, and local officials, we paired these with the CDC’s Social Vulnerability Index to predict the incidence of COVID-19 in case studies in Massachusetts, Wisconsin, Illinois, and New York City. We found that social capital predicted 41–49% of the variation in COVID-19 outbreaks, and up to 90% with controls in specific cases, highlighting its power as diagnostic and predictive tools for combating the spread of COVID.

Abstract: Disaster survivors and scholars alike argue that the presence, dearth, and type of social networks in a community can enable or limit disaster adaptation, response, evacuation, and recovery. But what kinds of networks can we examine in disaster studies? This chapter seeks to make social network analysis methods accessible to students and scholars of disaster policy. Using a network of disaster reconstruction committee members after Japan’s 2011 tsunami and earthquake, we demonstrate different techniques for network visualization, analysis, and statistics. First, we introduce readers to the logic and types of networks they may encounter, drawing from recent examples. Second, we lay out a process for analyzing networks, describing centrality measures, visualization, and statistical models. These accessible techniques allow us to identify what kinds of social ties affect recovery and how we can leverage these to improve disaster outcomes.

As directed therapy for Lp(a) emerges, it is important to understand patterns of Lp(a) testing in routine clinical practice. We set out to characterize Lp(a) testing across a large academic health system. Using electronic health record (EHR) data from 2014 to 2019, we compared patients who underwent Lp(a) testing to date-matched peers who had low density lipoprotein (LDL-C) assessment alone. We analyzed ordering provider characteristics and rates of initiation of new lipid lowering therapy (LLT) within 12 months after testing. Of 1,296 adults with Lp(a) test results, 629 (48.5%) had prior history of ASCVD and 667 (51.4%) did not. Compared with those with LDL-C testing alone, individuals who underwent Lp(a) testing were more like to have a myocardial infarction or ischemic stroke at a young age and multiple prior cardiovascular events. Though the majority of Lp(a) tests were ordered in outpatient encounters, a higher proportion of Lp (a) tests compared with LDL-C tests were performed in the inpatient setting. Neurology and psychiatry were the most common specialty to order Lp(a) tests in our cohort. There was a significantly increased initiation of LLT after Lp(a) testing compared with LDL-C testing across all medication types. Consistent with guidelines, Lp(a) testing is used in those with early onset ASCVD, and among those with multiple cardiovascular events. Lp (a) testing is associated with more aggressive LLT in following year. Further research is needed to characterize Lp(a) testing across larger populations.

Abstract: Hurricane Harvey was social media's first real stress test. Despite headlines that praise platforms like Facebook, Twitter, and Nextdoor for their role in disasters, we still know little about how community resilience differs across these platforms in a single event across a single measure of recovery. With an original dataset from a survey administered in Houston 39 days following Hurricane Harvey, this study explores the community resilience outcomes across users of Facebook, Twitter, and Nextdoor; more specifically, if users agreed that their neighbors engaged in neighborhood-level coordinated recovery efforts in the weeks following the storm. Controlling for collective efficacy, storm impact, amount of aid received, age, gender, income, education attainment, home tenure, and survey modality, findings from two multinomial logistic regression analyses reveal that individuals who used Nextdoor were more likely to receive help from neighbors to collect and pile debris following Hurricane Harvey compared to individuals who indicated they never used the platform, a finding not found among Facebook and Twitter users. This study identifies important differences in the resilience outcomes across these three platforms and discusses implications for future research and resilience policies.

Controlling for several factors, including politics and governance, health care quality, and demographic characteristics, we find that bonding and linking social capital reduce the toll of COVID-19 on communities. Public health officials and community organizations should prioritize building and maintaining strong social ties and trust in government to help combat the pandemic.

Abstract: The impact of disasters continues to grow in the early 21st century, as extreme weather events become more frequent and population density in vulnerable coastal and inland cities increases. Against this backdrop of risk, decision-makers persist in focusing primarily on structural measures to reduce losses centered on physical infrastructure such as berms, seawalls, retrofitted buildings, and levees. Yet a growing body of research emphasizes that strengthening social infrastructure, not just physical infrastructure, serves as a cost-effective way to improve the ability of communities to withstand and rebound from disasters. Three distinct kinds of social connections, including bonding, bridging, and linking social ties, support resilience through increasing the provision of emergency information, mutual aid, and collective action within communities to address natural hazards before, during, and after disaster events. Investing in social capital fosters community resilience that transcends natural hazards and positively affects collective governance and community health. Social capital has a long history in social science research and scholarship, particularly in how it has grown within various disciplines. Broadly, the term describes how social ties generate norms of reciprocity and trust, allow collective action, build solidarity, and foster information and resource flows among people. From education to crime, social capital has been shown to have positive impacts on individual and community outcomes, and research in natural hazards has similarly shown positive outcomes for individual and community resilience. Social capital also can foster negative outcomes, including exclusionary practices, corruption, and increased inequality. Understanding which types of social capital are most useful for increasing resilience is important to move the natural hazards field forward. Many questions about social capital and natural hazards remain, at best, partially answered. Do different types of social capital matter at different stages of disaster—e.g., mitigation, preparedness, response, and recovery? How do social capital’s effects vary across cultural contexts and stratified groups? What measures of social capital are available to practitioners and scholars? What actions are available to decision-makers seeking to invest in the social infrastructure of communities vulnerable to natural hazards? Which programs and interventions have shown merit through field tests? What outcomes can decision-makers anticipate with these investments? Where can scholars find data sets on resilience and social capital? The current state of knowledge about social capital in disaster resilience provides guidance about supporting communities toward more resilience.

Abstract: Anthropogenic climate change increasingly disrupts livelihoods, floods coastal urban cities and island nations, and exacerbates extreme weather events. There is near-universal consensus among scientists that in order to reverse or at least mitigate climate disruptions, limits must be imposed on anthropogenic sources of climate-forcing emissions and adaptation to changing global conditions will be necessary. Yet adaptation to current and future climate change at the individual, community, and national levels vary widely from merely coping, to engaging in adaptive change, to transformative shifts. Some of those affected simply cope with lower crop yields, flooded streets, and higher cooling bills. Others incrementally adapt to new environmental conditions, for example, by raising seawalls or shifting from one crop to another better suited for a hotter environment. The highest—and perhaps least likely—type of change involves transformation, radically altering practices with an eye toward the future. Transformative adaptation may involve a livelihood change or permanent migration; it might require shuttering whole industries and rethinking industrial policy at the national level. Entire island nations such as Fiji, for example, are considering relocating from vulnerable locations to areas better suited to rising sea levels. A great deal of research has shown how social capital (the bonding, bridging, and linking connections to others) provides information on trustworthiness, facilitates collective action, and connects us to external resources during disasters and crises. We know far less about the relationship between social capital and adaptation behaviors in terms of the choices that people make to accommodate changing environmental conditions. A number of unanswered but critical questions remain: How precisely does social capital function in climate change adaptation? To what degree does strong bonding social capital substitute for successful adaptation behaviors for individuals or groups? Which combinations of social factors make coping, adapting, and transforming most likely? How can social capital help migrating populations maintain cultural identity under stress? How can local networks be integrated into higher-level policy interventions to improve adaptation? Which political and social networks contribute to transformative responses to climate change at local, regional, and international levels? This article serves as a comprehensive literature review, overview of empirical findings to date, and a research agenda for the future.

36. Ethics and experiment:an ethnographic study of clinical gene therapy

Abstract: This thesis is an anthropological study of gene therapy. Gene therapy is an experimental medical technique, in which modified DNA is used as a therapeutic for diseases that have a whole or partial genetic cause. Clinical trials of gene therapies are underway in many countries around the world. However, social scientists have yet to devote much attention to this ethically contentious and medically complex field. This project aimed to identify and explore social and ethical factors shaping gene therapy practice in clinical settings. It is based on six months of participant observation in a London children’s hospital (the UKCH), thirty-two interviews with key actors in the gene therapy field, and scientific and policy document analysis. One of the main interests of this research is with the politics of ethics. The thesis shows that ‘ethical boundary work’ was central to establishing the credibility of gene therapy, and the authority of its practitioners. The politics of ethics can also be discerned in practice: the UK research ethics system structures scientific work but cannot account for the various, complex, and on-going ethical dilemmas that patients and practitioners face when undertaking gene therapy. The thesis explores some such dilemmas. It also identifies translation, the process of moving research from pre-clinical phases to clinical trials, as a key challenge for contemporary gene therapy, due to the material, technical, and social changes this shift entails. Translation requires building new relationships between patients, practitioners, regulators, and industry partners, and involves navigating conflicting or unclear expectations. As the first ethnographic exploration of gene therapy, this research contributes a new body of knowledge about the socio-ethical complexities of an under-examined scientific field.

In this article I present a brief history of HGT, showing how the ethical and practical viability of the field was achieved by key scientific and regulatory actors. These parties carefully articulated gene therapy’s scope, limiting it to therapeutic interventions on somatic cells, and cultivated alliances and divisions that bolstered the field’s legitimacy. At times these measures faltered, and then practitioners and sometimes patients would invoke an ethical imperative, posing gene therapy as the best solution to life and death problems.
