Abstract Introduction Abdominal angina is usually caused by atherosclerotic disease, and other causes are considered uncommon. This is the first report of a case of abdominal angina secondary to neoplastic vascular stenosis caused by local recurrence of an adenocarcinoma of the papilla of Vater. Case presentation An 80-year-old woman of Caucasian origin presented with abdominal pain and diarrhea. She had undergone a pancreaticoduodenectomy for adenocarcinoma of the papilla of Vater four years earlier. Computed tomography revealed a mass surrounding her celiac trunk and superior mesenteric artery. Her abdominal pain responded poorly to analgesic drugs, but disappeared when oral feedings were withheld. A duplex ultrasonography of the patient's splanchnic vessels was consistent with vascular stenosis. Parenteral nutrition was started and the patient remained pain free until her death. Conclusion Pain relief is an important therapeutic target in patients with cancer. In this case, abdominal pain was successfully managed only after the ischemic cause had been identified. The conventional analgesic therapy algorithm based on nonsteroidal anti-inflammatory drugs and opioids had been costly and pointless, whereas the simple withdrawal of oral feeding spared the patient of the discomfort of additional invasive procedures and allowed her to spend her remaining days in a completely pain-free state.

Abstract Introduction Lactating adenoma is a benign condition, representing the most prevalent breast lesion in pregnant women and during puerperium; in this paper, a case of a woman with lactating adenoma occurring during the first trimester of pregnancy is reported. There have been no reports in the literature, according to our search, focusing on magnetic resonance imaging findings in cases of lactating adenomas. Also the early onset of the lesion during the first trimester of pregnancy is quite unusual and possibly unique. Case presentation We report the case of a primiparous 30-year-old Caucasian woman, who noted an asymptomatic lump within her left breast during the 9th week of gestation, slightly increasing in size over the next few weeks. Ultrasound demonstrated a hypoecoic solid mass, hypervascularized and measuring 4 cm. On magnetic resonance imaging, performed in the first month after delivery, the lesion appeared as an ovoidal homogeneous mass, with regular margins and a significant contrast enhancement indicative of a giant adenoma. Conclusion Magnetic resonance imaging could play an important role in the differential diagnosis of pregnancy-related breast lumps, particularly during puerperium, thus avoiding unnecessary surgical biopsies.

Objective: This article reviews basic sonographic findings for distinguishing cystic lesions of the breast., Methods: We describe sonographic features of simple and complicated cysts in comparison with complex masses and intracystic carcinomas., Results: We correlate cystic lesion appearances with histologic patterns and illustrate the diagnostic and therapeutic management of cystic breast lesions., Conclusions: Sonography is a useful tool in distinguishing simple cysts from complicated cysts and complex masses of the breast.

Although the surgical treatment of breast cancer has become more conservative in the last decades, some patients still arrive at the first physical examination with advanced diseases and with large skin infiltration. We report an unusual case of giant invasive papillary carcinoma of the breast underwent mastectomy and reconstruction with a bipedicled transverse rectus abdominis myocutaneous (TRAM) flap.

The case of a 70-year-old female patient with family history of breast cancer come to the breast Unit for the presence of a nodular swelling in the right breast is discussed. On mammography and US a gross cystic neoformation with vascularized mural nodes was identified. Another contralateral solid nodular neoformation suggestive of malignancy was also present. Diagnostic completion with MRI confirmed the mixed solid, partly cystic nature of the right lesion, leading to the differential diagnosis with cystosarcoma phylloides and intracystic tumor. On the left side, MRI confirmed the presence of the second lesion whose morphologic and dynamic characteristics suggested a neoplastic lesion. At the anatomopathological examination the left lesion was shown to be an infiltrating ductal carcinoma; the right lesion was shown to be an intracystic tumor.

The case of a patient with a lesion in the right breast, is discussed. Retrospective evaluation of mammography and US previously performed elsewhere were not fully helpful in the differential diagnosis between benign and malignant and MRI was necessary. Morphological and dynamic characteristics could be indicative of an expansive/infiltrating lesion of the breast and support the staging. The histological examination established the diagnosis of primary breast lymphoma.

The typical presentation of breast cancer in a young woman is discussed. Diagnostic imaging of the breast showed the presence of an extensive pathological area, approximately 5 cm in size, at the level of the upper quadrants of left breast. On mammography the pathological sign was represented by numerous regional granular microcalcifications. On sonography an inhomogeneously hypoechoic area with intralesional hyperechoic spots was identified. On MRI a large area of pathological enhancement was visualized. For the tumor extent and the patient's young age neoadjuvant chemotherapy was performed. Post-treatment assessment showed good response to therapy, suggestive of possible better prognosis.

Aims: To reassess the relationship between magnetic resonance imaging (MRI) findings and surgical resection margins in an attempt to address the issue of appropriate surgical management of phyllodes tumors (PT)., Methods: Three female patients with a large palpable mass suspicious for phyllodes tumors were studied by mammography (MX), ultrasound (US) and dynamic MRI and then underwent surgery., Results: MRI demonstrated a rapidly and markedly enhancing multi-lobulated lesion. T1-weighted and T2-weighted sequences showed inhomogeneous signal intensity for the presence of cystic areas with internal septation and hemorrhage. Some areas of linear enhancement were present around the mass only in one case. Surgical management was mastectomy in one patient and wide excision in the other two patients. The margins in one of the latter patients were not clear, so mastectomy with immediate prosthetic reconstruction was subsequently performed. Pathological results showed 1 case of benign phylloides tumor, 1 case of borderline phylloides tumor and 1 case of malignant phylloides tumor., Conclusions: MRI enabled complete visualization of the tumor even in the region close to the chest wall, as well as clear delineation from healthy glandular tissue and may help to define the appropriate surgical management of phylloides tumor.

The case of a male patient with gynecomastia of the left mammary gland is discussed. The differential diagnosis was considered in relation to other possible benign and malignant lesions where there is swelling of the mammary gland in men. US was the examination of first choice. To complete the study of the diagnostic pattern, mammography was also performed. The definitive diagnosis was true nodular gynecomastia caused by chronic liver disease.

The case of a 57-year old woman, apparently in good health, in menopause with familial incidence of breast cancer is discussed. The patient came to the Senology Unit for annual control mammography. For diagnostic completion US was performed: it showed the presence of a hypoechoic neoformation highly suggestive of malignancy (class V BI-RADS). Therefore, US-guided needle aspiration biopsy was performed. It confirmed the malignant nature of the neoformation and surgery was planned. At definitive histological examination a small invasive ductal carcinoma, 6 mm in diameter, was diagnosed.

The case of a female patient with a normal mammography of first level screening and a control mammography showing the presence of a cluster of microcalcifications, is discussed. The case history is carefully examined also based on previous investigations. The patient decided to undergo stereotactic vacuum-assisted biopsy (mammotome biopsy) that was performed elsewhere. Indications, advantages and possible complications of the procedure are analyzed.

The case of a 41-year-old female patient with axillary lymphadenopathies and negative clinical and mammographic examinations is discussed. In the suspicion of a cup syndrome of breast origin, breast sonography was performed. While the absence of alterations was confirmed, a right parasternal swelling was highly suggestive of malignancy. On histology, a malignant parasternal nodule was diagnosed. Breast MRI was performed and an ipsilateral malignant focal lesion was visualized. Chemotherapy was administered but after a disease-free interval leptomeningeal and cerebral metastases appeared.

Although being predominantly anecdotal, breast carcinoma shows a considerable component of multifactorial genetic transmission, referable to dominant autosomal inheritance. Among the genes responsible for hereditary breast cancer two main genes (BRCA1 and BRCA2) have already been identified. At present, genetic tests to identify known inherited mutations associated with breast tumor predisposition, are available. Asymptomatic subjects with genetic BRCA1/2 mutations represent a group at high risk; therefore it is now indispensable to work out adequate strategies of prevention and/or of early diagnosis. Besides education (information) and genetic counselling, there are three perspectives of intervention for these subjects: bilateral prophylactic surgery, pharmacologic prophylaxis and periodical diagnostic monitoring; unfortunately all of them are still burdened by limitations.

The role of MR-mammography in radial scar was assessed in 4 female patients. All 4 patients showed mammographic findings of radial scar and underwent dynamic MR-mammography. MR-mammographic findings were compared with histopathological results. On MR-mammography a focal enhancing lesion with irregular margins, plateau or rapid wash-out time/signal intensity curves, with maximum signal intensity > 100% were documented in the 4 patients. The histological diagnosis identified i case of infiltrating lobular carcinoma, 1 case of atypical hyperplasia and 2 cases of typical hyperplasia. In the preliminary study, MR-mammography was superior to mammography in the assessment of the extent and margins of the lesions. However, in the 4 study patients it could not accurately differentiate between carcinoma and benign lesions. It should be observed that lesions suggestive of radial scar have to be removed by surgery.

A case of diabetic mastopathy studied with MR-mammography is described. It concerns a female patient with long-lasting (10-years) insulin-dependent diabetes mellitus. A non-tender, palpable firm-to hard mass of the left breast was found and studied with mammography, ultrasonography and MR-mammography. Surgical excision of the mass was performed. The mammogram revealed dense fibroglandular tissue bilaterally. Ultrasonography showed hypoechoic solid tissue in the region of palpable breast mass with marked posterior acoustic shadowing. MR-mammography showed a hypointense area of fibrosis with poor enhancement. Quantitative analysis revealed a typically benign gradual type dynamic curve. MR-mammography can be useful to differentiate diabetic mastopathy from malignant lesions. In women with diabetic mastopathy MR-mammography, clinical examination and careful follow-up may prevent surgical excision.
