3. Disseminated Leishmaniasis, a Severe Form of Leishmania braziliensis Infection.

Abstract

Disseminated leishmaniasis (DL) is an emergent severe disease manifesting with multiple lesions. To determine the relationship between immune response and clinical and therapeutic outcomes, we studied 101 DL and 101 cutaneous leishmaniasis (CL) cases and determined cytokines and chemokines in supernatants of mononuclear cells stimulated with leishmania antigen. Patients were treated with meglumine antimoniate (20 mg/kg) for 20 days (CL) or 30 days (DL); 19 DL patients were instead treated with amphotericin B, miltefosine, or miltefosine and meglumine antimoniate. High levels of chemokine ligand 9 were associated with more severe DL. The cure rate for meglumine antimoniate was low for both DL (44%) and CL (60%), but healing time was longer in DL (p = 0.003). The lowest cure rate (22%) was found in DL patients with >100 lesions. However, meglumine antimoniate/miltefosine treatment cured all DL patients who received it; therefore, that combination should be considered as first choice therapy. [ABSTRACT FROM AUTHOR]

Abstract

The article aimed to investigate the paradigms used in academic production expressed in theses on the Formation of Biology Teachers, defended in graduate programs of Education and / or Science Teaching existing in Brazil in the period 2007-2016. 19 documents were examined. It is believed that this article may motivate reflections on the training of Biology teachers, within the scope of the national scenario, in addition to allowing the analysis and understanding of how the methodologies are being applied for the collection and study of data of the developed research. The work was guided by the identification of the methodological approaches that were applied in the works. The results presented some terms used in scientific research to designate a paradigm, in addition to identifying the paradigms most used by researchers. O artigo teve como finalidade investigar os paradigmas utilizados na produção acadêmica expressa em teses sobre a Formação de Professores de Biologia, defendidas em programas de pós-graduação de Educação e/ou Ensino de Ciências existentes no Brasil no período 2007-2016. Foram examinados 19 documentos. Acredita-se que este artigo poderá motivar reflexões sobre a formação de professores de Biologia, no âmbito do cenário nacional, além de permitir a análise e compreensão de como estão sendo aplicadas as metodologias para a coleta e estudo de dados das pesquisas desenvolvidas. O trabalho foi norteado pela identificação das abordagens metodológicas que foram aplicadas nos trabalhos. Nos resultados foram apresentados alguns termos utilizados na pesquisa científica para designar paradigma, além da identificação dos paradigmas mais utilizados pelos pesquisadores.

Abstract

Background: Leishmania skin test (LST) evaluates the delayed type hypersensitivity to Leishmania antigens (LA) and has been used for diagnosis of cutaneous leishmaniasis (CL). In CL patients LST is usually positive but a small percentage have negative LST. The aim of this study was to determine the clinical and immunologic features and response to antimony therapy in LST-negative CL patients.Methods: We compare the clinical presentation, response to therapy, and immune response of CL patients with negative vs positive LST.Results: The clinical presentation was similar in both groups but LST-negative patients had a lower cure rate. In the lesions, LST-negative patients displayed less inflammation and necrosis, and higher frequency of CD8+ T cells. Mononuclear cells from LST-negative patients had a poor T helper 1 cell (Th1) response but levels of interleukin-1β (IL-1β), IL-6, IL-17, granzyme B, and metalloproteinase-9 (MMP-9) were similar to the LST-positive group upon stimulation with LA. Leishmania internalization and killing by macrophages were similar in both groups. Cure of disease was associated with restoration of Th1 response.Conclusions: In LST-negative patients, impaired Th1 response is associated with therapeutic failure. Increased frequency of CD8+ T cells and high production of inflammatory cytokines, granzyme B, and MMP-9 contributes to immunopathology. [ABSTRACT FROM AUTHOR]

Abstract

Background: Skin lesions from patients infected with Leishmania braziliensis has been associated with inflammation induced by cytotoxic CD8+ T cells. In addition, CD8+ T cell-mediated cytotoxicity has not been linked to parasite killing. Meanwhile, the cytotoxic role played by natural killer (NK) cells in cutaneous leishmaniasis (CL) remains poorly understood.Methods: In this study, we observed higher frequencies of NK cells in the peripheral blood of CL patients compared with healthy subjects, and that NK cells expressed more interferon-γ, tumor necrosis factor (TNF), granzyme B, and perforin than CD8+ T cells.Results: We also found that most of the cytotoxic activity in CL lesions was triggered by NK cells, and that the high levels of granzyme B produced in CL lesions was associated with larger lesion size. Furthermore, an in vitro blockade of granzyme B was observed to decrease TNF production.Concclusions: Our data, taken together, suggest an important role by NK cells in inducing inflammation in CL, thereby contributing to disease immunopathology. A leishmaniose cutânea (LC) é uma doença parasitária inflamatória caracterizada pela presença de lesões ulceradas na pele. Pacientes com LC infectados por L. braziliensis produzem altos níveis de TNF, citocina que contribui para dano tecidual e desenvolvimento da úlcera. Infiltrado de células mononucleares é encontrado na lesão de pacientes com LC, com presença de linfócitos T e B e fagócitos mononucleares. A maioria dos trabalhos tem dado ênfase à resposta imune das células T e pouca atenção tem sido dada para a contribuição de monócitos na imunopatologia observada na LC. Recentemente, três subpopulações de monócitos, foram descritas baseadas na expressão das moléculas CD14 e CD16: Monócitos clássicos (CD14++CD16-), intermediários (CD14+CD16+) e não-clássicos (CD14++CD16+). Objetivo deste estudo foi caracterizar fenotipicamente e funcionalmente as subpopulações de monócitos de pacientes com LC. Material e Método: Células mononucleares do sangue periférico foram obtidas de indivíduos sadios, pacientes com leishmaniose cutânea com lesão recente (LCR) e paciente com leishmaniose cutânea com úlcera estabelecida. A caracterização das subpopulações de monócitos foi determinada por citometria de fluxo. Resultados: Nós observamos que a frequência das populações de monócitos intermediários e não-clássicos estavam aumentadas em indivíduos com LCR e em pacientes com LC. Expressão de MHC classe II foi aumentada em monócitos intermediários, sugerindo que essas células podem apresentar antígeno para células T. Também observamos que monócitos clássicos e intermediários produzem mais TNF que os não-clássicos em resposta a antígeno solúvel de Leishmania e LPS. CCL2 é uma quimiocina conhecida por desempenhar importante papel na resposta imune contra Leishmania, sendo observada que frequência de monócitos expressando CCL2 foi significantemente maior no sangue periférico de pacientes com LC quando comparados aos indivíduos sadios, indicando que células expressando CCL2 podem migrar para o sítio da lesão. Análises de biópsias de pacientes com LC mostraram que a população de monócitos predominante encontrada na lesão desses indivíduos são os monócitos não-clássicos .Conclusão: A identificação de células que contribuem para a imunopatologia observada na LC pode ajudar para desenvolver novas formas de imunoterapia para essa doença.
