7. Markov Chain Approximations to Stochastic Differential Equations by Recombination on Lattice Trees

Abstract

We revisit the classical problem of approximating a stochastic differential equation by a discrete-time and discrete-space Markov chain. Our construction iterates Caratheodory's theorem over time to match the moments of the increments locally. This allows to construct a Markov chain with a sparse transition matrix where the number of attainable states grows at most polynomially as time increases. Moreover, the MC evolves on a tree whose nodes lie on a "universal lattice" in the sense that an arbitrary number of different SDEs can be approximated on the same tree. The construction is not tailored to specific models, we discuss both the case of uni-variate and multi-variate case SDEs, and provide an implementation and numerical experiments.

Abstract

This work concerns continuous-time, continuous-space stochastic dynamical systems described by stochastic differential equations (SDE). It presents a new approach to compute probabilistic safety regions, namely sets of initial conditions of the SDE associated to trajectories that are safe with a probability larger than a given threshold. The approach introduces a functional that is minimised at the border of the probabilistic safety region, then solves an optimisation problem using techniques from Malliavin Calculus, which computes such region. Unlike existing results in the literature, the new approach allows one to compute probabilistic safety regions without gridding the state space of the SDE.

15. Carath\'eodory Sampling for Stochastic Gradient Descent

Abstract

Many problems require to optimize empirical risk functions over large data sets. Gradient descent methods that calculate the full gradient in every descent step do not scale to such datasets. Various flavours of Stochastic Gradient Descent (SGD) replace the expensive summation that computes the full gradient by approximating it with a small sum over a randomly selected subsample of the data set that in turn suffers from a high variance. We present a different approach that is inspired by classical results of Tchakaloff and Carath\'eodory about measure reduction. These results allow to replace an empirical measure with another, carefully constructed probability measure that has a much smaller support, but can preserve certain statistics such as the expected gradient. To turn this into scalable algorithms we firstly, adaptively select the descent steps where the measure reduction is carried out; secondly, we combine this with Block Coordinate Descent so that measure reduction can be done very cheaply. This makes the resulting methods scalable to high-dimensional spaces. Finally, we provide an experimental validation and comparison.

Abstract

Given a discrete probability measure supported on $N$ atoms and a set of $n$ real-valued functions, there exists a probability measure that is supported on a subset of $n+1$ of the original $N$ atoms and has the same mean when integrated against each of the $n$ functions. If $ N \gg n$ this results in a huge reduction of complexity. We give a simple geometric characterization of barycenters via negative cones and derive a randomized algorithm that computes this new measure by "greedy geometric sampling". We then study its properties, and benchmark it on synthetic and real-world data to show that it can be very beneficial in the $N\gg n$ regime. Abstract

The thesis goal is to explore the relations between Formal Verification techniques in Computer Science and Stochastic Analysis in Mathematics. They both deal with probabilistic dynamical systems, implying that the number of connections is significant. The Formal Verification studies dynamical systems' properties, which are specified in terms of logic statements. This discipline has been successfully applied to multiple industry problems, such as ``verification of'' Software, Microprocessors or Neural Networks. The models studied have a countable states space and this can be considered the main obstacle to extend Formal Verification techniques for the computation of likelihoods in the case of Stochastic Differential Equations. However, the most studied properties in Formal Verification have a clear characterization in Mathematics. In this thesis, we focus on the safety property ``always'', which can be characterized as an exit time in the theory of Stochastic Analysis. The main difficulties are twofold. Firstly, if we are particularly interested in discretizing the states space - going from a continuous, uncountable space to a finite countable one - it is very likely to have an explosion of the cardinality of the states space. This leads to problems both in time and memory complexity. We find a new characterization of the barycenter of a discrete probability measure, which allows us to introduce a new algorithm for the recombination problem. Then, we use the algorithm to build recombining trees in order to approximate weakly stochastic processes, solving the exponential explosion of the cardinality of the states space. Secondly, one has to consider the approximation error: if the probability to exit from a safety region has not an analytical closed-form, a numerical scheme must be considered and therefore it is important to analyse the error. We introduce a grid-free approach for the computation of Probabilistic Safety Regions using Malliavin Calculus.

Abstract

Aim: To assess weight loss and cardiorenal outcomes by baseline body mass index (BMI) in VERTIS CV. Methods: Patients with type 2 diabetes and atherosclerotic cardiovascular (CV) disease were randomized to ertugliflozin or placebo. These post hoc analyses evaluated cardiometabolic and cardiorenal outcomes (a composite of death from CV causes or hospitalization for heart failure [HHF], CV death, HHF and an exploratory composite kidney outcome including ≥40% estimated glomerular filtration rate [eGFR] decrease) by baseline BMI, using conventional clinical categories and Cox proportional hazards models. Results: In total, 8246 adults were randomized (mean age 64.4 years, diabetes duration 13.0 years, BMI 32.0 kg/m2, 61% with BMI >30 kg/m2). Absolute body weight reduction was greater with ertugliflozin versus placebo at 3 and 5 years in the overall population (p < 0.001) and across BMI subgroups. Ertugliflozin increased the proportion of participants achieving ≥5% and ≥10% body weight reduction (ertugliflozin 34.9% and 13.6%, placebo 19.4% and 4.1%; odds ratio [95% confident interval, CI], 2.21 [1.76–2.77] and 3.65 [2.39–5.57], respectively) at 5 years. No significant difference was observed in the effect of ertugliflozin on HHF across BMI subgroups (Pinteraction = 0.61). Similarly, no significant difference was observed in the effect of ertugliflozin on the kidney composite outcome across BMI subgroups (Pinteraction = 0.39). Results were similar for other CV outcomes, and safety was consistent with the known ertugliflozin profile. Conclusion: Weight loss was observed across baseline BMI and was sustained over 5 years of follow‐up. The effects of ertugliflozin on HHF and kidney composite were consistent across baseline BMI. Abstract

Background and Aims Hyperglycaemia during gestational diabetes (GD) predisposes women and their offspring to later cardiometabolic disease. The hyperglycaemia-mediated epigenetic changes remain to be elucidated. Methyltransferase MLL1-induced trimethylation of histone 3 at lysine 4 (H3K4me3) activates inflammatory and oxidative phenotype. This epigenetic mark in GD women and its transmission to the offspring were investigated. Methods Peripheral blood mononuclear cells (PBMC) were collected from GD and control (C) women and also from adolescents born to women of both groups. Endothelial human umbilical vein endothelial cells (HUVEC) and cord blood mononuclear cells (CBMC) were from umbilical cords. The NF-κBp65 and NOX4 expressions were investigated by reverse transcription quantitative polymerase chain reaction and immunofluorescence (IF). MLL1 and H3K4me3 were investigated by immunoblotting and IF. H3K4me3 on NF-κBp65 and NOX4 promoters was studied by chromatin immunoprecipitation. Superoxide anion generation was measured by electron spin resonance spectroscopy. Plasma cytokines were measured by enzyme-linked immunosorbent assay. To investigate the role of MLL1, HUVEC were exposed to inhibitor MM102 or siRNA transfection. Results PBMC, CBMC, and HUVEC showed an increase of NF-κBp65 , IL-6 , ICAM-1 , MCP-1 , and VCAM-1 mRNAs. These findings were associated with H3K4me3 enrichment in the promoter of NF-κBp65. Elevated H3K4me3 and cytokine levels were observed in GD adolescents. MLL1 drives H3K4me3 not only on NF-kB p65 , but also on NOX4 promoter. Inhibition of MLL1 blunted NF-κBp65 and NOX4 by modulating inflammatory and oxidative phenotype. Conclusions Such proof-of-concept study shows persistence of MLL1-dependent H3K4me3 in offspring born to GD women, suggesting an epigenetic-driven transmission of maternal phenotype. Abstract

With the increasing burden of diabetes as a cause of macro- and microvascular disease linked to the epidemics of obesity, attention is being paid to dysglycaemic states that predict and precede the development of type 2 diabetes. Such conditions, termed pre-diabetes, are characterized by fasting plasma glucose, or plasma glucose levels on an oral glucose tolerance test, or values of glycated haemoglobin intermediate between 'normal' values and those characterizing diabetes. These last are by definition associated, in epidemiological terms, with a higher incidence of microvascular disease—mostly retinopathy. Pre-diabetes overlaps with the components of the 'metabolic syndrome'—among which are excess visceral adiposity; hypertension; hypertriglyceridaemia; high levels of small, dense low-density lipoproteins; and metabolic-associated fatty liver disease. There is little doubt that pre-diabetes has important prognostic implications, especially for the occurrence of myocardial infarction, ischaemic stroke, and peripheral arterial disease. It is disputed, however, whether pre-diabetes is itself an actionable disease entity, in addition to the risk factors characterizing it. Because of this uncertainty, the latest European Society of Cardiology guidelines chose not to include pre-diabetes as a treatment target for atherosclerotic cardiovascular disease, at variance from the three previous editions of such guidelines. Abstract

We have explored applications of Augmented Reality (AR) for external beam radiotherapy to assist with treatment planning, patient education, and treatment delivery. We created an AR development framework for applications in radiotherapy (RADiotherapy Augmented Reality, RAD-AR) for AR ready consumer electronics such as tablet computers and head mounted devices (HMD). We implemented in RAD-AR three tools to assist radiotherapy practitioners with: treatment plans evaluation, patient pre-treatment information/education, and treatment delivery. We estimated accuracy and precision of the patient setup tool and the underlying self-tracking technology, and fidelity of AR content geometric representation, on the Apple iPad tablet computer and the Microsoft HoloLens HMD. Results showed that the technology could already be applied for detection of large treatment setup errors, and could become applicable to other aspects of treatment delivery subject to technological improvements that can be expected in the near future. We performed user feedback studies of the patient education and the plan evaluation tools. Results indicated an overall positive user evaluation of AR technology compared to conventional tools for the radiotherapy elements implemented. We conclude that AR will become a useful tool in radiotherapy bringing real benefits for both clinicians and patients, contributing to successful treatment outcomes.

41. Effects of ertugliflozin on uric acid and gout‐related outcomes in persons with type 2 diabetes and cardiovascular disease: Post hoc analyses from VERTIS CV.

Abstract

Aim: To conduct post hoc analyses of the VERTIS CV (NCT01986881) trial to explore the effects of ertugliflozin on serum uric acid (UA) and gout‐related outcomes. Materials and Methods: Participants with type 2 diabetes and atherosclerotic cardiovascular disease were randomised (1:1:1) to placebo, ertugliflozin 5 mg or ertugliflozin 15 mg. Mean UA over time (260 weeks) was evaluated for pooled ertugliflozin versus placebo overall, and by baseline quintile of UA (≤4.3 mg/dL [≤255.8 µmol/L], >4.3–5.1 mg/dL [>255.8–303.4 µmol/L], >5.1–5.8 mg/dL [>303.4–345.0 µmol/L], >5.8–6.9 mg/dL [>345.0–410.4 µmol/L] and >6.9 mg/dL [>410.4 µmol/L]), glycated haemoglobin level, albuminuria status, estimated glomerular filtration rate and KDIGO (Kidney Disease: Improving Global Outcomes in Chronic Kidney Disease) risk category. The effect of ertugliflozin on a composite of gout onset or initiation of anti‐gout medication was assessed. Results: The mean UA levels at baseline were 5.67 and 5.62 mg/dL in the placebo and ertugliflozin groups, respectively. Ertugliflozin reduced UA over Weeks 6–260 compared with placebo, with least squares mean (LSM) changes (95% confidence interval [CI]) from baseline at Week 260 of 0.07 mg/dL (−0.02, 0.15) and −0.19 mg/dL (−0.25, −0.13) in the placebo and pooled ertugliflozin groups, respectively. At Week 260, placebo‐adjusted LSM change (95% CI) from baseline in UA was −0.26 mg/dL (−0.36, −0.16) with ertugliflozin. Ertugliflozin was associated with reductions in UA across baseline UA quintiles compared with placebo. The incidence of the composite of gout‐related outcomes was 84/2539 (3.3%) for placebo and 133/5091 (2.6%) for ertugliflozin (hazard ratio for the composite 0.76 [95% CI 0.580, 1.002]). Conclusions: Ertugliflozin was generally associated with lowering UA overall and across subgroups compared with placebo, and numerically reduced rates of gout‐related outcome events. Abstract

Background: Menopause, a natural phase in a woman's life, often adversely affects physical, mental, sexual, and emotional well-being due to low estrogen levels. This study examines the impact of vaginal ovules with tocopherol acetate (Filme Gyno-V® ovules, manufactured by Panin Srl and distributed by Hulka Srl, Italy), 500 mg per ovule, on vaginal health in pre- and menopausal women. Methods: Fifty women aged 50–70 were divided into menopausal (28) and premenopausal (22) cohorts and treated with the ovules for two weeks, with assessments before and after treatment. Results: The findings showed that distressing symptoms of vaginal atrophy, such as dryness, itching, and pain during intercourse, were resolved post-treatment. A molecular analysis revealed a reduction in Escherichia coli in both cohorts and an increase in three species of Lactobacillus in premenopausal patients. Conclusions: This study concludes that Filme Gyno-V ovules may benefit vaginal health by alleviating atrophy symptoms and promoting healthy vaginal microbiota. [ABSTRACT FROM AUTHOR]
