8. Viability of the overset method for geometrical sensitivity studies

Abstract

In the following thesis the overset method, also called chimera or overlapping meshes, is discussed and applied toa formula race car, in order to calculate its aerodynamic map. The proposed method would allow reducing set-uptime through automation and avoided re-meshing process. a A theoretical background is presented before thediscussion of the way this kind of approach has been set-up in Star-CCM+. Results are obtained and discussedfor various car positions. Further investigations are finally suggested to further assess the viability of the method.

Abstract

We describe a case of spontaneous esophageal perforation (Boerhaave's syndrome) that was admitted at our department with acute clinical symptoms: dyspnea, thoracic pain and vomit after episode of alcohol abuse. Pneumothorax was suspected: early chest X-rays revealed left sided pleural effusion with complete collapse of the omolateral lung and pneumomediastinum. Successive esophagoscopy showed a 1-cm longitudinal perforation on the left side of the lower esophagus. Perforation was repaired by direct suture and reinforced with endoprosthesis. Patient was discharged on the 45th postoperative day without complications occurred after 1-year period. 12. Dual left anterior descending coronary artery from right aortic sinus: report of a case of recurrent unstable angina after CABG

Abstract

Anomalies of the left coronary artery are very rare, with an incidence range between .3% and 1.64%. The diagnosis is generally incidental during coronary angiogram, coronary artery bypass operation, or autopsy. However, sometimes this anomaly is not recognized during CABG operation and can be responsible for the recurrence of angina after CABG operation and even compromise the outcome. We presented a case in which the dual left anterior coronary artery from the right aortic sinus occasionally was shown in a coronary angiogram after CABG operation; the angiogram was performed because of the recurrence of angina.

19. ECMO support for the treatment of cardiogenic shock due to left ventricular free wall rupture.

Abstract

Left ventricular free wall rupture (LVFWR) is still an uncommon catastrophic complication after acute myocardial infarction (MI), and it is one of the most frequent causes of sudden cardiac death. Immediate surgical repair is the treatment of choice. When LVFWR presents acutely with tamponade and cardiogenic shock in emergency department, salvage with a good outcome is still possible by timely pericardiocentesis and extracorporeal membrane oxygenation (ECMO) support. We report a case of cardiac rupture with tamponade and cardiogenic shock in which cardiopulmonary support with portable ECMO was used to rescue the patient before the operation.
