- Abstract
Background: Community-acquired pneumonia (CAP) has a potential complication of bacteremia. The objective of this study was to define the clinical outcomes of patients with CAP and bacteremia treated with and without a macrolide. Materials and methods: Secondary analysis of the Community-Acquired Pneumonia Organization database of hospitalized patients with CAP. Patients with a positive blood culture were categorized based on the presence or absence of a macrolide in their initial antimicrobial regimen, and severity of their CAP. Outcomes included in-hospital all-cause mortality, 30-day mortality, length of stay, and time to clinical stability. Results: Among 549 patients with CAP and bacteremia, 247 (45%) were treated with a macrolide and 302 (55%) were not. The primary pathogen was Streptococcus pneumoniae (74%). Poisson regression with robust error variance models were used to compare the adjusted effects of each study group on the outcomes. The unadjusted 30-day mortality was 18.4% in the macrolide group, and 29.6% in the non-macrolide group (adjusted relative risk (aRR)0.81; 95% confidence interval (CI)0.50–1.33; P = 0.41). Unadjusted in-hospital all-cause mortality was 7.3% in the macrolide group, and 18.9% in the non-macrolide group (aRR 0.54, 95% CI 0.30–0.98; P = 0.043). Length of stay and time to clinical stability were not significantly different. Conclusions: In-hospital mortality, but not 30-day mortality, was significantly better in the macrolide group. Our data support the use of a macrolide in hospitalized patients with CAP and bacteraemia

13. La visión del alumnado de educación física: 5º y 6º de educación primaria del colegio Compañía de María (Zaragoza)

- Abstract
En el presente trabajo se analiza la visión del alumnado de Educación Física en los cursos 5º y 6º de Educación Primaria del colegio Compañía de María (Zaragoza). Para ello, se ha elaborado un cuestionario dirigido a este alumnado. Ahí se tratan diferentes dimensiones como Recibir, Responder, Valorar, Organización, Caracterización y Dificultad. Los resultados obtenidos reflejan que el alumnado valora positivamente la asignatura, mostrando interés durante el desarrollo de las clases. Este interés es reflejado en su actitud, su predisposición a atender, su participación, animando a sus compañeros y trabajando duro. Además, no se manifiestan grandes diferencias entre cursos ni entre géneros.

El Instituto Plan Agropecuario (IPA) desarrolló un modelo para representar a una explotación ganadera criadora extensiva sobre un suelo representativo de basalto: el MEGanE (Modelo de una Explotación Ganadera Extensiva). Los resultados de la simulación, tanto fitotécnicos como zootécnicos muestran gran coherencia con resultados de investigación nacional. Asimismo, el MEGanE pudo reproducir una situación de crisis forrajera conocida, con resultados coherentes a una realidad posible. El MEGanE fue diseñado para transmitir conceptos, con términos de fácil comprensión por los usuarios. Simulation models can be considered as learning tools, according to the idea of «flight simulator», where users would benefit by experimenting with possible realities, even more if they are built with the potential stakeholders in a participatory way. The IPA (Agricultural Planning Institute) developed a model to represent an extensive breeder cattle farm on a representative basaltic soil: the MEGanE (Model of an Extensive Stock Breeding Farm). The results of the simulation, both for forage production and zootechnics, show great consistency with national research information. Also, the MEGanE was able to reproduce a known drought crisis, with results consistent with a possible reality. This work presents productive and reproductive results of the simulations made with an extensive livestock farm model on basaltic soils. The scenari were constructed varying the initial grass height (GH) and the system stocking rate (SR). The model is chaotic, dynamic, empirical and deterministic. It was built in a participatory way with a group of local stakeholders of the basaltic region of the country (Uruguay). A simplified version of it was utilized, where only one parcel and only one animal category were simulated each time. No adaptive management was implemented into the simulation process. These models represent a Breeding (Br) and a Rearing (Re) extensive systems, where the exclusive feed source is the natural pasture. All scenari were simulated with average pasture growth, estimated by satellite remote sensing for the series 2000-2010, for a representative farm basaltic region. Annual results of pregnancy rate and live weight gain per animal were the two main outputs analyzed for the Br and Re simulated systems, respectively. In the scenari building the two input variables changed stepwise were the initial GH (going from 2 to 12 cm) as indicator of the forage availability, and the initial number of animals (going from 0.1 to 1.9 animal/ha) as indicator of the SR. The initial situation for the Br simulation was cows of 360 kg of average live weight. The start of the simulation was at the beginning of autumn, and the calving and mating seasons were spring and summer, respectively. For the Re simulation, the initial situation was defined by heifers with an average live weight of 150 kg. Simulation started also at the beginning of autumn. For the Br simulations results of pregnancy rates higher than 80% are only achieved in the scenari of low and mean SR ( 10 cm). If the initial GH on autumn is 5 cm, to achieve that pregnancy rate level, the SR must be less than 0.9 cows/ha. For the Re simulation, results of gain per animal bigger than 100 kg/heifer are attained in a continuum of situations going from 3 cm and 0.1 animals/ha to 12 cm and 1.5 animals/ha. The results of simulation are consistent with the real situations on basaltic soils, showing that low forage availability (< 5 cm in silico) in the early autumn combined with a high SR (> 0.9 animals/ha in silico) compromise seriously the productive and reproductive performances of the system.

El uso de simulaciones permite el manejo de grandes cantidades de información de distinto origen y ello puede mejorar las decisiones y hacerlas más pertinentes. Desde hace algunos años se han concretado las posibilidades de simular sistemas complejos, en forma dinámica, espacialmente explícita e integrando información cualitativa por medio de sistemas multiagentes y otros tipos de simulaciones que integran al menos alguna de estas características, y permiten explorar la evolución de estos sistemas en distintos escenarios. Al mismo tiempo se ha enfatizado la necesidad de aumentar la capacidad de adaptación de los productores y de las cadenas productivas al aumento de variabilidad climática que se prevé tenga lugar en los próximos decenios. Esto tiene especial relevancia en socio-eco-sistemas donde innumerables variables que tienen su propia dinámica interaccionan de una forma intrincada. En este trabajo se describen dos modelos que simulan la ganadería en el Norte del Uruguay a nivel de explotaciones. Ambos han considerado la participación de involucrados externos al proyecto, sea en cuanto a examinar la estructura y la dinámica de la simulación, sea en contrastar los resultados contra su experiencia. El primero de ellos, Arapey, compara distintas estrategias de ganaderos entre los años 1970 y 2004 y SequiaBasalto simula las interacciones entre el crecimiento del pasto y el desempeño animal cuando se aplican distintas estrategias, teniendo en cuenta la dinámica estacional y la variabilidad climática. Se muestran avances hacia simulaciones disponibles en la web con distintos formatos según los usuarios a los que se apunta y se discute su uso como herramientas de aprendizaje y de fomento de la capacidad de adaptación.

This paper focuses on the collective design and immediate execution of an agent-based model (ABM) by dynamically interpreting activity diagrams of agent behaviors. To reach this objective, we have implemented an ABM of livestock producers facing drought conditions in Uruguay. The first step consists in implementing a standard ABM with pasture growth, herd dynamics and simple agents roughly imitating farmers' strategies. The second step is more participative since it consists in assessing the model with the real cattle farmers. As it appears in a majority of modeling processes, this evaluation phase requires feedback on model design. In order to make this assessment more lively and efficient, we have conceived a tool for drawing diagrams and interpreting them immediately. Thanks to this new opportunity, the actors have quickly understood how the model worked and were able to criticize and modify it. Thus, this innovative modeling tool enables the involvement of stakeholders in co-designing ABM for participatory foresight studies. We hope it will facilitate the emergence of new and more efficient practices for farm management that account for climate changes.

Dynamics in agrarian systems of Uruguay and Argentina present some positive aspects as well as other potentially devastating. Traditional producers have a production strategy based on looking for a balance between cattle and agriculture production, alternating pasture and crops. A new actor: investment-fund-managers rent the land for agriculture production, and from our team¿s discussions emerged that they follow a strategy similar to that of financial capital: decide what to do in terms of the expected net profit. Economical, ecological and social consequences could be expected. Modeling and simulating with Multi-Agent-Systems was used for exploring the system¿s evolution with the objective of improving our understanding of the agrarian system and to contribute to the envision of possible effects on land use caused by changes in product prices and/or policy changes. The model considers the soil resource as having productive potential and assumes that each traditional producer annually decides whether to change or not its production activity over 25% of its land units or to rent to investment fund managers. A six year database with historical production activity revenues and product¿s prices was used for the simulation, where each simulated year randomly chooses from this database. The first results of these simulations generate questions about the dynamics of the natural resources, challenge the survival of traditional farmers and anticipate landscape changes associated to economic, ecological and social changes. A strong variability was observed from year to year in respect to land use. Results show that if the current price structure is maintained as well as the relation between net profits of agriculture and cattle, then a tendency to expand Investment Fund Managers¿ lands will occur. We conclude that the newly arrived Investment Fund Managers tend to induce a rent activity in traditional producers as well as a substantial increase in agricultural activities (decreasing cattle activities). The historical cattle-agricultural model well known in Uruguay and Argentina has been challenged by market-imposed conditions.The simulation shows that social effects should also be foreseen.

Se realizaron una serie de experimentos virtuales usando un modelo multi-agentes que incluye la dinámica i) de la producción estacional del pasto sobre suelos de basalto, ii) de la estructura del rodeo, iii) del comportamiento climático, iv) de las estrategias aplicadas y la interacción entre estas variables. El modelo es determinístico y dinámico, incluye retroacciones y es caótico, para ciertos rangos de los parámetros manifiesta gran sensibilidad a la situación inicial, con régimen cuasi periódico. La estrategia elegida usa como indicador para la toma de decisiones la altura del pasto. Se usan precios promedio del período 2001-2010, a lo largo de cien años. Se descartan los primeros 30 que son de ajuste del sistema. Se estudió la sensibilidad a tres parámetros: 1) carga de ajuste en invierno 0,7 a 1,4 UG/ha, 2) clima medio 0,5 a 1,2 del promedio, 3) amplitud de variabilidad climática interanual, 0 (clima estacionario) a 1 variabilidad simulada con el modelo Century normalizado. El control fue variabilidad interanual nula, carga de ajuste en invierno 0,9 y clima promedio 1999-2008. Las variables anuales de resultado fueron: ganancia, carga media, altura del pasto, condición corporal y porcentaje de parición. Los resultados indican que con clima medio y sin variabilidad climática interanual la variabilidad para todas las variables elegidas se incrementa fuertemente cuando el ajuste de carga invernal se hace por encima de 0,9 UG/ha. Las ganancias crecen hasta ese valor de ajuste - sin variabilidad interanual - y luego se estabilizan aumentando sensiblemente su variabilidad. Cuando se introduce variabilidad climática interanual los resultados reflejan esta variabilidad crecientemente a medida que los ajustes invernales se realizan a cargas mayores, incluyendo crecientemente ganancias negativas y colapsos del sistema. Estrategias rígidas, solo repoblarse a partir de los nacimientos en el predio por ejemplo, son inviables con la variabilidad actual simulada.

- Abstract
Du fait de la globalisation des échanges, la société rurale uruguayenne se transforme profondément. Ces changements se répercutent sur l'élevage et la gestion de ses terres de parcours. Au travers d'une modélisation participative fondée sur les systèmes multi-agents, les auteurs présentent les facteurs de changement tels qu'ils sont perçus par les acteurs locaux : l'augmentation du prix des produits agricoles liée à la globalisation des échanges, mais également : l'internationalisation des centres de décision ; l'exode rural et l'attrait de la vie urbaine ; l'émergence des " pools de siembra ", nouveaux acteurs de la vie rurale ; les changements biotechniques et climatiques. Ils montrent que la construction de modèles est également un outil de partage de l'information et de formation des acteurs locaux, et permet en outre de dégager des éléments de politique publique durable.

- Abstract
Background: Once-daily atazanavir/ritonavir demonstrated similar antiviral efficacy to twice-daily lopinavir/ritonavir over 48 weeks, with less gastrointestinal disturbance and a better lipid profile, in treatment-naive patients., Methods: International, multicenter, open-label, 96-week noninferiority randomized trial of atazanavir/ritonavir 300/100 mg once daily vs lopinavir/ritonavir 400/100 mg twice daily, each in combination with fixed-dose tenofovir/emtricitabine 300/200 mg once daily, in antiretroviral-naive, HIV-1-infected patients. The primary end point was the proportion of patients with HIV RNA <50 copies/mL at 48 weeks. Results through 96 weeks are reported., Results: Of 883 patients enrolled, 440 were randomized to atazanavir/ritonavir and 443 to lopinavir/ritonavir. At week 96, more patients receiving atazanavir/ritonavir achieved HIV RNA <50 copies/mL (74% vs 68%, P < 0.05) in the intent-to-treat analysis. On both regimens, 7% of subjects were virologic failures by 96 weeks. Bilirubin-associated disorders were greater in patients taking atazanavir/ritonavir. Treatment-related gastrointestinal adverse events were greater in patients taking lopinavir/ritonavir. Mean changes from baseline in fasting total cholesterol, non-high-density lipoprotein cholesterol, and triglycerides at week 96 were significantly higher with lopinavir/ritonavir (P < 0.0001)., Conclusions: Noninferiority of atazanavir/ritonavir to lopinavir/ritonavir was confirmed at 96 weeks. Atazanavir/ritonavir had a better lipid profile and fewer gastrointestinal adverse events than lopinavir/ritonavir.

- Abstract
A committee of six scientific organizations from Argentina developed guidelines for the management of patients with community-acquired pneumonia (CAP).The objective of this study was to evaluate the level of adherence with the recommended care suggested by the guidelines in patients hospitalized with CAP in Argentina. Using quality indicators the management of 436 patients who were hospitalized in 12 Argentinean institutions was evaluated. The care given among the following areas was reviewed: need for hospitalization, need for oxygen therapy, empiric antibiotic therapy, switch therapy, hospital discharge, and prevention. The level of adherence was classified as optimal (>90%), intermediate (60% to 90%), and low (<60%).The selection of the empiric antibiotic therapy and the administration of antibiotics between the first 8 hours after arrival had an adherence greater to 80%. A low level of adherence was found in the areas of switch therapy (53%), prevention of CAP with pneumococcal vaccine (51%) and smoking cessation offered (29%). Using quality indicators it is possible to identify specific areas of management in patients with CAP to a low level of adherence with national guidelines. In Argentina interventions to improve switch therapy and prevention of CAP should be developed.
