A new method for noninvasive assessment of tear film surface quality (TFSQ) is proposed. The method is based on high-speed videokeratoscopy in which the corneal area for the analysis is dynamically estimated in a manner that removes videokeratoscopy interference from the shadows of eyelashes but not that related to the poor quality of the precorneal tear film that is of interest. The separation between the two types of seemingly similar videokeratoscopy interference is achieved by region-based classification in which the overall noise is first separated from the useful signal (unaltered videokeratoscopy pattern), followed by a dedicated interference classification algorithm that distinguishes between the two considered interferences. The proposed technique provides a much wider corneal area for the analysis of TFSQ than the previously reported techniques. A preliminary study with the proposed technique, carried out for a range of anterior eye conditions, showed an effective behavior in terms of noise to signal separation, interference classification, as well as consistent TFSQ results. Subsequently, the method proved to be able to not only discriminate between the bare eye and the lens on eye conditions but also to have the potential to discriminate between the two types of contact lenses. Videokeratoscopy techniques rely on a number of factors in order to achieve accurate estimates of corneal surface topography. Good tear film quality, minimal reflections from eyelashes, and minimal eye movements are essential for corneal topography estimates to be reliable. However, in practice, these ideal conditions may not always be fulfilled, especially in cases of subjects diagnosed with dry eye syndrome, having narrow palpebral apertures, long eyelashes, or nystagmus (uncontrolled eye movements). Such nonoptimal conditions of image acquisition result in poorer estimates of corneal topography. The aim of this paper was to devise a technique that would provide more accurate estimation of corneal topography in such situations and particularly when the source of signal interference is strong. This was achieved by developing a set of algorithms that extract the interference from the acquired raw videokeratoscopic image and filter the topography according to the interference location. The experiments carried out with test surfaces and real corneas showed that this new technique leads to a significant improvement in the topography estimator. Additionally, it is an interference indication procedure that, in the future, could be used for the purpose of tear film quality estimation. It has classically been accepted that the healing of narrow wounds in epithelia occurs by the formation of a contractile actin cable, while wide wounds are resurfaced by lamellipodia-dependent migration of border cells into the denuded area. To further investigate the general validity of this idea, we performed systematic experiments of the roles of wound geometry, wound size, and extracellular matrix (ECM) in wound healing in monolayers of bovine corneal endothelial cells, a system shown here to predominantly display any of the two healing mechanisms according to the experimental conditions. We found that, in this system, it is the absence or presence of the ECM on the wound surface that determines the specific healing mode. Our observations demonstrate that, independent of their size and geometry, wounds created maintaining the ECM heal by migration of cells into the wound area, while ECM removal from the wound surface determines the predominant formation of an actin cable. While the latter mechanism is slower, the actin cable permits the maintainance of the epithelial phenotype to a larger extent during the healing process, as also confirmed by our finding of a more conserved localization of cadherin and vinculin. We also introduce a model that simulates experimental findings about the dynamics of healing mechanisms, both for the maintenance or removal of the ECM on the wound surface. An algorithm for estimating the corneal limbus from videokeratoscopic images is proposed. After the image is transformed to a polar grid, a novel edge-detection procedure, suitable for the detection of the soft edge produced by the limbus, is used to locate the limbus. Outliers due to the eyelids, eyelashes, and videokeratoscopic rings are removed by taking advantage of the approximate circularity of the cornea. An ellipse which minimizes the sum of the squared algebraic errors is fitted to the remaining edge points. Comparisons between the proposed algorithm, a manual computer-based technique and an algorithm which uses conventional edge-detection techniques demonstrate the accuracy of the proposed algorithm. Hyperopia affects approximately 25% of the population. The aim of different heating modalities for the treatment of hyperopia is to steepen the central curvature of the cornea. Conductive keratoplasty (CK) involves the placement of radio-frequency (RF) lesions around a 7-mm-diameter ring concentric with the pupil of the eye. Dynamics of lesion formation during CK depend on corneal electrical impedance, which is expected to change during each 600-ms-long macropulse. The purpose of this study was to measure impedance dynamics during CK. RF lesions were made in in vitro porcine eyes at different power settings. Voltage and current measurements were acquired using a high-speed computer-based data acquisition system. Root-mean-square voltages ([V.sub.RMS]) and currents ([I.sub.RMS]) were calculated for each micropulse, and impedance was determined by calculating the quotient [V.sub.RMS]/[I.sub.RMS]. Initial corneal impedance in vitro was approximately 2000 [ohm]. During the macropulse, impedance decreased initially due to increased mobility of conductive ions. At higher power settings (e.g., > 70%, or maximum peak-to-peak voltage of 233 V), impedance increased after the initial decrease, indicative of local water vaporization and/or tissue coagulation. Preliminary impedance data obtained for in vivo porcine eyes were similar in magnitude to the in vitro values. Index Terms--Cornea, impedance measurements, radio-frequency, thermal damage.

We describe a new, nonapplanating, real-time scanning slit confocal microscope that produces unique real-time video images of the in vivo human cornea. This new real-time slit scanning confocal microscope produces en face, single-video-frame images (2-micrometer sections) with high contrast through the full thickness (500 micrometers) of the normal cornea. No frame averaging or digital image enhancement is required. The images of superficial epithelial cells, wing cells, basal epithelial cells, corneal innervation, nuclei of stromal keratocytes, and the cell bodies of the stromal keratocytes in the posterior stromal region demonstrate the unique real-time imaging characteristics. The microscope is equipped with water-immersion microscope objectives of high numerical aperture. The microscope objective does not flatten or distort the cornea; a polymer gel is used to couple the microscope objective optically to the cornea.
