Background: The aim of this paper is to evaluate the impact on the quality of life of the treatment of nasal vestibule tumors by interventional radiotherapy (IRT-brachytherapy) through a patient reported outcome questionnaire. Methods: We prospectively collected data about patients undergoing IRT according to our institutional schedule of 44 Gy delivered in 14 fractions twice a day. We recorded both acute toxicity data, using the Common Terminology Criteria for Adverse Events (CTCAE) version 5.0, and quality of life data, using the 22-item Sino-Nasal Outcome Test (SNOT-22) at baseline (T0), at 1 month (T1), at 3 months (T3), and at 6 months (T6). Results: We enrolled 10 consecutive patients treated between February 2023 and October 2023. The decrease in terms of SNOT-22 mean value was statistically significant from T0 and T6 with a p-value < 0.001. A noteworthy clinical finding is that quality of life improved regardless of the occurrence of G1-G2 side effects. Conclusions: Using SNOT-22 on patients with nasal vestibule carcinoma treated with IRT has shown an improvement in quality of life that is not strictly dependent on the occurrence of expected G1-G2 side effects. Simple Summary: Conventional closed face masks (CFMs) are used in radiotherapy for head and neck cancer (HNC) and brain cancer (BC) but can cause discomfort and anxiety, affecting the patient experience. Recently, open-face masks (OFMs) have been introduced as an alternative. This study reviews and explores the application and use of OFMs in the treatment of HNC and BC, analyzing 19 relevant studies. The analysis shows heterogeneity in the type of OFMs used, especially in BC. In some cases, they are used alone, while in others, they are combined with complementary devices such as mouth bites. For both treatment sites, the review shows that OFMs, especially in combination with surface-guided radiotherapy (SGRT), offer significant advantages in terms of patient comfort and positioning accuracy. The results suggest that OFMs can achieve sub-millimeter and sub-degree reproducibility, which supports their clinical integration. Introduction: The main goal of radiotherapy (RT) is to deliver a precise dose to the target while sparing the surrounding normal tissue and minimizing side effects. Appropriate patient immobilization is crucial, especially for head and neck cancer (HNC) and Brain Cancer (BC). Conventional closed-face masks (CFMs), while effective in minimizing head motion, can cause significant discomfort, anxiety, and claustrophobia. Open-face masks (OFMs) have been developed to increase patient comfort while providing precise immobilization. Methods: Following the Preferred Reporting Items for Systematic Review and Meta-Analysis (PRISMA) extension for scoping reviews and the Arskey and O'Malley framework, an electronic search of EMBASE, PubMed, SCOPUS, and Web of Science was conducted to identify original studies reporting the use and description of OFMs in clinical practice up to April 2024. The inclusion criteria were English-language articles focusing on OFMs for HNC and BC patients undergoing RT. Results: Of 618 titles, 19 articles fulfilled the selection criteria. Most studies were comparative (n = 13) or observational (n = 6). The articles were categorized by treatment site, resulting in three groups: BC (n = 14, 68.4%), HNC (n = 4, 21.4%), and mixed (n = 2, 10.5%), which includes both BC and HNC. Of note, 82.4% (n = 16) of the included studies were published from 2020 onwards, emphasizing the recent adoption of OFM in clinical practice. Conclusions: The reviewed studies show that OFMs, in combination with SGRT, offer significant advantages in terms of patient comfort and positioning accuracy in HNC and BC treatments. Reproducibility in the sub-millimeter and sub-degree range can be achieved, which supports the use of OFMs in clinical practice. Future research should explore innovative combinations of immobilization and monitoring to further improve RT outcomes and ensure precise treatment while increasing patient comfort. Purpose: Nasopharyngeal cancers (NPC) are very aggressive, and the recurrence rate after radical therapy is high. This study aimed to evaluate the efficacy of brachytherapy (BT) also called interventional radiotherapy (IRT) in primary NPC in comparison with external beam radiotherapy (EBRT) alone. Material and methods: A systematic search was performed in Scopus, Cochrane, and PubMed databases. Clinical query based on PICO framework was as follows: In patients with NPC (P), is EBRT plus IRT (I) superior to EBRT alone (C) in terms of local control (LC) and toxicity (O)? Full articles evaluating the efficacy of IRT as a boost after EBRT in patients with NPC were considered. Results: Eight papers, including 1,320 patients fulfilled the inclusion criteria. The median 5-year LC for IRT group and no-IRT group was 98% (range, 95.8-100%) and 86% (range, 80.2-91%), respectively; the median 5-year overall survival (OS) for IRT group and no-IRT group was 93.3% (range, 89.2-97.5%) and 82.9% (range, 74.8-91.1%), respectively; the median 5-year DFS for IRT group and no-IRT group was 94.2% (range, 92.5-96%) and 83.9% (range, 73.3-94.6%), respectively; the median 5-year cancer-specific survival (CSS) for IRT group and no-IRT group was 96% (range, 94.5-97.5%) and 88.2% (range, 83.4-93.1%), respectively. G1-2 and G3-4 toxicities were similar in some articles, or significantly lower in patients treated with IRT in other papers. Conclusions: Data suggest that IRT may improve results of external beam radiotherapy in primary NPCs, especially when using new technologies. [ABSTRACT FROM AUTHOR]

Interventional radiotherapy (IRT, brachytherapy) is a highly effective treatment method for non-melanoma skin cancer (NMSC). Traditionally, the maximum depth of NMSC lesions considered eligible for contact IRT was 5 mm; however, following several national surveys and recent recommendations, such cut-off, lesions thicker than 5 mm may be treated by contact IRT. The use of image guidance in defining the actual depth in treating NMSC to correctly identify clinical target volume (CTV) and prevent unnecessary toxicity is of paramount importance. The aim of the paper was to describe a multilayer arrangement of catheters to treat NMSC lesions thicker than 5 mm, thus proposing an example of dynamic intensity modulated IRT, using different catheter-to-skin distance of sources to reach the best CTV coverage and maximally reduce the excess of dose to the skin. OBJECTIVE Interventional oncology (IO) offers minimal invasive oncological procedures to personalize therapeutic approaches. We aimed at assessing potential advantages of an IO multidisciplinary tumor board (INTER-BOARD) in terms of waiting time, procedural adverse events, and length of hospital stay. METHODS We retrospectively evaluated all patients managed at our institutional INTER-BOARD from January 2018 to June 2019. Outcome measurements such as number of patients managed in each semester, median interval time between initial assessment and admission to the hospital, total number of procedures, procedures type, adverse events, and complication-related days of hospital stay were analyzed. RESULTS The INTER-BOARD discussed 438 patients (mean age 64 years; range 27-92) during the study period. Overall, 203 procedures were performed in patients >65 years and 49 in patients >80 years. The number of patients discussed progressively increased over time 82 (18.7%) during the first semester, 120 (27.4%) during the second semester, and 236 (54.1%) in the third semester. Patients with 33 different cancer types were discussed and 22 different procedure types were performed. Mean hospitalization time was 4 days (progressively decreasing over time from 5 to 3 days). Adverse events occurred in 4% of cases, with a progressively decreasing rate over time (16%, 5%, and 3% for 1st, 2nd, and 3rd semester, respectively). CONCLUSION In our experience, the dedicated INTER-BOARD allowed optimization of patient care through critical evaluation of all treatment options offered by different specialists. A-48-year old man, affected by HIV, working as an electrician, presented a swollen and painful subungual lesion over the distal phalanx of the right thumb with an important limitation to his job. A cutaneous examination of the right thumb revealed an irregular erosive crusted mass formation about 2 cm×1 cm size with erythematous changes and complete destruction of the overlying nail plate. There was no evidence of regional lymphadenopathy; the patient was studied with computed tomography that did not show any distant disease. A conservative onychectomy was performed with removal of the lesion and with histological confirmation of SCC. However, after five months, the patient presented local recurrence over the thumb. Amputation was proposed, but the patient declined due to the negative impacts on his job. Thus, contact radiotherapy (i.e., brachytherapy, BT) was proposed. BT was delivered using a customized homemade surface mold with six plastic tubes arranged around. The total delivered dose was 40 Gy delivered in eight fractions, 5 Gy for daily using Iridium-192 High Dose Rate (HDR) source. Four years later, recently, the patient is free from disease and has a complete functionality of the thumb without any limitations in any movement and in his job.
