Air pollution and climate change are risk factors for noncommunicable diseases of paramount importance and of major concern in a population. Their complex interaction suggests the need for an integrated and participatory approach by health professionals and citizens. During the Italian BRIGHT-NIGHT (European Researchers' Night) at the Pisa Research Campus of National Research Council (CNR), a laboratory focusing on environmental epidemiology issues and open to the public was set up. A self-administered questionnaire was distributed, with the aim of investigating the individual perception, knowledge, opinions and attitudes in relation to 12 different environmental, natural and anthropogenic hazards. The questionnaire was completed by 44 subjects aged over 18 years (47.6% female, 56.8% university graduates, 61.4% employed). Air pollution and climate change were considered two priorities for environmental protection and public health by 72.1%, prompting about one third of the subjects to commit to increasing active commuting. The results of this pilot survey indicate a diffuse awareness of air pollution and climate change as main environmental determinants to be tackled in order to protect public health. The population of the Venafro Valley (Southern Italy) faces various type of air pollution problems (industrial facilities, traffic, and biomass combustion). To estimate exposure to various pollution sources, a multi-stage random forest model was used, integrating particulate matter (PM) data with satellite observations, land-use patterns, and meteorological information generating maps of PM2.5 concentration. Four distinct PM2.5 exposure categories were established using the quartile method. To assess the association between PM2.5 and cause-specific mortality and morbidity, a time-dependent and sex-specific Cox multiple regression analysis was conducted, adjusting for age classes. In addition, the hazard ratios were accompanied by a probability measure of the strength of the evidence toward a hypothesis of health risk associated with the exposure under study (1−p value). The whole cohort was exposed to PM2.5 annual levels exceeding the 5 µg/m3 limit recommended by the World Health Organization. Mortality excesses were observed in class 3 for both sexes for cardiac heart diseases. Excesses of cardiovascular diseases were observed for both sexes in class 3 and 4. The COVID‐19 pandemic has induced significant impairments, including sleep disturbances. The present study aimed to explore the impact of fear in relation to stress on sleep disorders among Italian adults and older participants in the second phase of the EPICOVID19 web‐based survey (January‐February 2021). Sleep disturbances during the pandemic were evaluated using the Jenkins Sleep Scale, perceived stress through the 10‐item Perceived Stress Scale and fear of contagion and about economic and job situation with four ad hoc items. The strength of the pathways between stress, sleep disturbances and fear was explored using structural equation modelling, hypothesising that stress was related to sleep disturbances and that fear was associated with both stress and sleep problems. Out of 41,473 participants (74.7% women; mean age 49.7 ± 13.1 years), 8.1% reported sleep disturbances and were more frequently women, employed in a work category at risk of infection or unemployed, and showed higher deprivation scores. Considering an a priori hypotheses model defining sleep and stress scores as endogenous variables and fear as an exogenous variable, we found that fear was associated with sleep problems and stress, and stress was associated with sleep problems; almost half of the total impact of fear on sleep quality was mediated by stress. The impact of stress on sleep quality was more evident in the younger age group, among individuals with a lower socioeconomic status and healthcare workers. In this work a meta-policy on health promotion is analysed, through content analysis of documents and interviews to stakeholders, with the aim of understanding the use of evidence and the knowledge conversion process in a multi-actor policy trajectory. We study the different sources of evidence and their interaction. Among our results, we describe a process of document analysis and classification which leads to the definition of mediated use of research evidence. The analysis of the policy trajectory helps to identify when the use of evidence is driven by the local level, and to recognise "pivot" individuals in the policy development.

From the initial COVID-19 outbreak, Italy was the first Western country to be seriously affected by the pandemic. Understanding vaccine hesitancy can help efforts to achieve broad vaccination coverage. The objectives of this research were to determine the extent of vaccine hesitancy in Italy and to understand the characteristics of those segments of the population with some hesitancy. Between January and February 2021, 41,473 subjects answered the second questionnaire delivered in phase II of the web-based EPICOVID19 survey. Among the included adult volunteers living in Italy, 4653 (11.2%) reported having previously received at least one dose of the COVID-19 vaccine. In the sample of 36,820 respondents, all not vaccinated (age 51.1 ± 13.5; 59.7% female; 63.6% high level of education), the comparison between hesitant and inclined participants was accompanied by percentages and odds ratios. A total of 2449 individuals were hesitant (6.7% of the unvaccinated ones). Hesitancy was higher among women (OR = 1.48; 95%CI: 1.36–1.62); it was highest in the 50–59 and 40–49 age groups and among those with a lower educational level. A higher level of education was associated with a lower proportion of hesitancy (5.54%) compared with 9.44% among respondents with a low level of education (OR = 0.56; 95%CI: 0.46–0.68). Hesitancy was most common in subjects who did not report fear of infection (12.4%, OR = 4.0; 95%CI: 3.46–4.61). The LIFE GIOCONDA is an ongoing project that aims to provide an innovative methodology to the authorities for supporting the environment & health policies by involving the young people in the decision-making processes. The project suggests a web platform able to relate air and noise pollution data in the schools with the students’ pollution awareness. GIOCONDA aims to enhance the awareness of students, teachers and local administrations on the noise issues in schools, presenting suitable tools to improve the public participative processes. This paper presents a new method that has been developed within the Project. It aims to evaluate the acoustic performances of a classroom and to suggest the use of a global indicator based on a group of acoustic parameters compared with their limit values. Whit the new method the comparison between different classrooms or different schools becomes possible, together with a homogeneous evaluation of the priority for planning noise mitigation actions. Several noise measurement campaigns have been performed to characterize the students’ exposure in eight Italian schools. The results are useful to describe the acoustic performances of classrooms.

Introduction Physical, chemical, and biological hazards in the environment are responsible for > 24% of the global burden of disease and for 36% of all deaths in children worldwide (Pruss-Ustun [...], Background: Chronic diseases are increasing among children in Latin America. Objective and Methods: To examine environmental risk factors for chronic disease in Latin American children and to develop a strategic initiative for control of these exposures, the World Health Organization (WHO) including the Pan American Health Organization (PAHO), the Collegium Ramazzini, and Latin American scientists reviewed regional and relevant global data. Results: Industrial development and urbanization are proceeding rapidly in Latin America, and environmental pollution has become widespread. Environmental threats to children's health include traditional hazards such as indoor air pollution and drinking-water contamination; the newer hazards of urban air pollution; toxic chemicals such as lead, asbestos, mercury, arsenic, and pesticides; hazardous and electronic waste; and climate change. The mix of traditional and modern hazards varies greatly across and within countries reflecting industrialization, urbanization, and socioeconomic forces. Conclusions: To control environmental threats to children's health in Latin America, WHO, including PAHO, will focus on the most highly prevalent and serious hazards-- indoor and outdoor air pollution, water pollution, and toxic chemicals. Strategies for controlling these hazards include developing tracking data on regional trends in children's environmental health (CEH), building a network of Collaborating Centres, promoting biomedical research in CEH, building regional capacity, supporting development of evidence-based prevention policies, studying the economic costs of chronic diseases in children, and developing platforms for dialogue with relevant stakeholders. http://dx.doi.org/10.1289/ehp.1408292

Abstract Background Cost-benefit analysis is a transparent tool to inform policy makers about the potential effect of regulatory interventions, nevertheless its use to evaluate clean-up interventions in polluted industrial sites is limited. The two industrial areas of Gela and Priolo in Italy were declared "at high risk of environmental crisis" in 1990. Since then little has been done to clean the polluted sites and reduce the health outcomes attributable to pollution exposure. This study, aims to quantify the monetary benefits resulting from clean-up interventions in the contaminated sites of Gela and Priolo. Methods A damage function approach was used to estimate the number of health outcomes attributable to industrial pollution exposure. Extensive one way analyses and probabilistic analyses were conducted to investigate the sensitivity of results to different model assumptions. Results It has been estimated that, on average, 47 cases of premature death, 281 cases of cancer and 2,702 cases of non-cancer hospital admission could be avoided each year by removing environmental exposure in these two areas. Assuming a 20 year cessation lag and a 4% discount rate we calculate that the potential monetary benefit of removing industrial pollution is €3,592 million in Priolo and €6,639 million in Gela. Conclusions Given the annual number of health outcomes attributable to pollution exposure the effective clean-up of Gela and Priolo should be prioritised. This study suggests that clean-up policies costing up to €6,639 million in Gela and €3,592 million in Priolo would be cost beneficial. These two amounts are notably higher than the funds allocated thus far to clean up the two sites, €127.4 million in Gela and €774.5 million in Priolo, implying that further economic investments - even considerable ones - could still prove cost beneficial.

In 2014, the Sardinian Regional Department of Hygiene, Health and Social Security promoted the Regional Prevention Plan, which embraced the protection of the population from exposure to indoor pollutants, including radon gas. The programme foresaw: the drafting of "Guidance for the construction/renovation of buildings"; radon monitoring and mapping activities during a dedicated campaign based on geological surveying; a radon health impact assessment; community involvement and a radon risks communication campaign. The objectives of the programme were focused to protect Sardinian population from radon risk, with special reference to vulnerable and susceptible subjects, spread knowledge about risks and the opportunities to reduce them. Using a Health Impact Assessment procedure, the number of attributable deaths was estimated based on radon exposure levels, also visible as preventable events by implementing preventive actions. The purpose of this article is to illustrate the activities carried out, with specific reference to the use of communication to develop each action, to understand strengths and weaknesses and the lessons to be applied in Sardinia and other areas. Monitoring and evaluation results indicate that the health of populations living in radon-exposed areas can be significantly improved by reducing exposure to radon and synergistic risk factors. It is essential to strengthen awareness-raising events using historical and acquired knowledge, and to monitor progress in order to reinforce further actions, as these schemes should be planned for the long term, with central coordination and continuous evaluation. The case of radon risk management and communication in Sardinia offers a number of suggestions and lessons learned, both for the continuation of the work that is expected to be very intense in the coming years, and for the problems that may arise in other regions and countries affected by radon risk.
