The biofortification of food crops for human consumption is a direct strategy for increasing dietary intake of selenium (Se). The aims of this study were the evaluation of different Se-fortification variables (concentration and number of foliar applications) for obtainment of fortified olives and the efficacy of two technological treatments (brining and dehydration) applied to them in the maintenance of this element. The research was carried out in two experiments: the results of first one were that the application of the highest Se concentration (75 mL L−1) on olive trees was useful to obtain fortified Gordal olives (650 µg kg−1) but the further brining process did not allow an acceptable retention of this nutrient in olive pulp for its dissolution in brines: the decrease was from 92 to 99%. The second experiment concerned in different number of foliar applications of the best previously tested Se-solution (75 mL·L−1) to obtain fortified Carolea olives, that were then submitted to different drying processes to became edible. The available selenium in the final products was better retained in olives dried without any preliminary treatment prior to drying and in those pre-treated by dried salt, with different trends linked to foliar application number.

Background: Radiation-induced health risks are broadly questioned in the literature. As cone beam computed tomography (CBCT) is increasingly used in non-dental examinations, its effective dose needs to be known. This study aimed to review the published evidence on effective dose of non-dental CBCT for diagnostic use by focusing on dosimetry system used to estimate dose. Materials and methods: A systematic review of the literature was performed on 12 November 2017. All the literature up to this date was included. The PubMed and web of science databases were searched. Studies were screened for inclusion based on defined inclusion and exclusion criteria according to the preferred reporting items for systematic reviews. Results: Fifteen studies met the inclusion criteria and were included in our review. Thirteen and two of them examined one and two anatomical areas, respectively. The anatomical areas were: ear (6), paranasal sinuses (4), ankle (3), wrist (2), knee (1), and cervical spine (1). Effective dose was estimated by different methods: (i) RANDO phantom associated with thermoluminescent dosimeters (6), metal oxide semiconductor field-effect transistor dosimeters (3), and optically stimulated luminescent dosimeters (1). (ii) Scanner outputs, namely computed tomography dose index (1) and dose area product (2). (iii) Monte Carlo simulations (2). Conclusion: CBCT of extremities, cervical spine, ears and paranasal sinuses was found to be a low-dose volumetric imaging technique. Effective doses varied significantly because of different exposure settings of CBCT-units and different dosimetry systems used to estimate dose.

Hepatocellular carcinoma (HCC) is the principal primary liver tumor, representing the third largest cause of cancer-associated death worldwide. The actual reference standard systemic treatment for advanced HCC is represented by sorafenib, a multi-targeted orally active small-molecule tyrosine kinase inhibitor. Sorafenib has exhibited a good general safety profile in multiple clinical trials. However, adverse drug-associated events are common, occasionally severe, and special attention should be paid to cardiovascular adverse events, particularly in patients with risk factors or known heart disease. In the present study, the case of a patient with no known cardiovascular risk factors affected by highly enhancing advanced HCC in cirrhotic liver, who died during successful sorafenib monotherapy, is reported.

At the state-of-the-art, electric propulsion systems based on hub-motors allow mechanical components to be removed, increasing also the vehicle stability. However, since motors and wheels are directly coupled, when a high propulsion torque is delivered, in such a configuration the motors could work in low efficiency operative points, negatively affecting the vehicle consumptions. In this paper, a torque distribution strategy is defined to minimize these consumptions. The propulsion configuration under investigation is a four-wheel drive that adopts two different couples of Brushless DC (BLDC) hub motors. The proposed research highlights how the electric efficiency is improved when the propulsion torque required by the vehicle is delivered by the BLDCs in an asymmetric way, according to the motors operative conditions. Since this torque repartition dynamically changes while the vehicle is running, the effects on the vehicle stability are also evaluated. Such an analysis is performed considering, as reference, a Class A vehicle, principally designed for the urban mobility.

Salivary gland tumours are rare, representing only 3% of all head and neck neoplasms, with the parotid gland being the most common site (80 %). The risk of malignancy is inversely proportional to the size of the gland: lesions arising in the sublingual or minor salivary glands are more likely to be malignant, whereas parotid gland neoplasms are mostly benign. Fine needle aspiration cytology and core needle biopsy are considered the most accurate modalities for the diagnosis of a salivary gland neoplasm; however, they are not always conclusive due to procedural sampling errors and for the presence of a cytological / histological overlap between benign and malignant tumours. Moreover, they cannot be easily performed for parotid deep portion localisation. The role of magnetic resonance imaging (MRI) is growing and advanced techniques (diffusion-weighted and dynamic contrast-enhanced perfusion-weighted imaging) can provide useful additional information for the assessment of salivary gland neoplasms. The aim of this review is to present the main MRI and clinical features of salivary gland tumours to improve their comprehensive evaluation and characterisation.

Occurrence of multiple primary lung cancers (MPLC) in individuals undergoing low-dose computed tomography (LDCT) screening has not been thoroughly addressed. We investigated MPLC in subjects recruited in the ITALUNG randomized clinical trial. Cases of cytologically/histologically proven MPLC detected at screening LDCT or follow-up CT were selected and pathologically re-evaluated according to the WHO 2015 classification. Overall 16 MPLC were diagnosed at screening LDCT (n=14, all present at baseline) or follow-up CT (n=2) in six subjects (4 in one subject, 3 in two and 2 in three subjects), representing 0.43% of the 1,406 screenees and 15.8% of the 38 subjects with at least one screen-detected primary lung cancer. MPLC included 9 adenocarcinomas in three subjects and a combination of 7 different tumour histotypes in three subjects. MPLC, mostly adenocarcinomas, are not uncommon in smokers and ex-smokers with at least one LDCT screen detected primary lung cancer.

Electric vehicles represent a valid solution to solve the air pollution problem. The typical propulsion system of an electric vehicle includes a high speed-low torque electric motor combined with some mechanical parts, such as gearbox, transmission and differential. The indirect coupling between driving wheels and engine allows the electric motor to work close to its maximum operative efficiency point. A different traction system is based on using a hub motor for each wheel drive instead of a single high speed-low torque motor. This allows removing all the mechanical transmission components, reducing weight and increasing space saving. However, in this configuration the hub motor works on all its operating range, affecting adversely the efficiency. The aim of this paper is to analyze efficiency, weight, and cost of different propulsion systems of an electric car designed principally for the urban mobility. The propulsion system is based on hub motors, and two- and four-wheel drive configurations are either evaluated. Furthermore, in the case of the four-wheel drive, configurations with equal or different hub motor features are analysed. The main results of this study points out that hybrid configurations have better performance with respect to the two-wheel drive or the homogenous four-wheel drive solutions.

This paper describes the implementation of a Test Lab purposely designed for the trial and validation of different energy flow management methods to be applied in Energy and Comfort Management Systems for Smart Homes and Smart Buildings. The Test Lab is located in a student apartment within the Campus of the University of Calabria. It represents the nucleus of a larger testing system addressed to the experimentation of microgrids. Some preliminary experiments have been performed to validate the operation and the effectiveness of the implemented system.

To evaluate the effective dose and image quality of horizontal CBCT in comparison with multislice spiral CT (MSCT) in scans of the head, cervical spine, ear and dental arches.A head and neck Alderson-RandoThe CBCT effective dose was 248, 249, 361, 565 and 688 µSv in the cervical spine, head, ear, dental arches with small field of view and dental arches with medium field of view, respectively. The MSCT effective dose was 3409, 1892, 660 and 812 µSv in the cervical spine, head, ear and dental arches, respectively. The modulation transfer function was 0.895 vs 0.347, 0.895 vs 0.275, 0.875 vs 0.342 and 0.961 vs 0.352 for CBCT vs MSCT in the cervical spine, head, ear and dental arches, respectively. Head and cervical spine MSCT showed greater CNR than CBCT, whereas CNR of the ear and dental arches showed comparable values.CBCT was preferable to MSCT for the ear and dental arches volumetric imaging due to its lower radiation dose and significantly higher spatial resolution. In the case of cervical spine and head imaging, MSCT should be generally recommended if a high contrast resolution is required, despite the greater radiation exposure.

Background: The effect of thrombus aspiration on mortality is still controversial, with results which are often inconsistent in different randomized trials, real world registries and different follow-up duration. The aim of this analysis was to assess the effect on 30-day and 1-year mortality of thrombus aspiration during primary percutaneous coronary intervention (pPCI) for ST-elevation myocardial infarction (STEMI) compared with conventional PCI. Methods: We used data from all the consecutive STEMI patients treated either with conventional PCI or thrombus aspiration between January 1, 2004 and January 1, 2012. Propensity matching score was calculated on the basis of several baseline and procedural characteristics in order to predict the probability for each patient of having been treated with thrombus aspiration. This propensity score analysis was used in order to select a cohort of patients treated with thrombus aspiration matched one-to-one with patients treated with conventional PCI. Results: In total, 744 (53.1%) patients out of 1,400 enrolled were treated with thrombus aspiration. In the matched cohort, at 30-day follow-up 6.3% of patients in the conventional PCI group died compared to 4.7% in the thrombus aspiration group. The unadjusted hazard ratio (HR) for 30-day mortality was 1.01 (95% CI 0.33–3.14, p = 0.985). In the same cohort, 10.7% of patients died at 1-year in the conventional PCI group compared to 5.2% in the thrombus aspiration group. The 1-year unadjusted hazard ratio for mortality was 0.47 (95% CI 0.25–0.90, p = 0.025). The HR changed and was no longer significant after adjustment for differences in the use of glycoprotein (GP) IIb/IIIa inhibitors, lesion pre-dilatation and pre-procedural TIMI flow: 0.71 (95% CI 0.36–1.39, p = 0.322). Conclusions: Thrombus aspiration does not influence 30-day mortality, however it is associated with 1-year survival benefit. GP IIb/IIIa inhibitors and thrombus aspiration may have an important synergistic role in leading to this long-term benefit.

Viene valutato il flusso ematico cerebrale mediante indagine Xenon-TAC in 11 traumi cranio-cerebrali gravi (GCS all'ingresso uguale od inferiore a 8) con quadro TAC morfologico di lesione diffusa secondo la classificazione di Marshall. La quantificazione della perfusione ematica cerebrale, confrontata con i valori attesi di norma permette, fin dalla fase acuta del trauma attendibile giudizio prognostico che si è dimostrato statisticamente significativo sia per il sottogruppo di pazienti con favorevole evoluzione clinica che per quelli con sfavorevole evoluzione valutata al follow-up di 3 e 6 mesi, rispetto ai valori stimati su un campione di pazienti sani. In particolare l'analisi statistica (eseguita con il “Chi Test”) ha dimostrato significatività di valori flussimetrici sia a livello dei nuclei della base che in sede corticale mentre minor significatività è stata rilevata per i valori di flusso valutati a livello dell sostanza bianca e globalmente in sede emisferica.

To analyze risk factors for type 2 diabetes among women attending menopause clinics in Italy for counselling about the menopause.Women attending a network of first-level outpatient menopause clinics in Italy for general counselling about menopause or treatment of menopausal symptoms.Cross-sectional study with no exclusion criteria. Type 2 diabetes was defined according to National Diabetes Data Groups Indications and the fasting blood glucose at an oral glucose tolerance test within the previous year.Out of the 44 694 considered in this analysis, 808 had a diagnosis of diabetes type 2 (1.8%). In comparison with women aged50 years, the multivariate odds ratios (OR) of type 2 diabetes were 1.31 (95% confidence interval (CI), 0.99-1.74) for women aged 50-52 years, 1.66 (95% CI, 1.27-2.17) at 53-56 years and 2.84 (95% CI, 2.20-3.67) in women agedor = 57 years. Type 2 diabetes was less frequently reported in more educated women (OR high school/university vs. primary school = 0.44 (95% CI, 0.36-0.55)). Being overweight was associated with an increased risk of type 2 diabetes. In comparison with women reporting a low level of physical activity, the multivariate OR of type 2 diabetes was 0.67 (95% CI, 0.54-0.84) for women reporting regular physical activity. In comparison with premenopausal women, the multivariate OR of type 2 diabetes was 1.38 (95% CI, 1.03-1.84) in women with natural menopause. This finding was present also after allowing for the potential confounding effect of age. The multivariate OR of diabetes for users of hormonal replacement therapy was 0.58 (95% CI, 0.46-0.73).This large cross-sectional study suggests that postmenopausal women are at higher risk of type 2 diabetes after allowance for the effect of age. Other main determinants of risk of type 2 diabetes in women around menopause were low socioeconomic status and being overweight. Diabetes was found less frequently in those taking hormone replacement therapy.

OBJECTIVE To obtain data on correlates of climacteric symptoms in women around menopause attending menopause clinics in Italy. METHODS Since 1997 a large cross sectional study has been conducted on the characteristics of women around menopause attending a network of first level menopause outpatient's clinics in Italy. A total of 66,501 (mean age 54.4 years) women are considered in the present paper. RESULTS The odds ratios of moderate and severe hot flashes/night sweats were lower in more educated women and (for severe symptoms only) in women reporting regular physical activity. Depression, difficulty to sleep, forgetfulness and irritability tended to be less frequent in more educated women and (depression only) in women reporting regular physical activity. Parous women reported more frequently these symptoms. CONCLUSIONS This large study confirms in Southern European population that low education, body mass index and low physical activity are associated with climacteric symptoms. Parous women are at greater risk of psychological symptoms.

The cytotoxic activity on tumoral cells of thymopentin (TP5), a synthetic pentapeptide corresponding to residues 32–36 of thymopoietin, was evaluated after inclusion in multilamellar liposomes. These were made by all neutral (zwitterionic) phospholipids as well as those containing positively (stearylamine, SA) or negatively charged (DPPS) lipids. Vesicles were also prepared containing cholesterol, and were characterized by light scattering dimensional analysis and differential scanning calorimetry. The effect of charged species and of the presence of cholesterol on TP5 incorporation was studied. For the different kinds of liposomes, loaded with sodium fluoresceinate, the rate of interaction after incubation with lymphocytes in a normal or tumoral (IM9 lymphoblastoid cells) state was also evaluated. TP5-loaded vesicles were assayed on two lines of tumoral cells (IM9 and K562) to assess the ability of liposomal carriers to potentiate the cell growth inhibitory activity of the drug. SA-containing positive liposomes appeared as the most efficacious, resulting in 87% inhibition on IM9 cells, with an amount of liposomes containing 100 ng of TP5.

Age at menopause is population-specific, with a range from the early 40s in Yucatan Mayan women to the early 50s for the Ede population in the Netherlands. Even within the same population we may observe some differences. For instance, the mean age at menopause for Indonesians in Central Java is 50.2 years in the educated population and 46.0 years in the rural population. Also typical and atypical climacteric symptoms are different among different populations, e.g. the vasomotor symptoms cause virtually no difficulties in Asian women and the psychological impact of the menopause is also much less of a problem in Asian than in Western women [1–3].

A representative sample of the general population residing in the city of Florence was invited to participate in the local section of the EPIC study with two major aims: i) to carry out a population-based survey on dietary and life-style habits in this urban area of Tuscany, Central Italy; ii) to compare these results with a large series of EPIC volunteers residing in the same municipality in order to evaluate the differences between the two groups. A random sample of 500 residents (250 women) aged 40-64 years, was invited to participate in the study; 362/500 (72.4%) accepted and followed the EPIC protocol. The distribution of selected individual characteristics (including measured weight and height) showed a high prevalence of being overweight in men (52.2%) and obesity in both sexes (17.4% in men and 12.5% in women). A dietary pattern characterized by a high consumption of red meat, processed meats, olive oil and wine emerged in both sexes; the consumption of vegetables and fresh fruit was approximately 200 and 300 g/day, respectively. The estimated mean intakes of macronutrients reflected this pattern, with a high mean intake of total fat and protein. The mean contribution to total caloric intake provided by fat was 30.9% and 33.6% in these randomly sampled men and women, respectively. The results were compared with those of 9,123 Florence residents aged 40-64 years and enrolled as EPIC volunteers. Current smokers and less educated subjects were less represented among male volunteers, who, in general, showed a healthier dietary pattern (more fresh fruit and less spirits). Female volunteers were taller and heavier and consumed more fresh fruit but also more beef and less carbohydrates. Other statistically significant differences emerged, but the absolute values of these differences were usually modest and the two groups appeared remarkably similar. Overall, our results suggest that the large EPIC-Florence cohort was not strictly selected and showed a total caloric intake and a range of dietary variability similar to that of the general population of the same area.
