Abstract

BACKGROUND: Chronic constipation represents a widespread medical condition, mainly in elderly people. The management is compelling as these patients frequently have comorbidity and polypharmacy. Polyethylene glycol (PEG) is commonly used but does not resolve some complaints, mainly concerning pain and bloating. Therefore, a new medical device containing PEG plus simethicone, an antifoaming agent, has been developed. The present survey explored its use in clinical practice. METHODS: A group of 64 Italian gastroenterologists visited 2426 patients with chronic primary constipation. In about two/third, gastroenterologists freely prescribed the new medical device, and in the remaining patients, other treatments were freely chosen by doctors. Patients were followed up for three months. The number of evacuations, stool characteristics, patients’ perception of illness assessed by VAS, frequency of patients with pain and intestinal symptoms, and discontinuing patients was observed during the experience. RESULTS: The new medical device was significantly better than other therapies in improving all parameters (P<0.05 for all parameters). CONCLUSIONS: The present survey demonstrated that a new medical device containing PEG plus simethicone represents a favorable option in managing patients with chronic primary constipation as significantly and safely improves intestinal transit, reduces stool consistency, reduces symptom severity, and is highly satisfactory.

Abstract

Direct antivirals are available for treating recurrent hepatitis C (RHC). This study reported outcomes of 424 patients with METAVIR F3–F4 RHC who were treated for 24 weeks with sofosbuvir/ribavirin and followed for 12 weeks within the Italian sofosbuvir compassionate use program. In 55 patients, daclatasvir or simeprevir were added. Child–Pugh class and model of end stage liver disease (MELD) scores were evaluated at baseline and 36 weeks after the start of therapy. The sustained viral response (SVR) was 86.7% (316/365) in patients who received sofosbuvir/ribavirin and 98.3% (58/59) in patients who received a second antiviral (P < 0.01). In patients treated with sofosbuvir/ribavirin, a significant difference in SVR was observed between patients diagnosed with METAVIR F4 (211/250; 84.4%), METAVIR F3 (95/105; 90.5%) and fibrosing cholestatic hepatitis (10/10; 100%) (P = 0.049). A significant association was found between patients who worsened from Child–Pugh class A and who experienced viral relapse (4/26 vs. 8/189, P = 0.02). In patients with a baseline MELD score <15, a significant association was found between maintaining a final MELD score <15 and the achievement of SVR (187/219 vs. 6/10, P = 0.031). This real-world study indicates that sofosbuvir/ribavirin treatment for 24 weeks was effective, and the achievement of SVR was associated with a reduced probability of developing worsening liver function.

Abstract

The diagnosis of Alzheimer's disease is still subject to error, and a completely reliable diagnosis may be obtained only at autopsy. Although much is known about the changes that occur within the brain during Alzheimer's disease, these changes are not detectable from outside the living patient, and the physician making a clinical diagnosis must rely upon more variable signs in order to arrive at a satisfactory diagnosis. The two pathologic signs characteristic of Alzheimer's disease are the neurofibrillary tangles and the senile plaques. Neurofibrillary tangles are composed of abnormal nerve filaments, within which several different components may be detected using specific antibodies. One of the structures that bind specific antibodies, called an epitope, has been given the name tau. Antibodies to the tau epitope react with neurofibrillary tangles, as well as with the so-called 'neuropil threads' found in many dystrophic nerve processes within an Alzheimer's brain. (Dystrophic nerves have disordered function.) Previous research has shown that structures resembling these neuropil threads may be seen in olfactory mucosa biopsy specimens obtained from patients with Alzheimer's disease. (Olfactory mucosa is mucosal tissue in the nose, which is involved in olfaction, or the sense of smell.) Researchers have now demonstrated that in addition to a similar appearance, these neuropil threads in the olfactory mucosa are also recognized by antibodies to tau. The tests were performed on olfactory biopsies from eight patients with probable Alzheimer's disease and six normal age-matched control subjects. Nerve cell processes positive for the tau epitope could be identified in the olfactory mucosa of all eight patients with Alzheimer's disease and in none of the control subjects. The authors suggest that this test may prove to be a useful method for improving the reliability of the diagnosis of Alzheimer's disease. Abstract

Knowledge of the food web structure and complexity are central to better understand ecosystem functioning. A food-web approach includes both species and energy flows among them, providing a natural framework for characterizing species’ ecological roles and the mechanisms through which biodiversity influences ecosystem dynamics. Here we present for the first time a high-resolution food web for a marine ecosystem at Potter Cove (northern Antarctic Peninsula). Eleven food web properties were analyzed in order to document network complexity, structure and topology. We found a low linkage density (3.4), connectance (0.04) and omnivory percentage (45), as well as a short path length (1.8) and a low clustering coefficient (0.08). Furthermore, relating the structure of the food web to its dynamics, an exponential degree distribution (in- and out-links) was found. This suggests that the Potter Cove food web may be vulnerable if the most connected species became locally extinct. For two of the three more connected functional groups, competition overlap graphs imply high trophic interaction between demersal fish and niche specialization according to feeding strategies in amphipods. On the other hand, the prey overlap graph shows also that multiple energy pathways of carbon flux exist across benthic and pelagic habitats in the Potter Cove ecosystem. Although alternative food sources might add robustness to the web, network properties (low linkage density, connectance and omnivory) suggest fragility and potential trophic cascade effects.

Abstract

The paper presents 24 patients treated between 1997 and 2001 in the E.N.T. Clinic of University of Genoa, Italy because of deep neck space infections. The organism responsible for these infections usually includes either aerobic or anaerobic microorganism, whose synergistic activity provokes virulence and contributes to the violent nature of infection. Initial management of deep neck infections consists of intensive parenteral antibiotic therapy. Another fundamental part of treatment is surgical drainage and debridement of the deep-seated infection.

Abstract

El proceso de animalización que sufrieron los moriscos españoles durante los siglos XVI y XVII se evidencia abiertamente como una fase de un más amplio procedimiento de deshumanización, aquel fenomeno de negación y sustracción de las cualidades humanas que la minoría padeció forzosamente por parte de los feroces opositores cristiano-viejos. Dentro de un proyecto de arquitectura demonizadora realizada por los autores de la literatura apologética de la expulsión, el ejercicio de superponer los animales a los seres humanos se interpreta como un método de imponer una estrategia de dominación que pretende fabricar una indispensable imagen vilipendiada del grupo otro. El lenguaje se vuelve, entonces, un despiedado instrumento de opresión psicológica y social que colabora a una gestión torcida de la representación de la alteridad hispanomusulmana. Las ofensas de los tratados de los apologistas se estructuran a través de una amplia gama de epítetos enlazados al mundo animal, cada uno de los cuales tiene forma y sustancia al asociarse a un tipo específico de acusación acuñada contra los miembros de la comunidad minoritaria. A modo de ejemplo, a una hipotética actitud de sexualidad perversa se relaciona cierto tipo de animales sexualmente activos, a la tendencia morisca al criptoislamismo, o sea, la propensión hacia el ocultamiento de su tradición cultural y religiosa, se enlaza la dimensión de los animales nocturnos que habitan en la oscuridad, a una supuesta conducta conspiradora se vinculan los animales venenosos que corrompen el cuerpo de la nación española. Este trabajo, pues, procura asentar una reflexión acerca del uso metáforico de unas imagenes del mundo animal relacionadas a los componentes de la comunidad morisca, una meditación que explore un lenguaje estratégicamente aprovechado por los detractores apologistas y utilizado conscientemente como medio fundamental para llevar a cabo una implacable propaganda discriminatoria.
