Background: Visceral adipose tissue (VAT) has a hazardous influence on systemic inflammation, insulin resistance and an adverse metabolic profile, which increases the risk of developing non-alcoholic fatty liver disease (NAFLD) and chronic complications of diabetes. In our study we aimed to evaluate the association of VAT and the triglyceride glucose (TyG) as a proxy of insulin resistance surrogated with metabolic and liver risk factors among subjects diagnosed with metabolic syndrome (MetS). Methods: A cross-sectional study was performed including 326 participants with MetS (55-75 years) from the PREDIMED-Plus study. Liver-status markers, VAT and TyG were assessed. Participants were stratified by tertiles according to VAT (n = 254) and TyG (n = 326). A receiver operating characteristic curve was used to analyse the efficiency of TyG for VAT. Results: Subjects with greater visceral fat depots showed worse lipid profile, higher homeostatic model assessment for insulin resistance (HOMA-IR), TyG, alanine transaminase (ALT), fibroblast growth factor-21 (FGF-21), fatty liver index (FLI) and hepatic steatosis index (HSI) compared with participants in the first tertile. The multi-adjusted linear-regression analyses indicated that individuals in the third tertile of TyG (>9.1-10.7) had a positive association with HOMA-IR [beta = 3.07 (95% confidence interval (CI) 2.28-3.86; p trend < 0.001)], ALT [beta = 7.43 (95% CI 2.23-12.63; p trend = 0.005)], gamma glutamyl transferase (GGT) [beta = 14.12 (95% CI 3.64-24.61; p trend = 0.008)], FGF-21 [beta = 190.69 (95% CI 93.13-288.25; p trend < 0.001)], FLI [beta = 18.65 (95% CI 14.97-22.23; p trend < 0.001)] and HSI [beta = 3.46 (95% CI, 2.23-4.68; p trend < 0.001)] versus participants from the first tertile. Interestingly, the TyG showed the largest area under the receiver operating curve (AUC) for women (AUC = 0.713; 95% CI 0.62-0.79) compared with men (AUC = 0.570; 95% CI 0.48-0.66). Conclusions: A disrupted VAT enlargement and impairment of TyG are strongly associated with liver status and cardiometabolic risk factors linked with NAFLD in individuals diagnosed with MetS. Introducción: el hígado graso no alcohólico (HGNA) es una enfermedad hepática crónica más prevalente en los países occidentales. Objetivos: evaluar factores de riesgo asociados a HGNA en hombres y mujeres diagnosticados con SM estratificados según terciles del índice de esteatosis hepática (HSI). Métodos: análisis transversal del estudio PREDIMED Plus (incluyendo únicamente información del nodo Navarra-Nutrición). Un total de 278 participantes (141 hombres y 137 mujeres) con SM fueron estratificados según terciles de HSI. El estudio analizó variables clínicas, bioquímicas e información sobre el estilo de vida, como grado de adherencia a la dieta mediterránea y práctica de actividad física. Resultados: el análisis multivariante reveló que las mujeres tienen 4,54 unidades más de HSI (95% IC: 3,41 a 5,68) respecto a los hombres. Ambos sexos mostraron un incremento en los niveles de triglicéridos, TG/colesterol HDL e índice triglicéridos-glucosa (TyG) entre los terciles de HSI. Asimismo, se observó una asociación negativa entre la actividad física y el HSI (hombres: r = -0,19, p = 0,025; mujeres: r = -0,18, p = 0,031). La grasa visceral mostró una asociación positiva con el HSI en ambos sexos (hombres: r = 0,64, p < 0,001; mujeres: r = 0,46, p < 0,001). La adherencia a la dieta mediterránea es menor en los pacientes con HSI más elevado (hombres r = -0,18, p = 0,032; mujeres r = -0,19, p = 0,027). Conclusiones: las mujeres presentan mayor riesgo de daño hepático y se sugieren importantes diferencias de sexo en relación a la EHGNA. La adherencia a la dieta mediterránea y la actividad física se reafirman como factores clave frente a la regulación de enfermedades cardiovasculares entre las que se encuentra la EHGNA. El riesgo de esteatosis hepática en población adulta obesa y con SM disminuye. - Abstract
[Background]: Overall quality of dietary carbohydrate intake rather than total carbohydrate intake may determine the risk of cardiovascular disease (CVD)., [Objective]: We examined 6- and 12-mo changes in carbohydrate quality index (CQI) and concurrent changes in several CVD risk factors in a multicenter, randomized, primary-prevention trial (PREDIMED-Plus) based on an intensive weight-loss lifestyle intervention program., [Methods]: Prospective analysis of 5373 overweight/obese Spanish adults (aged 55–75 y) with metabolic syndrome (MetS). Dietary intake information obtained from a validated 143-item semiquantitative food-frequency questionnaire was used to calculate 6- and 12-mo changes in CQI (categorized in quintiles), based on 4 criteria (total dietary fiber intake, glycemic index, whole grain/total grain ratio, and solid carbohydrate/total carbohydrate ratio). The outcomes were changes in intermediate markers of CVD., [Results]: During the 12-mo follow-up, the majority of participants improved their CQI by increasing their consumption of fruits, vegetables, legumes, fish, and nuts and decreasing their consumption of refined cereals, added sugars, and sugar-sweetened beverages. After 6 mo, body weight, waist circumference (WC), systolic and diastolic blood pressure (BP), fasting blood glucose, glycated hemoglobin (HbA1c), triglyceride levels, triglycerides and glucose (TyG) index, and TyG-WC decreased across successive quintiles of improvement in the CQI. After 12 mo, improvements were additionally observed for HDL cholesterol and for the ratio of total to HDL cholesterol. Favorable improvements (expressed in common units of SD and 95% CI) for quintile 5 compared with quintile 1 of CQI change were observed for most risk factors, including TyG-WC (SD −0.20; 95% CI −0.26, −0.15), HbA1c (SD −0.16; 95% CI −0.23, −0.10), weight (SD −0.12; 95% CI −0.14, −0.09), systolic BP (SD −0.11; 95% CI −0.19, −0.02) and diastolic BP (SD −0.11; 95% CI −0.19, −0.04)., [Conclusions]: Improvements in CQI were strongly associated with concurrent favorable CVD risk factor changes maintained over time in overweight/obese adults with MetS. This trial was registered as ISRCTN 89898870.

Trabajo presentado en la Jornada Científica CIBERESP (Consorcio de Investigación Biomédica en Red de Epidemiología y Salud Pública), celebrada en Madrid (España), el 14 de noviembre de 2019, AIM: Physical activity practiced regularly is known to improve the well-being and to reduce the risk of age-related diseases. We aimed to determine the impact of long-term physical activity on memory maintenance and search for peripheral blood markers related to cognitive function that may be detectable at middle-age METHODS: We performed neuropsychological analyses to veteran amateur rugby players (n = 24, age: 46–68 y) and middleaged sedentary controls (n = 25, age: 47–67 y). We analyzed the expression of selected genes in whole blood mRNA of player and control groups and young sedentary controls (n = 21, age: 17–25 y). Trabajo presentado en el FENS Regional Meeting, celebrado en Belgrado (Sebia), del 10 al 13 de julio de 2019, Aim: Physical exercise performed regularly is known to improve health and to reduce the risk of age-related diseases. We had previously reported improved immediate memory responses in a cohort of veteran rugby players as compared to control subjects with low physical activity. We hypothesized that long-term physically active middle-aged men have developed brain resilience that can be detected with the analysis of peripheral blood markers. Methods: We tested the expression of selected genes of longevity, inflammation, redox homeostasis, and trophic signaling in whole blood mRNA. Analyses were performed on blood samples of veteran rugby players (n=24) and controls (n=25) aged 45-65 year, and also young controls (aged 15-25 years) with low physical activity (n=21). Physical activity and other lifestyles were thoroughly recorded with standardized questionnaires. Results: Middle-aged control subjects showed lower levels of expression of SIRT1, SIRT3 and CAT than the young controls, although rugby players maintained the expression levels of these genes at a young-like level. No differences were detected in the inflammatory genes, although there was a tendency towards a decrease in trophic and transduction factors in middle-aged groups as compared to the young controls. A statistical study of Spearman’s correlations supported a positive effect of sporting activity on memory and executive functions through SIRT1, SIRT3 and antioxidant gene expression levels in the veteran rugby players. Conclusions: Our results suggest the involvement of the SIRT1-SIRT3 axis, and the downstream antioxidant and neuroprotective signaling, in the anti-aging resilience of the brain mediated by physical exercise, Ajut Mario Sàlvia i Ferret 2014, lnstitut d’Estudis Catalans; SAF2016‐77703, MINECO and ERDF; 2017-SGR-106, AGAUR.

- Abstract
[Background]: Inflammation is known to be related to the leading causes of death including cardiovascular disease, several types of cancer, obesity, type 2 diabetes, depression-suicide and other chronic diseases. In the context of whole dietary patterns, the Dietary Inflammatory Index (DII®) was developed to appraise the inflammatory potential of the diet. [Objective]: We prospectively assessed the association between DII scores and all-cause mortality in two large Spanish cohorts and valuated the consistency of findings across these two cohorts and results published based on other cohorts., [Design]: We assessed 18,566 participants in the “Seguimiento Universidad de Navarra” (SUN) cohort followed-up during 188,891 person-years and 6790 participants in the “PREvencion con DIeta MEDiterránea” (PREDIMED) randomized trial representing 30,233 person-years of follow-up. DII scores were calculated in both cohorts from validated FFQs. Higher DII scores corresponded to more proinflammatory diets. A total of 230 and 302 deaths occurred in SUN and PREDIMED, respectively. In a random-effect meta-analysis we included 12 prospective studies (SUN, PREDIMED and 10 additional studies) that assessed the association between DII scores and all-cause mortality., [Results]: After adjusting for a wide array of potential confounders, the comparison between extreme quartiles of the DII showed a positive and significant association with all-cause mortality in both the SUN (hazard ratio [HR] = 1.85; 95% CI: 1.15, 2.98; P-trend = 0.004) and the PREDIMED cohort (HR = 1.42; 95% CI: 1.00, 2.02; P-trend = 0.009). In the meta-analysis of 12 cohorts, the DII was significantly associated with an increase of 23% in all-cause mortality (95% CI: 16%–32%, for the highest vs lowest category of DII)., [Conclusion]: Our results provide strong and consistent support for the hypothesis that a pro-inflammatory diet is associated with increased all-cause mortality. - Abstract
[Importance] High-quality dietary patterns may help prevent chronic disease, but limited data exist from randomized trials about the effects of nutritional and behavioral interventions on dietary changes., [Objective] To assess the effect of a nutritional and physical activity education program on dietary quality., [Design, Setting, and Participants] Preliminary exploratory interim analysis of an ongoing randomized trial. In 23 research centers in Spain, 6874 men and women aged 55 to 75 years with metabolic syndrome and no cardiovascular disease were enrolled in the trial between September 2013 and December 2016, with final data collection in March 2019., [Interventions] Participants were randomized to an intervention group that encouraged an energy-reduced Mediterranean diet, promoted physical activity, and provided behavioral support (n = 3406) or to a control group that encouraged an energy-unrestricted Mediterranean diet (n = 3468). All participants received allotments of extra-virgin olive oil (1 L/mo) and nuts (125 g/mo) for free., [Main Outcomes and Measures] The primary outcome was 12-month change in adherence based on the energy-reduced Mediterranean diet (er-MedDiet) score (range, 0-17; higher scores indicate greater adherence; minimal clinically important difference, 1 point)., [Results] Among 6874 randomized participants (mean [SD] age, 65.0 [4.9] years; 3406 [52%] men), 6583 (96%) completed the 12-month follow-up and were included in the main analysis. The mean (SD) er-MedDiet score was 8.5 (2.6) at baseline and 13.2 (2.7) at 12 months in the intervention group (increase, 4.7 [95% CI, 4.6-4.8]) and 8.6 (2.7) at baseline and 11.1 (2.8) at 12 months in the control group (increase, 2.5 [95% CI, 2.3-2.6]) (between-group difference, 2.2 [95% CI, 2.1-2.4]; P < .001)., [Conclusions and Relevance] In this preliminary analysis of an ongoing trial, an intervention that encouraged an energy-reduced Mediterranean diet and physical activity, compared with advice to follow an energy-unrestricted Mediterranean diet, resulted in a significantly greater increase in diet adherence after 12 months. - Abstract
[Purpose]: To assess the association between the consumption of non-soy legumes and different subtypes of non-soy legumes and serum uric acid (SUA) or hyperuricemia in elderly individuals with overweight or obesity and metabolic syndrome. [Methods]:A cross-sectional analysis was conducted in the framework of the PREDIMED-Plus study. We included 6329 participants with information on non-soy legume consumption and SUA levels. Non-soy legume consumption was estimated using a semi-quantitative food frequency questionnaire. Linear regression models and Cox regression models were used to assess the associations between tertiles of non-soy legume consumption, different subtypes of non-soy legume consumption and SUA levels or hyperuricemia prevalence, respectively., [Results]: Individuals in the highest tertile (T3) of total non-soy legume, lentil and pea consumption, had 0.14 mg/dL, 0.19 mg/dL and 0.12 mg/dL lower SUA levels, respectively, compared to those in the lowest tertile (T1), which was considered the reference one. Chickpea and dry bean consumption showed no association. In multivariable models, participants located in the top tertile of total non-soy legumes [prevalence ratio (PR): 0.89; 95% CI 0.82–0.97; p trend = 0.01, lentils (PR: 0.89; 95% CI 0.82–0.97; p trend = 0.01), dry beans (PR: 0.91; 95% C: 0.84–0.99; p trend = 0.03) and peas (PR: 0.89; 95% CI 0.82–0.97; p trend = 0.01)] presented a lower prevalence of hyperuricemia (vs. the bottom tertile). Chickpea consumption was not associated with hyperuricemia prevalence., [Conclusions]: In this study of elderly subjects with metabolic syndrome, we observed that despite being a purine-rich food, non-soy legumes were inversely associated with SUA levels and hyperuricemia prevalence.

- Abstract
Physical exercise performed regularly is known to improve health and to reduce the risk of age-related diseases. Furthermore, there is some evidence of cognitive improvement in physically active middle-aged and older adults. We hypothesized that long-term physically active middle-aged men may have developed brain resilience that can be detected with the analysis of peripheral blood markers. We aimed to analyze the activation of pathways potentially modulated by physical activity in a cohort of healthy amateur rugby players (n = 24) and control subjects with low physical activity (n = 25) aged 45¿65 years. We had previously reported neuropsychological improvement in immediate memory responses in the player group compared to the controls. Here, we tested the expression of selected genes of longevity, inflammation, redox homeostasis, and trophic signaling in whole blood mRNA. Analyses were also performed on blood samples of young (aged 15¿25 years) control subjects with low physical activity (n = 21). Physical activity and other lifestyle factors were thoroughly recorded with standardized questionnaires. Interestingly, middle-aged control subjects showed lower levels of expression of SIRT1, SIRT3, CAT, and SOD1 than the young controls, although rugby players maintained the expression levels of these genes at a young-like level. Middle-aged players showed lower levels of IL1B than the non-physically active groups. However, there was a tendency towards a decrease in trophic and transduction factors in middle-aged groups as compared to the young controls. A statistical study of Spearman¿s correlations supported a positive effect of sporting activity on memory and executive functions, and on peripheral gene expression of SIRT1, SIRT3 and downstream genes, in the middle-aged rugby players. Our results indicate that the SIRT1-SIRT3 axis, and associated neuroprotective signaling, may contribute to the anti-aging resilience of the brain mediated by physical exercise.

We aimed to determine the impact of long-term physical activity on memory maintenance and search for peripheral blood markers related to cognitive function that may be detectable at middle-age METHODS: We performed neuropsychological analyses to veteran amateur rugby players (n = 24, age: 46–68 y) and middleaged sedentary controls (n = 25, age: 47–67 y). - Abstract
Aim: Physical exercise performed regularly is known to improve health and to reduce the risk of age-related diseases. We had previously reported improved immediate memory responses in a cohort of veteran rugby players as compared to control subjects with low physical activity. We hypothesized that long-term physically active middle-aged men have developed brain resilience that can be detected with the analysis of peripheral blood markers. Methods: We tested the expression of selected genes of longevity, inflammation, redox homeostasis, and trophic signaling in whole blood mRNA. Analyses were performed on blood samples of veteran rugby players (n=24) and controls (n=25) aged 45-65 year, and also young controls (aged 15-25 years) with low physical activity (n=21). Physical activity and other lifestyles were thoroughly recorded with standardized questionnaires. Results: Middle-aged control subjects showed lower levels of expression of SIRT1, SIRT3 and CAT than the young controls, although rugby players maintained the expression levels of these genes at a young-like level. No differences were detected in the inflammatory genes, although there was a tendency towards a decrease in trophic and transduction factors in middle-aged groups as compared to the young controls. A statistical study of Spearman’s correlations supported a positive effect of sporting activity on memory and executive functions through SIRT1, SIRT3 and antioxidant gene expression levels in the veteran rugby players. Conclusions: Our results suggest the involvement of the SIRT1-SIRT3 axis, and the downstream antioxidant and neuroprotective signaling, in the anti-aging resilience of the brain mediated by physical exercise

- Abstract
Background & aims: Legumes, a low-energy, nutrient-dense and low glycemic index food, have shown beneficial effects on glycemic control and adiposity. As such, legumes are widely recommended in diabetic diets, even though there is little evidence that their consumption protects against type 2 diabetes. Therefore the aim of the present study was to examine the associations between consumption of total legumes and specific subtypes, and type 2 diabetes risk. We also investigated the effect of theoretically substituting legumes for other protein- or carbohydrate-rich foods. Methods: Prospective assessment of 3349 participants in the PREvención con DIeta MEDiterránea (PREDIMED) study without type 2 diabetes at baseline. Dietary information was assessed at baseline and yearly during follow-up. We used Cox regression models to estimate hazard ratios (HRs) and 95% confidence intervals (95% CIs) for type-2 diabetes incidence according to quartiles of cumulative average consumption of total legumes, lentils, chickpeas, dry beans and fresh peas. Results: During a median follow-up of 4.3 years, 266 new cases of type 2 diabetes occurred. Individuals in the highest quartile of total legume and lentil consumption had a lower risk of diabetes than those in the lowest quartile (HR: 0.65; 95% CI: 0.43, 0.96; P-trend = 0.04; and HR: 0.67; 95% CI: 0.46–0.98; P-trend = 0.05, respectively). A borderline significant association was also observed for chickpeas consumption (HR 0.68; 95% CI: 0.46, 1.00; P-trend = 0.06). Substitutions of half a serving/day of legumes for similar servings of eggs, bread, rice or baked potato was associated with lower risk of diabetes incidence. Conclusions: A frequent consumption of legumes, particularly lentils, in the context of a Mediterranean diet, may provide benefits on type 2 diabetes prevention in older adults at high cardiovascular risk.
