Vulva, vajina ve perine rekonstrüksiyonu plastik cerrahlar, jinekologlar, çocuk cerrahları, ürologlar ve genel cerrahlar için yönetimi en güç klinik durumlardan biridir. Sıklıkla vajinal agenezi nedeniyle olsa da bu alandaki onkolojik problemlerden sonra da rekonstrüksiyon gereksinimi doğar. Bu bölgenin rekonstrüksiyonunda pek çok yöntem tanımlanmıştır. Nörovasküler pudendal uyluk flebi ile vajina rekonstrüksiyonu yapılabilmektedir. Biz çalışmamızda primer amenore nedeniyle başvuran ve doğumsal vajina agenezisi tanısı alan toplam yedi bayan olguda uyguladığımız nörovaskuler pudendal uyluk flebi sonuçlarımızı sunduk. Hiç bir olgumuzda erken yada geç dönemde bir komplikasyon gelişmedi. 3 olguda yeni oluşturulan vajina içinde kıl gelişimi gözlense de bu durum olgularda hiç bir şikayete yol açmadı.Tümolguların post-operatif 6. ayındaki kontrollerde duyusal ve seksüel fonksiyonu tam olan yeni vajina oluşturulduğu görüldü. Bu flep; uygulamasındaki kolaylık, donör alanda kabul edilebilir ve gizlenebilir iz yaratması, postoperatif dönemde stent kullanma gerekliliği olmaması, dayanıklı, güvenilir bir flep olması ve hem kozmetik hem de fonksiyonel olarak istenilen sonuçların elde edilmesinden dolayı vajina rekonstrüksiyonundaki en ideal yöntemlerden biri olduğu sonucuna vardık Reconstruction of the vulva, vagina and perineum is still a challenge for the plastic and reconstructive surgeons, gynecologists, pediatric surgeons, urologists and general surgeons. The major indications for the reconstruction of the vagina are vaginal agenesis and post-oncological surgery. Many techniques have been described for the reconstruction of this anatomical region. The neurovascular pudendal thigh flap is one of these. In this report we presented the results of the neurovascular pudendal thigh flap in seven female patients with primary amenorrhea secondary to congenital vaginal agenesis. There were no early or late complications in our series. Only hair growth was observed in three patients but this caused no complaints. Satisfactory functional, sensory and cosmetical results were achieved in all patients within the post-operative six months. We concluded that neurovascular pudendal thigh flap is one of the most ideal techniques in reconstruction of the vagina due to its relatively simple technique, acceptable scar formation in donor area, lack of need for a stent in post-operative period, durable and reliable characteristics of the flap and successful functional and cosmetical results.

Servikal özafagus defektlerinin onarımı, rekonstrüktif cerrahinin en zor konularından biridir. Bu defektlerhipofarinks tümörleri ve primer servikal özafagus tümörleri sonucu gelişir. İdeal rekonstrüksiyon yönteminin,onkolojik cerrahi ile aynı seansta yapılabilir, erken fonksiyon sağlayabilir ve düşük morbidite oranına sahip birteknik olması gereklidir. Bu çalışmada üç farklı teknikle servikal özafagus onarımları, literatür bilgileriylebirlikte ayrıntılı olarak sunulmuştur. Olguların biri pektoralis major kas deri flebi, diğeri deltopektoral fleple veen son olgu da servikohumeral flep ve deri grefti ile rekonstrükte edilmiştir. Olguların tümünde oral alımı erkendönemde sağlanmıştır.Yutma fonksiyonun en erken zamanda sağlanmasının, yöntem seçiminde temel belirleyiciolduğunu düşünmekteyiz.Sonuç olarak bu çalışmada her ne kadar üç farklı yöntem, farklı cerrahların seçimi olarak sunulmuş ise de, buyöntemler kendi avantaj ve dezavantajlarına sahiptir. Deltopektoral flepde parsiyel nekroz gelişmesi,servikohumeral flebin ise geciktirme işlemine ihtiyaç duyması nedeniyle, sunulan yöntemler içinde pektoralismajor kas deri flebi ile onarım daha başarılı bunmuştur, Reconstruction of defects of the cervical esophagus is one of the most difficult tasks in reconstructive surgery. These defects are generally seen after tumors of hypopharynx and cervical esophagus. The method of reconstruction, which will be selected, therefore, must be reliable, a one-stage procedure with minimal morbidity and one that restores function in the shortest time possible. In this article, we present our experience with three different techniques: the pectoralis major musculocutaneous flap, the deltopectoral flap and a combination of cervicohumeral flap and skin grafting. Oral feeding was established early in all cases. We are of the opinion that the time for swallowing ability after operation is the one of the most important factors in determining the technique chosen for reconstruction of defects of the cervical esophagus. In conclusion, although three different techniques were presented in this study according to the surgeons choice, each technique has its own advantages and disadvantages. Since the partial necrosis in deltopectoral flap and necessity of the delay process in cervicohumeral flap were complications of the techniques, pectoralis major myo-cutaneous flap may be accepted as a more suitable reconstruction technique.

Tam kalınlıktaki olar defektleri estetik ve fonksiyonel olarak tek seansta tamiri cerrahlar için her zaman zorluk yar atmıştır. Günümüze kadar pek çok yöntem literatürde tanımlanmakla birlikte ideal yöntem arayışı devam etmektedir. Alar bölgedeki tam kalınlıktaki defektler doğumsal anomaliler, tümör cerrahisi, travma ve yanık yaralanması sonucu olabilir. Burun anatomisinin kompleks olmasında dolayı bu bölgenin rekonstrüksiyonu güçtür. Burnun konturları değişken, konkav ve konveks yüzeye sahip olup kendine özgü deri yapısı ve rengi vardır. Alar bölge, burnun alt ünitelerinden biri olup hem deri, hem kıkrdak hemde mukozal yüzey içerir. Bu çalışmada tam kalınlıkta alar bölgede defekti olan 16 olguda tek seanslı rekonstrüksiyon deneyimlerimiz sunulmuştur. Olgulara 3 tür operasyon uygulanmıştır: Kulakdan alınan deri-kıkırdak kompozit grefti, nazolabial turnover deri flebi ve tam kalınlıkta deri grefti uygulaması ve son olarak da üç loblu deri flebi. Bu yöntemler içinde en çok komplikasyon kompozit greft grubunda olmuştur ki bunlarda greft büzüşmesi ve nekrozdur. Çalışmamız sonucunda üç-loblu deri flebi ile büyük defektler de dahi başarılı onarım yapılabileceğine inanmaktayız.

Bu teknik, özellikle meme düzeltme ve çok büyük olmayan memelerin küçültülmesindepek çok cerrah tarafından başarı ile uygulanmaktadır. Ancak aşırı büyük memelerde özellikle dikey izin iyileşmeproblemleri ile ilgili olarak kullanımı tartışmalıdır. Bu tekniğe en iyi aday hasta elastik derisi olan, orta derecedehipertrofik (800 gr.dan az) ve yağ ve bez karışımından oluşan memelerdir.Yöntem: Bu çalışmada, 1000 gr.dan fazla miktarda doku rezeksiyonu yaptığımız toplam 11 hastadaki aşırı büyükmemelerde uyguladığımız dikey izli tekniğin erken ve geç dönem sonuçları sunulmuştur.Bulgular: Hastaların yaşları 20 ile 46 arasında değişmekte olup ortancası 32 idi. Eksize edilen doku miktarı 1000ile 1780 gr. arasında olup ortalaması 1322 gr. idi. Tüm hastalar aynı cerrahi ekip tarafından ameliyat edildiler.İzlem süresince olgularda erken dönemde iki komplikasyon gözlendi: parsiyel meme başı - areola nekrozu vehematom. Aşırı sigara içimi öyküsü olan nekrozlu hasta hipertrofik skar ile iyileşti. Uzun dönem sonuçlarındahastalarda herhangi bir komplikasyon gözlenmedi.Sonuç: Sonuç olarak dikey izli meme küçültme yönteminin, 1000 gr. dan fazla rezeksiyon yapılacak aşırı büyükmemelerde güvenli bir şekilde uygulanabileceği görüşündeyiz, Aim: The vertical scar mammaplasty is one of the most frequently performed operations among reduction mammoplasties. Method: In this study, we present our late term results with vertical scar reduction mammaplasty in heavy sized breast with resection of 1000g or more per breast in eleven patients. Eleven patients with heavy sized 22 breasts in which more than 1000g of tissue was resected are evaluated. Results: Median age of the patients was 32 years (20-46 years) and the average amount of resected specimen was 1322g (1000-1780g) per breast. All patients were operated by the same team and senior surgeon. During the follow up period 2 major complications in the early period were observed: partial nipple-areola necrosis and hematoma. Secondary healing of a partial nipple areola necrosis in a heavy smoker patient resulted in periareolar hypertrophic scar. There were no late term complications. Conclusion: We think that the vertical mammaplasty technique can safely be applied to heavy breasts and large reductions over 1000 gr of tissue.

Sıklıkla erkeklerde görülen ve patogeneziembriyolojik gelişimdeki defektlerle açıklanmaya çalışılan bu deformite ile ilgili olarak literatürde yirmiden azsayıda yayınlanmış çalışma vardır. Bizim çalışmamızda, kadın erkek sayısı eşit olup olguların hepsi estetikyakınmalarından dolayı doktora başvurmuşlardır. Olguların hepsine lokal anestezi altında eliptik eksizyonyapılmıştır. Alınan cerrahi örneklerin patolojik incelemeleri normal dudak mukozası olarak rapor edilmiştir.Olguların hiçbirinde post-operatif erken yada geç komplikasyon görülmeyip istenilen kozmetik sonuç eldeedilmiştir. Bir olguda parsiyel çift dudak deformitesi saptanmış olup, bilgilerimize göre literatürde parsiyel çiftdudak deformitesine ait bir yayın yoktur. Sonuç olarak bu çalışmada, doğumsal çift dudağa sahip toplam 8 olgusunulmuş ve bu deformitenin etyolojisi, görülme sıklığı ve tedavi yöntemleri literatürdeki bulgular eşliğindetartışılmıştır, Although congenital double lip deformity is not an uncommon condition, this deformity was presented rarely in the literature. Congenital double lip deformity may be present either as an isolated anomaly or a component of a syndrome. However less than twenty reports were presented in the literature, males were affected predominantly and pathogenesis of this deformity was explained by the embryological defects. In our study male to female ratio was equal and all patients were operated since the aesthetical reason. Elliptical excision was performed under the local anesthesia in all patients. All specimens were evaluated pathological and reported as “normal lip mucosa”. There were no early or late post-operative complication and aesthetical results were achieved in all patients. One case of our patients had partial congenital lip deformity, best of our knowledge; there was no data about the partial congenital lip deformity. Finally, in this study we present eight cases with congenital double lip and discuss the etiology, incidence and treatment modalities according to the data presented in the literature.

Rinoplasti ameliyatında istenilen sonucun elde edilmesi ve komplikasyonların en aza indirilmesi, burnun anatomisinin en iyi şekilde bilinmesine ve hastadaki mevcut probleme uygun cerrahi tekniğin seçilmesine bağlıdır. Rinoplasti için başvuran olgunun ameliyat öncesi değerlendirilmesi fizik bakı ve hastanın fotoğrafları ile olmaktadır. Bifid burun yada bulböz burun ucu gibi anomalilerin değerlendirilmesi için herhangi bir objektif radiodiagnostik test tanımlanmamıştır. Bu çalışmada rinoplasti için başvuran 34 olgunun burun uçları ameliyat öncesi ultrasonografi (USG) ile değerlendirilmiş ve interdomal mesafe ve interdomal yağ yastıkçığı boyutları ölçülmüştür. Ameliyat öncesi altı olguda bulböz burun tanısı konmuş olup, bulböz burun olan olgularda interdomal mesafe ve interdomal yağ yastıkçığı boyutları normal olgulara göre daha fazla olduğu saptandı. Tüm olgular açık rinoplasti tekniği ile ameliyat edilmiş olup, interdomal bölge görüntülenmiş, interdomal yağ yastıkçığı tüm olgularda alınmış ve patolojik olarak incelenmiştir. Ameliyat sonrası IYY'nın alınmış olması herhangi bir komplikasyon yaratmadığı görüldü. Burun ucunun USG kullanılarak değerlendirilmesi basit, ucuz ve kolay uygulanabilir bir yöntemdir. İnterdomal yağ yastıkçığı yeni tanımlanmış bir anatomik yapı olup USG ile görüntülenebilmektedir ve bulböz burunun tanımlanmasında objektif bir kriter olarak kullanılabilir. IYY'nın eksizyonu ve interdomal mesafenin düzeltimi açık rinoplasti tekniğiyle daha kolay olacağından, bu bölgenin USG ile ameliyat öncesi değerlendirilmesi cerrahi tekniğin seçimi açısından yol gösterici olabilir.

Background: Hypospadias is one of the most challenging anomalies in the point of reconstruction. More than 600 techniques have been described in the literature until now. These techniques can be considered as either single or multiple staged. Every technique has some advantages and disadvantages. Although one stage techniques are mostly performed in primary hypospadias, there is no consensus in management of secondary hypospadias for “salvage” operations. More complication rates have beenreported in secondary cases. Material and methods: Between 1995 to 2000, we performed two stage hypospadias repair described by Bracka, in twelve patients who had history of two or more hypospadias repairs. Initially the patients were operated to create a bed for neo-urethra by a full thickness skin graft from retro-auricular region or buccal mucosa. Six months later, neo-urethra was created using this graft and the meatus was relocated to the anatomical region. To prevent fistula formation, waterproofing flap was performed to cover of neo-urethra. Results: Partial graft loss seen as secondary to infection was observed in one patient after the first operation and fistulas were seen in two patients after the second operation. Conclusions: These complication rates were lower than the reported salvage hypospadias repair techniques found in the literature. We concluded that two-stage hypospadias repair of Bracka is a good alternative in salvage hypospadias repair.
