Abstract

This is an introduction to Hall algebras from the perspective of $2$-Segal spaces or decomposition spaces, as introduced by Dyckerhoff and Kapranov and G\'{a}lvez-Carrillo, Kock and Tonks, respectively. We explain how linearizations of the $2$-Segal space arising as the Waldhausen $\mathcal{S}_{\bullet}$-construction of a proto-exact category recover various previously known Hall algebras. We use the $2$-Segal perspective to study functoriality of the Hall algebra construction and explain how relative variants of $2$-Segal spaces lead naturally to representations of Hall algebras., Comment: 32 pages

2. Pressure-Induced Separation of a Laminar Boundary Layer over a Partially-Slip Wall

Abstract

The characteristics of pressure-induced laminar separation bubbles (LSBs) over a partially-slip wall, compared with that over a canonical no-slip wall, are studied using direct numerical simulation. Three cases, two utilizing linear Robin-type slip boundary conditions of differing slip length ($\Lambda$), and one non-slip are compared. For the partial-slip cases, a streamwise distribution of slip profile is employed ensuring smooth transition between no-slip and partial-slip (transition from no-slip to a constant slip length takes 5$\delta$, where $\delta$ is the inflow boundary layer thickness). The constant target slip length is maintained for $20\delta$ upstream and during the onset of flow separation. The separation is induced by a wall-normal velocity profile applied at the top boundary. All cases are performed at $Re_\delta = U_{\infty}\delta/\nu = 455$. Initial results indicate that as slip length increases, separation and reattachment are delayed. Most notably, the formation and shedding of roller vortices is mitigated as slip length increases, resulting in a less turbulent wake, despite that self-similarity of the plane shear layer is maintained., Comment: 7 pages, 9 figures

3. Separation of a Laminar Boundary Layer Subjected to Pressure Gradients with Spanwise Variations

Abstract

The characteristics of three-dimensional laminar separation bubbles (LSBs), compared with their quasi-two-dimensional counterparts, are studied using direct numerical simulation. A three-dimensional suction-blowing distribution is applied on the top boundary to induce flow separation. Three levels of suction strength (i.e., adverse pressure gradient (APG) strength) are tested. For each suction level, the spanwise extent of the suction is varied using a Gaussian distribution centered about the domain centerline. Two non-uniform suction distributions are compared with uniformly-applied suction across the full span (implying a two-dimensional LSB), totaling nine cases. All cases are performed at $Re_\delta = U_{\infty}\delta/\nu = 1000$ based on the inflow boundary layer thickness ($\delta$) and free-stream velocity ($U_{\infty}$). Initial results indicate stark contrast between three-dimensional and two-dimensional LSBs. Most notably, the role of suction width is determining the topology of the LSB and its reattachment mechanism. Meanwhile, APG is responsible for determining the size of the LSB and level of turbulence at reattachment. Abstract

Nasogastric tubes (NGTs) are feeding tubes that are inserted through the nose into the stomach to deliver nutrition or medication. If not placed correctly, they can cause serious harm, even death to patients. Recent AI developments demonstrate the feasibility of robustly detecting NGT placement from Chest X-ray images to reduce risks of sub-optimally or critically placed NGTs being missed or delayed in their detection, but gaps remain in clinical practice integration. In this study, we present a human-centered approach to the problem and describe insights derived following contextual inquiry and in-depth interviews with 15 clinical stakeholders. The interviews helped understand challenges in existing workflows, and how best to align technical capabilities with user needs and expectations. We discovered the trade-offs and complexities that need consideration when choosing suitable workflow stages, target users, and design configurations for different AI proposals. We explored how to balance AI benefits and risks for healthcare staff and patients within broader organizational and medical-legal constraints. We also identified data issues related to edge cases and data biases that affect model training and evaluation; how data documentation practices influence data preparation and labelling; and how to measure relevant AI outcomes reliably in future evaluations. We discuss how our work informs design and development of AI applications that are clinically useful, ethical, and acceptable in real-world healthcare services.

Abstract

We construct differential graded enhancements of the zigzag algebras which were used by Khovanov, Seidel and Thomas to produce categorical braid group actions. These enhancements are related to $p$-differential graded structures by a version of Koszul duality. We prove that the minimal model $A_\infty$-structure on the zigzag algebras is {\em not} formal. Abstract

Nirmatrelvir-ritonivir (Paxlovid) recently received emergency use authorization for the treatment of coronavirus disease 2019 (COVID-19). Literature has linked numerous cutaneous adverse effects to nirmatrelvir and ritonavir, the copackaged tablets within Paxlovid. A review and comparison of these adverse effects to the common cutaneous manifestations of COVID-19 is provided. Numerous drug-to-drug interactions exist between nirmatrelvir-ritonivir and commonly-used medications within dermatology.

Introduction: Cardiac memory refers to T-wave inversions that result when normal ventricular activation resumes following a period of abnormal ventricular activation.Case Report: We present a case of a 29-year-old man with a pacemaker who presented with new, deep symmetric T-wave inversions caused by cardiac memory.Discussion: Abnormal ventricular activation is most commonly induced by ventricular pacing but can also occur in the setting of transient left bundle branch blocks, ventricular tachycardia, and intermittent ventricular pre-excitation.Conclusion: Recognition of this phenomenon may help to reduce unnecessary admissions, cardiac testing, and cardiac catheterizations.

35. Twiddler’s Syndrome

Abstract

Twiddler’s syndrome refers to a rare condition in which a pacemaker or automatic implantable cardioverter-defibrillator (AICD) malfunctions due to coiling of the device in the skin pocket and resultant lead displacement. This image is the chest radiograph (CXR) of a 54-year-old male who presented to the emergency department with chest pain five months after his AICD was placed. The CXR shows AICD leads coiled around the device and the absence of leads in the ventricle consistent with Twiddler’s syndrome. Patients with twiddler’s syndrome should be admitted for operative intervention.

Abstract

Firearm injuries are the leading cause of death among children aged 0 to 17 years in the United States. This study examines the factors associated with recurrent firearm injury among children who presented with a nonfatal firearm injury in the St. Louis, Missouri, region. Visual Abstract. This study examines the factors associated with recurrent firearm injury among children who presented with a nonfatal firearm injury in the St. Louis, Missouri, region. Background: Firearm injuries are the leading cause of death among children aged 0 to 17 years in the United States. Objective: To examine the factors associated with recurrent firearm injury among children who presented with acute (index) nonfatal firearm injury in the St. Louis region. Design: Multicenter, observational, cohort study. Setting: 2 adult and 2 pediatric level I trauma hospitals in St. Louis, Missouri. Participants: Pediatric patients aged 0 to 17 years presenting with an index firearm injury between 2010 and 2019. Measurements: From the St. Louis Region-Wide Hospital-Based Violence Intervention Program Data Repository, we collected data on firearm-injured patient demographics, hospital and diagnostic information, health insurance status, and mortality. The Social Vulnerability Index was used to characterize the social vulnerability of the census tracts of patients' residences. Analysis included descriptive statistics and time-to-event analyses estimating the cumulative incidence of experiencing a recurrent firearm injury. Results: During the 10-year study period, 1340 children presented with an index firearm injury. Most patients were Black (87%), non-Hispanic (99%), male (84%), and between the ages of 15 and 17 years (67%). The estimated risk for firearm reinjury was 6% at 1 year and 14% at 5 years after initial injury. Male children and those seen at an adult hospital were at increased risk for reinjury. Limitation: Our data set does not account for injuries occurring outside of the study period and for reinjuries presenting to nonstudy hospitals. Conclusion: Children who experience an initial firearm injury are at high risk for experiencing a recurrent firearm injury. Interventions are needed to reduce reinjury and address inequities in the demographic and clinical profiles within this cohort of children. Primary Funding Source: National Institutes of Health. Objectives To improve firearm injury encounter classification (new vs follow-up) using machine learning (ML) and compare our ML model to other common approaches. Materials and Methods This retrospective study used data from the St Louis region-wide hospital-based violence intervention program data repository (2010-2020). We randomly selected 500 patients with a firearm injury diagnosis for inclusion, with 808 total firearm injury encounters split (70/30) for training and testing. We trained a least absolute shrinkage and selection operator (LASSO) regression model with the following predictors: admission type, time between firearm injury visits, number of prior firearm injury emergency department (ED) visits, encounter type (ED or other), and diagnostic codes. Our gold standard for new firearm injury encounter classification was manual chart review. We then used our test data to compare the performance of our ML model to other commonly used approaches (proxy measures of ED visits and time between firearm injury encounters, and diagnostic code encounter type designation [initial vs subsequent or sequela]). Performance metrics included area under the curve (AUC), sensitivity, and specificity with 95% confidence intervals (CIs). Results The ML model had excellent discrimination (0.92, 0.88-0.96) with high sensitivity (0.95, 0.90-0.98) and specificity (0.89, 0.81-0.95). AUC was significantly higher than time-based outcomes, sensitivity was slightly (but not significantly) lower than other approaches, and specificity was higher than all other methods. Discussion ML successfully delineated new firearm injury encounters, outperforming other approaches in ruling out encounters for follow-up. Conclusion ML can be used to identify new firearm injury encounters and may be particularly useful in studies assessing re-injuries. [ABSTRACT FROM AUTHOR]

Abstract

Annually, thousands of U.S. students fail high school introductory biology. The language demands of biology are large, and science teachers are often unprepared to support students' language needs. Here, we describe a 4-week summer high school introductory biology course executed in a large West Coast city. Our aim was to help 33 students recover their biology credit. A centerpiece of the 4-week course was the embedding of metacognitive language support tools in class lectures and assessments. Of 29 regular attendees, 28 passed with a C or better. Student science learning was reliably associated with use of the language support tools.

50. Disability Identification and Self-Efficacy among College Students on the Autism Spectrum

Abstract

The number of youth on the autism spectrum approaching young adulthood and attending college is growing. Very little is known about the subjective experience of these college students. Disability identification and self-efficacy are two subjective factors that are critical for the developmental and logistical tasks associated with emerging adulthood. This study used data from the National Longitudinal Transition Study-2 (NLTS2) to examine the prevalence and correlates of disability identification and self-efficacy among college students on the autism spectrum. Results indicate nearly one-third of these students do not report seeing themselves as disabled or having a special need. Regression models of the correlates of disability identification and self-efficacy indicate that the odds of disability identification were significantly lower among students who were Black and those with higher functional skills. Some colleges are now offering support programs targeted at students on the autism spectrum. If students are required to self-identify as being on the autism spectrum before qualifying for services then this may unintentionally exclude a large proportion of those in need. Colleges might consider options for reducing stigma by creating services to support social and academic success that are marketed to all students without required self-disclosure with respect to disability status.
