7. Important Events in the Development of Academic Advising in the United States

Abstract

The development of academic advising parallels the history of higher education and reflects decades of student personnel work. Changes in funding, curricula, students, and faculty roles have all affected the means by which students have been advised. The evolution of advising eventually led to the formation of NACADA in 1979. Since 2001, when I last documented the history of academic advising in the "Mentor", I have expanded the benchmark information and references. I also added key advising events in the new millennium. Abstract

Objective: The HOME BP (Home and Online Management and Evaluation of Blood Pressure) trial aimed to test a digital intervention for hypertension management in primary care by combining self-monitoring of blood pressure with guided self-management. Design: Unmasked randomised controlled trial with automated ascertainment of primary endpoint. Setting: 76 general practices in the United Kingdom. Participants: 622 people with treated but poorly controlled hypertension (>140/90 mm Hg) and access to the internet. Interventions: Participants were randomised by using a minimisation algorithm to self-monitoring of blood pressure with a digital intervention (305 participants) or usual care (routine hypertension care, with appointments and drug changes made at the discretion of the general practitioner; 317 participants). The digital intervention provided feedback of blood pressure results to patients and professionals with optional lifestyle advice and motivational support. Target blood pressure for hypertension, diabetes, and people aged 80 or older followed UK national guidelines. Main outcome measures: The primary outcome was the difference in systolic blood pressure (mean of second and third readings) after one year, adjusted for baseline blood pressure, blood pressure target, age, and practice, with multiple imputation for missing values. Results: After one year, data were available from 552 participants (88.6%) with imputation for the remaining 70 participants (11.4%). Mean blood pressure dropped from 151.7/86.4 to 138.4/80.2 mm Hg in the intervention group and from 151.6/85.3 to 141.8/79.8 mm Hg in the usual care group, giving a mean difference in systolic blood pressure of −3.4 mm Hg (95% confidence interval −6.1 to −0.8 mm Hg) and a mean difference in diastolic blood pressure of −0.5 mm Hg (−1.9 to 0.9 mm Hg). Results were comparable in the complete case analysis and adverse effects were similar between groups. Within trial costs showed an incremental cost effectiveness ratio of £11 ($15, €12; 95% confidence interval £6 to £29) per mm Hg reduction. Conclusions: The HOME BP digital intervention for the management of hypertension by using self-monitored blood pressure led to better control of systolic blood pressure after one year than usual care, with low incremental costs. Implementation in primary care will require integration into clinical workflows and consideration of people who are digitally excluded. Trial registration: ISRCTN13790648.

17. Human papillomavirus screening in pediatric victims of sexual abuse

Abstract

Latent infection of human papillomavirus (HPV) in children may be a poor screening test for sexual abuse. This virus has many forms, resulting in genital warts or non-symptomatic disease. Of 40 children with proven or suspected sexual abuse and no physical sign of genital warts, only two were positive for HPV 16. More than half had physical signs of abuse and 13% were positive for Chlamydia infection, another sexually transmitted disease. These low rates of HPV detection in this high-risk population suggest that this is not a justified screening test for sexual abuse., Objective. To evaluate for the presence of subclinical human papillomavirus (HPV) in cases of suspected sexual abuse in children. Design. Prospective data collection via interviews, physical examination, colposcopic examination, and tissue sampling by a surface swab technique. Setting. A total of 40 pediatric patients ranging in age from 1 to 16 years who were referred to the Special Assessment and Management Clinic at Cardinal Glennon Children's Hospital, St Louis, MO, for probable or confirmed sexual abuse. Interventions. In addition to colposcopic examination for physical signs of abuse, the patients were screened for evidence of sexually transmitted diseases, including syphilis, gonorrhea, and Chlamydia. At that time, surveillance sampling of the throat, vaginal introitus, and/or rectum by a simple, rapid surface swab technique was performed to detect the presence of HPV. Measurements. Template DNA was extracted from cotton swabs and analyzed using polymerase chain reaction analysis. Results. Human [Beta]-globin sequences were detected in 58 (83%) of 70 specimens obtained from 40 patients, indicating successful processing had occurred. Using a consensus L1 primer-probe set capable of detecting multiple HPV genotypes, 2 (3%) of 58 samples from 2 (5%) of 40 patients were positive for HPV 16. None of the other 56 specimens yielded evidence of HPV. Appropriate positive and negative controls were included in each assay. Conclusions. Our results suggest that subclinical HPV infection is possible, but not commonly associated with sexual abuse in children from St Louis, MO. In this group of children without condyloma, HPV 16 was the only type identified. Abstract

While the officially reported incidence of juvenile offenses in the People's Republic of China is significantly lower than that in much of the remainder of the world community, youthful offenders account for about 75% of all criminal cases. This article provides some preliminary insights into juvenile justice in China based on the data available. The study examines survey data from the Chinese Ministry of Public Security and establishes that juveniles in China have a greater fear of crime than their elders and are less willing to cooperate with and less trusting of the justice system. Next, the analysis examines statistical data, both national and provincial, and establishes that youthful offenders account for a disproportionate amount of total crime committed. The work briefly examines crime causation theories forwarded by Chinese scholars and concludes by focusing on gongdu (work-study) schools, examining their philosophy, structure, and their apparent success in reforming juvenile offenders.

44. Long-term consequences of foster care for adult well-being

Abstract

The object of the present study was to examine the adult functioning of individuals who were in foster care placements during childhood. The data for the analyses were a part of the National Survey of Families and Households. The study compared the adult well-being of 107 former foster children and 12,910 non-foster placed adults. It was predicted that former foster care placement would be associated with poorer adult functioning. The results of the study indicate that former foster children did report lower scores on life happiness, higher depression scores, lower levels of esteem, lower scores on marital happiness, less intimate paternal and maternal relationships, and higher incidence of social isolation than adults who were never in foster care placements. These findings persisted after controlling for background variables, socioeconomic and marital status. The present study also attempted to delineate the aspects of foster care placement that adversely affect later life functioning. It was predicted that older age at first placement, multiple disruptions from the parental home, more frequent contact with the biological parents, failure in family functioning, and longer length of time in foster care placements would be associated with lower levels of adult well-being. The results of the analyses which examine the relationship between foster care factors and adult well-being suggest that foster care variables are not able to significantly predict the variance in adult well-being.
