6. The Development of Children's Autobiographical and Deliberate Memory through Mother-Child Reminiscing

Abstract

Children's use of appropriate techniques for remembering and the effectiveness of deliberate strategies improve throughout elementary school. However, relatively little is known about the contextual factors that may play a role in the development of these skills as children enter formal school. Building upon findings from the mother-child reminiscing literature, the current study was designed to examine concurrent and longitudinal associations between maternal elaborative reminiscing style, children's autobiographical memory, and children's deliberate memory skills. Fifty-one children entering kindergarten, drawn from three schools in the Southeastern region of the United States, were assessed with a battery that included tasks for measuring autobiographical memory and deliberate memory. In a parent-child reminiscing task, parent-child dyads discussed two jointly-experienced events, and parents were categorized as higher or lower in their elaborative reminiscing style. The results reveal an association between parents' reminiscing style and their children's performance on the Free Recall with Organizational Training Task, in which both spontaneous and trained strategy use and recall are measured. Although elaborative reminiscing style was not associated with children's spontaneous strategy use or recall performance at school entry, children with higher elaborative mothers displayed higher levels of strategy use and recall scores after training than did children with lower elaborative mothers. These findings highlight linkages between parents' elaborative style and children's uptake and successful use of strategic organizational training, underscoring the role that parent-child reminiscing conversations play in the socialization of cognition.

17. Characterizing Highly Frequent Users of a Large Canadian Urban Emergency Department

Abstract

Introduction: Highly frequent users (HFU) of the emergency department (ED) are a poorly defined population. This study describes patient and visit characteristics for Canadian ED HFU and patient subgroups with mental illness, substance misuse, or ≥ 30 yearly ED visits. Methods: We reviewed health records from a random selection of adult patients whose visit frequency comprised the 99th percentile of yearly ED visits to The Ottawa Hospital. We excluded scheduled repeat ED assessments. We collected the following: 1) patient characteristics – age, sex, and comorbidities; and 2) ED visit characteristics – diagnosis category, length of stay, presentation time, consultation services, and final disposition. Two reviewers collected data, and we performed an inter-rater review to measure agreement.  Results: We analyzed 3,164 ED visits for 261 patients in all subgroups overall. Within the HFU random selection, mean age was 53.4 ± 1.3, and 55.6% were female. Most patients had a fixed address (88.9%), and family physician (87.2%). Top ED diagnoses included musculoskeletal pain (9.6%), alcohol intoxication (8.5%), and abdominal pain (8.4%). Allied health (social work, geriatric emergency medicine, or community care access centre) was consulted for 5.9% of visits. In 52.7% of these cases, allied health services were not available at the time of presentation.  Conclusion: HFU are a complex population who represent a marked proportion of annual ED visits. Our data indicate that there are opportunities to improve the current approaches to care. Future work examining ED-based screening and multi-disciplinary approaches for HFU may help reduce frequent ED presentations, and better serve this vulnerable population.

Abstract

The diagnosis of pheochromocytoma or paraganglioma (PGL) during pregnancy is extremely rare, with 2 large case series suggesting that the prevalence is between 0.0002% and 0.007%. Here, we present a case of a 38-year-old woman who presented during pregnancy with clinical features suggestive of preeclampsia and was found to have a norepinephrine-secreting inferior laryngeal nerve PGL, which was diagnosed after pregnancy. She underwent uncomplicated surgical resection and genetic testing revealed a succinate dehydrogenase subunit B (SDHB) pathogenic variant. In conclusion, PGLs diagnosed during pregnancy and hypersecreting head and neck PGLs are both rare clinical entities. Hyperfunctioning PGLs may mimic pregnancy-induced hypertension or preeclampsia. Metanephrine testing should be considered in patients with atypical features and can be reliably assessed using nonpregnant reference ranges. 34. Effects of home-, school-, and individual-level factors on children’s deliberate memory development in elementary school

Abstract

Children enter kindergarten with a variety of experiences and skills. In this transition to formal school, they are expected to adapt quickly to new demands such as remembering specific pieces of information, knowing when to retrieve this information, and understanding how to use this information to complete specific tasks. These skills have been referred to as children’s deliberate memory skills and are thought to serve children’s long term academic success. However, limited research has focused on specific aspects of children’s everyday contexts that play a role in the development of these skills – such as adult-to-child language exchanges in home and school settings. Therefore, the goals of the current study were to (a) understand the role of children’s every day, lived experiences such as parent–child reminiscing and teacher–child linguistic exchanges (i.e., cognitive processing language in classrooms) on the initial acquisition and sustained use of mnemonic strategies across the kindergarten and first-grade years, and (b) describe the interplay between individual-level factors – such as other components of children’s cognition – and these adult-to-child scaffolding practices on children’s memory development. Drawing on a sample of 79 children nested in 10 kindergarten classrooms, children’s deliberate memory skills were assessed at 6 timepoints from kindergarten entry to the end of first grade. Kindergarten teachers’ instruction was recorded using GoPro cameras during regular mathematics and language arts lessons; these recordings were subsequently coded for the prevalence use of cognitive processing language (Coffman et al., 2008; 2019). Abstract

Introduction Breast care nurses (BCNs) provide clinical and supportive care to meet the complex needs of people diagnosed with breast cancer (BC), which differ depending on a diagnosis of early or metastatic disease. By specifically designing a Cancer Nurse Self-Assessment Tool (CaN-SAT) that identifies learning needs, tailored education and training can be provided to upskill nurses and ensure optimal supportive care for patients. Objectives To develop and psychometrically test tools for early and metastatic breast cancer (CaN-SAT-eBC and CaN-SAT-mBC) that assess the skill levels of BCNs. Methodology A three-phase study was conducted. Phase 1: An expert working group was formed to develop the domains and item content for both tools. Phase 2: The Content Validation Index (CVI) was used to assess the relevance and clarity of each item on the two tools with BCN experts and nurse educators. Consensus of 0.78 across expert groups was required for an item to be included in the tools. Items rated less than 0.78 were revised or removed based on open-ended comments. Phase 3: The tools were tested for reliability with BCNs and Cronbach's alpha scores of ≥ 0.70 were considered good reliability. Results Each tool has undergone two rounds of content validation. The final versions of these tools included 18 domains, 123 items (CaN-SAT-eBC) and 22 domains, 145 items (CAN-SAT-mBC). For the eBC tool, two items were modified, and one removed based on CVI scores, while no items rated less than 0.78 in the mBC tool. The reliability testing is ongoing, and results will be available by March 2024. Conclusion The CaN-SATs are comprehensive tools for selfassessment of skills for BCNs. These tools provide opportunities for nurses to identify individual learning needs, as well as for researchers and educators to develop tailored training for optimal care provision. Abstract

Introduction With an increasing number of new metastatic Breast Care Nurse (mBCN) roles in Australia a pilot program was developed through a clinical and academic collaboration to provide specialised training and support for mBCNs to better meet complex patient care needs. The MBCNTP was designed by an expert group and consisted of 60+ hours of online learning, a 3-day practicum attended in-person or via a telepresence robot, followed by 12 months of clinical supervision. Objectives To i) measure the effectiveness of the MBCNTP on participant skill levels related to nursing care; ii) investigate the impact of the MBCNTP at the service level; and iii) explore participants' experience of the program and mode of participation. Methodology The evaluation employed a longitudinal design guided by Kirkpatrick's Evaluation Model. The Cancer Nurse Self-Assessment tool for metastatic breast cancer was used to measure skills at three intervals: pre- MBCNTP (T0); immediately post-practicum (T1); and 12 months post-practicum (T2). A retrospective audit of nurse activity data was conducted to evaluate the impact of the MBCNTP at the service level and analysed using descriptive statistics. Qualitative interviews were conducted at all timepoints, and data were analysed using an iterative, realist approach to thematic analysis. Results Eight mBCNs participated in the pilot MBCNTP. Equivalent outcomes were achieved regardless of participation in the practicum in-person or via robot. Self-assessed skills significantly improved at each timepoint, with mean scores at T2 indicating proficient practice in 16/20 domains. Nurse activity data showed an increased number of supportive care referrals from the mBCN post MBCNTP. Participants valued observing a comprehensive mBCN service and reported increased knowledge and confidence in providing care. Conclusion The MBCNTP has filled an identified gap in formal education opportunities for mBCNs.
