Abstract

Despite the increasing trend in liver resections for non-colorectal non-neuroendocrine liver metastases (NCNNLM), the role of surgery for these liver malignancies is still debated. Registries are an essential, reliable tool for assessing epidemiology, diagnosis, and therapeutic approach in a single hub, especially when data are dispersive and inconclusive, as in our case. The dissemination of this preliminary survey would allow us to understand if the creation of an International Registry is a viable option, while still offering a snapshot on this issue, investigating clinical practices worldwide. The steering committee designed an online questionnaire with Google Forms, which consisted of 37 questions, and was open from October 5th, 2022, to November 30th, 2022. It was disseminated using social media and mailing lists of the Italian Society of Endoscopic Surgery and New Technologies (SICE), the Association of Italian Surgeons in Europe (ACIE), and the Spanish Chapter of the American College of Surgeons (ACS). Overall, 141 surgeons (approximately 18% of the total invitations sent) from 27 countries on four continents participated in the survey. Most respondents worked in general surgery units (62%), performing less than 50 liver resections/year (57%). A multidisciplinary discussion was currently performed to validate surgical indications for NCNNLM in 96% of respondents. The most commonly adopted selection criteria were liver resectability, RECIST criteria, and absence of extrahepatic disease. Primary tumors were generally of gastrointestinal (42%), breast (31%), and pancreaticobiliary origin (13%). The most common interventions were parenchymal-sparing resections (51% of respondents) of metachronous metastases with an open approach. Major post-operative complications (Clavien–Dindo > 2) occurred in up to 20% of the procedures, according to 44% of respondents. 9. Stability theory of TASE-Runge-Kutta methods with inexact Jacobian

Abstract

This paper analyzes the stability of the class of Time-Accurate and Highly-Stable Explicit Runge-Kutta (TASE-RK) methods, introduced in 2021 by Bassenne et al. (J. Comput. Phys.) for the numerical solution of stiff Initial Value Problems (IVPs). Such numerical methods are easy to implement and require the solution of a limited number of linear systems per step, whose coefficient matrices involve the exact Jacobian $J$ of the problem. To significantly reduce the computational cost of TASE-RK methods without altering their consistency properties, it is possible to replace $J$ with a matrix $A$ (not necessarily tied to $J$) in their formulation, for instance fixed for a certain number of consecutive steps or even constant. However, the stability properties of TASE-RK methods strongly depend on this choice, and so far have been studied assuming $A=J$. In this manuscript, we theoretically investigate the conditional and unconditional stability of TASE-RK methods by considering arbitrary $A$. To this end, we first split the Jacobian as $J=A+B$. Then, through the use of stability diagrams and their connections with the field of values, we analyze both the case in which $A$ and $B$ are simultaneously diagonalizable and not. Healthy subjects had the lowest number of altered spectra, and patients with nonfunctioning kidney transplants the highest. Differences were significant for group HD-Tx vs group C (P=0.017) and group HD vs group C (P=0.015), but not between nonfunctioning kidney-transplant recipients and dialysis patients (group HD-Tx vs group HD).Conclusion: Although dialysis appears to be a weaker trigger for clonal expansion of T cells, our data suggest that the utilization of complementarity-determining region-3 spectratyping analysis of the TCR repertoire might be useful to monitor specific immunoactivation in patients before and after kidney transplantation. 14. A Systematic Review on the Physical, Physiological, Perceptual, and Technical-Tactical Demands of Official 3 × 3 Basketball Games.

Abstract

PURPOSE: To systematically review the physical, physiological, perceptual, and technical-tactical demands of official 3 × 3 basketball games. METHODS: The PRISMA (Preferred Reporting Items for Systematic Reviews and Meta-Analyses) guidelines were followed. Three electronic databases (PubMed, Scopus, and Web of Science) were systematically searched to identify studies assessing physical, physiological, perceptual, and technical-tactical demands of 3 × 3 games. Data were also coded according to player sex and tournament phase. Quality assessment of the included studies was performed using a modified Downs and Black checklist. RESULTS: Thirteen articles were finally included, with a mean quality of 8.6 (1.1) out of 11. Three-by-three basketball games have an intermittent profile (1:1 work-rest ratio), with a duration of ∼15 minutes, and are characterized by short (6-8 s) ball possessions and considerable physical (17-33 accelerations, 24-44 decelerations, 62-94 changes of directions, and 17-24 jumps per game) and physiological (lactate: ∼6.2 mmol·L-1) demands. Overall, the game performance profile is similar in males and females, with minor changes happening across tournament phases. Several key technical-tactical indicators were identified as discriminating winning and losing teams, such as better shooting and defensive efficiency, low number of turnovers, and implementing tactical actions involving more players, passing first, and ending possessions with shots from outside of the arch from the top of the key. CONCLUSIONS: Three-by-three basketball is an intermittent, physically demanding sport characterized by quick plays and specific tactical constraints. This review provides information that should be considered by performance staff to improve training prescription, game tactical plans, and for player selection and talent identification.

Abstract

Despite the increased popularity and number of international competitions for 3 × 3 basketball, the precise physical demands of this sport are not well described. Therefore, this study aimed to quantify the physical demands of 3 × 3 basketball games according to game outcome and competition phase. Following an observational design, video footage from 27 games were analysed from 104 international 3 × 3 basketball players (n = 52 male and n = 52 female players) belonging to 26 national teams (n = 13 male and n = 13 female teams) during the 2019 FIBA 3 × 3 World Cup. Manual frame-by-frame time-motion analyses were conducted to determine the relative frequency (n · min-1) and duration (% of live playing time) for several physical demand variables to make comparisons according to game outcome (win vs. loss) and competition phase (group games vs. finals games). Linear mixed models for repeated measures and effect size analyses revealed non-significant, trivial-small differences in physical demands between games that were won and lost. Regarding competition phase, male players completed more high-intensity activity (sprinting, high-intensity specific movements, and jumping) but spent a greater proportion of playing time jumping and performing recovery activities (standing/ walking) during finals games than group games (P < 0.05, small), while female players performed more low-intensity activity (jogging and low-intensity specific movements) during group games than finals games (P < 0.05, small). These findings indicate that the physical capabilities of male and female 3 × 3 basketball players may not be the determining factor for team success in games and players can mostly maintain activity outputs across phases of tournament play conducted at the highest international standard.

Following an observational design, video footage from 27 games of the International Basketball Federation 3 × 3 World Cup 2019 were analyzed from 104 international 3 × 3 basketball players ( n = 52 male and n = 52 female players) resulting in a total of 216 (104 male and 112 female) individual game samples. Manual frame-by-frame time-motion analyses determined the relative frequency (n·min -1 ) and duration (%) for several physical demands at different intensities, according to sex, during the live playing time and in ball possession phases. Linear mixed models for repeated measures and effect size (ES) analyses revealed small non-significant differences in the intermittent profile of 3 × 3 basketball games according to sex (total movements per minute, male = 39.3 (38.6-40.1); female = 40.2 (39.5-41.0), estimated marginal means with 95% confidence intervals). Female competitions had significantly greater number of low-intensity activities (LIA, small ES) and high-intensity activities (HIA, small ES) performed per minute over longer games ( small ES), whereas male players had more recovery activities ( small ES). During ball possession, male players spent a larger amount of time performing LIA ( small ES) than female players, who displayed both the greatest number of HIA and the highest percentage of playing time performed at high intensity ( small ES). Overall, these findings suggest that basketball coaches should design sex-specific training sessions based on the specific match demands.

Abstract

Background & Aims: The ANSWER study reported that long-term albumin administration in patients with cirrhosis and uncomplicated ascites improves survival. During treatment, serum albumin increased within a month and remained stable thereafter. In this post hoc analysis, we aimed to determine whether on-treatment serum albumin levels could guide therapy. Methods: Logistic regression was used to assess the association between baseline serum albumin and mortality, as well as to determine on-treatment factors associated with mortality and to predict the achievement of a given on-treatment serum albumin level. Survival was assessed by Kaplan-Meier estimates and second-order polynomial regression. Patients whose on-treatment serum albumin remained below normal were compared with a subset of patients from the control arm matched by principal score. Results: Baseline serum albumin was closely associated with 18-month mortality in untreated patients; albumin treatment almost effaced this relationship. On-treatment serum albumin and MELD-Na at month 1 were the sole independent variables associated with mortality. Second-order polynomial regression revealed that survival improved in parallel with increased 1-month on-treatment serum albumin. Kaplan-Meier estimations showed that any value of 1-month on-treatment serum albumin (0.1 g/dl intervals) in the range 2.5–4.5 g/dl discriminated patient survival. In the normal range of serum albumin, the best discriminant value was 4.0 g/dl. Compared to untreated patients, survival even improved in patients whose on-treatment serum albumin remained below normal. Conclusion: Baseline serum albumin per se should not guide the decision to start albumin therapy. Conversely, 1-month on-treatment serum albumin levels are strongly associated with outcomes and could guide the use of albumin – 4.0 g/dl being the target threshold. Abstract

Here, we analyze future projections of cyclone activity in the Mediterranean region at the end of the twenty-first century based on an ensemble of state-of-the-art fully-coupled Regional Climate System Models (RCSMs) from the Med-CORDEX initiative under the Representative Concentration Pathway (RCP) 8.5. Despite some noticeable biases, all the RCSMs capture spatial patterns and cyclone activity key characteristics in the region and thus all of them can be considered as plausible representations of the future evolution of Mediterranean cyclones. In general, the RCSMs show at the end of the twenty-first century a decrease in the number and an overall weakening of cyclones moving across the Mediterranean. Five out of seven RCSMs simulate also a decrease of the mean size of the systems. Moreover, in agreement with what already observed in CMIP5 projections for the area, the models suggest an increase in the Central part of the Mediterranean region and a decrease in the South-eastern part of the region in the cyclone-related wind speed and precipitation rate. These rather two opposite tendencies observed in the precipitation should compensate and amplify, respectively, the effect of the overall reduction of the frequency of cyclones on the water budget over the Central and South-eastern part of the region. A pronounced inter-model spread among the RCSMs emerges for the projected changes in the cyclone adjusted deepening rate, seasonal cycle occurrence and associated precipitation and wind patterns over some areas of the basin such as Ionian Sea and Iberian Peninsula. The differences observed appear to be determined by the driving Global Circulation Model (GCM) and influenced by the RCSM physics and internal variability. Abstract

Scores in upper GI bleeding (UGIB) are used to stratify death risk and need for hospitalization at admission, but a tool that incorporates dynamic changes during the hospital stay is lacking. We aimed to develop a death risk score that considers changes in clinical status during hospitalization and compare its performance with existing ones.A multicenter cohort study enrolling patients with UGIB in 50 Italian hospitals from January 2014 to December 2015 was conducted. Data were collected and used to develop a risk score using logistic regression analyses. Performance curves (area under the receiver-operating characteristic [AUROC] curves), sensitivities, specificities, positive and negative predictive values, and outcomes classified as low, intermediate, and high death risk were calculated. The score's performance was externally validated and then compared with other scores.We included 1852 patients with nonvariceal UGIB in the development cohort and 912 in the validation cohorts. The new score, which we named the Re.Co.De (rebleeding-comorbidities-deteriorating) score, included 10 variables depicting the changes in clinical conditions while in the hospital. The mortality AUROC curves were .93 (95% confidence interval, .91-.96) in the derivation cohort and .94 (95% confidence interval, .91-.98) in validation cohort. In a comparison of AUROC curves with other scores, the new score showed a significant performance compared with pre- and postendoscopy scores. Patients with low and high scores had 30-day mortality rates of .001% and 48.2%, respectively.The Re.Co.De score has a higher performance for predicting mortality in patients with UGIB compared with other scores, correctly identifying patients at low and high death risk while in the hospital through a dynamic re-evaluation of clinical status.
