1. Leveraging Video Vision Transformer for Alzheimer's Disease Diagnosis from 3D Brain MRI

Abstract

Alzheimer's disease (AD) is a neurodegenerative disorder affecting millions worldwide, necessitating early and accurate diagnosis for optimal patient management. In recent years, advancements in deep learning have shown remarkable potential in medical image analysis. Methods In this study, we present "ViTranZheimer," an AD diagnosis approach which leverages video vision transformers to analyze 3D brain MRI data. By treating the 3D MRI volumes as videos, we exploit the temporal dependencies between slices to capture intricate structural relationships. The video vision transformer's self-attention mechanisms enable the model to learn long-range dependencies and identify subtle patterns that may indicate AD progression. Our proposed deep learning framework seeks to enhance the accuracy and sensitivity of AD diagnosis, empowering clinicians with a tool for early detection and intervention. We validate the performance of the video vision transformer using the ADNI dataset and conduct comparative analyses with other relevant models. Results The proposed ViTranZheimer model is compared with two hybrid models, CNN-BiLSTM and ViT-BiLSTM. CNN-BiLSTM is the combination of a convolutional neural network (CNN) and a bidirectional long-short-term memory network (BiLSTM), while ViT-BiLSTM is the combination of a vision transformer (ViT) with BiLSTM. The accuracy levels achieved in the ViTranZheimer, CNN-BiLSTM, and ViT-BiLSTM models are 98.6%, 96.479%, and 97.465%, respectively. ViTranZheimer demonstrated the highest accuracy at 98.6%, outperforming other models in this evaluation metric, indicating its superior performance in this specific evaluation metric. Conclusion This research advances the understanding of applying deep learning techniques in neuroimaging and Alzheimer's disease research, paving the way for earlier and less invasive clinical diagnosis.

Abstract

BackgroundIn 2011, the American Board of Medical Specialties established clinical informatics (CI) as a subspecialty in medicine, jointly administered by the American Board of Pathology and the American Board of Preventive Medicine. Subsequently, many institutions created CI fellowship training programs to meet the growing need for informaticists. Although many programs share similar features, there is considerable variation in program funding and administrative structures.ObjectivesThe aim of our study was to characterize CI fellowship program features, including governance structures, funding sources, and expenses.MethodsWe created a cross-sectional online REDCap survey with 44 items requesting information on program administration, fellows, administrative support, funding sources, and expenses. We surveyed program directors of programs accredited by the Accreditation Council for Graduate Medical Education between 2014 and 2021.ResultsWe invited 54 program directors, of which 41 (76%) completed the survey. The average administrative support received was $27,732/year. Most programs (85.4%) were accredited to have two or more fellows per year. Programs were administratively housed under six departments: Internal Medicine (17; 41.5%), Pediatrics (7; 17.1%), Pathology (6; 14.6%), Family Medicine (6; 14.6%), Emergency Medicine (4; 9.8%), and Anesthesiology (1; 2.4%). Funding sources for CI fellowship program directors included: hospital or health systems (28.3%), clinical departments (28.3%), graduate medical education office (13.2%), biomedical informatics department (9.4%), hospital information technology (9.4%), research and grants (7.5%), and other sources (3.8%) that included philanthropy and external entities.ConclusionCI fellowships have been established in leading academic and community health care systems across the country. Due to their unique training requirements, these programs require significant resources for education, administration, and recruitment. There continues to be considerable heterogeneity in funding models between programs. Our survey findings reinforce the need for reformed federal funding models for informatics practice and training.

Abstract

IntroductionAbout one-third of critically ill patients with acute kidney injury (AKI) develop persistently decreased kidney function, known as acute kidney disease (AKD), which may progress to chronic kidney disease (CKD). Although sepsis is the most common cause of AKI, little is known about sepsis-associated AKD.MethodsUsing data from a large randomized trial including 1341 patients with septic shock, we studied patients with stage 2 or 3 AKI on day 1 of hospitalization. We defined AKD as a persistently reduced glomerular filtration rate for >7 days. In addition to clinical data, we measured several urinary biomarkers (tissue inhibitor of metalloproteinases-2 and insulin-like growth factor-binding protein 7 [TIMP-2∗IGFBP7], neutrophil gelatinase-associated lipocalin [NGAL], kidney injury molecule-1 [KIM-1], liver-type fatty acid binding protein, and type 4 collagen) at 0, 6, and 24 hours, to predict AKD.ResultsOf 598 patients, 119 (19.9%) died within 7 days, 318 (53.2%) had early reversal of AKI within the first 7 days, whereas 161 (26.9%) developed AKD. In patients with early reversal, 45 (14.2%) had relapsed AKI after early reversal, and only about one-third of these recovered. Among patients developing AKD, only 15 (9.3%) recovered renal function prior to discharge. Male sex, African American race, and underlying CKD were more predominant in patients developing AKD. None of the biomarkers tested performed well for prediction of AKD, although NGAL modestly increased the performance of a clinical model.ConclusionsAKD is common in patients with septic shock, especially among African American males and those with underlying CKD. Existing AKI biomarkers have limited utility for predicting AKD but might be useful together with clinical variables. Novel predictive biomarkers for renal recovery are needed.

Abstract

Manual ventilation, most commonly with a bag-valve mask, is a form of short-term ventilation used during resuscitative efforts in emergent and out-of-hospital scenarios. However, compared to mechanical ventilation, manual ventilation is an operator-dependent skill that is less well controlled and is highly subject to providing inappropriate ventilation to the patient. This article first reviews recent manual ventilation guidelines set forth by the American Heart Association and European Resuscitation Council for providing appropriate manual ventilation parameters (e.g., tidal volume and respiratory rate) in different patient populations in the setting of cardiopulmonary resuscitation. There is then a brief review of clinical and manikin-based studies that demonstrate healthcare providers routinely hyperventilate patients during manual ventilation, particularly in emergent scenarios. A discussion of the possible mechanisms of injury that can occur during inappropriate manual hyperventilation follows, including adverse hemodynamic alterations and lung injury such as acute barotrauma, gastric regurgitation and aspiration, and the possibility of a subacute, inflammatory-driven lung injury. Together, these injurious processes are described as manual-ventilation-induced lung injury (MVILI). This review concludes with a discussion that highlights recent progress in techniques and technologies for minimizing manual hyperventilation and MVILI, with a particular emphasis on tidal-volume feedback devices. Abstract

The rising application of extracorporeal membrane oxygenation (ECMO) has emphasized the need for consistent and standardized terminology, especially concerning peripheral percutaneous cannulation of the pulmonary artery (PPC-PA). The Extracorporeal Life Support Organization (ELSO) Nomenclature Task Force produced the ELSO Maastricht Treaty for extracorporeal life support (ECLS) Nomenclature to address this challenge. However, adherence to nomenclature recommendations has been poor in publications describing PPC-PA. We aim to describe common nomenclature errors and provide a user-guide for abbreviations that can be used by authors, reviewers, and journal staff to ensure properadherence to standardized nomenclature in publications describing PPC-PA. Abstract

Traditional document‐based systems engineering (SE) practices have proven inadequate to address modern systems' increasing complexity, leading to shortcomings in SE outcomes. To address these limitations, model‐based systems engineering (MBSE) has emerged with an emphasis on the system model as the primary SE artifact. Despite its potential, MBSE has not achieved widespread adoption. This study explores MBSE adoption challenges through the lens of the diffusion of innovations theory to identify factors hindering its adoption. The study's methodology includes a survey distributed to SE professionals focusing on perceptions of attributes of MBSE identified by the diffusion of innovations theory, current use of models and MBSE, and basic demographic information. Results highlighted that respondents recognize the relative advantage of MBSE in improving data quality and traceability, but perceived complexity and compatibility with existing practices still present barriers to adoption. Subpopulation analysis reveals that those who are not already involved in MBSE efforts face the additional adoption obstacles of limited trial opportunities and tool access (chi‐squared test of independence between these populations resulted in p = 0.00). The survey underscores the potential for closer alignment between MBSE and existing SE methodologies to improve the perceived compatibility of MBSE. Future studies would benefit from examining additional variables identified by the diffusion of innovations theory, incorporating control questions to differentiate between perceptions of SE generally and MBSE specifically, identification of better methods to assess current MBSE use by participants, and measures to broaden the participant scope. Finally, the imminent introduction of SysML v2 presents a unique opportunity to reshape perceptions of MBSE to improve the adoption rate of MBSE and enhance its accessibility among SE professionals. 47. Emergency Department Activation of Interventional Cardiology to Reduce Door-to-Balloon Time

Abstract

Background: Despite American College of Cardiology (ACC) and American Heart Association (AHA) guidelines, many hospitals have door-to-balloon times in excess of 90 minutes. Emergency Department (ED) activation of interventional cardiology has been described as an important strategy to reduce door-to-balloon time. However, prior studies on ED activation have been in suburban hospitals with door-to-balloon times near the ACC/AHA targeted times.Objective: To determine if ED activation of interventional cardiology could significantly improve reperfusion times and reach the ACC/AHA target of 90 minutes or less in a safety net hospital, a Level I trauma center and teaching hospital served as a primarily uninsured and underinsured patient population with door-to-balloon times ranking in the lowest quartile of United States hospitals.Methods: In this study, door-to balloon times before and after implementation of ED activation were compared by retrospective chart review.Results: Eighty patients were included in the study, 48 before and 32 after ED activation of interventional cardiology. Median door-to-balloon time decreased from 163.5 minutes before to 130 minutes after ED activation, a significant difference of 33.5 minutes (p=0.028). Door-to-balloon time on nights, weekends and holidays decreased from a median of 165.5 minutes to 130 minutes, a reduction of 35.5 minutes, which also reached statistical significance (p=0.029).Conclusion: ED activation of interventional cardiology produced a statistically significant reduction in door-to-balloon time. However, the reduction was not enough to achieve a door-to-balloon time of less than 90 minutes. Safety net hospitals with door-to-balloon times in the lowest quartile nationally may require multiple strategies to achieve targeted myocardial reperfusion times.
