Here we report the use of resistance-guided retreatment of patients who failed prior NS5A inhibitor-containing regimens in the GEHEP-004 cohort. This is the largest direct-acting antiviral (DAA)-resistance cohort study conducted in Spain. We aim to provide indications on how to use resistance information in settings where sofosbuvir/velpatasvir/voxilaprevir may not be available., [Methods] GEHEP-004 is a prospective multicenter cohort enrolling HCV-infected patients treated with interferon (IFN)-free DAA regimens. Prior to retreatment, population-based sequencing of HCV NS3, NS5A and NS5B genes was performed. After receiving a comprehensive resistance interpretation report, the retreatment regimen was chosen and the sustained virological response (SVR) at 12 weeks after treatment completion (SVR12) was recorded., [Results] A total of 342 patients experiencing virological failure after treatment with sofosbuvir/ledipasvir±ribavirin (54%), sofosbuvir/daclatasvir±ribavirin (23%), or paritaprevir-ritonavir/ombitasvir±dasabuvir±ribavirin (20%) were studied. After a resistance report, 186 patients were retreated. An SVR12 was achieved for 88.1% of the patients who failed after sofosbuvir/ledipasvir±ribavirin, 83.3% of the patients who failed after sofosbuvir/daclatasvir±ribavirin, 93.7% of the patients who failed after paritaprevir-ritonavir+ombitasvir±dasabuvir±ribavirin., [Conclusions] In our study, we show how resistance-guided retreatment in conjunction with an interpreted report allows patients to achieve SVR rates close to 90%. We hypothesize that SVR rates may even be improved if resistance data are discussed between experienced virologists and treating clinicians. We believe that our data may be relevant for countries where the access to new DAA combination regimens is limited., [Lay summary] Hepatitis C infection can be cured with currently available antiviral agents. Only a small proportion of patients experience treatment failure, however, in absolute numbers, a high number of patients may require retreatment. Highly effective combinations of antivirals are also available for retreatment. However, these antivirals might not be available in resource-limited settings. * This record is given in both English and Spanish Microbiological methods for the analytical control of antibiotic residues are based on detecting the only common characteristic among all antibiotics, their ability to inhibit bacterial growth. Therefore, over the past 30 years in Spain, they have played an important role in the control of these residues within the framework of the National Residue Monitoring Plan in food. Its simplicity, flexibility, low cost and multi-residue detection and sample processing capabilities made them the methods of choice for undertaking the analysis of antibiotic residue screening. The validation of some of the antibiotic residue screening methods is complex given that intact tissue is used as a test sample. Therefore, the five-plate screening method validation was completed through collaborative trials in which the National Reference Laboratory and the official control laboratories participated, so that their validation was shared and the accreditation of the method was facilitated according to the ISO/IEC 17025 standard. As a National Reference Laboratory for antibiotic residues in food, the National Centre for Food (CNA) of the Spanish Agency for Food Safety and Nutrition (AESAN) has carried out a whole series of activities to serve the official control laboratories, including: laboratory communications, conferences, courses, intercomparison and collaborative tests and supplying materials for analysis. The current development of multi-residue physical-chemical instrumental methods and with detection limits better adjusted to the maximum antibiotic residue limits established in the European Union has resulted in a turning point and has limited the applicability of traditional microbiological methods for detecting antibiotic residues in the official control of the same, ES; PDF; pfefsa@aesan.gob.es

BACKGROUND Various studies and two meta-analysis have shown that a variable stiffness colonoscope improves cecal intubation rate. However, there are few studies on how this colonoscope should be used. OBJECTIVE The aim of this study was to identify factors related to the advancement of the colonoscope when the variable stiffness function is activated. METHODS Prospective study enrolling consecutive patients referred for colonoscopy. The variable stiffness colonoscope (Olympus CF-H180DI/L®) was used. We performed univariate and multivariate analyses of factors associated with the success of the variable stiffness function. RESULTS After the data inclusion period, 260 patients were analyzed. The variable stiffness function was used most in the proximal colon segments (ascending and transverse colon 85%; descending/sigmoid colon 15.2%). The body mass index was lower in patients in whom the endoscope advanced after activating the variable stiffness than those in which it could not be advanced (25.9 + or - 4.8 vs. 28.3 + or - 5.4 kg/m2, p = 0.009). The endoscope advanced less frequently when the stiffness function was activated in the ascending colon versus activation in other segments of the colon (25% vs. 64.5% ascending colon vs. other segments; p < 0.05). In the multivariate analysis, only the colon segment in which the variable stiffness was activated was an independent predictor of advancement of the colonoscope. CONCLUSIONS The variable stiffness function is effective, allowing the colonoscope advancement especially when applied in the transverse colon, descending colon and sigmoid. However, when used in the ascending colon it has a lower effectiveness.

Introduction Variable stiffness colonoscope may be useful in performing colonoscopies in nonsedated patients or under endoscopist-controlled sedation. The objective of this study is to evaluate whether this instrument facilitates colonoscopy in patients under deep sedation monitored by an anaesthesiologist. Methods Prospective and randomized study enroling consecutive patients referred for colonoscopy under deep sedation monitored by an anaesthesiologist. In group I, a variable stiffness colonoscope was used, whereas in group II, a standard colonoscope was used. The main variable was the need to change the position of the patient during the endoscopy. Results Fifty-six patients were included in group I (variable stiffness colonoscope) and 54 in group II (standard colonoscope). The caecum was reached in 92.9% of patients in group I and in 90.7% of group II (P=0.7). The time required to reach the caecum was significantly less in group I (6.14±3.5 vs. 7.7±3.8; P=0.035). The variable stiffness colonoscope was effective in 66.7% of cases. Changing the position of the patient was necessary in 12.5% of cases in group I compared with 33.3% of cases in group II (P=0.01). Conclusion The variable stiffness colonoscope avoids the need to change the patient's position and reduces caecal intubation time in patients undergoing colonoscopy under deep sedation controlled by an anaesthesiologist.

Familial adenomatous polyposis (FAP) is mainly characterized by the development of a large number of polyps in the gastrointestinal tract and by the risk of developing adenocarcinomas. We present the case of a woman diagnosed with FAP and liver metastases. Histological analysis revealed both diseases to be secondary to a neuroendocrine tumor. To date, only three cases showing the simultaneous occurrence of these two entities have been published. Currently, there is no genetic basis to explain the coexistence of these two diseases, both of which have a very low prevalence.
