This study aimed to assess whether a specific adjustable compression garment (ACG) system (Coolflex Standard Calf and Coolflex Standard Foot; Sigvaris Inc.) promotes self-care in patients with chronic leg oedema. Secondary aims were to assess the effectiveness of this ACG in reducing oedema, improving patient reported outcomes, and determining the patients' degree of satisfaction with the handling of the wrap. This was a multicentre prospective observational study. The study included 99 adult patients aged 18 – 90 years presenting with chronic oedema of the lower extremity, which encompasses conditions such as lymphoedema, venous oedema, and phlebolymphoedema. At baseline, all patients received an ACG. After two to three days and after six weeks, their overall satisfaction with the therapy was assessed. Leg volume was determined in a contactless manner during the baseline and follow up visits. All patients completed the cross cultural adaptation of the Lymphoedema Functioning, Disability and Health Questionnaire for Patients with Lymphoedema of the Lower Extremity in Germany (Lymph-ICF-UG). A total of 86 patients completed the study and were followed up for six weeks. At the final six week follow up, 82 (95.3%) of the 86 subjects indicated that they were able to put on the wrap independently or with a little help from relatives. The overall satisfaction rate was 88.0% (95% confidence interval [CI], 79.64 – 93.9%). The mean leg volume reduction from baseline to six weeks was –4.7% (95% CI –6.3 – –3.0%; p <.001). Lymph-ICF-UG scores and scores in all domains improved significantly from baseline to the final follow up. Compression is the cornerstone of treatment for lymphoedema; however, in patients with Stage III lymphoedema that causes a significant alteration of limb shape and size, additional creative problem solving is required in order to find a solution that fits the person, their limb and their personal circumstances. Medical adaptive compression (MAC) systems provide solutions to complex situations. We present two cases of individuals who completed an inpatient rehabilitation stay for intensive treatment of their lymphoedema and were fitted with MAC devices to ensure long-term success. Introduction: The preference for endovascular techniques in treating varicose veins, particularly in the great saphenous vein (GSV), has increased due to their minimally invasive nature and reduced complications. Post-operative care, especially involving compression therapy, remains crucial to improve outcomes, prevent varicose vein recurrence, and enhance overall recovery. This study aimed to evaluate the efficacy of eccentric compression therapy compared to alternative post-operative care methods following endovenous laser treatment (EVLT) for GSV insufficiency. Methods: This prospective randomized clinical trial encompassed 88 EVLT procedures for GSV insufficiency. The participants were divided into two groups, each receiving different postoperative compression methods, and were evaluated over a specified period. The primary outcome was the pain scale after EVLT; meanwhile, the secondary outcome measured in the present study was the rate of GSV occlusion after EVLT. Results: Both groups underwent all EVLT procedures successfully without any complications. At the one-month duplex ultrasound (DUS) follow-up, the sapheno-femoral junction occlusion rates were 97% (43 out of 44) for group A (eccentric compression plus gradual compression stocking) and 95% (42 out of 44) for group B (only gradual compression stocking). Ecchymosis was observed in only 12 patients across both groups, accounting for an overall occurrence of 13.6%. Group A patients reported significantly lower analgesic usage (10%) compared to group B (18%), although this difference did not reach statistical significance. Analysis of postoperative pain data utilizing the visual analog scale (VAS) showed a median value of 5.5 in group B patients, which decreased to 3.1 with the application of eccentric compression. Moreover, there was less ecchymosis in group A observed by one week. Conclusion: This study contributes to the ongoing discourse on the efficacy of postoperative compression in varicose vein treatment. Chronic venous insufficiency (CVI) causes severe symptoms and complications in the general population, but the prevalence, related risk factors, and treatment of CVI are unknown among nurses. The demographics and occupational factors of nurses from a university hospital were collected by questionnaires, and the presentation of CVI was confirmed by the specialist vascular surgeons. A total of 1606 participants were enrolled, and the prevalence of CVI was 7.5%. After multivariate adjustment, CVI was positively related to deep venous thrombosis history (OR 6.44, 95% CI 2.73-15.22), increased standing time (OR 2.21, 95% CI 1.20-4.10), and increased time in night shift rotation (OR 2.01, 95% CI 1.29-3.14). CVI was negatively related to oral contraceptives (OR.43, 95% CI.21-.87). Of the participants, 72.5% with CVI received compression therapy. For them, wearing compression stockings with a pressure of >20 mmHg or for >4 h/day significantly increased the rate of symptom relief, while the length of compression stockings made no difference. Thus, it was beneficial for nurses to spend less time standing and shorten their time in night shift rotation. Abstract Background During prolonged standing, insufficient calf muscle pumping accompanies venous stasis and hypertension in the lower legs, resulting in valve dysfunction, venous wall problems, and sub-sequent inflammation. Compression therapy, which includes medical compression stockings (MCS) and mechanical intermittent pneumatic compression (IPC), is one of the most effective therapeutic interventions for treating chronic venous diseases. This study aimed to compare the therapeutic effect among resting, IPC and MCS alone, and IPC with MCS in long-standing workers (> 8 h daily). Methods This crossover trial was conducted with 39 participants with complaints of leg edema and pain whose work involved standing for more than 8 h daily. Four treatment protocols were established for each visit as follows: protocol A (not wear MCS during work and rest without IPC after work), protocol B (wear MCS during work and rest without IPC after work), protocol C (not wear MCS during work and treat with IPC after work), and protocol D (wear MCS during work and treat with IPC after work). The primary outcome was the visual analogue scale (VAS) score for leg pain. The secondary outcomes were leg volume (mL), circumference (cm), extracellular fluid/total body fluid (ECF/TBF), and extracellular water/total body water (ECW/TBW) through bioelectrical impedance analysis. Outcomes were assessed before work (T0), after work (T1), and 60 min after intervention (T2). Results All four protocols had significantly increased leg pain after work (T0-1) but improved 60 min after intervention (T1-2), particularly protocol C (decreased VAS by 1.9). When leg swelling was compared at T0 and T1, protocols A and C showed significant increases in leg volume and circumference, indicating significant work-induced edema, whereas protocols B and D showed no change or even a decrease. After interventions, leg volume and circumference significantly decreased in protocols A and C, although protocols B and C did not show significant improvement. The ECF/TBF and ECW/TBW of all protocols decreased after interventions. Conclusions Leg pain and edema after prolonged standing (T1-T2) in adults were safely and effectively improved by both IPC alone and IPC with MCS. Although the use of MCS during the workday did not show improvement in leg pain immediately after work (T0-T1), both MCS with resting and MCS with IPC decreased leg pain at T1-T2 and prevented leg edema at T0-T1. Objectives: This study aims to compare the effectiveness of compression bandages and compression stockings in the treatment of venous ulcers following venous ulcer surgery., Method: A total of 170 patients who underwent venous ulcer surgery were included in this retrospective study. Patients were divided into two groups: the first group used compression bandages, and the second group used compression stockings. In all patients, a sterile cotton dressing from the venous ulcer care kit was first applied directly over the open ulcer to protect the wound bed. Both groups were followed for 3 months. Demographic and clinical characteristics such as age, gender, smoking status, hypertension, peripheral arterial disease (PAD), and diabetes mellitus (DM) were recorded. Primary endpoints included changes in ulcer size, calf circumference, NPRS pain scores, and VCSS scores. Measurements were taken before treatment, at 1 week, at 1 month, and at 3 months. The data were statistically analyzed using appropriate methods., Results: The results showed that the compression bandage group had a more significant reduction in ulcer size compared to the compression stocking group. The ulcer size in the bandage group decreased from 16.5 ± 2.2 cm 2 to 7.9 ± 5.1 cm 2 after 3 months, while in the stocking group, it reduced from 16.1 ± 1.8 cm 2 to 9.6 ± 3.1 cm 2 ( p < .001). Additionally, the calf circumference showed a greater reduction in the bandage group, from 41.3 ± 1.9 cm to 35.3 ± 2.9 cm, compared to the stocking group, which showed a reduction from 40.7 ± 1.8 cm to 38.1 ± 2.4 cm ( p < .001). NPRS pain scores also decreased more significantly in the bandage group, with scores dropping from 8.0 ± 1.1 to 2.9 ± 2.8, while the stocking group saw a decrease from 7.9 ± 1.2 to 5.4 ± 2.4 ( p < .001). VCSS scores improved in both groups, but the bandage group showed a more marked improvement from 19.6 ± 5.1 to 5.2 ± 8.3 compared to the stocking group, which improved from 18.0 ± 5.1 to 7.1 ± 4.7 ( p < .001)., Conclusion: The findings suggest that compression bandages may be more effective than compression stockings in the post-surgical treatment of venous ulcers, particularly in terms of reducing ulcer size, calf circumference, and pain, as well as improving VCSS scores. Edema in pregnancy often occurs. Nearly 80% of all pregnancies can occur in leg edema which is a small problem that can provide initial symptoms for various diseases and pathological conditions and can be an indicator of serious chronic disease. Various methods of management to prevent and treat edema in pregnancy are by compression stocking and foot massage. The purpose of this study was to analyze the effect of foot massage and compression stocking on pain intensity and the degree of edema in pregnant women Trimester III. The type of research is quasi experiment and using a pretest-posttest control group design with 3 treatment groups, namely foot massage, compression stocking and a combination of foot massage and compression stocking. The population of this study was all Trimester III pregnant women with edema in Pekanbaru City. The sampling technique used purposive sampling with inclusion criteria. This research was conducted on April 9 to September 15, 2018 in Pekanbaru City Health Center namely Rumbai Health Center, Umbansari Health Center and Melur Health Center. Data analysis using Annova Test. The results showed that in the combination group of foot massage and compression stocking the decrease in the average pain intensity was 1.8 and the decrease in the average degree of edema was 1.6 mm. There is the effect of foot massage, compression stocking and a combination of foot massage and compression stocking on pain intensity and degree of edema (p -value = 0.00. The group that most significantly reduces pain intensity and the degree of edema is a combination of foot massage and compression stocking. Recommended for health workers in order to use the method of foot massage, compression stocking and a combination of foot massage and compression stocking in preventing and reducing edema in pregnant women. Background: Reduced venous return is an important trigger of vasovagal syncope (VVS). Elastic compression stockings (ECS) can modify venous return and be of therapeutic interest; however, evidence for ECS efficacy in VVS is scarce. This randomized controlled trial was designed to address the issue.Methods: COMFORTS-II is a multicenter, triple-blind, parallel design, randomized controlled trial aimed to assess the efficacy of ECS in preventing VVS recurrences. Using central online randomization, 268 participants will be allocated to 2 arms (1:1 ratio), wearing intervention ECS (25-30 mm Hg pressure) or sham ECS (≤10 mm Hg pressure). All participants will receive standard VVS treatment in the form of education, and lifestyle modification recommendations (drinking 2-3 l/d of fluids and consuming 10 g/d-roughly half a tablespoon-of table salt). Adherence to ECS treatment will be evaluated through diary sheets, and compared between study arms. Follow-up continues for 1 year, and is conducted via a 24/7 phone line available to patients and trimonthly visits. The co-primary outcomes are proportion of participants with any syncopal recurrence and time to first syncopal episode. Secondary outcomes include frequency of VVS spells, time intervals between recurrences, and incidence of any patient-reported adverse effects.Conclusion: To the best of our knowledge, COMFORTS-II is the first clinical trial to assess ECS efficacy among patients with VVS, addressing an important gap in evidence for VVS treatments. Background: Varicose veins are a common chronic venous disorder that leads to a significantly reduced quality of life and high healthcare resource burden. There is a large amount of research-based data and opinions regarding varicose veins. There are also some common myths and misconceptions about the treatment options. Current Concepts: Management options include compression therapy, open venous surgery, and endovenous therapy. An appropriate treatment option must be selected on a case-by-case basis based on the symptoms, severity, and duplex ultrasonography findings. Venous intervention should be considered if the condition is symptomatic and often, there is no need for urgent surgery or endovenous therapy because the symptoms usually progress slowly and severe cardiac problems are rare. According to previous studies, there is no difference between the endovenous therapy modalities and the traditional surgical method (i.e., high ligation and stripping) in terms of recurrence rates. Therefore, case-specific factors, such as anatomy and economic factors, should be considered. When compression therapy is considered, graduated compression stocking with suitable pressure should be prescribed. Venoactive drugs can be suggested in addition to compression therapy for symptomatic varicose veins or venous ulcers. Discussion and Conclusion:. Correct information regarding treatment for varicose veins based on the best available evidence must be provided to patients. Proper level of pressure or compression generated by medical stocking or hosiery is the key element for successful treatment or management of chronic venous disorders such as oedema, leg ulcers, etc. However achieving the recommended compression level and, more importantly, sustaining it using stockings has been a major challenge to the health practitioners supervising the treatment. This work aims to investigate and design a smart compression stocking using shape-memory polymer that allows externally controlling the pressure level in the wrapped position on the leg. Based on thermodynamical rubber theories, we first derived several criteria that have to be satisfied simultaneously in order to achieve the controlled pressure adjustment using external heat stimuli. We then presented a case where such a stocking is developed using a blend yarn consists of selected shape-memory polyurethane and nylon filaments. Extensive experimental work has also been conducted to demonstrate the feasibility and explore the influencing factors involved.

Objectives: Compression stockings (CSs) are effective in the treatment of chronic venous and lymphatic diseases. Noncompliance with wearing them remains a hindrance to good results. This study aims to quantify the issue of noncompliance and to identify the reasons behind it. Methods: Over a period of 6 months, a set of questionnaire to assess the compliance to CSs was conducted on patients seen during their follow-up visits and of whom CSs were prescribed previously. Results: A total of 50 patients were recruited in this study. Females formed the majority of participants (76%), with a mean age of 42 years. The body mass index of our participants ranged between normal (18%), overweight (52%), and obese (30%). Most (78%) of our patients used CSs, while 22% did not. Duration of use ranged as follows: 40% of the patients used them for 8 weeks. 71% of our patients encountered difficulty in finding appropriate CSs. Conclusions: Noncompliance with CSs is a true challenge. The reasons behind it start from difficulty in finding appropriate size/quality, to patient-related characteristics. These need to be frequently assessed by the prescribing doctor and addressed.

• A two-step orthogonal simulation test was designed to analyze the influence of the material parameters of the GCS and soft tissues on the lower limb-stocking interfacial pressure in the different parts of the lower limb. • The Young's modulus in the wale direction of the GCS and the Poisson's ratio of the GCS are the parameters significantly affecting the lower limb-stocking interfacial pressure in the lower limb. • The mechanical property in the knee section of the GCS has a remarkable influence on the lower limb-stocking interfacial pressure in the calf and in the entire lower limb. Graduated compression stocking (GCS) plays an important role in the treatment of venous disease in the lower limb. However, the effect of the variation in the mechanical properties of the GCS and the soft tissues on the treatment of the venous disease in the lower limb remains unclear. The aim of the present study was to investigate the influence of the material properties of the GCS and soft tissues on the lower limb-stocking interfacial pressure using the orthogonal simulation test. A three-dimensional finite element (FE) model of the lower limb was established using the MRI dataset of a 40-year-old volunteer. The bones, the skin, the veins and the skeletal muscles were reconstructed in the FE model. The FE model of the GCS was generated using the information provided by the manufacturer. Then the parameter sensitivity analysis was performed using a two-step orthogonal simulation test. The first-step orthogonal test showed that the variation in the Young's modulus in the wale direction of the GCS induced a change of 0.37 mmHg in the lower limb-stocking interfacial pressure in the ankle section. The second-step orthogonal test showed that the variations in the Young's modulus in the wale direction of the GCS in the knee section induced the changes of 0.05 mmHg, 0.15 mmHg and 0.60 mmHg in the interfacial pressure in the ankle, the calf and the knee, respectively. In conclusion, the Young's modulus in the wale direction of the GCS and the Poisson's ratio of the GCS are the parameters significantly influencing the lower limb-stocking interfacial pressure. The interfacial pressure in the ankle is not sensitive to the Young's modulus in the wale direction of the GCS in the knee section. However, the interfacial pressures in the calf and knee are sensitive to the Young's modulus in the wale direction of the GCS in the knee section. The benefit of lower limb compression therapy is not limited to chronic venous insufficiency or/and lymphoedema. Thanks to its anti-edema and anti-inflammatory effects, compression therapy is considered a beneficial adjuvant therapy to treat atypical wounds, inflammatory dermatoses, cellulitis, and traumatic wounds in the absence of contraindications. Strict contraindications are limited to severe peripheral arterial disease and decompensated heart failure. The variability of commercially available compression materials and systems, such as short-stretch bandages, multi-component systems, zinc oxide bandages, medical adaptive compression systems, ulcer compression stockings or medical compression stockings, facilitates the adaptation of compression therapy to the individual needs of each patient. The aim of this study was to test whether an investigational two layer stocking exerting 27–29 mmHg pressure at the medial supramalleolar level, but without compression in the foot and heel, is easier to put on and take off than a standard stocking of the same compression class (23–32 mmHg), and also to assess the prevention of diurnal oedema with both types of stocking. This was an open label randomised controlled trial, which included 47 patients. All participants were at least 65 years of age and suffered from chronic venous disease class C3 – C6 in one leg. The primary end point was donning success; secondary endpoints were doffing success, prevention of diurnal oedema over one day, and the comfort of wearing the stocking. Patients were randomly allocated to one of two groups. Both types of compression stocking were compared in each group for ease of donning and doffing in the manner of a crossover study. Subsequently, patients wore the stocking type assigned to their group for a whole day to evaluate comfort and the effect on diurnal leg volume. All participants were able to don the investigational stocking unaided, compared with 75% for the standard stocking (p <.001). Unaided removal success was 100% with the investigational stocking vs. 66% for the standard stocking (p <.001). There was no significant difference in leg volume reduction between the study groups after a day of wear. The investigational stocking was also rated as being more comfortable than the standard stocking (p <.001). The investigational stocking, which has no compression in the foot or heel area, is significantly easier to don and doff, with no inferiority in oedema prevention, compared with a standard stocking of the same compression class. Objectives: To analyze the effect of graduated compression stockings on the venous reflux in the lower limbs of pregnant women. Method: A controlled randomized clinical trial was performed with sixty women: intervention group (n = 30), who used compression stockings, and control group (n = 30). Using duplex-ultrasound, the reflux time and peak reflux velocity in the great saphenous vein and small saphenous vein were analyzed. Results: Great saphenous vein reflux times in the intervention group were 0.13 s at the beginning (initial) and 0.04 s at the end of pregnancy (final) in the right leg and 0.02 s and 0.34 s (p < 0.0001) in the control group. No patient in the intervention group experienced pathological reflux at the end of the pregnancy. There was a significant difference in the reflux time measured from both the great saphenous vein and small saphenous vein and peak reflux velocity between the groups. Objectives: To evaluate the effectiveness of compression stockings in controlling the varicose veins in pregnant women. Method: A prospective controlled randomized clinical trial was performed, including 60 women: intervention group (n = 30), who used compression stockings, and control group (n = 30). Diameters of the great saphenous vein and small saphenous vein in the lower limbs of pregnant women in an orthostatic position were analyzed using Duplex-ultrasound. The symptomatology and CEAP were evaluated. Results: Great saphenous vein diameters in the intervention group were 0.37 cm initial and 0.32 cm final (p < 0.0001) in the right leg and 0.28 cm and 0.38 cm (p < 0.0001) in the control group. CEAP classification presented worsening in the control group (p < 0.0001). The signs and symptoms in the control vs. intervention group: pain (86.67% vs. 23.33%; p < 0.0001), edema (70.00% vs. 33.33%; p = 0.0045), and leg heaviness (93.33% vs. 13.33%; p < 0.0001). Introduction Venous ulcers are a serious clinical consequence of chronic venous insufficiency (CVI). The basis for successful management is compression therapy such as compression bandages, compression stockings and/or elastic tubular compression devices. Aim The purpose of this study is to undertake a retrospective review on the effectiveness of compression therapies in a wound care nurse clinic in Hong Kong. Method Patients in the clinic who presented with lower limb ulcers which showed either the signs and symptoms of CVI with an ankle brachial index >0.8 or where CVI was confirmed by Duplex scan were included in this study (Figure 1). The search period was from the start of treatment (Week 0) up to 24 weeks. Results Time to heal was compared by using the log-rank test; 152 wounds healed within 24 weeks, with an overall healing rate of 93.3%. A total of 90.2% of wounds healed with compression bandages, 93.5% of wounds healed with compression stockings, and 98% of wounds healed with elastic tubular compression devices. The mean healing time was 10 weeks, 8 weeks and 9 weeks respectively. Discussion In view of the various wound sizes between the three groups, there was relatively less difference in the overall wound healing rate among the three groups - relative risk (RR)<1. For ulcers sized >4cm² to ≤12cm², the difference in the wound healing rate between using compression bandages rather than compression stockings was found to be only 0.8% (RR=1.08). Conclusion Taking into account the result from our study and considering both economic factors and patients' convenience, elastic tubular compression devices may be more suitable for our group of patients. This study quantitatively analyzed the effects of 3 h of constant sitting on skeletal muscle oxygenation in the lower extremities, using near-infrared time-resolved spectroscopy (NIRTRS). The effects of compression stockings were also evaluated. Eleven healthy men (age, 30.0 ± 6.7 years) maintained their knee joints at 90° flexion during 3 h of constant sitting and wore a compression stocking on either the right or left leg. The side the stocking was worn was chosen randomly. Subsequently, leg circumference and extracellular water were measured. After 3 h of sitting, both factors increased significantly in uncompressed limbs. Furthermore, intracellular water and muscle oxygenation had significantly decreased. In contrast, extracellular water had not increased in the limbs wearing compression stockings. Furthermore, the increased circumference of compressed limbs was significantly smaller than that of uncompressed limbs. Decreases in oxygenated hemoglobin and total hemoglobin were significantly smaller in compressed limbs than in uncompressed limbs (oxy-Hb; p = 0.021, total-Hb; p = 0.013). Three hours of sitting resulted in decreased intracellular water and increased extracellular water in the lower extremities, leading to reduced blood volume and oxygenation levels in skeletal muscle. A series of self-designed and wove compreesion stockings were used in this preesure comparison experiment. In order to compare the differences of the garment preesure values exerted by the compression stockings with diferent structure parameters among three methods. The experiments were carried out with Flexi force sensors. The preesure value of nine pairs of compreesion stockings on five subjects and the model leg was collected, and the tensile fore of the every section of these stockings was collected to calculate the preesure according to Laplace's equation. The data analysis results show that the preesure values of the compresion stockings obtained by comparing the three methods have great diferences in various testing methods. There is a significant correlation between proceesing parameters and stitch density (SD). The preesure design of the compression stockings should be based on the actual wearing of the preesure. Compression treatment for the patients with chronic disorders such as venous ulcers and varicose veins needs the proper and adequate level of pressure sustainability. This has been a great challenge for health practitioners and stocking manufacturers even till today. There is an imperious need of any research, where internal compression pressure can be controlled or readjusted externally. In line with this, for the first time this study is focused mainly to design and optimize the smart stocking structure by integrating the stress-memory polymeric filament as a main load bearing element. Six different structures were employed to prepare the stocking fabric tubes. All the structures were investigated for pressure analysis and studied the effect of physical parameters such as temperature, strain, and leg radius. It is possible to control the level of massage effect by varying the stocking structures. An empirical relationship is derived, which provides the knowledge for how to control the stocking pressure with structural modifications like never done before. The effect of massage function on blood flow velocity in the popliteal vein on lower limb was objectively measured by Doppler ultrasound scanning. This study also sheds the insight of stocking structural modification for pressure control and provide the benchmark for achieving the efficient compression. This advanced stress-memory polymeric filaments based multifunctional compression stocking provides static pressure, massage effect, and easy size fitting in a more controlled manner for smart compression therapy. This study was conducted to identify the effect of elastic compression stockings on hypotension in patients with intradialytic hypotension. Subjects of this study were 59 patients (29 in the experimental group, 30 in the control group). Elastic compression stockings were applied to patients in the experimental group during hemodialysis. To evaluate effects of treatment, systolic blood pressure, diastolic blood pressure, pulse rate, and hypotension symptom scores were measured. Results indicated the decrease in systolic blood pressure in the experimental group was less than that in the control group. Hypotension symptom scores were significantly decreased in the experimental group. Considering these results, the application of elastic compression stockings can be regarded as an effective nursing intervention to help prevent intradialytic hypotension in patients undergoing hemodialysis. Biocompatibility of six different compression stockings and cytotoxic effects were determined using HaCaT keratinocytes, L929 mouse fibroblasts, primary adult and juvenile keratinocytes Cells were quantified using a luminometric ATP assay and the photometric BCA test. Cytotoxic effects were determined by LDH release. An area-based extraction ratio of 1.25 cm2:mL could be shown to be superior to the weight-based extraction of test material. Extraction medium should be an acidic sweat solution as this helps to recreate in vivo conditions. Monolayer cultures of HaCaT keratinocytes or L929 mouse fibroblasts should be used for testing. Primary adult keratinocytes or primary juvenile keratinocytes can also be used. For the latter, testing under DMEM with FCS is recommended to achieve comparable results. It was found that the compression stockings tested exhibited no negative influence on cell viability in vitro and no direct cytotoxic effects measured as release of LDH. Objective: After foam sclerotherapy in the truncal saphenous vein, the clinical effects of additional eccentric compression has not yet been explored., Methods: Between April 2020 and February 2021, we enrolled 42 patients (84 limbs) who underwent bilateral endovenous combined therapy for great saphenous vein (GSV) reflux. Each patient received the same type of endovenous ablation in both above-the-knee GSVs (laser, radiofrequency, cyanoacrylate glue), and combined foam sclerotherapy was performed on both below-the-knee GSVs. Subsequently, we conducted a prospective randomized, single-blind, within-person study in which each patient's bilateral truncal saphenous vein of the calves underwent two different compression therapies: wearing of a regular class II compression stocking on one side (RC group) and additional eccentric compression on the other side (AC group). The primary end point was the occlusion range (score, 0-10) of the below-the-knee truncal GSV after foam sclerotherapy. The secondary outcomes were the pain score (visual analog scale score range, 0-10) of the paired limb, the required number of additional foam sclerotherapy sessions, compliance with compression therapy, and procedure-related complications., Results: For the above-the-knee GSV, endovenous laser treatment (n = 44), endovenous radiofrequency ablation (n = 14), and endovenous cyanoacrylate glue ablation (n = 26) were performed. The mean subcompression pressure of the medial calf in the supine and standing positions were 16.7 ± 2.34 mm Hg and 24.5 ± 6.6 mm Hg in the RC group and 38.5 ± 5.5 mm Hg and 45.3 ± 8.2 mm Hg in the AC group, respectively (P = .000). The secondary outcomes of pain score, number of additional foam sclerotherapy sessions, and pigmentation were not significantly different statistically between the two groups. The patient-reported satisfaction scores (range, 0-10) on compression at 24 hours postoperatively were 8.03 ± 1.9 for the AC group and 7.98 ± 1.9 for the RC group (P = .317; Wilcoxon signed ranks test). In both groups, the closure rate of the above-the-knee GSV at 1 month postoperatively was 100%. No procedure-related complications were identified within 1 month postoperatively, including no deep vein thrombosis, numbness, or skin necrosis requiring additional medical attention., Conclusions: The 24 hours of additional eccentric compression on the truncal GSV compared with the use of a conventional knee-level stocking only did not yield any clinical advantages in terms of the occlusion range, postoperative pain, need for additional sclerotherapy, or skin pigmentation after foam sclerotherapy.
