4. Impact of proton pump inhibitors on cytochrome P450 activity assessed by 13C-aminopyrine breath test in patients with cirrhosis

Abstract

Background: Chronic use of proton pump inhibitors (PPIs) in patients with impaired liver function may worsen cytochrome P450 (CYP450) activity, predisposing them to clinically relevant drug–drug interactions. The 13C-aminopyrine breath test (13C-ABT) is a non-invasive tool to study CYP450-dependent liver function. Aims: To assess 13C-ABT modifications with different PPIs in patients with cirrhosis. Methods: Sixty consecutive patients with HCV-related cirrhosis and indication to start PPI therapy were randomised to receive omeprazole 20mg/day, esomeprazole 20mg/day, lansoprazole 15mg/day, pantoprazole 40mg/day or rabeprazole 20mg/day. 13C-ABT was performed at baseline and on the 15th day of PPI therapy. Results: At baseline, mean values of max 13C% dose/h and 13C% cum dose at 120minutes did not differ significantly among groups. On the 15th day of therapy, max 13C% dose/h and 13C% cum dose at 120minutes did not significantly differ with respect to baseline for pantoprazole (P=0.184 and P=0.309, respectively) or rabeprazole (P=0.536 and P=0.286, respectively), but were significantly decreased on omeprazole (P=0.013 and P=0.015, respectively), esomeprazole (P=0.009 and P=0.001, respectively), and lansoprazole (P=0.033 and P=0.035, respectively). Conclusions: In patients with cirrhosis, omeprazole, esomeprazole and lansoprazole inhibit microsomal activity while pantoprazole and rabeprazole do not have a significant impact. Should our data be confirmed in larger cohort studies, pantoprazole and rabeprazole could be safely recommended for patients with cirrhosis.

Abstract

Background and Aims: The Diverticular Inflammation and Complication Assessment (DICA) endoscopic classification of diverticulosis and diverticular disease (DD) is currently available. It scores severity of the disease as DICA 1, DICA 2 and DICA 3. Our aim was to assess the agreement on this classification in an international endoscopists community setting. Methods: A total of 96 doctors (82.9% endoscopists) independently scored a set of DD endoscopic videos. The percentages of overall agreement on DICA score and a free-marginal multirater kappa (κ) coefficient were reported as statistical measures of interrater agreement. Results: Overall agreement in using DICA was 91.8% with a free-marginal kappa of 88% (95% CI 80-95). The overall agreement levels were: DICA 1, 85.2%; DICA 2, 96.5%; DICA 3, 99.5%. The free marginal κ was: DICA 1 = 0.753, DICA 2 = 0.958, DICA 3 = 0.919. The agreement about the main endoscopic items was 83.4% (k 67%) for diverticular extension, 62.6% (k 65%) for number of diverticula for each district, 86.8% (k 82%) for presence of inflammation, and 98.5 (k 98%) for presence of complications. Conclusions: The overall interrater agreement in this study ranges from good to very good. DICA score is a simple and reproducible endoscopic scoring system for diverticulosis and DD.

Abstract

Allergy is defined as a hypersensitivity reaction due to specific antibody-mediated or cell-mediated immunologic mechanisms. Epidemiological studies are showing a dramatic increase of allergies in industrialized countries in the last few decades, while remaining stable in developing countries. In 1989 Strachan, hypothesized that the increase in allergic disorders was the result of a lack of infections in early infancy, and in 1998 Wold suggested that, rather than a decrease in viral or bacterial infections, an altered normal intestinal colonization pattern in infancy, could be responsible for the increase in allergies. Germ-free mice were shown to mount an exaggerated allergic airway reaction compared with that seen in colonized mice, indicating the important role of microbe-host interactions in the development of allergic diseases. Infants with food allergies are found to exhibit an imbalance between "beneficial"and potentially harmful bacteria, i.e., decreased Lactobacilli, Bifidobacteria and Enterococcus species and increased coliforms, Staphylococcus aureus and Clostridium species, suggesting that microbial inhabitants of the human body, may play either a pathogenic or protective role in allergies. Based on this data, many clinical trial addressing the use of probiotics in the context of allergic disorders, have been conducted in children. However, currently, no conclusive item may be drawn.

Abstract

Irritable bowel syndrome (IBS) is the commonest functional gastrointestinal disorder, affecting between 10% and 20% of the adult population worldwide. Recently, IBS research has focused on organic components of causative factors such as gut microbiota that in turn may trigger at mucosal level low-grade inflammation and immune activation. To date, four main lines of reasoning support the role of gut microbiota in the pathogenesis of IBS: 1) IBS develops in up to 30% of individuals recovering from acute gastroenteritis "post-infective IBS"; 2) small intestinal bacterial overgrowth (SIBO) is characterized by symptoms like IBS; 3) quantitative and qualitative alterations in gut microbiota "dysbiosis "have been reported in subjects with IBS; 4) IBS symptoms can be improved by treatments (antibiotics, probiotics, prebiotics) that target the microbiota. These arguments open the way to new therapeutic options to treat patients with IBS by the administration of antibiotics, probiotics and perhaps prebiotics. Antibiotic treatment results in significant improvement rates in IBS symptoms; however, side or systemic effects, drug resistance and cost Limit the chronic use. Many studies report a positive effect of probiotics, however, at present randomized placebo-controlled trials taking into account well-defined and measurable endpoints and a long follow-up should be planned. 49. Risk factors in gastric cancer.

Abstract

State of the Art: Gastric cancer (GC) is still a major health problem worldwide due to its frequency, poor prognosis and limited treatment options. At present prevention is likely to be the most effective means of reducing the incidence and mortality from this disease. The most important etiological factors implicated in gastric carcinogenesis are diet and Helicobacter pylori (H. pylori) infection. High intake of salted, pickled or smoked foods, as well as dried fish and meat and refined carbohydrates significantly increased the risk of developing GC while fibers, fresh vegetables and fruit were found to be inversely associated with GC risk. Epidemiological investigations (retrospective, case-control and prospective) and several meta-analyses have demonstrated that concurrent or previous H. pylori infection is associated with an increased risk of GC in respect to uninfected people. H. pylori colonizes gastric mucosa where it induces a complex inflammatory and immune reaction that on time leads to a severe mucosal damage i.e., atrophy, intestinal metaplasia (IM) and dysplasia. The risk of GC is closely related to the grade and extension of gastric atrophy, IM and dysplasia. Perspectives and Conclusions: Today a plausible program for GC prevention means: (1) a correct dietary habit since childhood increasing vegetables and fruit intake, (2) a decrease of H. pylori spread improving family and community sanitation and hygiene, (3) a search and treat H. pylori strategy in offspring of GC, (4) a search and treat H. pylori strategy in patients with chronic atrophic gastritis and intestinal metaplasia (IM), (5) a careful endoscopic and histologic follow-up if precancerous lesions persist irrespective of H. pylori eradication.
