There are limited resources for guidance on the transition from fellowship into a new faculty role in pediatric infectious diseases. This review aims to address this gap and provides a framework for a successful transition that is composed of four essential pillars—(1) stepping into your role, (2) finding your niche, (3) building your network, and (4) self-care—all of which are supported by strong mentorship/sponsorship and continual realignment with one's personal mission statement. In addition to providing general principles and guidance, this review also outlines specific steps that a junior faculty member can take to expand their influence and build a successful, fulfilling career in pediatric infectious diseases. Aims: Children of immigrants have a higher incidence of infectious disease than native children. Our aim was to assess the role of parental socioeconomic position for diagnoses of infectious disease among children of immigrants. Methods: Data from the Norwegian Patient Registry (on diagnoses from secondary/tertiary care), Medical Birth Registry of Norway, and Statistics Norway were linked by the national personal identification number. Seven diagnostic infection categories were included from 2008 onwards. The study population included children born in Norway aged 0–10 years between 2008 and 2018 (N = 988,647). Hazards of infection diagnoses by parental region of origin (adjusted for sex, birth year, parental education, household income and mother's parity) and by parental education and household income were assessed by Cox regression. Results: High parental education was associated with lower hazard of infection diagnoses among children of Norwegian-born parents, but associations were less consistent among children with immigrant parents. Lower household income was related to hazard of most infectious diagnoses among children with both Norwegian-born and immigrant parents. Assessed by region diagnoses of viral and bacterial infections and infections of the musculoskeletal system and soft tissue were not associated with household income. Parental education and household income did not explain differences in hazard of infection diagnoses between children born to immigrant versus Norwegian-born parents. Conclusions: Socioeconomic disadvantage did not explain differences in hazard of being diagnosed with infectious disease in secondary/tertiary care between children with immigrant versus Norwegian-born parents. Objective: To evaluate the differences and similarities in clinical picture, laboratory findings and outcomes between children's with Kawasaki Disease (KD) versus multisystem inflammatory syndrome (MIS-C). Methods: We conducted a retrospective, comparative study from children with Kawasaki Disease (KD) hospitalized in Sinaloa Pediatric Hospital from January 1, 2004, to March 31, 2020, and patients with multisystem inflammatory syndrome (MIS-C) according with World Health Organization (WHO) case definition criteria between May 1, 2020 and May 31, 2021. Demographic characteristics, epidemiological data, clinical features, laboratory findings, type of treatment and clinical outcomes were compared among both groups. Results: Eighty-one patients were included (62 patients with KD and 19 with MIS-C). several clinical and laboratory differences were found among these two entities. Median age was lower in KD vs. MIS-C (25 vs 79 months). Those finding more frequent in KD were male gender (64.5 vs. 47.4%), Mucocutaneous features (93.5 vs. 63.2%): Oral changes (83.9 vs. 63.2%) and extremity changes (77.4 vs. 57.9%); complete form of KD was (75.8 vs. 47.4%), Coronary artery aneurysm (16.1 vs. 11.8%). Secondly, findings that were more frequent in MIS-C than KD were Gastrointestinal involvement (89.4 vs. 9.6%), shock (57.9 vs. 3.2%), neurological symptoms (63.1 vs. 11.2%), kidney involvement (52.6 vs. 16.1%), heart disease in general (52.9% vs 29%): Myocardial dysfunction (23.5 vs. 11.3%) and pericardial effusion (17.6 vs. 2.9%). Lymphocyte count (2.07 + 2.03 vs. 4.28 + 3.01/mm3), platelet count (197.89 + 187.51 vs. 420.37 + 200.08/mm3); serum albumin (2.29 + 0.65 vs. 3.33 + 0.06g/dL), and CPR (21.4 + 11.23 vs. 14.26 + 12.37 mg/dL). KD vs. MIS-C types of Treatment: IVIG (96.8 vs. 94.7%), systemic steroids (4.82 vs. 94.7%), IVIG resistance (19.4 vs. 15.8). Finally, mortality in KD was 0% and 5.3% in MIS-C. Conclusion: Similarities were found in both groups such as fever, rash, and conjunctivitis. Nevertheless, significant differences such as severity of clinical presentation with multi-organ involvement and worst inflammatory response were found more frequently in MIS-C group than KD group, requiring more fluid replacement, use of inotropic agents and higher steroids dosages. Also, mortality rate was higher in patients with MIS-C than patients with KD. Similar results have been observed in other studies where both disorders were compared. [ABSTRACT FROM AUTHOR]

Hepatitis in dogs could be due to infectious, toxic or metabolic causes. Among infectious causes, leptospira, canine adenovirus (CAV 1) and babesia are responsible at varying levels, depending on the local geo-climatic conditions. This study, conducted on 40 dogs with clinical signs of hepatitis in Thrissur, Kerala, revealed occurrence of 2.5 per cent (1/40) for CAV 1, 20 per cent (8/40) for leptospirosis and 22.5 per cent (9/40) for babesiosis. The aetiologies were confirmed by polymerase chain reaction (PCR) targeting the E3 gene for CAV 1, microscopic agglutination test (MAT) for leptospirosis and peripheral blood smear examination for babesiosis. Haematological profile of CAV 1 infected animals revealed thrombocytopaenia, lymphopaenia, monocytopaenia and granulocytosis. Serum biochemical analysis divulged elevated alanine aminotransferase (ALT), alkaline phosphatase (ALP), aspartate aminotransferase (AST), total bilirubin and creatinine, with decreased total protein and albumin. Significant decrease in volume of packed red cells (VPRC), haemoglobin (Hb) and total erythrocyte count (TEC), with significant increase in total leukocyte count (TLC) were evident in dogs positive for leptospirosis. Serum biochemical analysis revealed significant increase in ALP, AST and blood urea nitrogen (BUN) and significant decrease in total protein. Severe anaemia and thrombocytopenia with mild leucocytosis and significant increase in ALP and total bilirubin, with hypoproteinemia were noticed in animals with babesiosis. [ABSTRACT FROM AUTHOR]

Aim: Approximately 96 out of every 1000 children born die worldwide, due to infectious diseases of gastrointestinal and respiratory origin. In Ecuador, 23.2% of all infant deaths in 2018, were due to infectious diseases. Material and method: In the present study, a descriptive research approach was adopted with the objective of examining the specific characteristics of the study population, made up of 20 families with children under 5 years of age who attended the Tulcán Health Center 8 due to infectious diseases. The importance of this study lies in its contribution to scientific knowledge about the situation of infectious diseases in children under 5 years of age in Tulcan Health Center 8. Lyme disease is an infectious disease caused by Borrelia Burgdorferi infection. It is a zoonosis transmitted by ticks and is common in Europe. Its clinical presentation includes an early manifestation, which can cause local and systemic symptoms, and a late form. Erythema migrans, arthritis, facial palsy, meningitis and carditis are possible complications of the disease. We report a case of a 4-year child who presented with fever and upper and lower limbs maculopapular cutaneous rash. Left otoscopy showed the presence of a black, round-shaped foreign body attached to the postero-superior aspect of the external auditory canal (EAC), which was identified as a tick. Prompt removal of the tick was proposed. Considering the age of the patient, general anesthesia was indicated. After orotracheal intubation, the lesion was completely removed under microscopic vision. Subsequently, intravenous antibiotic therapy was continued; the day after, the patient was transferred to a pediatric department where attentive monitoring and therapy were continued, with complete resolution of the symptoms. The number of physicians who are underrepresented in medicine within the pediatric infectious diseases workforce remains disproportionate compared to the US population. Physician workforce diversity plays an important role in reducing health care disparities. Pathways to careers in pediatric infectious diseases require that a diverse pool of students enter medicine and subsequently choose pediatric residency followed by subspecialty training. Efforts must be made to expose learners to pediatric infectious diseases earlier in the education timeline. Along with recruitment and creation of pathways, cultures of inclusivity must be created and fostered within institutions of learning along the entire spectrum of medical training. Aim: The initial cases of COVID-19 appeared in December 2019 and Spain was one of the most affected countries during the first wave (March to June). Since then, HIV HGM BioBank has been restructured as an established Paediatrics and Adults HIV_COVID-19 BioBank that aims at the long-term storage of samples obtained from not only HIV-1, but also from COVID-19 patients and HIV-1_COVID-19 coinfected patients. Methods: HIV HGM BioBank holds high quality biological samples from newborns, children, adolescents and adults with their associated clinical data. Research groups trying to establish large networks focused on research on specific clinical problems in epidemiology, biology, routes of transmission and therapies, are potential users of the clinical samples and of associated data of HIV-1_COVID-19 HGM BioBank. Results: The HIV HGM BioBank is an academic and ethical enterprise complying with all the legal regulatory rules to provide service to the society. HIV_COVID-19 HGM BioBank has been repurposed to offer an important resource for global research of COVID-19 in newborns, children, adolescents, adults and elders to study the biological effect of the pandemic. Conclusion: Herein, we present a description of how HIV HGM BioBank has rapidly become an indispensable structure in modern biomedical research, including COVID-19 research. Multisystemic inflammatory syndrome (MIS-C) diagnosis remains difficult because the clinical features overlap with Kawasaki disease (KD). The study aims to highlight the clinical and laboratory features and outcomes of patients with MISC whose clinical manifestations overlap with or without KD. This study is a retrospective analysis of a case series designed for patients aged 1 month to 18 years in 28 hospitals between November 1, 2020, and June 9, 2021. Patient demographics, complaints, laboratory results, echocardiographic results, system involvement, and outcomes were recorded. A total of 614 patients were enrolled; the median age was 7.4 years (interquartile range (IQR) 3.9–12 years). A total of 277 (45.1%) patients with MIS-C had manifestations that overlapped with KD, including 92 (33.3%) patients with complete KD and 185 (66.7%) with incomplete KD. Lymphocyte and platelet counts were significantly lower in patients with MISC, overlapped with KD (lymphocyte count 1080 vs. 1280 cells × μL, p = 0.028; platelet count 166 vs. 216 cells × 103/μL, p < 0.001). The median serum procalcitonin levels were statistically higher in patients overlapped with KD (3.18 vs. 1.68 µg/L, p = 0.001). Coronary artery dilatation was statistically significant in patients with overlap with KD (13.4% vs. 6.8%, p = 0.007), while myocarditis was significantly more common in patients without overlap with KD features (2.6% vs 7.4%, p = 0.009). The association between clinical and laboratory findings and overlap with KD was investigated. Age > 12 years reduced the risk of overlap with KD by 66% (p < 0.001, 95% CI 0.217–0.550), lethargy increased the risk of overlap with KD by 2.6-fold (p = 0.011, 95% CI 1.244–5.439), and each unit more albumin (g/dl) reduced the risk of overlap with KD by 60% (p < 0.001, 95% CI 0.298–0.559). Conclusion: Almost half of the patients with MISC had clinical features that overlapped with KD; in particular, incomplete KD was present. The median age was lower in patients with KD-like features. Lymphocyte and platelet counts were lower, and ferritin and procalcitonin levels were significantly higher in patients with overlap with KD. What is Known: • In some cases of MIS-C, the clinical symptoms overlap with Kawasaki disease. • Compared to Kawasaki disease, lymphopenia was an independent predictor of MIS-C. What is New: • Half of the patients had clinical features that overlapped with Kawasaki disease. • In patients whose clinical features overlapped with KD, procalcitonin levels were almost 15 times higher than normal. • Lethargy increased the risk of overlap with KD by 2.6-fold in MIS-C patients. • Transient bradycardia was noted in approximately 10% of our patients after initiation of treatment. This review highlights the clinical and epidemiologic characteristics of coronavirus disease 2019 (COVID-19) in children and neonates and contrasts these features with other common respiratory viruses. Although the majority of infections in children are mild, there are many important, as yet, unanswered questions (specifically, the attack rate in children and the role of children as vectors of infection) that will have a major impact on disease in adults. There are no distinctive clinical characteristics that will allow the infectious disease consultant to make the diagnosis without laboratory testing. Severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) appears to be less common, with lower morbidity and mortality than respiratory syncytial virus or influenza, and causes less-severe disease in children with cancer than these more common viruses. The range of severity of infection during pregnancy is comparable to infection in nonpregnant cohorts. Intrauterine infection has been documented but is uncommon. Background Hand, foot, and mouth disease (HFMD) is a common reportable infectious disease that is highly contagious among children in China. This study aimed to characterize the epidemics of HFMD and the serotypes of enterovirus (EV) after the introduction of EV-A71 vaccines in Shanghai, a city in Eastern China. Results A total of 2271 HFMD cases were recruited in this study from May 2017 through October 2020. Among these cases, a male-to-female ratio of 1.6:1 was observed, and the cases were mainly in 1-4 years old (63.1%). Children of all ages had a relatively similar time span between the onset of HFMD and the initial medical visit (P = 0.5192). The cases were reported year-round with peaks in the summer (2018 and 2019) and fall (2017), which was consistent with previous epidemics of the reported HFMD cases in the Shanghai municipality. Among the specimens that tested positive for EV (n = 1855), CV-A6 was predominantly detected (71.1%), followed by CV-A16 (14.2%) and EV-A71 (7.0%). Notably, the number of HFMD cases infected with EV-A71 increased in 2019. Furthermore, 9.2% of the cases had comorbidities, mostly convulsion, bronchopneumonia, and pneumonia; however, they were not correlated with the EV serotypes. In addition, 31.2% (709/2271) of the cases were vaccinated with EV-A71 vaccines. The time span differed significantly between the time of vaccination and the onset of the disease across the groups based on whether the onset was before or after vaccination (P < 0.001). Conclusions CV-A6 is the predominant EV serotype in the epidemic of HFMD in Shanghai; in addition, CV-A16 and EV-A71 may be moderately prevalent. The changing trends in the presence of EV serotypes contributes to the periodicity of the HFMD epidemic. In addition, the minority of HFMD cases may have comorbidities, regardless of the EV serotype. The use of the EV-A71 vaccine has affected the HFMD epidemic. And serotype-specific protection by the EV-A71 vaccine may promote vaccination in children infected with EV-A71 compared to those infected with non-EV-A71 serotypes, which would further change the epidemic scenario of HFMD. Background Telehealth (TH) practices among pediatric infectious disease (PID) specialists prior to the coronavirus disease 2019 (COVID-19) pandemic are largely unknown. Methods In 2019, the Pediatric Infectious Diseases Society (PIDS) Telehealth Working Group surveyed PIDS members to collect data on the use of TH modalities, adoption barriers, interest, extent of curbside consultations (CCs), and reimbursement. Results Of 1213 PIDS members, 161 (13.3%) completed the survey, and the responses of 154 (12.7%) from the United States were included in our report. Medical school (63.6%) and hospital (44.8%) were the commonest work settings with 16.9% practicing in both of them. The most common TH modalities used were synchronous provider-patient virtual visits (20.8%) and synchronous provider-provider consultations (13.6%). TH services included outpatient consultations (48.1%), vaccine recommendations (43.5%), inpatient consultations (39.6%), and travel advice (39.6%). Barriers perceived by respondents included reimbursement (55.8%), lack of experience with TH (55.2%), lack of institutional support (52.6%), lack of administrative support (50%), and cost of implementation (48.7%). Most of the respondents (144, 93.5%) were interested in implementing a wide range of TH modalities. CCs accounted for 1–20 hours/week among 148 respondents. Conclusions Most of the PIDS survey respondents reported low utilization of TH and several perceived barriers to TH adoption before the COVID-19 pandemic. Nonetheless, they expressed a strong interest in adopting different TH modalities. They also reported spending considerable time on non-reimbursed CCs from within and outside their institutions.
