1. A decrease in Fkbp52 alters autophagosome maturation and A152T-tau clearance in vivo

Abstract

The failure of the autophagy-lysosomal pathway to clear the pathogenic forms of Tau exacerbates the pathogenesis of tauopathies. We have previously shown that the immunophilin FKBP52 interacts both physically and functionally with Tau, and that a decrease in FKBP52 protein levels is associated with Tau deposition in affected human brains. We have also shown that FKBP52 is physiologically present within the lysosomal system in healthy human neurons and that a decrease in FKBP52 expression alters perinuclear lysosomal positioning and Tau clearance during Tau-induced proteotoxic stress in vitro. In this study, we generate a zebrafish fkbp4 loss of function mutant and show that axonal retrograde trafficking of Lamp1 vesicles is altered in this mutant. Moreover, using our transgenic HuC::mCherry-EGFP-LC3 line, we demonstrate that the autophagic flux is impaired in fkbp4 mutant embryos, suggesting a role for Fkbp52 in the maturation of autophagic vesicles. Alterations in both axonal transport and autophagic flux are more evident in heterozygous rather than homozygous fkbp4 mutants. Finally, taking advantage of the previously described A152T-Tau transgenic fish, we show that the clearance of pathogenic A152T-Tau mutant proteins is slower in fkbp4+/− mutants in comparison to fkbp4+/+ larvae. Altogether, these results indicate that Fkbp52 is required for the normal trafficking and maturation of lysosomes and autophagic vacuoles along axons, and that its decrease is sufficient to hinder the clearance of pathogenic Tau in vivo.

Abstract

Moderate-size quantum computers are now publicly accessible over the cloud, opening the exciting possibility of performing dynamical simulations of quantum systems. However, while rapidly improving, these devices have short coherence times, limiting the depth of algorithms that may be successfully implemented. Here we demonstrate that, despite these limitations, it is possible to implement long-time, high fidelity simulations on current hardware. Specifically, we simulate an XY-model spin chain on the Rigetti and IBM quantum computers, maintaining a fidelity of at least 0.9 for over 600 time steps. This is a factor of 150 longer than is possible using the iterated Trotter method. Our simulations are performed using a new algorithm that we call the fixed state Variational Fast Forwarding (fsVFF) algorithm. This algorithm decreases the circuit depth and width required for a quantum simulation by finding an approximate diagonalization of a short time evolution unitary. Crucially, fsVFF only requires finding a diagonalization on the subspace spanned by the initial state, rather than on the total Hilbert space as with previous methods, substantially reducing the required resources. We further demonstrate the viability of fsVFF through large numerical implementations of the algorithm, as well as an analysis of its noise resilience and the scaling of simulation errors., Comment: Main text: 14 pages, 11 Figures. 3. Variational Hamiltonian Diagonalization for Dynamical Quantum Simulation

Abstract

Dynamical quantum simulation may be one of the first applications to see quantum advantage. However, the circuit depth of standard Trotterization methods can rapidly exceed the coherence time of noisy quantum computers. This has led to recent proposals for variational approaches to dynamical simulation. In this work, we aim to make variational dynamical simulation even more practical and near-term. We propose a new algorithm called Variational Hamiltonian Diagonalization (VHD), which approximately transforms a given Hamiltonian into a diagonal form that can be easily exponentiated. VHD allows for fast forwarding, i.e., simulation beyond the coherence time of the quantum computer with a fixed-depth quantum circuit. It also removes Trotterization error and allows simulation of the entire Hilbert space. We prove an operational meaning for the VHD cost function in terms of the average simulation fidelity. Moreover, we prove that the VHD cost function does not exhibit a shallow-depth barren plateau, i.e., its gradient does not vanish exponentially. Our proof relies on locality of the Hamiltonian, and hence we connect locality to trainability. 4. Polynomial Scaling of Numerical Diagonalization of the 1D Transverse Field Ising Model into a Commuting Basis using the Pauli Product Representation

Abstract

We report numerical results on the diagonalization of 1D transverse field Ising model. Numerical simulations using the Pauli product representation yield diagonalization from 3 spins to 22 spins in the transverse field Ising model with the number of global Jacobi unitary transformations and number of final terms in diagonalized spin z representation both grew polynomial with the number of spins. These results computed on a classical computer show promise in constructing a quantum circuit to simulate diagonalized generic many-particle Hamiltonians using polynomial number of gates.

Abstract

The identification of novel multifunctional Dirac materials has been an ongoing effort. In this connection quasi 2-dimensional (BEDT-TTF)-based charge transfer salts are widely discussed. Here, we report about the electronic structure of $\alpha$-(BEDT-TTF)$_2$I$_3$ and $\kappa$-(BEDT-TTF)$_2$I$_3$ under a hypothetical substitution of iodine with the halogens bromine, chlorine and fluorine. The decreasing size of the anion layer corresponds to applying chemical strain which increases tremendously in the case of (BEDT-TTF)$_2$F$_3$. We performed structural optimization and electronic structure calculations in the framework of density functional theory, incorporating, first, the recently developed strongly constrained and appropriately normed semilocal density functional SCAN, and, second, van der Waals corrections to the PBE exchange correlation functional by means of the dDsC dispersion correction method. In the case of $\alpha$-(BEDT-TTF)$_2$F$_3$ the formation of over-tilted Dirac-type-II nodes within the quasi 2-dimensional Brillouin zone can be found. Abstract

(BEDT-TFF)$_2$I$_3$ charge transfer salts are reported to show superconductivity and pressure induced quasi two-dimensional Dirac cones at the Fermi level. By performing state of the art ab initio calculations in the framework of density functional theory, we investigate the structural and electronic properties of the three structural phases $\alpha$, $\beta$ and $\kappa$. \edit{We furthermore report about the irreducible representations of the corresponding electronic band structures, symmetry of their crystal structure, and discuss the origin of band crossings. Additionally, we discuss the chemically induced strain in $\kappa$-(BEDT-TTF)$_2$I$_3$ achieved by replacing the Iodine layer with other Halogens: Fluorine, Bromine and Chlorine. In the case of $\kappa$-(BEDT-TTF)$_2$F$_3$, we identify topologically protected crossings within the band structure. Abstract

BackgroundFirst medical contact (FMC)-to-balloon time is associated with outcome of ST-elevation myocardial infarction (STEMI). We assessed the impact on mortality and the determinants of indirect vs. direct transfer to the cardiac catheterization laboratory (CCL).MethodsWe analyzed data from 2,206 STEMI patients consecutively included in a prospective multiregional percutaneous coronary intervention (PCI) registry. The primary endpoint was 1-year mortality. The impact of indirect admission to CCL on mortality was assessed using Cox models adjusted on FMC-to-balloon time and covariables unequally distributed between groups. A multivariable logistic regression model assessed determinants of indirect transfer.ResultsA total of 359 (16.3%) and 1847 (83.7%) were indirectly and directly admitted for PCI. Indirect admission was associated with higher risk features, different FMCs and suboptimal pre-PCI antithrombotic therapy.At 1-year follow-up, 51 (14.6%) and 137 (7.7%) were dead in the indirect and direct admission groups, respectively (adjusted-HR 1.73; 95% CI 1.22–2.45). The association of indirect admission with mortality was independent of pre-FMC and FMC characteristics. Older age, paramedics- and private physician-FMCs were independent determinants of indirect admission (adjusted-HRs 1.02 per year, 95% CI 1.003–1.03; 5.94, 95% CI 5.94 3.89–9.01; 3.41; 95% CI 1.86–6.2, respectively).ConclusionsOur study showed that, indirect admission to PCI for STEMI is associated with 1-year mortality independent of FMC to balloon time and should be considered as an indicator of quality of care. Indirect admission is associated with higher-risk features and suboptimal antithrombotic therapy. Older age, paramedics-FMC and self-presentation to a private physician were independently associated with indirect admission. Our study, supports population education especially targeting elderly, more adequately dispatched FMC and improved pre-CCL management.

Abstract

Abstract Helping neurons to compensate for proteotoxic stress and maintain function over time (neuronal compensation) has therapeutic potential in aging and neurodegenerative disease. The stress response factor FOXO3 is neuroprotective in models of Huntington’s disease (HD), Parkinson’s disease and motor-neuron diseases. Neuroprotective compounds acting in a FOXO-dependent manner could thus constitute bona fide drugs for promoting neuronal compensation. However, whether FOXO-dependent neuroprotection is a common feature of several compound families remains unknown. Using drug screening in C. elegans nematodes with neuronal expression of human exon-1 huntingtin (128Q), we found that 3ß-Methoxy-Pregnenolone (MAP4343), 17ß-oestradiol (17ßE2) and 12 flavonoids including isoquercitrin promote neuronal function in 128Q nematodes. MAP4343, 17ßE2 and isoquercitrin also promote stress resistance in mutant Htt striatal cells derived from knock-in HD mice. Interestingly, daf-16/FOXO is required for MAP4343, 17ßE2 and isoquercitrin to sustain neuronal function in 128Q nematodes. This similarly applies to the GSK3 inhibitor lithium chloride (LiCl) and, as previously described, to resveratrol and the AMPK activator metformin. Daf-16/FOXO and the targets engaged by these compounds define a sub-network enriched for stress-response and neuronally-active pathways. Collectively, these data highlights the dependence on a daf-16/FOXO-interaction network as a common feature of several compound families for prolonging neuronal function in HD.

Obstructive coronary artery disease is the main cause of death worldwide. By tracking events and gaining feedback on patient management, the most relevant information is provided to public health services to further improve prognosis. To create an inclusive and accurate registry of all percutaneous coronary intervention (PCI) procedures performed in France, to assess and improve the quality of care and create research incentives. Also, to describe the methodology of this French national registry of interventional cardiology, and present early key findings. The France PCI registry is a multicentre observational registry that includes consecutive patients undergoing coronary angiography and/or PCI. The registry was set up to provide online data analysis and structured reports of PCI activity, including process of care measures and assessment of risk-adjusted outcomes in all French PCI centres that are willing to participate. More than 150 baseline data items, describing demographic status, PCI indications and techniques, and in-hospital and 1-year outcomes, are captured into local reporting software by medical doctors and local research technicians, with subsequent encryption and internet transfer to central data servers. Annual activity reports and scoring tools available on the France PCI website enable users to benchmark and improve clinical practices. External validation and consistency assessments are performed, with feedback of data completeness to centres. Between 01 January 2014 and 31 December 2022, participating centres increased from six to 47, and collected 364,770 invasive coronary angiograms and 176,030 PCIs, including 54,049 non-ST-segment elevation myocardial infarction cases and 31,631 ST-segment elevation myocardial infarction cases. Fifteen studies stemming from the France PCI registry have already been published. This fully electronic, daily updated, high-quality, low-cost, national registry is sustainable, and is now expanding. Abstract

Objectives: The aim of the study was to assess 24-month efficacy and safety of a novel drug-eluting stent (DES) for femoropopliteal interventions with an innovative stent design and abluminal reservoir technology releasing the amphilimus formulation (sirolimus plus fatty acid) for efficient drug transfer and optimized release kinetics. Background: DES releasing paclitaxel exhibited good patency rates after femoropopliteal interventions. No benefit has been reported when sirolimus or everolimus were used for antiproliferative stent coating. Methods: Within a multicenter, first-in-man, single-arm study, 100 patients with symptomatic femoropopliteal disease (Rutherford category 2-4, mean lesion length 5.8 ± 3.9 cm, 35.0% total occlusions) were treated with the NiTiDES stent (Alvimedica). Two-year follow-up included assessment of primary patency (defined as absence of clinically driven target lesion revascularization or binary restenosis with a peak systolic velocity ratio >2.4 by duplex ultrasound), safety, functional, and clinical outcomes. Results: At 24 months, Kaplan-Meier estimates of primary patency and freedom from clinically driven target lesion revascularization were 83.4% (95% CI: 73.9%-89.6%) and 93.1% (95% CI: 85.3%-96.9%), respectively. Over the study period, 3 deaths were reported with no major limb amputation. Functional and clinical benefits were sustained, as 82.1% of patients fell into Rutherford category 0 or 1 at 24 months, which was associated with preserved improvements in all walking disability questionnaire scores. Conclusions: The 2-year results of the ILLUMINA (Innovative siroLimus seLf expanding drUg-eluting stent for the treatMent of perIpheral disease: evaluation of safety aNd efficAcy) study demonstrate a sustained treatment benefit with a novel sirolimus-eluting stent that also compares favorably to other femoropopliteal intervention trials. Abstract

OBJECTIVES: The aim of the study was to assess 24-month efficacy and safety of a novel drug-eluting stent (DES) for femoropopliteal interventions with an innovative stent design and abluminal reservoir technology releasing the amphilimus formulation (sirolimus plus fatty acid) for efficient drug transfer and optimized release kinetics. BACKGROUND: DES releasing paclitaxel exhibited good patency rates after femoropopliteal interventions. No benefit has been reported when sirolimus or everolimus were used for antiproliferative stent coating. METHODS: Within a multicenter, first-in-man, single-arm study, 100 patients with symptomatic femoropopliteal disease (Rutherford category 2-4, mean lesion length 5.8 ± 3.9cm, 35.0% total occlusions) were treated with the NiTiDES stent (Alvimedica). Two-year follow-up included assessment of primary patency (defined as absence of clinically driven target lesion revascularization or binary restenosis with a peak systolic velocity ratio >2.4 by duplex ultrasound), safety, functional, and clinical outcomes. RESULTS: At 24months, Kaplan-Meier estimates of primary patency and freedom from clinically driven target lesion revascularization were 83.4% (95%CI: 73.9%-89.6%) and 93.1% (95%CI: 85.3%-96.9%), respectively. Over the study period, 3 deaths were reported with no major limb amputation. Functional and clinical benefits were sustained, as 82.1% of patients fell into Rutherford category 0 or 1 at 24months, which was associated with preserved improvements in all walking disability questionnaire scores. CONCLUSIONS: The 2-year results of the ILLUMINA (Innovative siroLimus seLf expanding drUg-eluting stent for the treatMent of perIpheral disease: evaluation of safety aNd efficAcy) study demonstrate a sustained treatment benefit with a novel sirolimus-eluting stent that also compares favorably to other femoropopliteal intervention trials. Abstract

Objective Dizziness is associated with hypertension but there are numerous other causes. The aims of the present study were to describe the characteristics and the clinical correlates of dizziness in a large cohort of hypertensive patients, and to test its prognostic value for all-cause, cardiovascular, and stroke mortality. Methods A total of 1716 individuals from the OLD-HTA Lyon's cohort of hypertensive patients recruited in the 1970s were categorized according to the absence or the presence of dizziness. The dizziness group was subdivided into vertigo and other dizziness excluding vertigo. Results Multiple regression analysis demonstrated that presence of dizziness was predicted by age, female sex, coronary artery disease, and the absence of microalbuminuria. During 30 years of follow-up, we observed 956 deaths, 508 of which with a cardiovascular cause, and 114 fatal acute strokes. In the multivariate Cox regression model, the presence of dizziness had no impact on the risk for all-cause mortality [hazard ratio 0.91; 95% CI (0.78-1.06)], cardiovascular mortality [hazard ratio 0.86; 95% CI (0.70-1.05)], or stroke mortality [hazard ratio 1.27; 95% CI (0.85-1.90)]. In an analysis of the different subgroups of dizziness, only vertigo had a prognostic impact. The increased risk was particularly marked on stroke death with a hazard ratio of 2.43 (95% CI 1.33-4.46) vs. patients without dizziness and 2.22 (95% CI 1.21-4.06) vs. patients with dizziness excluding vertigo. Conclusion Hypertensive patients with dizziness did not have a high-risk profile at baseline, but those with vertigo must be carefully followed over years because of the higher stroke mortality.

Abstract

AIMS Our aim was to report on a survey initiated by the European Association of Percutaneous Cardiovascular Interventions (EAPCI) concerning opinion on the evidence relating to dual antiplatelet therapy (DAPT) duration after coronary stenting. METHODS AND RESULTS Results from three randomised clinical trials were scheduled to be presented at the American Heart Association Scientific Sessions 2014 (AHA 2014). A web-based survey was distributed to all individuals registered in the EuroIntervention mailing list (n=15,200) both before and after AHA 2014. A total of 1,134 physicians responded to the first (i.e., before AHA 2014) and 542 to the second (i.e., after AHA 2014) survey. The majority of respondents interpreted trial results consistent with a substantial equipoise regarding the benefits and risks of an extended versus a standard DAPT strategy. Two respondents out of ten believed extended DAPT should be implemented in selected patients. After AHA 2014, 46.1% of participants expressed uncertainty about the available evidence on DAPT duration, and 40.0% the need for clinical guidance. CONCLUSIONS This EAPCI survey highlights considerable uncertainty within the medical community with regard to the optimal duration of DAPT after coronary stenting in the light of recent reported trial results. Updated recommendations for practising physicians to guide treatment decisions in routine clinical practice should be provided by international societies.

Abstract

Using French transcatheter aortic valve replacement (TAVR) registries linked with the nationwide administrative databases, the study compared the rates of long-term mortality, bleeding, and ischemic events after TAVR in patients requiring oral anticoagulation with direct oral anticoagulants (DOACs) or vitamin K antagonists (VKAs). The choice of optimal drug for anticoagulation after TAVR remains debated. Data from the France-TAVI and FRANCE-2 registries were linked to the French national health single-payer claims database, from 2010 to 2017. Propensity score matching was used to reduce treatment-selection bias. Two primary endpoints were death from any cause (efficacy) and major bleeding (safety). A total of 24,581 patients who underwent TAVR were included and 8,962 (36.4%) were treated with OAC. Among anticoagulated patients, 2,180 (24.3%) were on DOACs. After propensity matching, at 3 years, mortality (hazard ratio [HR]: 1.37; 95% confidence interval [CI]: 1.12-1.67; P < 0.005) and major bleeding including hemorrhagic stroke (HR: 1.64; 95% CI: 1.17-2.29; P < 0.005) were lower in patients on DOACs compared with those on VKAs. The rates of ischemic stroke (HR: 1.32; 95% CI: 0.81-2.15; P = 0.27) and acute coronary syndrome (HR: 1.17; 95% CI: 0.68-1.99; P = 0.57) did not differ among groups. In these large multicenter French TAVR registries with an exhaustive clinical follow-up, the long-term mortality and major bleeding were lower with DOACs than VKAs at discharge. Abstract

L’utilisation de la voie intra nasale (IN) connaît un essor récent et représente une alternative sûre, efficace et non invasive à l’analgésie par voie intraveineuse (IV) recommandée en situation d’urgence. En milieu hostile et/ou en situation multi-victimes, l’abord IV est difficilement réalisable. Intégrées au sein des colonnes d’assaut du Groupe d’Intervention de la Gendarmerie Nationale, les équipes médicales de la 1reAntenne Médicale Spécialisée agissent en zone d’exclusion dans un environnement souvent dégradé. Ce contexte particulier nécessite le juste soin, afin de ne pas surexposer le patient et l’équipe soignante au risque environnant. Nous rapportons la prise en charge d’un traumatisé en situation hostile, présentant une fracture déplacée de membre. L’utilisation de la voie IN a permis l’extraction du patient avant la réduction en ambiance sécurisée. La gestion précoce de la douleur ne bénéficie pas d’un consensus en situation sanitaire exceptionnelle. L’emploi du sufentanil IN semble un moyen sûr efficient et non invasif dans la gestion de l’analgésie en milieu hospitalier. De nouvelles études en milieu pré-hospitalier devraient permettre de valider son efficacité et sa tolérance, préalable indispensable à son emploi en milieu hostile.

Abstract

Souvent évoqué comme l’arme du pauvre, le feu est une arme de terreur en raison de sa facilité de mise en œuvre, de son potentiel destructeur et de sa symbolique. Le groupe d’intervention de la gendarmerie nationale, unité des forces d’interventions spécialisées (FIS), est régulièrement confronté à cette problématique. Le feu représente un paramètre opérationnel à prendre en compte, en tant qu’arme ou moyen de diversion. Il complexifie les schémas tactiques des FIS et rend plus difficiles les conditions d’un assaut. Outre le soutien de la colonne d’assaut, l’objectif des équipes médicales des FIS est la mise en œuvre de gestes de survie adaptés dès la zone d’exclusion ainsi que la gestion du flux d’évacuation des blessés. Aux classiques plaies balistiques pénétrantes viennent s’ajouter des brûlés, intoxiqués aux fumées et polytraumatisés par défenestration. En raison de contraintes d’emport, les équipes médicales tactiques ont des moyens limités pour ces prises en charge spécifiques. Le renfort d’opérateurs de premiers secours au combat permet cependant, selon la situation tactique et les contraintes du flux d’évacuation, de réaliser une prise en charge ciblée, suivant la doctrine du service de santé des armées. Un abord des voies aériennes supérieures, un remplissage vasculaire ou une analgésie précoce sont ainsi envisageables en zone d’exclusion. Une collaboration étroite entre le médecin responsable des FIS et le Directeur des secours médicaux facilite l’extraction des victimes vers la zone de soutien. En cas de lésions hémorragiques associées, l’évacuation du brûlé doit se faire prioritairement dans une structure à capacité de damage control surgery.
