Abstract

Catchment urbanisation results in urban streams being exposed to a multitude of stressors. Notably, stressors originating from diffuse sources have received less attention than stressors originating from point sources. Here, advances related to diffuse urban stressors and their consequences for stream benthic communities are summarised by reviewing 92 articles. Based on the search criteria, the number of articles dealing with diffuse urban stressors in streams has been increasing, and most of them focused on North America, Europe, and China. Land use was the most common measure used to characterize diffuse stressor sources in urban streams (70.7 % of the articles characterised land use), and chemical stressors (inorganic nutrients, xenobiotics, metals, and water properties, including pH and conductivity) were more frequently reported than physical or biological stressors. A total of 53.3 % of the articles addressed the impact of urban stressors on macroinvertebrates, while 35.9 % focused on bacteria, 9.8 % on fungi, and 8.7 % on algae. Regarding ecosystem functions, almost half of the articles (43.5 %) addressed changes in community dynamics, 40.3 % addressed organic matter decomposition, and 33.9 % addressed nutrient cycling. When comparing urban and non-urban streams, the reviewed studies suggest that urbanisation negatively impacts the diversity of benthic organisms, leading to shifts in community composition. These changes imply functional degradation of streams. The results of the present review summarise the knowledge gained to date and identify its main gaps to help improve our understanding of urban streams.

Abstract

Robust demonstration case-studies are needed to evaluate whether improvements in ecosystem condition are translated into improvements in ecosystem services. This research is essential for effectively scaling-up nature-based solutions across Europe and providing the evidence to support transformation agendas in society and industries, and ultimately supporting the ambitions of the European Green Deal. Clear guidance and standardised indicators help monitor the impact of freshwater and wetland restoration measures on European Green Deal goals. These monitoring indicators should include environmental indicators (e.g. for biodiversity, greenhouse gas emissions and water storage capacity) as well as socio-economic indicators such as stakeholder representation, private finance mobilisation and job creation. A combined Before-After-Control-Intervention (BACI) monitoring design is recommended to provide robust evidence and attribute change to the restoration measures. Benchmark (cost or effort) and local/regional context data (e.g. land use, governance) are also needed alongside monitoring of impacts to not just measure what has happened at a site but to understand why it has happened.

Abstract

International audience; Globally, streams represent major CO2 emitters, accounting for approximately 70% of the total flux from inland waters. Hence, a proper understanding of carbon dioxide (CO2) uptake and emissions from streams is crucial for accurately estimating aquatic global carbon emissions. However, diurnal dynamics of these fluxes are still not comprehensively quantified, contributing to a high uncertainty in regional and global carbon budgets. Within the EuroRun project, the 1st Collaborative European Freshwater Science Project for Young Researchers, we aimed to assess spatial and seasonal variability of day- and night-time CO2 fluxes of 34 streams across 11 European countries. CO2 fluxes were measured with drifting flux chambers once per season at midday and midnight. The median CO2 fluxes during the day amounted to 1.4 µmol m-2 h-1 (IQR 0.5 to 3.1) and during the night to 2.1 µmol m-2 h-1 (IQR 0.95 to 3.75). The CO2 fluxes during night exceeded those during day by up to eight times. Interestingly, these differences occurred throughout all seasons and showed no latitudinal patterns. Our results highlight the importance of night-time fluxes to accurately assess CO2 fluxes from streams. As most studies are performed during the day, our survey indicates that global carbon budgets are currently underestimating the amount of CO2 released by streams.

Abstract

International audience; Globally, streams and rivers emit a significant amount of methane, a highly potent greenhouse gas. However, little is known about stream sediment microbial communities, driving the net methane balance in these systems, especially on their distribution and composition at large spatial scales. Within the project Euro Methane we investigated the diversity and abundance of methanogenic archaea and methane-oxidizing bacteria across 16 European streams (from northern Spain to central Sweden) via 16S rRNA sequencing and qPCR. We determined environmental drivers of both abundance and community composition and explored the link to measured potential methane production and oxidation rates of the respective sediments. We found that the community composition of methane-oxidizing bacteria significantly differed among the studied streams, while methanogenic archaea were more homogeneously distributed. Beyond the overall diversity trends, indicator species for stream types were identified. Methanogenic Methanosaeta sp. and methane-oxidizing Methyloglobulus sp. increased with geographical latitude and dominated in headwater streams (orders 1-3) with high oxygen levels and high proportions of pristine land within the catchment, while methanogenic Methanomethylovorans sp. and methane-oxidizing Methylocaldum spp. were more common in larger streams (orders 4-6) with higher discharge and agricultural influence. Potential methane production rates significantly increased with abundance of methanogenic archaea, while potential methane oxidation rates did not show significant correlations with methane oxidizing bacteria, presumably due to the more diverse physiological capabilities of this microbial group. Our study represents a holistic large-scale biogeographical overview of two microbial groups to enhance our understanding of the methane cycle within a heretofore understudied ecosystem.

Abstract

Large-scale studies are needed to identify the drivers of total mercury (THg) and monomethyl-mercury (MeHg) concentrations in aquatic ecosystems. Studies attempting to link dissolved organic matter (DOM) to levels of THg or MeHg are few and geographically constrained. Additionally, stream and river systems have been understudied as compared to lakes. Hence, the aim of this study was to examine the influence of DOM concentration and composition, morphological descriptors, land uses and water chemistry on THg and MeHg concentrations and the percentage of THg as MeHg (%MeHg) in 29 streams across Europe spanning from 41°N to 64 °N. THg concentrations (0.06–2.78 ng L−1) were highest in streams characterized by DOM with a high terrestrial soil signature and low nutrient content. MeHg concentrations (7.8–159 pg L−1) varied non-systematically across systems. Relationships between DOM bulk characteristics and THg and MeHg suggest that while soil derived DOM inputs control THg concentrations, autochthonous DOM (aquatically produced) and the availability of electron acceptors for Hg methylating microorganisms (e.g. sulfate) drive %MeHg and potentially MeHg concentration. Overall, these results highlight the large spatial variability in THg and MeHg concentrations at the European scale, and underscore the importance of DOM composition on mercury cycling in fluvial systems.

13. 4CPS-162 Infliximab serum concentrations, antibody formation and clinical response in psoriatic patients

Abstract

BackgroundA large variability in biopharmaceutical kinetics exists between patients and even within a patient over time. Also, adequate thorough concentrations are linked to response in psoriasis.Therapeutic drug monitoring of biopharmaceuticals (i.e. infliximab–IFX) together with clinical response allows targeted cost-effective dose-adjustments.PurposeThe aim of the present study was to evaluate the real-life association between IFX exposure and clinical outcomes in patients with psoriasis.Material and methodsProspective study in psoriatic patients receiving IFX, between October 2013 and November 2016. We measured Cminand antibodies towards IFX (ATI) using an enzyme-linked immunosorbent assay (ELISA) kit. Data on demographic, analytical and pharmacological variables and Psoriasis Area Severity Index (PASI) were recorded. Mixed models were estimated to evaluate association between IFX through concentrations (Cmin) and clinical response. Statistical analysis was carried out using R.ResultsWe used a total of 155 Cminvalues and ATI from 33 patients (33% females). Weight: 88.2 Kg (±23.5), BMI: 31 Kg/m2(±2.2), PASI at blood sampling: 2.2 (±3.2), PASI score reduction:% (±) (normal weight: 79% (±32.4), overweight: 78.2% (±35) and obese: 76.3% (±31)). Percentage of PASI 75, 90 and 100 response: 78.8%, 60.6% and 54.5%, respectively.The median Cminwas 2.4 mg/L (±2.2) (normal weight: 1.64 mg/L, overweight: 2.68 mg/L and obese: 2.68 mg/L). Six patients tested ATI positive and had undetectable Cmin. Patients achieving PASI 75 had a significantly higher Cminthan non-responders (2.86 vs 1.58 mg/L, p<0.001). Similar results were obtained for PASI 90 and 100 responses.PASI score was significantly influenced by Cmin(IRR: 0.79, 95% CI: 0.69 to 0.91). This remained significant when adjusting by sex, BMI, diagnose, baseline PASI, leukocyte count, ATI status and immunomodulator treatment (IRR: 0.80, 95% CI: 0.70 to 0.93). Same results were obtained for PASI 90 and 100 responses (OR: 1.79, 95% CI: 1.14 to 2.81; OR:.1.79, 95% CI: 1.18 to 2.71 respectively).ConclusionPASI score and achievement of PASI 90 response or higher were significantly influenced by IFX Cmin.The percentage of patients achieving PASI 75 or higher decreased with BMI, while Cminvalues increased.No conflict of interest

Abstract

Globally, inland waters emit over 2 Pg of carbon per year as carbon dioxide, of which the majority originates from streams and rivers. Despite the global significance of fluvial carbon dioxide emissions, little is known about their diel dynamics. Here we present a large-scale assessment of day- and night-time carbon dioxide fluxes at the water-air interface across 34 European streams. We directly measured fluxes four times between October 2016 and July 2017 using drifting chambers. Median fluxes are 1.4 and 2.1 mmol m−2 h−1 at midday and midnight, respectively, with night fluxes exceeding those during the day by 39%. We attribute diel carbon dioxide flux variability mainly to changes in the water partial pressure of carbon dioxide. However, no consistent drivers could be identified across sites. Our findings highlight widespread day-night changes in fluvial carbon dioxide fluxes and suggest that the time of day greatly influences measured carbon dioxide fluxes across European streams.

Large-scale studies are needed to identify the drivers of total mercury (THg) and monomethyl-mercury (MeHg) concentrations in aquatic ecosystems. Studies attempting to link dissolved organic matter (DOM) to levels of THg or MeHg are few and geographically constrained. Additionally, stream and river systems have been understudied as compared to lakes. Hence, the aim of this study was to examine the influence of DOM concentration and composition, morphological descriptors, land uses and water chemistry on THg and MeHg concentrations and the percentage of THg as MeHg (%MeHg) in 29 streams across Europe spanning from 41°N to 64 °N. THg concentrations (0.06-2.78 ng L-1) were highest in streams characterized by DOM with a high terrestrial soil signature and low nutrient content. MeHg concentrations (7.8-159 pg L-1) varied non-systematically across systems. Relationships between DOM bulk characteristics and THg and MeHg suggest that while soil derived DOM inputs control THg concentrations, autochthonous DOM (aquatically produced) and the availability of electron acceptors for Hg methylating microorganisms (e.g. sulfate) drive %MeHg and potentially MeHg concentration. Overall, these results highlight the large spatial variability in THg and MeHg concentrations at the European scale, and underscore the importance of DOM composition on mercury cycling in fluvial systems.

Abstract

Background: Some patients with severe asthma may benefit from treatment with biologics, but evidence has been mostly collected from randomized controlled trials (RCTs), in which patients' characteristics are different from those encountered in asthma patients in the real-world setting. The aim of this study was to describe the clinical features of complete responders versus non-complete responders to long-term treatment with biologics in patients with severe asthma attended in routine daily practice., Methods: Data of a cohort of 90 patients with severe asthma who were treated with biologics (omalizumab, benralizumab, and mepolizumab) for at least 12 months and were followed up to March 2022. Data recorded included clinical characteristics and effectiveness of treatment (exacerbation, Asthma Control Test [ACT] score, lung function, use of maintenance oral corticosteroids [mOCS]), FeNO, and blood eosinophils at baseline, at 12 months, and at the end of follow-up. Complete response is considered if, in addition to not presenting exacerbations or the use of mOCS, the ACT score was >20 and, the FEV 1 >80% predicted., Results: An improvement in all asthma control parameters was observed after 12 months of treatment and a mean follow-up of 55 months. After 12 months of treatment 27.2% of patients met the criteria of complete response and this percentage even increased to 35.3% at the end of follow-up. Long-term complete response was associated to better lung function with mepolizumab and omalizumab treatment and to less previous exacerbations in the benralizumab group. The main cause of not achieving a complete response was the persistence of an airflow obstructive pattern., Conclusions: This study shows that omalizumab, benralizumab, and mepolizumab improved the clinical outcomes of patients with severe asthma in a clinic environment with similar effect sizes to RCTs in the long term follow-up. Airflow obstruction, however, was a predictor of a non-complete response to biologics.

17. Impact of COVID19 pandemic on patients with rare diseases in Spain, with a special focus on inherited metabolic diseases.

Abstract

Introduction: The Covid-19 pandemic soon became an international health emergency raising concern about its impact not only on physical health but also on quality of life and mental health. Rare diseases are chronically debilitating conditions with challenging patient care needs. We aimed to assess the quality of life and mental health of patients with rare diseases in Spain, with a special focus on inherited metabolic disorders (IMD)., Methods: A prospective case-control study was designed, comparing 459 patients suffering from a rare disease (including 53 patients with IMD) and 446 healthy controls. Quality of life (QoL) and mental health were assessed using validated scales according to age: KINDL-R and the Pediatric Symptom Checklist (PSC) for children and the WhoQoL-Bref questionnaire, GAD and PHQ-9 in adults., Results: First, children and adults (but not adolescents) with IMD showed greater psychological effects than controls ( p  = 0.022, p  = 0.026 respectively). Second, when comparing QoL, only adult patients with IMD showed worse score than controls (66/100 vs 74,6/100 respectively, p  = 0.017). Finally, IMD had better quality of life than other rare neurological and genetic diseases ( p  = 0.008) or other rare diseases ( p  < 0.001 respectively) but similar alteration of the mental status., Conclusions: Our data show that the pandemic had a negative impact on mental health that is more evident in the group of patients with IMD. Young age would behave as a protective factor on the perception of QoL. Furthermore, patients with IMD show a better QoL than other rare diseases., (© 2023 The Authors.)

18. Duration of water flow interruption drives the structure and functional diversity of stream benthic diatoms.

Abstract

Flow cessation affects river ecosystems submitted to low precipitation and increased water demand, and creates unfavourable conditions to aquatic biological communities. Diatoms are amongst the most sensitive biological groups to hydric stress, making them good indicators of preceding hydrological conditions. We here analyse the response of diatom assemblages to the duration and frequency of non-flow periods in 23 Mediterranean temporary streams. All of them experienced a strong decrease in water flow during summer, leading to a period of flow cessation. In addition, other ten permanent streams (zero dry days during the study period) were included in the study for comparative purposes. Temporary and permanent streams showed similar diatom species richness, evenness, and alpha diversity. However, beta diversity was higher in temporary than permanent streams, regardless rare taxa (<1% occurrence) had similar numbers in the two types of streams. Alpha diversity, richness, and evenness of diatom assemblages in the temporary streams changed with the duration of the non-flow period. Durations of 50-100 days were associated to higher alpha diversity, richness, and evenness, but longer non-flow periods caused their decrease. Diatoms thriving under the most extreme conditions were mostly aerophilic, pioneer, and motile taxa. The proportion of aerophilic diatom taxa increased beyond 100 dry days, particularly in those sites receiving more intense solar radiation. Overall, the taxonomical and functional composition of diatom assemblages mostly responded to the duration of the non-flow period, irrespectively of these being consecutive or not. This study shows that diatom assemblages from temporary streams contain taxa adapted to non-flow conditions, but remain vulnerable to further reductions of water flow associated to climate or global change. 22. A method for converting a disease registry's case-load to a new classification of diagnostic codes.

Abstract

A method is described for converting the diagnostic codes of a disease registry from the eighth to the ninth revision of the International Classification of Diseases. The files of the Health Surveillance Registry contain records of nearly 100 000 cases with ICDA-8 coded disabilities. These data are used to determine incidence, prevalence, and other statistics relating to morbidity in the Province of British Columbia. The steps are outlined to convert the disability codes to ICD-9, and include the development of conversion tables, techniques for both computer and manual conversion of the codes for each case, and a description of the resources needed to complete the conversion within a set time-limit. The conversion methodology outlined here can be adapted to future revisions of disease classifications.
