2. Filtering Pseudo-Bleb Secondary to Sutured Posterior Chamber Intraocular Lens with Implications for Ocular Surgery in Marfan Syndrome

Abstract

A 51-year-old female Caucasian patient with a history of Marfan syndrome and multiple previous bilateral ocular surgeries presented with increasing discomfort, epiphora, and blurred vision in her right eye for a few months. On examination, we found an overhanging cystic Seidel-positive filtering pseudo-bleb with hypotony in her right eye and a smaller Seidel-negative filtering pseudo-bleb in the left eye secondary to sutured intraocular lens (IOL) in both eyes. Intraoperatively, two full-thickness scleral defects were found close to the limbus, suggesting a melting flap in the location of the previous sutured IOL implant in the right eye. The defects were plugged with two pieces of donor sclera and covered with a larger donor scleral patch, the ischemic conjunctiva was excised, and the remaining healthy conjunctiva was advanced and sutured along the limbus. At last follow-up, intraocular pressure and vision in the right eye increased to preoperative levels, and no pseudo-bleb or leak was detected.

Abstract Purpose The efficacy and safety of the trabecular micro-bypass stents (iStent and iStent inject) have been well documented in various open-angle glaucoma subtypes. However, their outcomes remain understudied in normal-tension glaucoma (NTG). The present study aimed to assess the 1-year outcomes related to the implantation of two second-generation trabecular micro-bypass stents (iStent inject) concomitant with cataract surgery (CE-TMS), exclusively in eyes with NTG. Methods This multi-center, consecutive case series included eyes with cataract and normal-tension glaucoma that underwent CE-TMS to reduce intraocular pressure or glaucoma medication use. The 12-month efficacy measures included change in average intraocular pressure (IOP) and medication burden. Safety included change in best-corrected visual acuity, cup-to-disc ratio, visual field mean-deviation and retinal nerve fiber layer thickness. Intra- or postoperative adverse events were noted. Results A total of 62 eyes with mild-to-severe NTG and average preoperative IOP of 15.82 ± 2.94 mmHg on 1.50 ± 1.28 glaucoma medications were included. Abstract

Objective To describe the development and implementation of a web-based high-quality data collection tool to track the outcomes of glaucoma treatments in routine practice.Methods and analysis This is a prospective observational registry study. An international steering committee undertook an iterative structured process to define a minimum, patient-centred data set designed to track outcomes of glaucoma treatment. The outcomes were coded into a web-based programme allowing easy access for rapid data entry. Clinicians receive personal reports enabling instant audit of their outcomes. Analyses of aggregated anonymised data on real-world outcomes are analysed and periodically reported with the goal of improving patient care.Results The minimum data set developed by the international steering committee includes the following: a baseline visit captures 13 mandatory fields in order to accurately phenotype each patient’s subtype of glaucoma and to allow comparison between services, and a follow-up visit includes only four mandatory fields to allow completion within 30 s.Currently, there are 157 surgeons in 158 ophthalmology practices across Australia and New Zealand who are registered. These surgeons are tracking 5570 eyes of 3001 patients and have recorded 67 074 visits. The median number of eyes per surgeon is 22 eyes with a range of 1–575. The most common glaucoma procedure, excluding cataract surgery, is iStent inject, with 2316 cases.Conclusion This software tool effectively facilitates data collection on safety and efficacy outcomes of treatments for different subgroups of glaucoma within a real-world setting. It provides a template to evaluate new treatments as they are introduced into practice.

(iStent inject®) with phacoemulsification.Materials and Methods: This multicenter retrospective study of iStent inject implantation with phacoemulsification included data from eight surgeons across Australia. Eyes with cataract and mild to advanced glaucoma [predominantly primary open-angle (POAG), primary angle closure (PAC), or normal-tension (NTG) glaucoma] or ocular hypertension (OHT) were included. Study assessments included intraocular pressure (IOP); number of ocular hypotensive medications; proportions of eyes with 0, 1, 2, or ≥ 3 IOP-lowering medications; IOP ≤ 15 mmHg or ≤ 18 mmHg; visual fields (VF); retinal nerve fiber layer thickness (RNFL); central corneal thickness (CCT); intraoperative complications; adverse events; and secondary surgeries.Results: A total of 273 eyes underwent surgery and had 36-month follow-up. At 36 months versus preoperative, mean IOP decreased by 15.5% (16.4± 4.6 mmHg to 13.9± 3.5 mmHg; p< 0.001), and 70.3% of eyes achieved IOP of ≤ 15 mmHg (versus 49.1% preoperatively; p< 0.001). The mean medication burden decreased by 68.5% (from 1.51± 1.17 to 0.48± 0.89 medications; p< 0.001); 71.4% of eyes were medication-free (versus 21.6% preoperatively; p< 0.001), while 6.2% of eyes were on ≥ 3 medications (versus 22.3% preoperatively; p< 0.001); 96.3% of eyes maintained or reduced medications vs preoperative. Significant IOP and medication reductions occurred across glaucoma subtypes (POAG, PAC, NTG, OHT): 13– 22% for IOP (p< 0.05 for all) and 42– 94% for medication (p< 0.05 for all). Favorable safety included few adverse events; stable VF, RNFL, and CCT; and filtering surgery in only 8 eyes (2.9%) over 3 years.Conclusion: In this multicenter cohort from 8 surgeons across Australia, significant IOP and medication reductions were sustained through 3 years after iStent inject implantation with phacoemulsification. To describe the efficacy and safety of iStent implantation prior to phacoemulsification in manual as well as femtosecond laser-assisted cataract surgery (FLACS), and highlight this approach as a reasonable, if not necessary, step to advance one's ability and confidence in the use of microinvasive glaucoma surgery (MIGS) technology in phakic patients.A retrospective consecutive case series of patients with open angle glaucoma or ocular hypertension who underwent iStent inject implantation followed by cataract surgery (manual or FLACS). All cases underwent postoperative video review and were assessed and classified for intraoperative lens injury and hyphaema. Postoperative data included intraocular pressure (IOP), medication usage and adverse events.Sixty-three eyes (Early implantation is safe, maximizes corneal clarity and angle visualization, avoids the risk of non-implantation due to surgical complications, and has a high success rate in both manual cataract surgery and the setting of FLACS.The conventional recommended approach of iStent implantation following cataract extraction has been adopted by many, however, with the advent of stand-alone procedures and concern about potential lens injury, there is an opportunity to gain experience with minimal risk in patients undergoing MIGS procedures combined with cataract surgery by implanting iStents at the start of the procedure. Abstract

Data on geospatial entities is increasingly available from various fields, such as GIS, CIM (City Information Modeling), and BIM (Building Information Modeling). They are described using different data models, such as IFC and CityGML, and are composed of both semantic and geometric data. Integrating this heterogeneous data to create a unified view of geospatial entities requires the use of technologies and methods adapted to each type of data. In this work, we propose a data integration methodology that leverages geospatial and semantic web technologies to provide data views suited to facilitating spatial and semantic navigation of these data. Concerning semantic data, we propose a model-driven data transformation process for limiting data loss during integration and reducing semantic heterogeneity between data from different urban information domains. For geometric data, we propose data transformations towards data formats for efficiently sharing, storing and visualizing 2D and 3D data. "There could be no society, no family, and no social recognition without children. The way in which children were perceived, integrated, and raised within the family and the community established the very foundations of Egyptian society. Childhood in Ancient Egypt is the most comprehensive attempt yet published to reconstruct the everyday life of children from the Predynastic period to the end of the New Kingdom. Drawing on a vast wealth of textual, iconographic, and archaeological sources stretching over a period of 3,500 years, Amandine Marshall pieces together the portrait of a society in which children were ever-present in a multiplicity of situations. The ancient sources are primarily the expressions of male adults, who were little inclined to take an interest in the condition of the child, and the feelings of young Egyptians and all that touches on their emotional state can never be deduced from the sources. Abstract

Future thinking (FT) is the cognitive ability that enables humans to mentally imagine and pre-experience future events. Despite a sharp increase in basic research on future thinking in the last decade, we note the lack of transfer to Human Factors and Ergonomics (HF&E), although the field is increasingly concerned by future artefacts and although theory building on future-oriented HF&E has begun (e.g. prospective ergonomics). This article gives an overview of key findings regarding future thinking, namely i) underlying cognitive mechanisms, ii) functions and obstacles, iii) common descriptors of future thinking, iv) recommendations as to the possible improvement of future thinking during user research and, v) methods for the assessment of future thinking abilities. Abstract

Sharing is a complex system of resource allocation at the crossroad of developing businesses (e.g., the 'sharing economy') and promising societal and environmental impacts. Yet, it is suffering from adoption problems (e.g., vandalism, unwarranted privatisation, reluctance) that call for more ergonomics research. In order to contribute to research on sharing within human factors and ergonomics, the evolution of sharing is synthesised to highlight its roots, developments and societal outcomes. A subset of sharing activities, sharing physical objects, is identified. In order to provide common ground to researchers and practitioners it is defined as 'a cooperative and distributive behavior where a user does not have exclusive interactions with a material resource which is, then, enjoyed in common'. A lack of research on shared physical objects within human factors and ergonomics is identified. Finally, seven shareability criteria (Governance, Utility, Balanced appropriation, Cooperativity, Durability, Decontamination, Onboarding/Support) are proposed in order to support practitioners when designing experiences that include sharing physical things. International audience; In the Greco-Roman catacombs of Alexandria, uniquely decorated tombs from the time when religious boundaries blurred and syncretistic beliefs flourished have long been known. But it was only in 1993 that researchers discovered faint traces of paintings on walls previously thought to be blank, or underneath other painted scenes: the hidden scenes could be partly made out and photographed using ultraviolet light. Abstract

Intraocular pressure (IOP) peaks and means have been considered important factors for glaucoma onset and progression. However, peak IOP detection depends only on appropriated IOP checks at office visits, whereas the mean IOP requires longitudinal IOP data collection and may be affected by the interval between visits. Also, IOP peak assessment is necessary to verify if the peak pressure of a given patient is in target range, to evaluate glaucoma suspect risk, the efficacy of hypotensive drugs and to detect early loss of IOP control. The water-drinking test has gained significant attention in recent years as an important tool to evaluate IOP peaks and instability. The main objective of this review was to present new findings and to discuss the applicability of the water-drinking test in glaucoma management.

Abstract

In this book we have set out to reconstruct and share the memories of these teaching experiences lived during the pandemic. With this we try to respond to a double need. First, the need not to forget what happened and how it was lived from one's own experiences overflowing with subjectivities. Second, the need to rescue these experiences and remember them, in other words, to make memories to think about the future. In this way, we maintain that the future of teaching and professional work cannot be thought of without taking into account past experiences and, in this sense, those experienced during the pandemic., En el presente libro nos hemos propuesto reconstruir y compartir las memorias de estas experiencias docentes vividas durante la pandemia. Con ello intentamos responder a una doble necesidad. Primero la necesidad de no olvidar lo que ocurrió y cómo se ha vivido desde las propias experiencias desbordadas de subjetividades. Segundo, la necesidad de rescatar estas experiencias y de rememorarlas, en otras palabras, de hacer memoria para pensar el futuro. De esta forma, sostenemos que el futuro del quehacer docente y profesional no se puede pensar sin tomar en cuenta las experiencias pasadas y, en este sentido, aquellas vividas durante la pandemia.

Abstract

Electric double layer transistor configurations have been employed to electrostatically dope single crystals of insulating SrTiO(3). Here we report on the results of such doping over broad ranges of temperature and carrier concentration employing an ionic liquid as the gate dielectric. The surprising results are, with increasing carrier concentration, an apparent carrier-density dependent conductor-insulator transition, a regime of the anomalous Hall effect, suggesting magnetic ordering, and finally the appearance of superconductivity. The possible appearance of magnetic order near the boundary between the insulating and superconducting regimes is reminiscent of effects associated with quantum critical behavior in some complex compounds.
