1. Serum Metal Levels Following Total Ankle Arthroplasty

Category: Ankle Arthritis; Basic Sciences/Biologics Introduction/Purpose: Wear debris from metal implants can result in local and systemic complications and has become a growing concern. Potential complications include implant loosening, peri-implant osteolysis, adverse local tissue reaction with destruction of surrounding soft tissues, and end-organ deposition leading to cardiac and neurologic compromise. Implant modularity is known to be an important factor in the development of elevated serum metal levels and associated complications. Such components are susceptible to mechanically assisted crevice corrosion and fretting damage that results in metal ion release. Little is known regarding circulating metal levels in patients following total ankle arthroplasty (TAA). The purpose of this study is to quantify post-operative serum metal levels in patients who have undergone primary TAA and to evaluate the impact of tibial component design. Methods: Twenty patients having undergone primary, unilateral TAA at a single institution from January 2010–January 2023 were recruited. Ten patients with a non-modular tibial component using the Stryker INFINITY Total Ankle System were included in the resurfacing cohort, and 10 patients with a modular tibial component using the Stryker INBONE Total Ankle System were included in the stemmed cohort. A control group consisted of 21 patients without implants from a previous study at the same institution. The tibial components of both implant systems are composed of titanium, aluminum, vanadium and plasma spray coated with unalloyed titanium. Talar components are fabricated from cobalt, chromium, molybdenum alloy. An ultra-high molecular weight polyethylene insert was used in both cohorts. Following study enrollment, subjects attended a one-time visit at minimum 12 months post-operative. Primary outcome measures included serum cobalt, chromium, and titanium levels measured by ICP-MS. Secondary outcome measures included patient-reported outcome measurement surveys. Results: There were no differences in age, gender, body mass index, or follow-up between the control, resurfacing implant, and stemmed implant cohorts. Serum titanium levels were greater in the resurfacing cohort (0.619 µg/L) and stemmed cohort (0.993 µg/L) than in the control cohort (0.150 µg/L) (p< 0.001 and p< 0.001 respectively; Table 1). Serum titanium levels were greater in the stemmed group than in the resurfacing group (p=0.003; Table 1). Serum cobalt and chromium levels did not differ between study cohorts or controls (p>0.05; Table 1). Patient-reported outcomes did not differ between groups (p>0.05). Conclusion: Results from this study provide the first evidence that TAA is associated with elevated serum metal levels. In this study, serum titanium levels were elevated following TAA while chromium and cobalt levels were not elevated. Patients with stemmed implants demonstrated higher titanium levels than patients with resurfacing implants. Possible explanations for these findings may be related simply to the greater surface area of the stemmed implants or greater release of metallic debris from tribocorrosion at the numerous modular junctions. These findings prompt the need for additional research in this area.

Abstract

Olfactory groove meningiomas (OGMs) are rare, intracranial tumors located in the anterior cranial fossa that grow along the nerves between the brain and the nose. This report highlights a case in which a 74-year-old Nigerian male presented with clinical features of anosmia and headache as well as pertinent CT and MRI findings that typically occur in female patients. The study reviews the importance of imaging as a means of achieving a timely diagnosis and accurately measuring the size and magnitude of the disease as it serves as a guide to surgeons when planning for intervention.

Abstract

Background: The coronavirus disease-2019 (COVID-19) pandemic significantly affected management of cardiovascular disease around the world. The effect of the pandemic on volume of cardiovascular diagnostic procedures is not known. Objectives: This study sought to evaluate the effects of the early phase of the COVID-19 pandemic on cardiovascular diagnostic procedures and safety practices in Asia. Methods: The International Atomic Energy Agency conducted a worldwide survey to assess changes in cardiovascular procedure volume and safety practices caused by COVID-19. Testing volumes were reported for March 2020 and April 2020 and were compared to those from March 2019. Data from 180 centers across 33 Asian countries were grouped into 4 subregions for comparison. Results: Procedure volumes decreased by 47% from March 2019 to March 2020, showing recovery from March 2020 to April 2020 in Eastern Asia, particularly in China. The majority of centers cancelled outpatient activities and increased time per study. Practice changes included implementing physical distancing and restricting visitors. Although COVID testing was not commonly performed, it was conducted in one-third of facilities in Eastern Asia. The most severe reductions in procedure volumes were observed in lower-income countries, where volumes decreased 81% from March 2019 to April 2020. Conclusions: The COVID-19 pandemic in Asia caused significant reductions in cardiovascular diagnostic procedures, particularly in low-income countries. Further studies on effects of COVID-19 on cardiovascular outcomes and changes in care delivery are warranted.

Abstract

Background:. The accurate assessment of physician academic productivity is paramount and is frequently included in decisions for promotion and tenure. Current metrics such as h-index have been criticized for being biased toward older researchers and misleading. The relative citation ratio (RCR) is a newer metric that has been demonstrated within other surgical subspecialties to be a superior means of measuring academic productivity. We sought to demonstrate that RCR is a valid means of assessing academic productivity among plastic surgeons, and to determine demographic factors that are associated with higher RCR values. Methods:. All Accreditation Council for Graduate Medical Education-accredited plastic and reconstructive surgery residency programs and faculty throughout the United States were compiled from the American Council of Academic Plastic Surgeons website. Demographic information was obtained for each surgeon via the program’s website, and RCR data were obtained utilizing iCite, a bibliometrics tool provided by the National Institutes of Health. Surgeons were excluded if any demographic or RCR data were unavailable. Results:. A total of 785 academic plastic surgeons were included in this analysis. Surgeons who belonged to departments with more than six members had a higher median RCR (1.23). Increasing academic rank (assistant: 12.27, associate: 24.16, professor: 47.58), chief/chairperson status (47.58), male gender (25.59) and integrated model of residency training program (24.04) were all associated with higher median weighted RCR. Conclusions:. RCR is a valid metric for assessing plastic surgeon academic productivity. Further research is warranted in assessing disparities among different demographics within academic plastic surgery.

Abstract

What allows a planet to be both within a potentially habitable zone and sustain habitability over long geologic time? With the advent of exoplanetary astronomy and the ongoing discovery of terrestrial-type planets around other stars, our own solar system becomes a key testing ground for ideas about what factors control planetary evolution. Mars provides the solar system's longest record of the interplay of the physical and chemical processes relevant to habitability on an accessible rocky planet with an atmosphere and hydrosphere. Here we review current understanding and update the timeline of key processes in early Mars history. We then draw on knowledge of exoplanets and the other solar system terrestrial planets to identify six broad questions of high importance to the development and sustaining of habitability (unprioritized): (1) Is small planetary size fatal? (2) How do magnetic fields influence atmospheric evolution? (3) To what extent does starting composition dictate subsequent evolution, including redox processes and the availability of water and organics? (4) Does early impact bombardment have a net deleterious or beneficial influence? (5) How do planetary climates respond to stellar evolution, e.g., sustaining early liquid water in spite of a faint young Sun? (6) How important are the timescales of climate forcing and their dynamical drivers? Finally, we suggest crucial types of Mars measurements (unprioritized) to address these questions: (1) in situ petrology at multiple units/sites; (2) continued quantification of volatile reservoirs and new isotopic measurements of H, C, N, O, S, Cl, and noble gases in rocks that sample multiple stratigraphic sections; (3) radiometric age dating of units in stratigraphic sections and from key volcanic and impact units; (4) higher-resolution measurements of heat flux, subsurface structure, and magnetic field anomalies coupled with absolute age dating. Understanding the evolution of early Mars will feed forward to understanding the factors driving the divergent evolutionary paths of the Earth, Venus, and thousands of small rocky extrasolar planets yet to be discovered.

Abstract

Introduction: Lung cancer is the most common cause of cancer deaths worldwide, accounting for 1.8 million deaths each year. Only 20% of lung cancer cases are reported to occur in low- and middle-income countries. An estimated 1.5% of all Ethiopian cancers involved the lung; however, no nationwide cancer registry exists in Ethiopia. Thus, accurate data on clinical history, histopathology, molecular characteristics, and risk factors for lung cancer are not available. The aim of this study was to describe the clinical, radiologic, and pathologic characteristics, including available molecular profiles, for lung cancer at Tikur Anbessa Specialized Hospital (TASH), the main tertiary referral center in Addis Ababa, Ethiopia. Methods: A cross-sectional study was conducted at TASH among 146 patients with pathologically confirmed primary lung cancer, diagnosed from 2015 to 2019 and recorded in the Addis Ababa Cancer Registry at TASH. Clinical data were extracted from patient medical records, entered into a Research Electronic Data Capture database, and analyzed using Statistical Package for the Social Sciences statistical software. Variables collected included sociodemographics, personal exposures, comorbidities, clinical manifestations at presentation, chest imaging results, diagnostic procedures performed, histopathological classification, cancer staging, and type of treatment (if any). A subset of lung biopsies fixed in formalin for 2 to 7 days, which could be retrieved from the files of the Pathology Department of TASH, were reviewed, and molecular analysis was performed using next-generation sequencing to identify the tumor-oncogenic drivers. Results: Among the 146 patients studied, the mean (SD) age was 54 plus or minus 13 years; 61.6% (n = 90) were male and 25.3% (n = 37) had a history of tobacco use. The most common clinical manifestations included cough (88.4%, n = 129), chest pain (60.3%, n = 88), and dyspnea (53.4%, n = 78). The median duration of any symptoms was 6 months (interquartile range: 3–12 mo). The most common radiologic features were lung mass (84.9%, n = 129) and pleural effusion (52.7%, n = 77). Adenocarcinoma accounted for 35.7% of lung cancers (n = 52) and squamous cell carcinoma 19.2% (n = 28) from those specimens was reported. Among patients on whom staging of lung cancer was documented, 92.2% (n = 95) of the subjects presented at advanced stages (stages III and IV). EGFR mutation, exons 19 and 20, was found in 7 of 14 tissue blocks analyzed. No specific risk factors were identified, possibly reflecting the relatively small sample size and limited exposures. Conclusions: There are marked differences in the presentation, risk factors, and molecular characteristics of lung cancer in Ethiopia as compared with other African and non-African countries. Adenocarcinoma was the most common histologic type of lung cancer detected in our study, similar to findings from other international studies. Nevertheless, compared with high-income countries, lung cancer in Ethiopia presents at a younger age, a later stage, and without considerable personal tobacco use. The relatively higher prevalence of EGFR mutation, from the limited molecular analyses, suggests that factors other than smoking history, such as exposure to biomass fuel, may be a more important risk factor. Country-specific screening guidelines and treatment protocols, in addition to a national tumor registry and greater molecular mutation analyses, are needed to improve prevention and management of lung cancer in Ethiopia.

Abstract

International audience; The geological units on the floor of Jezero crater, Mars, are part of a wider regional stratigraphy of olivine-rich rocks, which extends well beyond the crater. We investigate the petrology of olivine and carbonate-bearing rocks of the Séítah formation in the floor of Jezero. Using multispectral images and x-ray fluorescence data, acquired by the Perseverance rover, we performed a petrographic analysis of the Bastide and Brac outcrops within this unit. We find that these outcrops are composed of igneous rock, moderately altered by aqueous fluid. The igneous rocks are mainly made of coarse-grained olivine, similar to some Martian meteorites. We interpret them as an olivine cumulate, formed by settling and enrichment of olivine through multi-stage cooling of a thick magma body.

11. Resting and Exercise Energy Metabolism After Liver Transplantation for Nonalcoholic Steatohepatitis

Abstract

Background. Nonalcoholic steatohepatitis (NASH) is a leading indication for liver transplantation (LT). We hypothesized that weight gain after LT may be exacerbated by reduced metabolic rates due to the LT procedure, particularly during exercise. We aimed to compare resting and exercise energy expenditure between patients transplanted for NASH and nontransplant nonalcoholic fatty liver disease (NAFLD) subjects. Methods. NASH LT recipients (>1-year post, n = 14) and NAFLD controls (n = 13) underwent analysis of body composition, resting energy expenditure (REE), and exercise energy expenditure (VO2max), the latter using a ramped-Bruce protocol assessed by expired gas analysis and peak heart rate. Results. Participants were mean 61.5 ± 7.9 years, 48.1% men, and 66.7% white. Baseline comorbidities were similar between groups. Among men, mean REE adjusted for total (17.7 vs 18.8, P = 0.87) and lean body mass (23.5 vs 26.9, P = 0.26), as well as VO2 (20.1 vs 23.9, P = 0.29), was lower in NASH LT recipients compared with NAFLD controls, respectively, although not statistically significant. However, female NASH LT recipients had significantly lower mean REE than NAFLD controls when adjusted for total (14.2 vs 18.9, P = 0.01) and lean body mass (19.3 vs 26.5, P = 0.002), as well as significantly lower VO2max (14.4 vs 20.6, P = 0.017). Conclusions. NASH LT recipients, particularly women, have lower REE and exercise energy expenditure compared with nontransplant NAFLD patients. More aggressive diet and exercise programs for post-LT NASH recipients to account for reduced resting and exercise metabolic rates may attenuate weight gain in this vulnerable population.

Abstract

The 40Ar/39Ar dating method is among the most versatile of geochronometers, having the potential to date a broad variety of K-bearing materials spanning from the time of Earth’s formation into the historical realm. Measurements using modern noble-gas mass spectrometers are now producing 40Ar/39Ar dates with analytical uncertainties of ∼0.1%, thereby providing precise time constraints for a wide range of geologic and extraterrestrial processes. Analyses of increasingly smaller subsamples have revealed age dispersion in many materials, including some minerals used as neutron fluence monitors. Accordingly, interpretive strategies are evolving to address observed dispersion in dates from a single sample. Moreover, inferring a geologically meaningful “age” from a measured “date” or set of dates is dependent on the geological problem being addressed and the salient assumptions associated with each set of data. We highlight requirements for collateral information that will better constrain the interpretation of 40Ar/39Ar data sets, including those associated with single-crystal fusion analyses, incremental heating experiments, and in situ analyses of microsampled domains. To ensure the utility and viability of published results, we emphasize previous recommendations for reporting 40Ar/39Ar data and the related essential metadata, with the amendment that data conform to evolving standards of being findable, accessible, interoperable, and reusable (FAIR) by both humans and computers. Our examples provide guidance for the presentation and interpretation of 40Ar/39Ar dates to maximize their interdisciplinary usage, reproducibility, and longevity.

In normoxia, PHD2 hydroxylates proline residues on HIF-1α, rendering it inactive. However, in hypoxia, PHD2 is inactive, HIF-1α is stabilized and downstream effectors such as vascular endothelial growth factor and fibroblast growth factor-2 are produced to promote angiogenesis. In the present study we utilize RNA interference to PHD2 to promote therapeutic angiogenesis in a diabetic wound model, presumably by the stabilization of HIF-1α. Stented wounds were created on the dorsum of diabetic Lepr db/db mice. Mice were treated with PHD2 small interfering RNA (siRNA) or nonsense siRNA. Wounds were measured photometrically on days 0-28. Wounds were harvested for histology, protein, and RNA analysis. Diabetic wounds treated with siRNA closed within 21±1.2 days; sham-treated closed in 28±1.5 days. By day 7, Western blot revealed near complete suppression of PHD protein and corresponding increased HIF-1α. Angiogenic mediators vascular endothelial growth factor and fibroblast growth factor-2 were elevated, corresponding to increased CD31 staining in the treated groups. siRNA-mediated silencing of PHD2 increases HIF-1α and several mediators of angiogenesis. This corresponded to improved time to closure in diabetic wounds compared with sham-treated wounds. These findings suggest that impaired wound healing in diabetes can be ameliorated with therapeutic angiogenesis.

Abstract

SUMMARY Librarians and archivists need special training in the use of rare law books and manuscripts that includes not only the protective functions with respect to rare materials but also education in the history of law, its literature, and its historical methodology. The article describes two different training programs: one, at Yale, trains law students in the use of historical legal materials; and the other, at the University of Virginia Rare Book School, trains librarians, archivists, historians, collectors and dealers in the care and use of rare law books and manuscripts. The article concludes with suggestions for model curricula for law librarians in the use of rare materials.

20. Efficacy and feasibility of teledermatology for paediatric medical education

Abstract

We evaluated a teledermatology consultation service in the education of medical trainees. The selection of cases for consultation was at the discretion of the trainees, who could contact the study team for advice about obtaining photographs and submitting the case to a web-based system. Asynchronous structured feedback was provided to trainees by an academic paediatric dermatology consultant using a web-based interface. Efficacy was evaluated by examining the trainees' self-reported competency in clinical dermatology skills before and after teleconsultation. A total of 44 trainees (31 residents and 13 medical students) completed 50 consultations. Trainees reported significant improvement (mean improvement 22%, P < 0.002) in competency in five of the six areas assessed. In addition, 88% of trainees were very satisfied with the teaching methodology (≥5 on a 7-point scale) and 86% were very likely to apply the information in their future practice (≥5 on a 7-point scale). We believe that teledermatology has great potential in the education of medical trainees.

Abstract

Acute febrile neutrophilic dermatosis (Sweet's syndrome) is a rare inflammatory disease of unknown cause characterized by painful, erythematous plaques and a dermal infiltrate of mature neutrophils. Associated systemic disease is a common finding in patients with Sweet's syndrome, and in fact determines the prognosis; therefore, the diagnosis of Sweet's syndrome warrants a thorough medical evaluation. We found only 17 reported cases of Sweet's syndrome in children. Two cases of Sweet's syndrome in children are presented; these cases are typical, except for the tibial pain in the first patient and the marked leukocytosis in the second patient. Both patients responded promptly to systemic corticosteroid therapy, although both had relapse.

28. Evaluation of an online graduate nursing curriculum: examining standards of quality.

Abstract

Recent offerings of online courses have outpaced the evaluation of the quality of those offerings, particularly at the program level. The purpose of this project was to evaluate the quality of a set of 16 graduate nursing courses developed for three master's specialty programs. An interdisciplinary group of nursing faculty and instructional technologists collaborated in the development of a quality assessment tool and evaluated 16 online graduate nursing courses. Faculty members for each of the courses were interviewed as part of the process. The collaborative design and development process is discussed, as well as evaluation techniques and the data collection instrument. Examples of best practices and areas for improvement in online courses are presented. The findings will help others as they develop and evaluate online curricula. This project provides a model for interdisciplinary collaboration on the evaluation of online programs that can be reviewed, modified, and implemented at other institutions.
