26. 193 Audit on Inappropriate Prescribing of Proton Pump Inhibitors Amongst Patients over 65 Years in an Acute Rehabilitation Hospital.

Abstract

Background Proton pump inhibitors (PPIs) are one of the most frequently prescribed drug classes in the older person. Indications for PPI use are outlined in the NICE guidelines, however they are often prescribed without an appropriate indication resulting in increased healthcare costs and increased exposure to potential adverse clinical effects. Methods To determine the rate of inappropriate prescribing of PPI's, an audit was carried out to assess the incidence of inappropriate prescribing in an Acute Rehabilitation facility pre and post Education to Hospital Doctors. An audit was carried out, pre and post intervention, on patients over the age of 65 years admitted to an acute rehabilitation hospital between 2018 and 2019. Patient's medical charts were reviewed and data was collected on PPI prescribing. Results Cycle 1-Pre Education; 83 people included in the audit. Mean age was 80.8 years[SD ±9]. 43% of patients were on a PPI, all of which were prescribed the generic form. 83.4% of patients had no indication for PPI use. Of the 16.6%, indications included GORD, Barrett's oesophagus, PUD and PPI prophylaxis against NSAID's including aspirin. 77.7% were on therapeutic dose without any indication. 100% of patients were on PPI for longer than 6 weeks. Cycle 2, Post education: 86 patients were included in re-audit. Mean age was 81.5[SD ±10]. 60.4% of patients were on PPI. Of those, 55% had no indication for PPI. Of the 44% on PPI, indications were similar to those in Cycle 1. 83% of patients were on PPI longer than 6 weeks. 56.5% were on inappropriate dose of PPI. Conclusion This audit highlighted the inappropriate prescribing of PPI's in the older person. By providing education to Doctors about NICE PPI prescribing guidelines, overall rate of inappropriate prescribing of PPI decreased by 28.4% and accurate dosing of PPI improved by 21.2%. 41. Haemodynamic parameters for assessment of orthostatic intolerance in older people.

Abstract

Orthostatic hypotension (OH) is a frequent cause of orthostatic intolerance (OI) and related symptoms associa/-ted with the occurrence of syncope. The purpose of this study was to examine if older people haemodynamic profile with symptoms of OI is different to controls during active stand. The database includes sample of 45 participants, aged at least 60 years, where 46.7% (n=21) had symptomatic OI and 53.3% (n=24) did not. Haemodynamic parameters were registered during three phases: a pre-exercise, walking phase and a post-exercise stand. Different haemodynamic parameters such as, pulse rate (HR), systolic blood pressure (SBP), diastolic blood pressure (DBP) and cardiac output (CO) were extracted at each of the phases. Results showed differences along phases in both groups. Mean HR differences in phase 1–2 and phase 2–3 showed a statistical signification between symptomatic OI and non-symptomatic OI group. In addition, the combination of the variables rise/fall time-maximum/minimum amplitude in phases 2–3 showed opposite trends in men and women in symptomatic and non-symptomatic OI groups. 42. Depression: a modifiable factor in fearful older fallers transitioning to frailty?

Abstract

Objective Fear of falling is one of the most common fears among community-dwelling older people and is as serious a health problem as falls themselves. Understanding fear of falling in fallers transitioning to frailty may help us identify effective strategies to reduce it in this already vulnerable group of older people. Our aim was to evaluate the psychological factors associated with fear of falling in a group of fallers transitioning to frailty when compared with robust or non-frail fallers. Methods Cross-sectional design where 301 fallers underwent assessment at the Technology Research for Independent Living Clinic in Dublin () is seen. Fear of falling was measured using the Modified Falls Efficacy Scale, and frailty was measured using the Biological Syndrome Model. Psychological measures included assessment of anxiety, depression, loneliness, personality factors and cognition. Results Frailer fallers had increased fear of falling when compared with robust fallers ( p < 0.001). Age, female gender and lower cognitive scores were associated with greater fear of falling in the robust group. For frailer fallers, higher depression score was the only factor associated with fear of falling on multivariate analysis. The odds ratio of having case level depressive disorder (CESD-8 ≥ 4) if you were a frailer faller was significantly higher than if you were robust (OR = 2.6, CI 1.3-5.2, p = 0.006). Conclusion Fallers at a transitional level of frailty may represent a particularly vulnerable group psychologically who would benefit most from interventional stratagies with specific intervention components addressing depressive symptoms.
