Objectif : l’objectif de cette étude est de déterminer la prévalence et les facteurs associés à la lipodystrophie chez les personnes vivant avec le VIH sous antirétroviral suivies à l’hôpital Béthesda de Cotonou. Méthodes : Il s’est agi d’une étude transversale, descriptive et analytique. La population d’étude est constituée par les personnes vivant avec le VIH sous antirétroviral, suivies à l’hôpital Béthesda de Cotonou. La lipodystrophie a été définie par la présence de signes de lipoatrophie et/ou de lipohypertrophie objectivés par les patients et confirmés par l’examen clinique. Résultats : Au total 305 patients ont été inclus dans l’étude, dont 214 (79 %) de sexe féminin. L’âge moyen des patients était 38,1± 5,4 ans. La lipodystrophie était présente chez 213 patients soit une prévalence de 69,83%. Les facteurs associés à la lipodystrophie étaient le sexe féminin, l’âge de 40 ans et plus, la sédentarité, la consommation du tabac, l’apport en énergie anormal, la consommation du tabac, la consommation insuffisante en fruits et légumes, une durée de traitement antirétroviral de 1 an ou plus, un taux de CD4 inférieur à 200 cellules/mm3 au dépistage, l’absence d’appui alimentaire et de conseils nutritionnels, un IMC anormal et un tour de taille élevé. Conclusion : La lipodystrophie est fréquente chez les patients infectés par le VIH.

Abstract Background COVID-19 is an emerging contagious infection with polymorphic clinical manifestations. The purpose of this study was to describe the epidemiological, clinical, therapeutic features and identify the predictors of mortality among COVID-19 hospitalized cases in Parakou. Methods This was a cross-sectional, descriptive and analytic study. Systematic recruitment was used to include all patients hospitalized with COVID-19 from May 8, 2020, to December 31, 2021, whose medical records were available and usable. The variables studied were clinical and paraclinical signs, diagnostic and therapeutic means, evolution under treatment and prognostic factors. This study was approved by the Local Ethical Committee. The data were analyzed using Stata/MP 14.1 software. Results A total of 198 cases of COVID-19 were identified, 117 of whom were men. The mean age was 51.53 ± 19.51 years. The presenting signs were fever 146 (74.11%), cough 157 (79.70%) and dyspnea 118 (53.90%). It was severe COVID-19 in 108 cases (54.55%). Therapeutically, 95 patients (47.98%) had received the combination of Lopinavir/ritonavir and Ribavirin and 95 others (47.98%) received chloroquine. Recovery was noted in 151 (76.26%) patients. Mortality rate was 18.18%. Predictors of death were high blood pressure, presence of signs of severity, high-concentration mask ventilation used, and elevated transaminases. Conclusion COVID-19 was a reality in Parakou, with a significant number of severe cases requiring hospitalization. Several factors are associated with the prognosis of the disease.

INTRODUCTION: le syndrome métabolique est associé aux maladies cardiovasculaires. L'infection au VIH est devenue aujourd'hui une maladie chronique. L'objectif de cette étude est de déterminer la prévalence, les facteurs associés et prédisposant au syndrome métabolique chez les patients infectés par le VIH sous traitement antirétroviral. METHODES: il s'est agi d'une étude transversale, descriptive et analytique. La population d'étude est constituée des patients vivant avec le VIH sous antirétroviral suivis au Centre Hospitalier Universitaire de l'Ouémé-Plateau. Le syndrome métabolique a été défini selon les critères de la Fédération Internationale du Diabète. RESULTATS: la population était constituée de 244 patients. La prévalence du syndrome métabolique était de 18,03% avec une prédominance féminine (74,6%). La moyenne d'âge était de 40,7 , 9,71 ans. Les facteurs associés au syndrome métabolique étaient le sexe féminin, la sédentarité, l'antécédent d'HTA, le surpoids, l'apport énergétique élevé, l'apport lipidique élevé, la consommation d'alcool, la consommation de tabac et l'hypercholestérolémie. Les facteurs prédisposant au syndrome métabolique étaient la présence de l'HTA, le tour de taille élevé, l'hyperglycémie, l'hypocholestérolémie HDL et l'hypertriglycéridémie. CONCLUSION: le syndrome métabolique est fréquent chez les patients infectés par le VIH sous traitement antirétroviral. Une prévention prenant en compte les facteurs associés et prédisposant s'avère nécessaire.

Background: Although aerobic training (AT) and resistance training (RT) are recommended after stroke, the optimal dosage of these interventions and their effectiveness on balance, walking capacity, and quality of life (QoL) remain conflicting., Objectives: Our study aimed to quantify the effects of different modes, dosages and settings of exercise therapy on balance, walking capacity, and QoL in stroke survivors., Method: PubMed, CINHAL, and Hinari databases were searched for randomised controlled trials (RCTs) evaluating the effects of AT and RT on balance, walking, and QoL in stroke survivors. The treatment effect was computed by the standard mean differences (SMDs)., Results: Twenty-eight trials ( n = 1571 participants) were included. Aerobic training and RT interventions were ineffective on balance. Aerobic training interventions were the most effective in improving walking capacity (SMD = 0.37 [0.02, 0.71], p = 0.04). For walking, capacity, a higher dosage (duration ≥ 120 min/week; intensity ≥ 60% heart rate reserve) of AT interventions demonstrated a significantly greater effect (SMD = 0.58 [0.12, 1.04], p = 0.01). Combined AT and RT improved QoL (SMD = 0.56 [0.12, 0.98], p = 0.01). Hospital located rehabilitation setting was effective for improving walking capacity (SMD = 0.57 [0.06, 1.09], p = 0.03) compared with home and/or community and laboratory settings., Conclusion: Our findings showed that neither AT nor RT have a significant effect on balance. However, AT executed in hospital-located settings with a higher dose is a more effective strategy to facilitate walking capacity in chronic stroke. Hypertension is the strongest cardiovascular risk factor worldwide. May Measurement Month (MMM) is an international campaign for blood pressure (BP) screening initiated by the International Society of Hypertension. This work aims to estimate the proportion and the levels of awareness, treatment, and control of hypertension in participants of the MMM survey in Benin in 2018. A cross-sectional survey focused on people aged ≥18 years was conducted in May 2018 in nine rural and urban areas in Benin. A sampling of volunteers was done. BP was measured following the MMM protocol. Hypertension was defined as a systolic BP ≥140 mm Hg and/or a diastolic BP ≥90 mm Hg (mean of the second and third readings) and/or taking antihypertensive medication. Linear regression was used to identify BP associations. A total of 2035 people were screened, including 55.9% women. The mean age was 44.2 ± 15.9 years. The percentage with hypertension was 35.4%. Of 721 participants with hypertension, 56.2% were aware of their diagnosis, 39.7% were on antihypertensive medication, and 13.6% were controlled (<140/90 mmHg). The results confirm the significant proportion of hypertension in Benin. Introduction: The purpose of this work was to determine the absolute cardiovascular risk (ACVR) of women using hormonal contraception in Porto-Novo., Methods: We carried out a descriptive, cross-sectional study, including women at the time of renewal of a hormonal contraceptive method. Blood pressure, fasting venous blood glucose level, body mass index and electrocardiographic left ventricular hypertrophy were studied. The determination of ACVR was dual based on the World Health Organisation (WHO/ISH) and the European Society of Cardiology (ESC/ESH) models., Results: The mean age of the women was 35.3 ± 8.2 years. Blood pressure and blood glucose levels were high in 24 and 1.5% of cases, respectively. Left ventricular hypertrophy was present in 7.1% of cases. A high ACVR was found in 5.2% of these women, using the ESC/ESH model., Conclusion: The occurrence of women with high ACVR in this group raises the problem of cardiovascular eligibility to the contraceptive method used.

Double-chambered right ventricle (DCRV) is a very rare cardiac defect in which the right ventricle is divided by anomalous muscle bundle into two chambers. It is associated with other malformations in 80-90% of cases. Clinical presentation varies and depends on the extent of the intraventricular obstruction. We here report the case of a 16-year old teenager with isolated DCRV revealed by recurrent syncopes. The diagnosis was made using Doppler echocardiography. The patient underwent successful surgical resection of the abnormal muscle band.

Objectives: The study was initiated to assess patient knowledge about diabetes mellitus, identify food claims and finally to identify the daily practices., Materiel and Methods: The study was sectional and descriptive. The study population consists of diabetic seen in CNHU-HKM the outpatient Cotonou. Data collection is done by a questionnaire., Results: Deficiencies were noted in knowledge of patients on diabetes mellitus.Several allegations have been identified. Patients think that the diabetic should not eat fruits, rice or corn dough. The practice of physical activity is low. Difficulties were encountered preventing compliance with diet, the practice of physical activity and glycemic control., Conclusion: Of challenges remain to improve the knowledge, attitudes and practices of patients. Objective: We validated against invasive measurements a measure of central SBP based on the QKD interval. The present objective is a comparison to the SphygmoCor device., Materials and Methods: A first cohort was used to redefine an algorithm for estimating ascending aorta SBP (cSBP) with respect to SphygmoCor. This algorithm was then tested in a second cohort under static conditions and in a third cohort during a head-up tilt to study blood pressure variations., Results: The first cohort (36 patients, mean age: 46 ± 20 years, SBP: 114 ± 19 mm Hg) defined the estimation algorithm as cSBP =62.687+1.35 × meanBP-0.207 × heart rate-0.251 × height-0.1 × QKD (mean difference with SphygmoCor=0 ± 7 mmHg).In the second cohort (105 patients, mean age: 51 ± 19 years, SBP: 132 ± 22 mmHg), the average difference between the two techniques was 2 ± 6 mm Hg (R=0.93). The dynamic cohort included 31 patients (mean age: 49 ± 23 years, SBP: 110 ± 19 mmHg) with 224 pairs of measurement. We studied the changes in central blood pressures compared with the basal state of each of the two techniques during head-up tilt. With QKD, the average cSBP was 104 ± 19 mmHg, the average variation was -13.00 mmHg ± 15 (range 7-74 mmHg). With SphygmoCor, average cSBP was 10  mmHg ± 18, the average variation was -14 mmHg   ± 16 (range 2-81 mmHg). The variations in blood pressure with the two techniques were well correlated (R=0.93)., Conclusion: cSBP estimation based on the QKD interval is comparable in precision to the SphygmoCor. Its ability to measure cSBP variations suggests that it could be employed for ambulatory measurements over 24 h.
