Abstract

The aim was to examine social, university, health-related differences among heterosexual and Sexual Minority Medical students (SM-Meds), and explore predictors of help-seeking behavior (i.e., current psychological support and hypothetical university counseling use). A multicenter cross-sectional study was conducted in Italy (2018). Questionnaires included socio-demographic items and Beck Depression Inventory-II (BDI-II). Chi-squared tests and multivariable regressions adjusted for BDI-II score were performed (sample size = 2513). SM-Meds were 13.9%. SM-Meds with depressive symptoms were 40.5%. Both considering overall and non-depressed students, SM-Meds exercised less and had worst relationships with family and classmates. Psychological support was reported by 6.1% (SM-Meds: 12.8%) and hypothetical counseling use by 42.7% (SM-Meds: 47.1%). Multivariable models showed SM-Meds had significant higher likelihood of psychological support. Therefore, SM-Meds reported higher help-seeking behavior, lower physical activity and social support. Universities should promote more inclusive learning climate and activities to increase exercise, especially among SM-Meds.

Abstract

Background: More than 10 years have elapsed since human papillomavirus (HPV) vaccination was implemented. We did a systematic review and meta-analysis of the population-level impact of vaccinating girls and women against human papillomavirus on HPV infections, anogenital wart diagnoses, and cervical intraepithelial neoplasia grade 2+ (CIN2+) to summarise the most recent evidence about the effectiveness of HPV vaccines in real-world settings and to quantify the impact of multiple age-cohort vaccination. Methods: In this updated systematic review and meta-analysis, we used the same search strategy as in our previous paper. We searched MEDLINE and Embase for studies published between Feb 1, 2014, and Oct 11, 2018. Studies were eligible if they compared the frequency (prevalence or incidence) of at least one HPV-related endpoint (genital HPV infections, anogenital wart diagnoses, or histologically confirmed CIN2+) between pre-vaccination and post-vaccination periods among the general population and if they used the same population sources and recruitment methods before and after vaccination. Our primary assessment was the relative risk (RR) comparing the frequency (prevalence or incidence) of HPV-related endpoints between the pre-vaccination and post-vaccination periods. We stratified all analyses by sex, age, and years since introduction of HPV vaccination. We used random-effects models to estimate pooled relative risks. Findings: We identified 1702 potentially eligible articles for this systematic review and meta-analysis, and included 65 articles in 14 high-income countries: 23 for HPV infection, 29 for anogenital warts, and 13 for CIN2+. After 5–8 years of vaccination, the prevalence of HPV 16 and 18 decreased significantly by 83% (RR 0·17, 95% CI 0·11–0·25) among girls aged 13–19 years, and decreased significantly by 66% (RR 0·34, 95% CI 0·23–0·49) among women aged 20–24 years. Abstract

Background: research in the health sector is an activity associated with an uncertain return on investment. This study aims to outline a multidimensional framework for assessing the impact of research funded by public (or mixed public and private) sponsors. Methods: a focus group representing different stakeholders was created to develop a sense of purpose that would be shared by the intended beneficiaries of the research assessment process, and others with a direct or indirect interest in a program’s implementation. Results: a framework was designed that takes a holistic approach to the impact of research in which not only technical, but also economic, social and political dimensions are interrelated with the results achieved for the population and health services. Conclusions: the framework reflected the views of different stakeholders, favoring the development of the positive, proactive relationships essential to orienting the process of transforming the evidence emerging from research into action for health.

Abstract

In the field of healthcare, programming is of vital importance for the future development of health system with regard to the scarcity of resources and to the increasing needs of the population. Routinely and systematically collected health-related information represents a major potential within epidemiology, since it refers to a volume of population that can achieve a considerable size. Besides, the bigger the impact of data on management process, the better the quality. Health data storage is unevenly distributed among public health authorities and undergoes specific national and regional regulations, with current limitation of this information being separately organized and managed. The present paper has the following major objectives: the development of an “Integrated Epidemiological System” (IES) of health surveillance, the application of IES to different local authorities (municipal, local corporate and regional) and the evaluation of its reproducibility and application in terms of prevention and therapy. A thorough description is provided about the construction phases, consisting in supply, data quality evaluation, record linkage of archives with sanitary registers and in frequency estimates production. Design and application of various algorithms suitable for frequency estimates production and the health profile description of subjects living in the analyzed geographical areas are particularly emphasised. The sources used are sanitary registers, mortality data, hospital discharge forms, pharmaceutical prescriptions, ticket exemptions and immunization registries. Such sources are differently utilised in the three application experiences. Collected data are applied to all residents and longitudinal cohorts (elderly and children) are built. Results indicate the possibility of applying IES to the definition of a community health profile and its utilisation as a tool for the assessment of preventive and therapeutical programmes. In this paper, results achieved in the three applications are provided in detail. To summarise, the study highlighted how the integrated epidemiological system can be developed and replicated both on a local and on a regional level. Its usage makes it possible to meet the increasing needs expressed to health authorities’ facilities and represents a useful tool in the evaluation phase of preventive programmes and treatment processes. Its application to large populations through the construction of residing and historical longitudinal cohorts allows to identify, confirm or exclude possible factors associated to a higher risk of occurrence of particular events (i.g. even on an environmental study). To conclude, integrating data from multiple sanitary sources turns out to be a useful instrument we can take advantage of in different contexts, enabling to perform reasonably prompt analysis with limited resources.

Since Regione Veneto suspended compulsory vaccination for children in 2008, and because of an increasing disaffection of parents to the vaccine practice, the vaccination rates have been slowly but steadily decreasing. The aim of this study was to analyze internal and external factors of immuniza- tion reduction and to implement potential solutions of the prob- lem. Methods. Servizio Igiene e Sanita? Pubblica of ULSS 12 Venezi- ana (SISP ? Hygiene and Public Health Service) analyzed and addressed both, the reasons of parents who do not vaccinate their children and the internal problems regarding vaccination clinics management, information to families, procedures and guidelines and, in general, the communication skills of the vaccination staff. Results. A positive trend in vaccination rates was observed, espe- cially in Venice historical centre. Moreover the staff reported a better working atmosphere and benefit from sharing common goals and procedures, even though the workforce was reduced of about 30% in terms of equivalent unit (EU). Discussion. The continuous quality improvement method fol- lowed in this experience led to a steady increase in vaccination coverage in all territorial clinics, to a better adhesion of guide- lines and standard operating procedures and to a general pro- fessional empowerment of SISP staff. The service now offered to the population is better and more efficient, since the workforce has been reduced. The goal of this study was to describe and comment the experience of the Veneto Region in the bacterial invasive disease. Vaccina- tion coverage was 93% against pneumococcus and 95% against haemophilus influenzae type B. Regard to meningococcus C the coverage rate was 90.1% In children, 81% at 6 years of age and 78.2% at 15 years . The preliminary data of an active surveillance of invasive bacterial diseases show that the Streptoccoccus pneumoniae was the main agent involved and that its consequences were particularly serious in elderly subjects. With regard to Neisseria meningitidis, we observed a substantial reduction in the number of cases due to serogroup C and a concomitant rise in the percentage of cases due to serogroup B. The suspension of mandatory vaccination should be maintained , the monitoring of vaccination coverage and the active surveillance proved to be a very good assessment tools. Abstract

Road accidents are a major public health problem that affect all age groups but their impact is most striking among the young. The aim of this study is to quantify the burden of road traffic injuries, their mortality and direct in-patient economic costs and to identify the age classes at highest risk for severe road traffic injuries, through analysis of data collected by information systems of an Italian Local Health Authority.The study was conducted in a Local Health Authority of Veneto Region. Injured people were selected from Emergency Department (2006-2010). Data were linked to the Hospital Information System for hospital admissions and to the Mortality Registry to check 30-day mortality. The direct costs associated to hospitalizations were estimated through Diagnosis Related Group reimbursement rates. Multivariate analysis was performed using hospitalization and mortality as the dependent variables and gender, age, day of week when accident occurred as the independent variables. Traffic injury, hospitalization and mortality incidence rates were calculated by gender and age per 100,000 residents per year.The road traffic injuries were 9,192, decreasing from 2,112 in 2006 to 1,980 in 2010. Among injured persons 55.3% were male (68.1% among 15-19 age class); 41.7% young people aged 15-34 years (43.9% among male, 39.0% among female). Total hospitalisation rate was 5.9%. Overall mortality rate was 0.3% (0.9% among aged 65 or older). The cost of hospital admission was euro 2,742,505 (hospitalization mean cost euro 5,097). Risk of hospitalization and death was higher in male, in elderly and during week end. Young people aged 15-19 had the highest incidence of visits (2,258.4 per 100,000) and high hospitalisation weekend and mortality rates (respectively 101.5 and 8.5).Analysis at local level, using current data sources, permits to estimate the burden of injuries caused by road-traffic, to describe the characteristics of injured persons and finally to estimate costs of care. All this information could be used to make the population aware of its own risk for road accidents. Linkage of these data with police and transport data is required to focus prevention on higher risk groups and to adopt effective local road safety strategies.

Summary In June 2009, the World Health Organization (WHO) issued a pandemic alert concerning the spread of an influenza A (H1N1) virus that showed distinctive genetic characteristics vis-à-vis both seasonal influenza strains and vaccine strains. The main mutation occurred in the gene coding for hemagglutinin (HA). Mathematical models were developed to calculate the transmissibility of the virus; the results indicated a significant overlap with the transmissibility of previous pandemic strains and seasonal strains. The remarkable feature of A/(H1N1)pdm09, compared with seasonal strains, is its high fatality rate and its higher incidence among younger people. Data provided by the WHO on the number of deaths caused by A/(H1N1)pdm09 only include laboratory-confirmed cases. Some authors suggest that these data could underestimate the magnitude of the event, as laboratory confirmation is not obtained in all cases. It is important to bear in mind that the A/(H1N1)pdm09 virus is still circulating in the population. It is therefore essential to maintain its epidemiological and virological surveillance.

Abstract

Background Four percent of the world’s population suffers from depression, which is a major public health issue. Medical students are at risk, as their depressive symptoms (DS) prevalence is reported to be approximately 27% worldwide. Since few data on Italian medical students exist, this study aimed to estimate their DS prevalence and assess risk and protective factors. Methods The PRIMES was a multicentre cross-sectional study performed in 12 Italian medical schools. Questionnaires were self-reported and included 30 sociodemographic items and the Beck Depression Inventory-II (BDI-II). The primary outcome was the presence of DS (BDI-II score≥14).
