Control of denture plaque biofilms is a practical approach to preventing persistent oral infections such as denture stomatitis. Objectives. This study compared in vitro biofilm attachment and growth on a new denture material, Ultaire® AKP, with that on traditional denture materials including cobalt chrome (CoCr), polymethyl methacrylate (PMMA), and polyoxymethylene (POM). Methods. Microbial biofilms were grown with cultures of Candida albicans, Streptococcus mutans UA159, or a mixed Streptococcus spp. (S. mutans 700610/Streptococcus sanguinis BAA-1455) for 6 hours in a static protocol or 24 hours in a dynamic protocol for each material. Adherent biofilm cells were removed, and viable colony-forming units (CFUs) were enumerated. Confocal microscopy of the 24-hour Streptococcus spp. biofilms was used to determine biofilm mass and roughness coefficients. Results. The rank order of C. albicans attachment after 6 hours was CoCr > PMMA ∗ > Ultaire® AKP ∗ ( ∗ vs CoCr, p ≤ 0.05 ), and that for 24-hour biofilm growth was CoCr > Ultaire® AKP ∗ > PMMA ∗ ( ∗ vs CoCr, p ≤ 0.05 ). The rank order of S. mutans biofilm attachment was CoCr > POM > Ultaire® AKP ∗ > PMMA ∗ ( ∗ vs CoCr, p ≤ 0.05 ), and that for the 24-hour Streptococcus spp. biofilm growth was POM > Ultaire® AKP > PMMA > CoCr ∗ ( ∗ vs POM, p ≤ 0.05 ). Confocal images revealed structural differences in Streptococcus spp. biofilms on CoCr compared with the other test materials. Significantly lower roughness coefficients of Streptococcus spp. biofilms on Ultaire® AKP were noted, suggesting that these biofilms were less differentiated. Ultaire® AKP promoted significantly less C. albicans and S. mutans biofilm attachment than CoCr at 6 hours and C. albicans growth at 24 hours. Streptococcus spp. biofilms on Ultaire® AKP were less differentiated than those on other test materials. Conclusion. In addition to its material strength, Ultaire® AKP represents an attractive option for denture material in removable partial dentures.

In oral rehabilitation, the treatment of partial edentulism (PEd) is performed by removable partial dentures (RPD) or assembled prosthetic works (APW) composed of several components, fixed to the prosthetic field (Pa) and a removable one (Pb), in order to facilitate the daily hygiene but also the damping of the occlusal forces applied in mastication. Cobalt-Chromium alloys are materials used to manufacture modern prosthetic assembles. In order for this study to be relevant, it was necessary to standardize the design of the framework (Pa) in terms of shape and volume so that the experiment could be reproducible for the five Co-Cr alloys: 0-A (Co-Cr-Mo), 5-A and 10-A (Co-Cr-Mo-W), 15-A and 16.4-A (Co-Cr-W-Fe) and for the three fabrication methods of dental assembled prosthetic frameworks: refractory duplicate method (RD) resulting removable framework (Pb), direct construction method (DC) resulting removable framework (Pb-) and casting over metal method (CoM) resulting removable framework (Pb+). The time allocated to the adaptation process (AP), mechanical processing and sandblasting, in order to assemble the two components was between 43–70 min, even though the assembly between the Pa-framework and the complementary framework (Pb+) was not necessary, CoM-method hs been provide the elimination of AP step. By applying the arithmetic simple rule of three, the percentages for each of the three methods used were calculated, the values of the difference were obtained. The CoM method improves the joining precision between the components of the removable assembly of prosthetic frameworks by 91.7% compared to the RD method and by 80.62% compared to the DC method. According to the efficiency of the methods used in the precision of joining between frameworks components, their order is: casting over metal, direct construction and refractory duplicate method.

The aim of this study is the investigation of the effect of blade designs used in the universal testing machine on the shear bond strength values. Three different zirconia framework ceramics (Zirkonzahn Translucent, Upcera Super Translucent, Rainbow Trans) and veneering ceramics suggested by their manufacturers (Zirkonzahn ICE Porcelain, Cerabien ZR Porcelain, Rainbow Porcelain) were subjected to shear bond strength tests with three different blade designs. Metal-ceramic (EOS Cobalt Chrome SP2 — GC Initial Porcelain) was used as a control group. A total of 90 samples (n = 10) for experimental group with zirconia framework and 30 samples of control group (n = 10) were prepared. Shear bond strength tests were performed with wire-loop design in the first group, notched-rod in the second group, and knife-edge chisel designs in the third group. Statistical analysis of the data was made by the Kruskal–Wallis test and followed by Conover’s multiple comparison test. When the test results made with different blade designs were examined, no significant difference was found for zirconia samples. In the control group, a statistically significant difference was found between different blade designs. It was observed that the wire-loop group had significantly higher shear bond strength values compared to the notched-rod and knife-edge chisel groups. Used blade designs should be standardized in order to be able to evaluate the materials and methods used in the studies in the literature.

We present a rare case of symptomatic adverse local tissue reaction in a 54-year-old female patient who had undergone total hip arthroplasty with ceramic-on-ceramic bearing. Inflammatory periarticular mass and osteolysis developed in the absence of cobalt chrome alloy interfaces and a modular neck component. On the pathologic images, there was no clear evidence of gross metal staining of tissues, metal corrosion, and ceramic or metal wear particles. However, there were impingement scars on the titanium alloy femoral neck and acetabular cup associated with a high combined anteversion angle of 75° (stem: 40° and cup: 35°), suggesting titanium debris release in vivo. Immunohistochemical staining proved a predominant infiltration of CD4+ T cells and the corresponding IL-17A response to metal. We conclude that neck-rim impingement may lead to the development of adverse local tissue reaction (periarticular mass and osteolysis) due to a metal hypersensitivity with the production of proinflammatory cytokines (IL-17A) by CD4+ T cells even in ceramic-on-ceramic total hip arthroplasty.

Objective The aim of this study was to evaluate the amount of micromotion of dental implants under immediate loading supported by Titanium (Ti) and Cobalt‐Chrome (Co‐Cr) superstructures. Material and methods A model of tridimensional half‐edentulous maxilla with three dental implants was made using the Finite Element Analysis (FEA). Two standard and one zygomatic implants were connected to a superstructure with an elliptic section of 6x 3 mm (mm). Two study models were established. Model A: Titanium (Ti) alloy superstructure; Model B: Cobalt‐Chrome (Co‐Cr) alloy superstructure. To simulate an immediate‐loading situation, a friction coefficient of 0.71 was applied between the implant and the bone surface. An axial load of 252.04 [N] was applied on standard and zygomatic implants. Results The Micromotion of dental implants was similar in both superstructure situations. The amount of micromotion was slightly higher in B1 and B3 models (Co‐Cr alloy‐superstructure) compared with A1 and A3 models (Titanium alloy superstructure). The micromotion values in two groups were greater than 150 μm in the incisive region (standard implant) and molar region (zygomatic). In general, the micromotion was higher on the implant that received the load with respect to the other implants. The greater difference was observed when the load was applied on the standard implant A1 (Model A1 = 189.12 μm) compared with standard implant B1(Model B1 = 263.25 μm). Conclusions Within the limits of present study, all implants on different load application points showed micromotion; in general, the amount of micromotion was slightly higher in the implants connected with Co‐Cr alloy superstructure.

Aims Dual mobility (DM) implants have been shown to reduce the dislocation rate after total hip arthroplasty (THA), but there remain concerns about the use of cobalt chrome liners inserted into titanium shells. The aim of this study was to assess the clinical outcomes, metal ion levels, and periprosthetic femoral bone mineral density (BMD) at mid-term follow-up in young, active patients receiving a modular DM THA. Methods This was a prospective study involving patients aged < 65 years, with a BMI of < 35 kg/m2, and University of California, Los Angeles activity score of > 6 who underwent primary THA with a modular cobalt chrome acetabular liner, highly cross-linked polyethylene mobile bearing, and a cementless titanium femoral stem. Patient-reported outcome measures, whole blood metal ion levels (μg/l), and periprosthetic femoral BMD were measured at baseline and at one, two, and five years postoperatively. The results two years postoperatively for this cohort have been previously reported. Results A total of 43 patients were enrolled. At minimum follow-up of five years, 23 (53.4%) returned for clinical and radiological review, 25 (58.1%) had metal ion analysis performed, 19 (44.2%) underwent dual energy x-ray absorptiometry scans, and 25 (58%) completed a pain-drawing questionnaire. The mean modified Harris Hip Scores improved significantly from 54.8 (SD 19) preoperatively to 93.08 (SD 10.5) five years postoperatively (p < 0.001). One patient was revised for aseptic acetabular loosening. The mean cobalt levels increased from 0.065 μg/l (SD 0.03) to 0.08 (SD 0.05) and the mean titanium levels increased from 0.35 (SD 0.13) to 0.78 (SD 0.29). The femoral BMD ratio decreased in Gruen Zone 1 (91.9%) at five years postoperatively compared with the baseline scores at six weeks potoperatively. The femoral BMD ratio was maintained in Gruen zones 2 to 7. Conclusion The use of a modular DM component and a cementless, tapered femoral stem shows excellent mid-term survivorship with minimal concerns for corrosion and metal ion release in a cohort of young, active patients undergoing primary THA. Human bones are biological examples of functionally graded lattice capable to withstand large in vivo loading and allowing optimal stress distribution. Disruption of bone integrity may require biocompatible implants capable to restore the original bone structure and properties. This study aimed at comparing mechanical properties and biological behavior in vitro of uniform (POR-FIX) and graded (POR-VAR) Cobalt-chrome alloy lattice structures manufactured via Selective Laser Melting. In compression, the POR-VAR equivalent maximum stress was about 2.5 times lower than that of the POR-FIX. According to the DIC analysis, the graded lattice structures showed a stratified deformation associated to unit cells variation. At each timepoint, osteoblast cells were observed to colonize the surface and the first layer of both scaffolds. Cell activity was always significantly higher in the POR-VAR (p< 0.0005). In terms of gene expression, the OPG/RANKL ratio increased significantly over time (p< 0.0005) whereas IL1β and COX2 significantly decreased (7 day vs 1 day; p< 0.0005) in both scaffolds. Both uniform- and graded-porosity scaffolds provided a suitable environment for osteoblasts colonization and proliferation, but graded structures seem to represent a better solution to improve stress distribution between implant and bone of orthopedic implants.

Custom 3D printed total talus implants have been used successfully as a functional alternative to arthrodesis or amputation in cases of severe talar destruction or loss. However, the ideal material and construct still remains to be elucidated. Current models have been made from aluminum ceramic, cobalt chrome, stainless steel, titanium, or metal combinations. The implants may be constrained (subtalar arthrodesis) or unconstrained (press fit within mortise). They may also be combined with a tibial prosthesis or used in isolation. The majority of currently published case studies examine unconstrained and isolated implants. This case study presents satisfactory 1-y outcomes in 3 cobalt chrome constrained total talar implants used in combination with a tibial prosthesis, and a literature review of total talus replacements.

Adverse reactions to metallic debris from corrosion of polished cobalt-chromium–cemented femoral stems are reported. Cobaltism (systemic cobalt poisoning) has not been reported from this phenomenon. Three patients presented to their surgeon for ongoing care 10-20 years after primary metal-on-plastic hip arthroplasty with the same polished cobalt-chromium–cemented femoral stems (Heritage, Zimmer). Urine cobalt was elevated, and the patients had symptoms consistent with cobaltism. Quantitative-F16DG-PET-CT brain imaging was performed showing generalized and focal brain hypometabolism consistent with cobalt encephalopathy. At revision, all stems were well fixed and grossly corroded. At 1 year after revision, cobalturia and cognitive symptoms were resolved or improved. Mechanically assisted crevice corrosion at the polymethylmethacrylate interface is a complication of polished cobalt-chromium–cemented stems that can result in systemic cobalt exposure and toxic encephalopathy. Our cases had only minor periprosthetic symptoms. Patients implanted with polished cobalt-chromium–cemented stems warrant monitoring with urine cobalt. Patients with cobaltemia warrant an evaluation for toxic encephalopathy.

Objectives To examine the retention force of removable dental prosthesis (RDP) clasps made from polyetheretherketone (PEEK) and cobalt-chrome-molybdenum (CoCrMo, control group) after storage in water and artificial aging. Materials and methods For each material, 15 Bonwill clasps with retentive buccal and reciprocal lingual arms situated between the second pre- and first molar were manufactured by milling (Dentokeep [PEEKmilled1], NT digital implant technology; breCAM BioHPP Blank [PEEKmilled2], bredent), pressing (BioHPP Granulat for 2 press [PEEKpressed], bredent), or casting (remanium GM 800+ [CoCrMo], Dentaurum); N = 60, n = 15/subgroup. A total of 50 retention force measurements were performed for each specimen per aging level (initial; after storage [30 days, 37 °C] and 10,000 thermal cycles; after storage [60 days, 37 °C] and 20,000 thermal cycles) in a pull-off test. Data were statistically analyzed using one-way ANOVA, post hoc Scheffé and mixed models (p < 0.05). Results Initial, PEEKpressed (80.2 ± 35.2) and PEEKmilled1 (98.9 ± 40.3) presented the lowest results, while PEEKmilled2 (170.2 ± 51.8) showed the highest values. After artificial aging, the highest retention force was observed for the control group (131.4 ± 56.8). The influence of artificial aging was significantly higher for PEEK-based materials. While PEEKmilled2 and PEEKpressed showed an initial decline in retention force, all other groups presented no impact or an increase in retention force over a repetitive insertion and removal of the clasps. Conclusions Within the tested PEEK materials, PEEKmilled2 presented superior results than PEEKpressed. Although CoCrMo showed higher values after artificial aging, all materials exhibited sufficient retention to recommend usage under clinical conditions. Clinical relevance As RDPs are still employed for a wide range of indications, esthetic alternatives to conventional CoCrMo clasps are sought.

Oxidized zirconium (OxZr) femoral heads were developed to emulate the superior wear and scratch properties of ceramics while eliminating their unwanted tendency for brittle fracturing. Studies using OxZr have demonstrated reduced linear and volumetric wear of polyethylene (PE) liners when compared with cobalt chrome, ceramic, and steel femoral heads. The purpose of this systematic review was to determine the difference in revision rates, radiologically determined linear wear rates, and patient-reported outcomes among OxZr and alternative bearing materials. Following the Preferred Reporting Items for Systematic Review and Meta-Analysis protocols, a systematic review of published literature through April 24, 2018 was conducted. Among the 862 cases included at a mean weighted follow-up of 4.78 years, the revision THA rate and patient-reported outcomes were similar among OxZr and alternative bearing materials. The demands for aesthetics in implant-supported full-arch prosthesis increased the use of zirconia as framework material due to its aesthetics, biocompatibility and high survival rate. The aim of this study was to compare the mechanical response of Zirconia and CoCr custom-milled framework indicated for maxillary prosthetic rehabilitations using the Finite Element Method. To perform this simulation, a custom-milled framework design for an implant-supported full-arch fixed dental prosthesis was used. The geometries of bone, prosthesis, implants, abutments and prosthetic screw were modelled. The mechanical properties for each isotropic and homogeneous material were simulated. Two frameworks were simulated (YZTP and CoCr Alloy). A load of 500 N load was applied on the occlusal surface of the right upper first molar. The results were analysed in terms of displacement, von Mises stress and microstrain. After the simulation processing, it was not possible to observe difference for prosthesis displacement or stress concentration regarding the framework material. The use of YTZP exhibited the lowest stress magnitude for implant (60 MPa) near the load application site, in comparison with the metallic framework (76 MPa in the same region). The same behaviour was calculated for the microstrain results in peri-implant region. The use of YZTP to perform a custom-milled framework design for an implant-supported full-arch fixed dental prosthesis may have acceptable mechanical response for the analysed structures.

Introduction Femoral head-neck modularity in total hip arthroplasty (THA) is advantageous but taper corrosion at the trunnion can result in implant failure. We report two cases of acute catastrophic trunnion failure with a TMZF alloy cementless stem. Methods Demographic, clinical, radiographic and operative data including implant retrieval was recorded and is presented. Results Case 1: A 79 year old farmer presented with sudden onset of hip pain and an inability to weight bear. He underwent a cementless large diameter stemmed metal-on-metal system (MITCH acetabular component, 56mm cobalt chrome head 4.5 lateralised Accolade TMZF, Stryker) nine years previously. He denied symptoms prior to his presentation to the Emergency Department. Case 2: An 86 year old gentleman presented with sudden onset of hip pain and inability to weight bear. He underwent a cementless large diameter stemmed metal-on-poly THA (Trident acetabular component, X3 polyethylene insert, 44mm cobalt chrome head, 4.5 lateralised Accolade TMZF, Stryker)nine years previously. This man had been complaining of mild hip symptoms prior to presentation. Conclusion Patients that have received TMZF alloy cementless stems coupled with CoCr alloy heads are at risk of catastrophic trunnion failure. Importantly, background trunnion corrosion may occur silently and present emergently irrespective of surveillance.

Numerous biomechanical studies comparing titanium (Ti) and cobalt-chrome-alloy (CCM) rods are described in the literature. However, there is a dearth of literature comparing the two rod materials in adolescent idiopathic scoliosis (AIS). Therefore, the purpose of this study is to compare the correction rates of Ti and CCM rods in the treatment of AIS with double major curves. We enrolled 45 patients with AIS who underwent surgery between 2009 and 2012. We divided patients into two groups, Group A (n = 29) treated with six-millimeter Ti rods and Group B (n = 16) treated with six-millimeter CCM rods. The rod-derotation maneuver was used for correction. We measured pre- and postoperative indices of coronal alignment (Cobb’s angle, coronal balance, T1-tilt, clavicle angle) and sagittal alignment (sagittal vertical axis, thoracic kyphosis, lumbar lordosis). In our study, there were no significant differences between the two groups with respect to demographics or curve characteristics (P > 0.05). In Group A, thoracic and lumbar curvature correction rates were 71.2% and 66.8% respectively, and in Group B they were 71.2% and 73.3%, respectively (P = 0.664 and 0.09). There were no significant differences between the two groups in coronal or sagittal factors (P > 0.05) except for greater postoperative lumbar lordosis in the CCM group (P in vitro is different in biological condition.

Finite-element analysis. Intraoperative contouring of rods is a common procedure for spine surgeons to match the native curvature of the spine, but it may lead to premature weakening of the rod. This study investigated the effect of different bending methods on rod fatigue performance. Rod failure in the cervical spine is of clinical concern, particularly when spanning the cervicothoracic region and when considering corrective osteotomies for deformity correction and global spinal alignment. Finite-element models were developed to simulate rod bending (3.5 mm D, 40 mm L) to achieve a 23° angle with 3 different bending methods: French single, multiple bending, and in situ bending. Simulations were conducted in 4 steps: rod bending, rod spring back, residual stress relaxation, and F1717 mechanical test simulation. French single bending resulted in the highest residual stress concentrations for both titanium (TiAlV) and cobalt chrome (CoCr) at 783 MPa and 507 MPa, respectively. During F1717 test simulation, the French single bent rod had its highest tensile stress in the middle, with 917 MPa and 623 MPa, respectively, for TiAlV and CoCr, compared to in situ (580 MPa and 586 MPa for TiAlV and CoCr) and the French multiple bent rod (765 MPa and 619 MPa for TiAlV and CoCr). The computational model found that CoCr rods made the construct least prone to deformation. French single bend with TiAlV rods put the construct at highest risk of failure. CoCr rods led to minimal physical changes in microstructure while showing evidence of flattening.

The main requirement for powders for additive machines is the spherical shape of the particles. Such particles most compactly fit into a certain volume and provide the “fluidity” of the powder composition in the material supply systems with minimal resistance. Based on the peculiarities of the methods of obtaining spherical powders in order to obtain spherical granules of a regulated grain size, the technology of electroerosive dispersion, which is distinguished by relatively low energy costs and ecological cleanliness of the process, is proposed. The main advantage of the proposed technology is the use of waste as raw materials, which is much cheaper than the pure components used in traditional technologies. In addition, this technology is powder, which allows to obtain powder-alloys. The widespread use of the method of EED for the processing of metal waste into powders for the purpose of their reuse and application in additive technologies is hampered by the lack of complete information in the scientific and technical literature on the influence of the original composition, modes and media on the properties of powders and technologies of practical application. Therefore, the development of technologies for the reuse of EED powders and the evaluation of the effectiveness of their use requires the conduct of comprehensive theoretical and experimental studies. The purpose of this work was to obtain and study additive products from electroerosive cobalt-chromium powders of a specific particle size distribution and to study their microstructure. The granulometry of the obtained powders was determined on a laser analyzer of particle sizes “Analysette 22 NanoTec”. The microstructure of additive samples from cobalt-chromium powders (by transverse polishing) was investigated by optical microscopy on an inverted optical microscope OLYMPUS GX51. On the basis of completed studies, aimed at obtaining and studying additive products from electroerosive cobalt-chrome powders of a specific particle size distribution, and studying their microstructure, it was found that additive samples, obtained from a cobalt-chrome powder with an average particle size of 35,68 microns, have practically no pores.

Fretting corrosion at the head-neck taper junction was compared between silicon nitride (Si3 N4 ) and commercially available cobalt chrome (CoCrMo) femoral heads on titanium (Ti-6Al-4V) trunnions. An electrochemical setup was used to capture the fretting currents (characterized by oxide abrasion and repassivation) during cyclic loading. Onset load, pull-off force (disassembly load), short term and long term (1 million cycles) fretting currents were used to compare the fretting corrosion performance between the test group (Si3 N4 /Ti-6Al-4V) and the control group (CoCrMo/Ti-6Al-4V). The aim to find the perfect biomaterial for spinal implant has been the focus of spinal research since the 1800s. Spinal surgery and the devices used therein have undergone a constant evolution in order to meet the needs of surgeons who have continued to further understand the biomechanical principles of spinal stability and have improved as new technologies and materials are available for production use. The perfect biomaterial would be one that is biologically inert/compatible, has a Young’s modulus similar to that of the bone where it is implanted, high tensile strength, stiffness, fatigue strength, and low artifacts on imaging. Today, the materials that have been most commonly used include stainless steel, titanium, cobalt chrome, nitinol (a nickel titanium alloy), tantalum, and polyetheretherketone in rods, screws, cages, and plates. Current advancements such as 3-dimensional printing, the ProDisc-L and ProDisc-C, the ApiFix, and the Mobi-C which all aim to improve range of motion, reduce pain, and improve patient satisfaction. Spine surgeons should remain vigilant regarding the current literature and technological advancements in spinal materials and procedures. The progression of spinal implant materials for cages, rods, screws, and plates with advantages and disadvantages for each material will be discussed.

Study Design: This was a biomechanical study. Objective: Shape loss of surgical spine rods has been implicated as a factor leading to postsurgical loss of alignment correction. Our objective was to compare the degree of shape loss in surgical spine rods of different compositions under physiological conditions that were bent before or after being autoclaved. Methods: 10 CoCr and 10 commercially pure titanium (CPTi) surgical spine rods were contoured using a machine press. Five CoCr and 5 CPTi rods were bent before being autoclaved (preoperative bent group); 5 CoCr and 5 CPTi rods were bent after being autoclaved (intraoperative bent group). All rods were immersed in a phosphate-buffered saline bath at body temperature (37.2°C ± 2°C). Changes in radius of curvature were measured at different time intervals over an 8-week course using a high-definition scanner. Results: Each rod demonstrated shape loss in radius of curvature (range = 1.04-9.99 mm) over the duration of the study. Intraoperatively bent CPTi rods demonstrated the largest shape loss (range = 8.73-9.99 mm; median 9.33 mm; P < .01). Preoperatively bent CPTi (range = 1.04-1.71 mm; median = 1.39 mm; P < .01) and intraoperatively bent CoCr (range = 1.11-2.11 mm; median = 2.01 mm; P < .01) rods underwent the least amount of shape loss. Conclusion: CPTi spinal rods bent after autoclave may lead to considerable loss of alignment correction. In addition, our results suggest that preautoclave bent CPTi and CoCr spinal rods bent after autoclave may be a more ideal choice of implant because they may provide more resistance to shape loss over time.

Background Although literature exists regarding hip and knee arthroplasty outcomes in patients with skin allergy to metals, there is minimal information about skin allergy implications on shoulder arthroplasty outcomes. The purpose of this study was to determine the results, complications, and failure rate among patients with a self-reported metal allergy undergoing shoulder arthroplasty. Methods Fifty-two shoulder arthroplasties were performed at our Institution in 43 patients with self-reported metal allergies. Forty primary and 12 revision shoulder arthroplasties were performed using anatomic (30) and reverse (22) components. Retrospective chart review was performed to determine metal allergy history, implant composition, pain, motion, and complications. Radiographs were reviewed to determine mechanical failure rates. Average follow-up time was 65 months. Results Allergies reported included nickel (37), cobalt chrome (4), copper (2), zinc (1), titanium (1), gold (1), and nonspecific metal allergy (8); 8 patients reported multiple metal allergies. All components implanted in patients with nickel allergies contained nickel. At most recent follow-up, pain was rated as none or mild in 88% of shoulders. Active elevation improved from 80° to 141° and external rotation from 24° to 52°. Two revisions were performed for glenoid loosening (3.8%); both were revision cases with substantial glenoid bone loss. One patient with mild pain had a radiographically loose glenoid component 12 years after anatomic shoulder arthroplasty. Conclusion Results from this study suggest that shoulder arthroplasty in patients with self-reported metal allergy provides satisfactory pain relief and improved range of motion with low revision rates.

Purpose Evaluating the fitness accuracy and retentive force of cobalt-chrome (Co-Cr) alloy clasps fabricated using the selective laser melting (SLM) technique. Methods Premolar and molar abutment models with a 0.5-mm undercut depth, 1.5-mm-thick occlusal rest seats, and guiding planes were designed and fabricated using a milling machine. On these models, Akers clasps with 0.25- and 0.5-mm undercut depths were designed and fabricated with SLM and a traditional lost wax casting method. Based on the manufacturing methods, abutment types, and undercut depths, the clasps were divided into eight groups (10 per group). The fitness accuracy of the clasps was evaluated by measuring the gap distance between the clasps and abutments using a silicone film method. The initial retentive force and changes in retention up to 7,200 insertion/removal cycles of the clasps were also measured. The data were analyzed using multiple linear regression, paired t-tests, and one-way ANOVA (α=0.05). Results For both the SLM and cast clasps, the fitness accuracy of the rest was greater than that of the clasp tip and shoulder. No significant difference was found in the fitness accuracy between the SLM and cast clasps, regardless of the abutment type and undercut depth before or after insertion/removal cycles (p>0.05). There was also no significant difference in the initial retentive force between the SLM and cast clasps (p>0.05). After 7,200 insertion/removal cycles, the SLM clasp exhibited a greater residual retentive force (p Conclusion The SLM technique for manufacturing the clasps of removable partial dentures has promising clinical applications.

Background:Implant wear in joint replacements is influenced by the chemical and physical properties of human synovial fluid (HSF). In vitro testing for implant wear uses 25% weight bovine calf serum (25BCS) to substitute for HSF, due to similar rheology and total protein content. However, previous studies have shown differences in the macromolecular composition. We aimed to evaluate the differences in small molecule composition between fluids and correlate these differences with their effects on implant material wear.Methods:HSF was harvested from osteoarthritis patients undergoing primary knee replacement (n=14). Nuclear magnetic resonance (NMR) spectroscopy with linear regression modelling analysed the metabolites present in HSF and commercially sourced 25BCS and investigated the differences. Wear properties of the fluids were evaluated using a validated quantitative laboratory bench-test utilising a cobalt/chromium/molybdenum (CoCrMo) ball oscillating against a CoCrMo disc and analysing the resulting wear scar. The variation in metabolite levels in both HSF and 25BCS was correlated to the wear properties of the fluids.Results:Differences in the levels of metabolites, lipids, and glycosaminoglycans (GAG) were observed between HSF and 25BCS: significance was confirmed by O-PLS-DA (pConclusions:The small molecule concentration differences between the fluids questions the validity of 25BCS as a model for wear analysis. The demonstration of variable metabolites present in HSF which correlate with material wear has implications for implant failure and targeted therapeutic manipulation of these metabolites. Recent progress in metal additive manufacturing methods allowed fabrication of parts with complex geometry while attaining comparable or superior to parts fabricated using conventional methods. Considering its efficient use of material, potential for light-weighing and role in closing the digital to physical gap, additive manufacturing emerged as a promising technology. In AM, the commonly used methods are selective laser melting (SLM) and Electron Beam Melting (EBM). Cobalt Chrome (CoCr) is gaining popularity as a biomaterial parts produced via AM. In this study, mechanical and corrosion properties of additively manufactured CoCr alloy are investigated and compared to those of the same alloy produced via casting. Samples produced using AM (SLM & EBM) and casting are compared in terms of mechanical properties (tensile strength, yield strength & hardness) and corrosion resistance. The microstructures were observed using SEM and corrosion resistance were tested using EIS. The mechanical properties of SLM printed parts are found to be more superior than cast produced part due to the smaller grain size of SLM printed parts. The grain size for SLM printed part is 30 µm while for cast is 4000 µm. For yield strength, the values were 840 MPa and 730 MPa for both SLM & EBM as-built samples while for cast sample was at 450 MPa. The yield strength values decreased to 622 MPa and 560 MPa for both SLM & EBM respectively post HIP-ing. For tensile strength, the values were 1070 MPa and 960 MPa for both SLM & EBM respectively while for cast samples, the value was at 655 MPa. As for hardness values (HRC), they were at 38 and 47 for both SLM & EBM respectively while for cast samples, the value was at 30. Post HIP-ing, hardness values (HRC) decreased to 36 and 34 respectively. Meanwhile, the corrosion resistance, Rct value for AM sample is at 6.21 ± 3.91 MU cm−2 while for cast sample is at 3.74 ± 1.86 MU cm−2. In conclusion, AM produced parts fare better than conventional cast part in terms of mechanical properties (tensile strength, yield strength & hardness) and corrosion resistance.

The results of microanalysis of additive products from electro-erosive cobalt-chrome powders are presented. It has been established experimentally that the obtained samples of additive products from powders, obtained by electro-erosive dispersion of cobalt-chrome wastes in alcohol, have a microcrystalline structure with a porosity of less than 0,78%. The obtained experimental data can be used in the development of promising resource-saving technologies for the manufacture of machine parts.

Introduction: Post-implantation rod deformation is anticipated in scoliosis surgery but the difference in rod deformation between titanium and cobalt chrome rod has not been elucidated. This study aims to compare the difference in rod deformation between two groups. Materials and Methods: Twenty-one adolescent idiopathic scoliosis (AIS) patients were recruited from a single center. The over-contoured concave rods were traced prior to insertion. Post-operative sagittal rod shape was determined from lateral radiographs. Rod deformation was determined using maximal rod deflection and angle of the tangents to rod end points. The differences between pre- and post-operative rod contour were analysed statistically. Rod deformation and thoracic kyphosis between two types of implants were analysed. Results: Both rods exhibited significant change of rod angle and deflection post-operatively. Curvature of the titanium rod and cobalt chrome rod decreased from 60.5° to 37°, and 51° to 28° respectively. Deflection of titanium rod and cobalt chrome rod reduced from 28mm to 23.5mm and 30mm to 17mm respectively. There was no significant difference between titanium and cobalt chrome groups with regard to rod angle (p=0.173) and deflection (p=0.654). Thoracic kyphosis was increased from 20° to 26° in titanium group but a reduction from 25° to 23° was noticed in cobalt chrome group, but these findings were not statistically significant. Conclusion: There was no statistical difference in rod deformation between the two groups. Thus, the use of titanium rod in correction of sagittal profile is not inferior in outcome compared with cobalt chrome but with lower cost.

The aim of this in vitro study was to investigate the microgaps at the implant–abutment interface when zirconia (Zr) and CAD/CAM or cast Co–Cr abutments were used. Methods: Sixty-four conical connection implants and their abutments were divided into four groups (Co–Cr (milled, laser-sintered and castable) and Zirconia (milled)). After chewing simulation (300,000 cycles, under 200 N loads at 2 Hz at a 30° angle) and thermocycling (10,000 cycles, 5 to 50 °C, dwelling time 55 s), the implant–abutment microgap was measured 14 times at each of the four anatomical aspects on each specimen by using a scanning electron microscope (SEM). Kruskal–Wallis and pair-wise comparison were used to analyze the data (α = 0.05). Results: The SEM analysis revealed smaller microgaps with Co–Cr milled abutments (0.69–8.39 μm) followed by Zr abutments (0.12–6.57 μm), Co–Cr sintered (7.31–25.7 μm) and cast Co–Cr (1.68–85.97 μm). Statistically significant differences were found between milled and cast Co–Cr, milled and laser-sintered Co–Cr, and between Zr and cast and laser-sintered Co–Cr (p &lt, 0.05). Conclusions: The material and the abutment fabrication technique affected the implant–abutment microgap magnitude. The Zr and the milled Co–Cr presented smaller microgaps. Although the CAD/CAM abutments presented the most favorable values, all tested groups had microgaps within a range of 10 to 150 μm.

Loosening is considered as a main cause of implant failure in total knee replacement (TKR). Among the predictive signs of loosening, migration is the most investigated quantitative parameter. Several studies focused on the migration of the tibial component in TKR, while no reviews have been focused on the migration of the femoral component and its influence on patients’ clinical outcomes. The aim of this narrative review was (1) to provide information about of the influence of migration in femoral component of TKR prostheses, (2) to assess how migration may affect patient clinical outcomes and (3) to present alternative solution to the standard cobalt-chrome prostheses. A database search was performed on PubMed Central® according to the PRISMA guidelines for studies about Cobalt-Chrome femoral component migration in people that underwent primary TKR published until May 2020. Overall, 18 articles matched the selection criteria and were included in the study. Few studies investigated the femoral component through the migration, and no clear migration causes emerged. The Roentgen Stereophotogrammetric Analysis has been mostly used to assess the migration for prognostic predictions. An annual migration of 0.10 mm seems compatible with good long-term performance and good clinical and functional outcomes. An alternative solution to cobalt-chrome prostheses is represented by femoral component in PEEK material, although no clinical evaluations have been carried out on humans yet. Further studies are needed to investigate the migration of the femoral component in relation to clinical outcomes and material used.

Background Patients suffering from osteoarthritis undergo surgery to replace hip joints with hip prosthesis implants. Today most acetabular cups of hip prostheses are made of Ultra-High-Molecular-Weight-Polyethylene. However, these materials acting as acetabular cups of the implant have been recalled since patients have been feeling uncomfortable and abstained from physical activities. A newly introduced material, 30% Carbon Reinforced Polyetherketone, possess better isotropic mechanical properties and lower wear rates. Objective The research aims to compare the von-Mises stresses and deformation in static and dynamic loading of Ultra-High Molecular-Weight-Polyethylene to 30% Reinforced Carbon Fiber Polyetherketone using Finite Element Analysis. Material and methods An analytical study was performed to evaluate material selection and their contact performances of acetabular cups. Four pairs have been analyzed under loading conditions following ASTM F2996-13 and ISO 7206-4 standards. The acetabular cups options are made of 30% Carbon Reinforced Fiber Polyetherketone or Ultra-High-Molecular-Weight-Polyethylene. Besides, the femoral head and steam options are either Alumina Ceramic or Cobalt Chrome Molybdenum. Results The yield strength of Ultra-High-Molecular-Weight-Polyethylene is considerably small, resulting in the acetabular cup to fail when applied to high loading conditions. Carbon Reinforced Polyetherketone with Alumina Ceramic yielded 65% lower deformation at stumbling phase. Conclusion Since the study focuses on linear isotropic material properties, Alumina Ceramic dominates a higher elastic modulus than Cobalt Chrome Molybdenum, nominating it the best fit combination for lower von-Mises stresses, acting on the Carbon Reinforced Polyetherketone acetabular cup.

sBackgroundTitanium surface coating on cobalt-chromium (CoCr) alloy has characteristics desirable for an orthopedic implant as follows: strength, osteointegrative capability, and biocompatibility. Creating such a coated surface takes a challenging process and two dissimilar metals are not easily welded. In our study, we utilized additive manufacturing with a 3D printing called direct metal fabrication (DMF) and compared it to the plasma spraying method (TPS), to coat titanium onto CoCr alloy. We hypothesized that this would yield a coated surface quality as acceptable or better than the already established method of plasma spraying. For this, we compared characteristics of titanium-coated surfaces created by direct metal fabrication method (DMF) and titanium plasma spraying (TPS), both in vitro and in vivo, for (1) cell morphology, (2) confocal microscopy images of immunofluorescent assay of RUNX2 and fibronectin, (3) quantification of cell proliferation rate, (4) push-out biomechanical test, and (5) bone histomorphometry.MethodFor in vitro study, human osteoblast cells were seeded onto the coated surfaces. Cellular morphology was observed with a scanning electron microscope. Cellular proliferation was validated with ELISA, immunofluorescent assay. For in vivo study, coated rods were inserted into the distal femur of the rabbit and then harvested. The rods were biomechanically tested with a push-out test and observed for histomorphometry to evaluate the microscopic bone to implant ratio.ResultFor cell morphology observation, lamellipodia and filopodia, a cytoplasmic projection extending into porous structure, formed on both surfaces created by DMF and TPS. The proliferation of the osteoblasts, the DMF group showed a better result at different optic density levels (p= 0.035, 0.005, 0.001). Expression and distribution of fibronectin and Runx-2 genes showed similar degrees of expressions. The biomechanical push-out test yielded a similar result (p= 0.714). Histomorphometry analysis also showed a similar result (p= 0.657).ConclusionIn conclusion, DMF is a method which can reliably create a proper titanium surface on CoCr alloy. The resulting product of the surface shows a similar quality to that of the plasma spraying method, both in vivo and in vitro, in terms of biological and mechanical property.
