Abstract

International audience; The main problem for the patient who wants to have access to all of the information about his health is that this information is very often spread over many medical records. Therefore, it would be convenient for the patient, after being identified and authenticated, to use a kind of specific medical search engine as one part of the solution to this main problem. The principal objective is for the patient to have access to his or her medical information at anytime and wherever it has been stored. This proposal for secure "Google Like" access requires the addition of different conditions: very strict identity checks using cryptographic techniques such as those planned for the electronic signature, which will not only ensure authentication of the patient and integrity of the file, but also protection of the confidentiality and access follow-up. The electronic medical record must also be electronically signed by the practitioner in order to provide evidence that he has given his agreement and accepted responsibility for the content. This electronic signature also prevents any kind of post-transmission falsification. New advances in technology make it possible to envisage access to medical records anywhere and anytime, thanks to Grid and watermarking methodologies.

Abstract

International audience; When dealing with medical image management, there is a need to ensure information authenticity and dependability. Being able to verify the information belongs to the correct patient and is issued from the right source is a major concern. Verification can help to reduce the risk of errors when identifying documents in daily practice or when sending a patient's Electronic Health Record. At the same time, patient privacy issues may appear during the verification process when the verifier accesses patient data without appropriate authorization. In this paper we discuss the combination of watermarking with different identifiers ranging from DICOM standard UID to an Anonymous European Patient Identifier in order to improve medical image protection in terms of authenticity and maintainability.

16. Medical record search engines, using pseudonymised patient identity: an alternative to centralised medical records

Abstract

Purpose The purpose of our multidisciplinary study was to define a pragmatic and secure alternative to the creation of a national centralised medical record which could gather together the different parts of the medical record of a patient scattered in the different hospitals where he was hospitalised without any risk of breaching confidentiality. Methods We first analyse the reasons for the failure and the dangers of centralisation (i.e. difficulty to define a European patients' identifier, to reach a common standard for the contents of the medical record, for data protection) and then propose an alternative that uses the existing available data on the basis that setting up a safe though imperfect system could be better than continuing a quest for a mythical perfect information system that we have still not found after a search that has lasted two decades. Results We describe the functioning of Medical Record Search Engines (MRSEs), using pseudonymisation of patients' identity. The MRSE will be able to retrieve and to provide upon an MD's request all the available information concerning a patient who has been hospitalised in different hospitals without ever having access to the patient's identity. The drawback of this system is that the medical practitioner then has to read all of the information and to create his own synthesis and eventually to reject extra data. Conclusions Faced with the difficulties and the risks of setting up a centralised medical record system, a system that gathers all of the available information concerning a patient could be of great interest. This low-cost pragmatic alternative which could be developed quickly should be taken into consideration by health authorities.

46. Secure Teleassistance towards endless medical litigations: Identification of liabilities through a protocol using Joint Watermarking-Encryption Evidences.

Abstract

Teleassistance is defined by the help provided through a telemedicine network by a medical practitioner to one other medical practitioner faced to a difficult case. One of the main limiting factors of its development is the fear of the practitioners to be involved in a litigation. In such a situation, the main issue is to determine as quick and as certain as possible if the damage is in relation with the tort of negligence and the liabilities of each involved physician. After a brief summary of the legal context, we present a protocol combining joint watermarking-encryption and a third party to enforce exchange traceability and therefore to bring valuable electronic evidence in case of teleassistance litigations. Abstract

Objectives: Compiling individual records coming from different sources is necessary for multi-center studies. Legal aspects can be satisfied by implementing anonymization procedures. When using these procedures with a different key for each study it becomes almost impossible to link records from separate data collections.Methods: The originality of the method relies on the way the combination of hashing and enciphering techniques is performed: like in asymmetric encryption, two keys are used but the private key depends on the patient's identity.Results: The combination of hashing and enciphering techniques provides a great improvement in the overall security of the proposed scheme.Conclusion: This methodology makes stored data available for use in the field of public health, while respecting legal security requirements.
