The treatment options for aberrant right subclavian artery vary depending on the presence of Kommerell’s diverticulum. Because there is a tendency not to report mortalities of these rare cases in the literature, it is hard to reach a conclusion on treatments from the limited data on post-interventional results in these patients. Objective: We aimed to report our surgical experience and outcomes in patients operated for intracardiac mass in our clinic. Materials and Methods: Six patients were operated for an intracardiac mass between May 2006 and March 2015. Five patients were female and 1 was male; the mean age of the study population was 54 (34-82) years. One patient presented with dyspnea and tachycardia secondary to pulmonary thromboembolism (PTE) and one patient with syncope, while the other patients presented with constitutional symptoms such as fatigue and dyspnea. Definitive diagnosis was made by a combination of thoracoabdominal tomography (CT) and echocardiography in one patient, and transthoracic echocardiography (TTE) and/or transesophageal echocardiography (TEE) in the remaining patients. The time from the diagnosis to the operation of masses was 2.5 days (1-4 days) on average. Four patients aged over 45 years underwent coronary angiography, irrespective of gender. The operations were performed via cardiopulmonary bypass, under standard aorta bicaval cannulation, antegrade cold blood cardioplegia, and moderate hypothermia. All defects in the patients with the initial diagnosis of cardiac myxoma were primarily closed. One patient was operated for a right atrial mass (thrombus). None of the patients suffered an intraoperative complication. The patients were discharged after an average of 9.6 days. Results: Histopathological diagnosis was a thrombus secondary to hypercoagulopathy related to antipsychotic drug use in one patient, and myxoma in the other patients. All myxomas were located in the left atrium while thrombi were in the right atrium, extending to the inferior vena cava. None of the patients had a family history for intracardiac mass. All patients enjoyed a clear clinical benefit upon mass removal. No early or late-term mortality was observed. There were no recurrences between 1 to 106 months (17.6 months on average) during early or late follow-up. Conclusion: Intracardiac masses, with myxomas being in the first place, may cause embolism, syncope, palpitations, and dyspnea; they should thus be remembered in the differential diagnosis of other intracardiac pathologies and operated at an early stage as soon as they are diagnosed; the tumor mass should be totally excised with a wide resection. © 2016 by Gazi University Medical Faculty.

A 59-year-old male patient was admitted to our clinic with the complaint of chest pain. Coronary angiography revealed acute myocardial infarction due to a thrombosis at the right coronary artery. Herein, we report a case undergoing quadruple coronary bypass grafting and revascularization and coronary thrombectomy on right coronary artery due to atherosclerotic heart disease and inferior myocardial infarction.

Factor XII deficiency is an important hematological problem which is characterized by isolated prolongation of the activated partial thromboplastin time and presents with thrombosis and thromboembolism tendency rather than expected bleeding diathesis in clinical practice. A 55-year-old male patient who was scheduled for aortic coronary bypass graft surgery had elevated activated partial thromboplastin time (109.4 sec) as a laboratory finding. The patient was diagnosed with factor XII deficiency and operated in our clinic. No thromboembolic event was seen during follow-up. The patient was discharged in the seventh postoperative day uneventfully. Herein, we highlight the importance of this disease due to its rare occurrence and present our treatment strategy applied.

The ankylosing spondilitis a chronic rheumatic inflammatory disorder characterized by the involvement of vertebrae and sacroiliac joints. It may attack eyes, alimentery system, heart and kidneys extraarticulary, and it can cause cardiovasculary complications in a range of an asymptomatic course to high morbidity and mortality. Hence a timely diagnosis is of prime clinical importance. A female patient of 53 years of age applied to our hospital with angina pectoris and her medical history revealed a 16 years of follow-up due to ankylosing spondilitis in another health institution. The echocardiography established a left ventricular wall motion abnormality and a mild mitral insufficiency. In the elective coronary angiography the ectatic LAD was detected to be stenozed and a LIMA single vessel by-pass was planned for this patient. In this report, we would like to enroll our experience of coronary revascularization on an ectatic coronary artery with stenosis in an ankylosing spondylosis patient. ©Copyright 2014 by Gazi University Medical Faculty

PubMed ID: 25583007, BACKGROUND: Cryopreservation is known to have a detrimental effect on the motility, viability and membrane integrity of sperm cells. OBJECTIVE: The aim of this study was to investigate the effect of various amount of linoleic acid supplementation to the Tris extender, on bull sperm parameters, DNA integrity and oxidative stress after freeze-thawing. METHODS: Ejaculates were split into five aliquots and extended to a final concentration of 18x10 6 spermatozoa/ml with the base extender containing different doses of linoleic acid 0.125 ml, (L125); 0.250 ml (L250); 0.5 ml (L500), 1 ml (L1000) and no additive (control; L0). The extended samples were equilibrated slowly to 4°C for 4 h and then froze using a digital freezing machine. Frozen straws were thawed individually in water bath at 37°C for 30 s to analyse progressive motility and sperm motion characteristics as well as membrane integrity. Biochemical assays were performed in a spectrophotometer using commercial kits. DNA damage was evaluated by Comet Assay. RESULT: The addition of various linoleic acid did not improve the sperm subjective, CASA and progressive motilities, sperm motility characteristics and DNA integrity (P>0.05). PubMed ID: 9726620, Objective: Neurological injury due to transient cerebral ischemia is a potential complication of cardiovascular surgery. The neuroprotective effect of magnesium, when given subcutaneously before the ischemia, was assessed in a rat model of transient global cerebral ischemia. Methods: Thirty-six male Wistar albino rats were included to this randomized, controlled, prospective study. In 24 animals, ischemia was induced with four-vessel occlusion technique with the duration of 15 min. MgSO4 was given 600 mg/kg subcutaneously 48 h before the procedure in group 1 (n = 12). Similar volume of saline solution was used in animals of control group (group 2, n = 12). The animals in group 3 (sham group, n = 12) were anesthetized and subjected to operative dissections without vascular occlusion. Physiological parameters and somatosensory evoked-potentials (SEP) were monitored in animals before ischemia, during ischemia and in the first 30 min of reperfusion. Their neurological outcome had been clinically evaluated and scored up to 4 days postischemia. The intergroup differences were compared. Then the animals were sacrificed and their brains were processed for histopathological examination. Results: In group 3, SEP amplitudes did not change during the procedures, and all animals recovered without neurologic deficits. At the end of ischemic period, the average amplitude was reduced to 5 ± 3% of the baseline in all ischemic animals. This was followed by a gradual return to 87 ± 10% and 83 ± 8% of the initial amplitude after 30 min of reperfusion in group 1 and group 2, respectively (P > 0.05). The average neurological score was significantly higher in group 1 than in group 2 at 48, 72 and 96 h after the ischemic insult (P < 0.05). Histological observations were clearly correlated with the neurological findings. Conclusion: The results suggest that subcutaneous MgSO4 reduces cerebral injury and preserves neurologic function when given two days before the transient global ischemia in rats.
