AIMS: Diabetes is a known risk factor for coronary artery disease. There is accumulating evidence that coronary artery disease pathogenesis differs for individuals with type 1 diabetes. However, the genetic background has not been extensively studied. We aimed to discover genetic loci increasing coronary artery disease susceptibility especially in type 1 diabetes, to examine the function of these discoveries and to study the role of the known risk loci in type 1 diabetes. METHODS AND RESULTS: We performed the largest genome-wide association study to date for coronary artery disease in type 1 diabetes, comprising 4869 individuals with type 1 diabetes (cases/controls: 941/3928). Two loci reached genome-wide significance, rs1970112 in CDKN2B-AS1 (OR = 1.32, p=1.50 x 10-8), and rs6055069 on DEFB127 promoter (OR = 4.17, p=2.35 x 10-9), with consistent results in survival analysis. The CDKN2B-AS1 variant replicated (p=0.04) when adjusted for diabetic kidney disease in three additional type 1 diabetes cohorts (cases/controls: 434/3123). Background and purpose: Cerebral small vessel disease is characterized by progressive white matter hyperintensities (WMH) and cognitive decline. However, variability exists in how individuals maintain cognitive capabilities despite significant neuropathology. The relationships between individual cognitive reserve, psychological resilience and cognitive functioning were examined in subjects with varying degrees of WMH. Methods: In the Helsinki Small Vessel Disease Study, 152 subjects (aged 65–75 years) underwent a comprehensive neuropsychological assessment, evaluation of subjective cognitive complaints and brain magnetic resonance imaging with volumetric WMH evaluation. Background The aim of this study was to assess the association between endothelial function and early‐onset cryptogenic ischemic stroke (CIS), with subgroup analyses stratified by sex and age groups. Methods and Results We prospectively enrolled 136 consecutive patients aged 18 to 49 years (median age, 41 years; 44% women) with a recent CIS and 136 age‐ and sex‐matched (±5 years) stroke‐free controls. Endothelial function was measured with an EndoPAT 2000 device and analyzed as tertiles of natural logarithm of reactive hyperemia index with lower values reflecting dysfunction. We used conditional logistic regression adjusting for age, education, hypertension, diabetes mellitus, dyslipidemia, current smoking, heavy drinking, obesity, and diet score to assess the independent association between endothelial function and CIS. Patients in the lowest tertile of natural logarithm of reactive hyperemia index were more often men and they more frequently had a history of dyslipidemia; they were also more often obese, had a lower diet score, and lower high‐density lipoprotein cholesterol. In the entire cohort, we found no association in patients with endothelial function and CIS compared with stroke‐free controls. In sex‐ and age‐specific analyses, endothelial dysfunction was associated with CIS in men (adjusted odds ratio [OR], 3.50 for lowest versus highest natural logarithm of reactive hyperemia index tertile; 95% CI, 1.22–10.07) and in patients ≥41 years (OR, 5.78; 95% CI, 1.52–21.95). These associations remained significant when dyslipidemia was replaced with the ratio of total to high‐density lipoprotein cholesterol. Conclusions Endothelial dysfunction appears to be an independent player in early‐onset CIS in men and patients approaching middle age.

Introduction: Patients with small core infarction and salvageable penumbra are likely to benefit from endovascular treatment (EVT). As computed tomography perfusion imaging (CTP) is not always available 24/7 for patient selection, many patients are transferred to stroke centers for CTP. We compared automatically measured infarct core volume (NCCTcore) from the non-contrast computed tomography (NCCT) with ischemic core volume (CTPcore) from CTP and the outcome of EVT to clarify if NCCTcore measurement alone is sufficient to identify patients that benefit from transfer to stroke centers for EVT. Patients and methods: We included all consecutive stroke-code patients imaged with both NCCT and CTP at Helsinki University Hospital during 9/2016–01/2018. NCCTcore and CTPcore volumes were automatically calculated from the acute NCCT images. Follow-up infarct volume (FIV) was measured from 24 h follow-up NCCT to evaluate efficacy of EVT. To study whether NCCTcore could be used to identify patients eligible to EVT, we sub-grouped patients based on NCCTcore volumes (>50 mL and ≥ 70 mL). Results: Out of 1743 patients, baseline NCCTcore, CTPcore and follow-up NCCT was available for 288 patients. Median time from symptom onset to baseline imaging was 74 min (IQR 52–118), and time to follow-up imaging 24.15 h (22.25–26.33). Baseline NCCTcore was 20 mL (10–42), CTPcore 4 mL (0–16), and FIV 5 mL (1–49). Out of 288 patients, 23 had NCCTcore ≥ 70 mL and 26 had CTPcore ≥ 70 mL. NCCTcore and CTPcore performed similarly well in predicting large FIV (≥70 ml). Conclusion: NCCTcore is a promising tool to identify patients that are not eligible to EVT due to large ischemic cores at baseline imaging.

Traumatic brain injury (TBI) causes deficits in executive function (EF), as well as problems in behavioural and emotional self-regulation. Neurological music therapy may aid these aspects of recovery. We performed a cross-over randomized controlled trial where 40 persons with moderate-severe TBI received a 3-month neurological music therapy intervention (2 times/week, 60 min/session), either during the first (AB, n = 20) or second (BA, n = 20) half of a 6-month follow-up period. The evidence from this RCT previously demonstrated that music therapy enhanced general EF and set shifting. In the current study, outcome was assessed with self-report and caregiver-report questionnaires performed at baseline, 3-month, 6-month, and 18-month stages. The results showed that the self-reported Behavioural Regulation Index of the Behaviour Rating Inventory of Executive Function (BRIEF-A) improved more in the AB than BA group from baseline to 3-month stage and the effect was maintained in the 6-month follow-up. No changes in mood or quality of life questionnaires were observed. However, a qualitative content analysis of the feedback revealed that many participants experienced the intervention as helpful in terms of emotional well-being and activity. Our results suggest that music therapy has a positive effect on everyday behavioural regulation skills after TBI.

Mitochondrial acyl-coenzyme A species are emerging as important sources of protein modification and damage. Succinyl-CoA ligase (SCL) deficiency causes a mitochondrial encephalomyopathy of unknown pathomechanism. Here, we show that succinyl-CoA accumulates in cells derived from patients with recessive mutations in the tricarboxylic acid cycle (TCA) gene succinyl-CoA ligase subunit-β (SUCLA2), causing global protein hyper-succinylation. Using mass spectrometry, we quantify nearly 1,000 protein succinylation sites on 366 proteins from patient-derived fibroblasts and myotubes. Interestingly, hyper-succinylated proteins are distributed across cellular compartments, and many are known targets of the (NAD+)-dependent desuccinylase SIRT5. To test the contribution of hyper-succinylation to disease progression, we develop a zebrafish model of the SCL deficiency and find that SIRT5 gain-of-function reduces global protein succinylation and improves survival. Thus, increased succinyl-CoA levels contribute to the pathology of SCL deficiency through post-translational modifications., The pathomechanism of succinyl-CoA ligase (SCL) deficiency, a hereditary mitochondrial disease, is not fully understood. Here, the authors show that increased succinyl-CoA levels contribute to SCL pathology by causing global protein hyper-succinylation.

Background: Non-adherence to medications is common in bipolar disorder but its prevalence estimations have varied significantly. In addition, non-adherence is known to increase the risk of poor clinical outcomes. Therefore, we studied how common primary non-adherence for mood stabilizers and antipsychotics is in Finland and which factors are associated with it.Methods: Nationwide sample of persons diagnosed with bipolar disorder during 1987-2018 were identified from registers. Dispensings of their electronic prescriptions during 2015-2018 were followed up to define rates of primary non-adherence. Primary non-adherence was defined as having at least one non-dispensed mood stabi-lizer or antipsychotic prescription during 2015-2018. In a broader definition, non-adherence was defined as having >= 20 % of mood stabilizer and/or antipsychotic prescriptions non-dispensed. Adjusted logistic regression was used to assess risk factors for non-adherence.Results: The study cohort included 33,131 persons and 59.1 % had at least one non-dispensed mood stabilizer or antipsychotic prescription. 31.0 % of the cohort was non-adherent to >= 20 % of their mood stabilizer and/or antipsychotic prescriptions. Lithium and clozapine had the lowest proportions of non-dispensed prescriptions. Especially young age, recent bipolar disorder diagnosis, multiple hospitalizations due to bipolar disorder, and use of benzodiazepines or antidepressants were associated with an increased risk of non-adherence.Limitations: This study was based on register data, and patient-reported reasons for medication non-adherence could not be included.Conclusions: The majority of patients with bipolar disorder do not use their medications as prescribed. Patient -specific risk for non-adherence should be assessed and those at high risk for non-adherence should be fol-lowed closely.

Mice homozygous for the human GRACILE syndrome mutation (Bcs1l (c.A232G)) display decreased respiratory chain complex III activity, liver dysfunction, hypoglycemia, rapid loss of white adipose tissue and early death. To assess the underlying mechanism of the lipodystrophy in homozygous mice (Bcs1l(p.S)(78G)), these and wild-type control mice were subjected to a short 4-hour fast. The homozygotes had low baseline blood glucose values, but a similar decrease in response to fasting as in wild-type mice, resulting in hypoglycemia in the majority. Despite the already depleted glycogen and increased triacylglycerol content in the mutant livers, the mice responded to fasting by further depletion and increase, respectively. Increased plasma free fatty acids (FAs) upon fasting suggested normal capacity for mobilization of lipids from white adipose tissue into circulation. Strikingly, however, serum glycerol concentration was not increased concomitantly with free FM, suggesting its rapid uptake into the liver and utilization for fuel or gluconeogenesis in the mutants. The mutant hepatocyte mitochondria were capable of responding to fasting by appropriate morphological changes, as analyzed by electron microscopy, and by increasing respiration. Mutants showed increased hepatic gene expression of major metabolic controllers typically associated with fasting response (Ppargc1a, Fgf21, Cd36) already in the fed state, suggesting a chronic starvation-like metabolic condition. Despite this, the mutant mice responded largely normally to fasting by increasing hepatic respiration and switching to FA utilization, indicating that the mechanisms driving these adaptations are not compromised by the CIII dysfunction. Summary statement: Bcs1l mutant mice with severe CIII deficiency, energy deprivation and post-weaning lipolysis respond to fasting similarly to wild-type mice, suggesting largely normal systemic lipid mobilization and utilization mechanisms.

Introduction The prevalence of hypoalbuminemia, early changes of plasma albumin (P-Alb) levels, and their effects on mortality in cardiogenic shock are unknown. Materials and methods P-Alb was measured from serial blood samples in 178 patients from a prospective multinational study on cardiogenic shock. The association of hypoalbuminemia with clinical characteristics and course of hospital stay including treatment and procedures was assessed. Theprimary outcome was all-cause 90-day mortality. Results Hypoalbuminemia (P-Alb < 34g/L) was very frequent (75%) at baseline in patients with cardiogenic shock. Patients with hypoalbuminemia had higher mortality than patients with normal albumin levels (48% vs. 23%, p = 0.004). Odds ratio for death at 90 days was 2.4 [95% CI 1.5-4.1] per 10 g/L decrease in baseline P-Alb. The association with increased mortality remained independent in regression models adjusted for clinical risk scores developed for cardiogenic shock (CardShock score adjusted odds ratio 2.0 [95% CI 1.1-3.8], IABPSHOCK II score adjusted odds ratio 2.5 [95%CI 1.2-5.0]) and variables associated with hypoalbuminemia at baseline (adjusted odds ratio 2.9 [95%CI 1.2-7.1]). In serial measurements,albumin levels decreased at a similar rate between 0h and 72h in both survivors andnonsurvivors (ΔP-Alb -4.6 g/L vs. 5.4 g/L, p = 0.5). While the decrease was higher for patients with normal P-Alb at baseline (p 0.001 compared to patients with hypoalbuminemia at baseline), the rate of albumin decrease was not associated with outcome. Conclusions Hypoalbuminemia was a frequent finding early in cardiogenic shock, and P-Alb levels decreased during hospital stay. Low P-Alb at baseline was associated with mortality independently of other previously described risk factors. Thus, plasma albumin measurement should be part of the initial evaluation in patients with cardiogenic shock.

Background: Psychotherapy for depression aims to reduce symptoms and to improve psychosocial functioning. We examined whether some symptoms are more important than others in the association between depression and functioning over the course of psychotherapy treatment.Methods: We studied associations between specific symptoms of depression (PHQ-9) and change in social and occupational functioning (SOFAS), both with structural equation models (considering liabilities of depression and each specific symptom) and with logistic regression models (considering the risk for individual patients). The study sample consisted of adult patients (n symbolscript 771) from the Finnish Psychotherapy Quality Registry (FPQR) who completed psychotherapy treatment between September 2018 and September 2021.Results: Based on our results of logistic regression analyses and SEM models, the baseline measures of depression symptoms were not associated with changes in functioning. Changes in depressed mood or hopelessness, problems with sleep, feeling tired, and feeling little interest or pleasure were associated with improved func-tioning during psychotherapy. The strongest evidence for symptom-specific effects was found for the symptom of depressed mood or hopelessness. Limitations: Due to our naturalistic study design containing only two measurement points, we were unable to study the causal relationship between symptoms and functioning. Conclusions: Changes in certain symptoms during psychotherapy may affect functioning independently of un-derlying depression. Knowledge about the dynamics between symptoms and functioning could be used in treatment planning or implementation. Depressed mood or hopelessness appears to have a role in the dynamic relationship between depression and functioning.

Objectives The purposes of this study were to localize monocyte chemoattractant protein-1-induced protein-1 (MCPIP-1) and its suppressor mucosa-associated lymphoid tissue lymphoma translocation protein 1 (MALT-1) in gingival tissues and to profile their protein expression levels in relation to the clinical inflammation, Porphyromonas gingivalis colonization, and interleukin (IL)-8 levels. Materials and methods Study samples were collected from two independent study populations: (1) Gingival tissues were collected from eight periodontally healthy individuals and eight periodontitis patients to localize MCPIP-1 and MALT-1 immunohistochemically, and (2) forty-one gingival tissue samples with marginal, mild, or moderate to severe inflammation were collected from 20 periodontitis patients to determine MCPIP-1 and MALT-1 levels using immunoblots, P. gingivalis levels with qPCR, P. gingivalis gingipain activities with fluorogenic substrates, and IL-8 levels with multiplex technique. Results MCPIP-1 was detectable in the epithelium and in connective tissue, being especially prominent around the blood vessel walls in healthy periodontal tissues. MALT-1 was observed at all layers of gingival epithelium and especially around the accumulated inflammatory cells in connective tissue. No difference in gingival tissue MCPIP-1 and MALT-1 levels was observed in relation to the severity of gingival inflammation. MALT-1 levels were elevated (p = 0.023) with the increase in tissue P. gingivalis levels, and there was an association between MALT-1 and IL-8 levels (β = 0.054, p = 0.001). Conclusions Interactions of MALT-1 levels with gingival tissue P. gingivalis counts and IL-8 levels suggest that activation of MALT-1 can take part in P. gingivalis-regulated host immune responses. Clinical relevance Pharmacological targeting the crosstalk between immune response and MCPIP-1/MALT-1 may have benefits in periodontal treatment.

Background Occupational class inequalities in physical functioning and their changes after retirement are poorly understood. We examined occupational class trajectories in physical functioning 10 years before and after transition to old-age and disability retirement. We included working conditions and behavioural risk factors as covariates, given their established link to health and retirement.Methods We used the Helsinki Health Study cohort data from surveys 2000–2002 to 2017, and included 3901 women, who were employed by the City of Helsinki, Finland, and retired during the follow-up. Mixed-effect growth curve models were used to examine changes in RAND-36 Physical Functioning subscale (range 0–100) 10 years before and after the retirement date by occupational class.Results Old-age (n=3073) and disability retirees (n=828) lacked class differences in physical functioning 10 years before retirement. By retirement transition, physical functioning declined and class inequalities emerged, the predicted scores being 86.1 (95% CI 85.2 to 86.9) for higher class and 82.2 (95% CI 81.5 to 83.0) for lower class old-age retirees, and 70.3 (95% CI 67.8 to 72.9) for higher class and 62.2 (95% CI 60.4 to 63.9) for lower class disability retirees. Physical functioning declined and class inequalities slightly widened among old-age retirees after the retirement, whereas among disability retirees the decline plateaued and class inequalities narrowed over time after retirement. Physical work and body mass index somewhat attenuated the class inequalities after adjustment.Conclusions Class inequalities in physical functioning widened after old-age retirement and narrowed after disability retirement. Background: Hartmann's procedure is a treatment option for perforated acute diverticulitis, especially when organ dysfunction(s) are present. Its use has been criticized mostly out of fear of high permanent stoma rate. The aim of this study was to investigate the rate of stoma reversal, reasons behind non-reversal, and safety of reversal surgery. Methods: This was a single-center retrospective study of patients undergoing urgent Hartmann's pro-cedure due to acute diverticulitis between the years 2006 and 2017 with follow-up until March 2021. Results: A total of 3,319 episodes of diverticulitis in 2,932 patients were screened. The Hartmann's procedure was performed on 218 patients, of whom 157 (72%) had peritonitis (48 (22%) with organ dysfunction). At 2-years, 76 (34.9%) patients had died with stoma, 42 (19.3%) were alive with stoma, and 100 (45.9%) had undergone stoma reversal. The survival of patients with and without reversal were 100% and 42.7% at 1-year, 96.0% and 35.0% at 2-years and 88.9% and 20.7% at 5-years, respectively. The risk factors for nonreversal were old age, a need for outside assistance, low HElsinki Staging for Acute Diverticulitis stage, and higher C-reactive protein level upon hospital admission. The most common reasons for nonreversal in surviving patients were patient not willing to have the operation 18 (41%) and dementia 10 (23%). Twelve (12%) patients had a major complication after reversal (Clavien-Dindo IIIb-IV) and 90-day mortality after reversal was 0%. Conclusion: After the Hartmann's procedure for acute diverticulitis, one-third died, half underwent stoma reversal, and one-fifth did not undergo stoma reversal within 2 years. Patients who survive with stoma are either not willing to have reversal or have severe comorbidities excluding elective surgery. The Hartmann's procedure remains a viable option for high-risk patients with perforated acute diverticulitis. (c) 2022 The Author(s). Published by Elsevier Inc.

Background and hypothesis: Pathogenic understanding of the psychotic disorders converges on regulation of dopaminergic signaling in mesostriatocortical pathways. Functional connectivity of the mesostriatal pathways may inform us of the neuronal networks involved. Study design: This longitudinal study of first episode psychosis (FEP) (49 patients, 43 controls) employed seed-based functional connectivity analyses of fMRI data collected during a naturalistic movie stimulus. Study results: We identified hypoconnectivity of the dorsal striatum with the midbrain, associated with antipsychotic medication dose in FEP, in comparison with the healthy control group. The midbrain regions that showed hypoconnectivity with the dorsal striatum also showed hypoconnectivity with cerebellar regions suggested to be involved in regulation of the mesostriatocortical dopaminergic pathways. None of the baseline hypoconnectivity detected was seen at follow-up. Conclusions: These findings extend earlier resting state findings on mesostriatal connectivity in psychotic disorders and highlight the potential for cerebellar regulation of the mesostriatocortical pathways as a target of treatment trials.

Increased self-focus is a core factor in the psychopathology of depression. Cortical midline structures (CMS) are implicated in the neurobiology of self, depression and antidepressant treatment response. Mirtazapine, an antidepressant that increases serotonin and norepinephrine release, enhances processing of positive and attenuates processing of negative emotional information in healthy volunteers after a single dose. These early changes, which are opposite to the negative information bias in depression, may be important for the therapeutic effect of mirtazapine. It nevertheless remains unresolved whether/how mirtazapine specifically influences processing of self-referential emotional information. Half of the healthy volunteers ( n=15/30) received a single dose of mirtazapine, in an open-label design, two hours before functional magnetic resonance imaging (fMRI), and the other half was scanned as a control group without medication. During fMRI the participants categorized positive and negative self-referential adjectives. Mirtazapine attenuated responses to self-referential processing in the medial prefrontal cortex and the anterior cingulate cortex. Mirtazapine further decreased responses to positive self-referential processing in the posterior cingulate cortex and parietal cortex. These decreased responses of the CMS suggest that mirtazapine may rapidly improve the ability of the CMS to down-regulate self-referential processing. In depressed patients, this could lead to decreased self-focus and rumination, contributing to the antidepressant effect.

Non-alcoholic fatty liver disease (NAFLD) affects up to a quarter of the population in the Western countries. It is tightly associated with insulin resistance and the metabolic syndrome (’Metabolic NAFLD’) and increased risk of type 2 diabetes, cardiovascular disease and advanced liver disease. Liver fat is composed of several types of lipids, such as metabolically inert triglycerides and insulin resistance-inducing ceramides and diacylglycerols. Common polymorphisms in genes such as PNPLA3, MBOAT7 and TM6SF2 increase the risk of advanced liver disease, but do not affect insulin resistance (’Genetic NAFLD’). Studies I-III were performed to investigate whether a different composition of liver fat could explain this dissociation between the risk of liver disease and insulin resistance by determining the molecular lipid composition of liver biopsy samples of 125 patients undergoing bariatric surgery. In ’Metabolic NAFLD’, the liver was enriched in triglycerides, ceramides and diacylglycerols. In ’Genetic NAFLD’, the liver was also enriched in triglycerides, but not with ceramides or diacylglycerols. This marked difference in hepatic lipid composition could explain the dissociation between the risk of liver disease and insulin resistance in subtypes of NAFLD. Liver fat can originate from dietary fat or adipose tissue lipolysis, or hepatic de novo lipogenesis (DNL). Study IV aimed to determine the effects of dietary macronutrient composition (saturated or unsaturated fat or sugar) on overfeeding-induced (1000 kcal/day) changes in liver fat content and insulin resistance in 38 subjects. Methods included magnetic resonance imaging, stable isotope infusions, hyperinsulinemic-euglycemic clamps as well as plasma lipidomics, adipose tissue transcriptomics and fecal microbiome analyses. Overfeeding of saturated fat for 3 weeks increased liver fat content more than unsaturated fat by stimulating adipose tissue lipolysis. Saturated fat also increased insulin resistance, circulating ceramides and fecal gram-negative bacteria. Overfeeding of sugar increased liver fat content by stimulating DNL. The diets had distinct effects on the adipose tissue transcriptome. In conclusion, the magnitude and mechanisms underlying overfeeding-induced changes in liver fat content and insulin resistance depend on the macronutrient composition of the diet. Overfeeding of saturated fat was metabolically more harmful for the liver as compared to unsaturated fat or sugar. In conclusion, this series of studies demonstrated that NAFLD is a heterogenous disease. Both the common genetic polymorphisms and the macronutrient composition of the diet have a marked effect on hepatic lipid metabolism and the resulting phenotype. Se on vahvasti yhteydessä insuliiniresistenssiin ja metaboliseen oireyhtymään (‘metabolinen NAFLD’) sekä lisääntyneeseen tyypin 2 diabeteksen, valtimotaudin sekä edenneen maksasairauden riskiin. Maksan rasva koostuu useista rasvatyypeistä, kuten aineenvaihdunnallisesti neutraaleista triglyserideistä sekä insuliiniresistenssiä aiheuttavista keramideista ja diasyyliglyseroleista. Yleiset muutokset geeneissä, kuten PNPLA3, MBOAT7 ja TM6SF2, lisäävät edenneen maksataudin riskiä, mutta eivät vaikuta insuliiniresistenssiin (‘Geneettinen NAFLD’). Osatöissä I-III tutkittiin, selittääkö maksan erilainen rasvakoostumus tämän maksatauti- ja insuliiniresistenssiriskin irtikytkeytymisen määrittämällä se 125:n laihdutusleikkauspotilaan maksan koepalasta. ‘Metabolisessa NAFLD:ssa’ maksaan oli väkevöitynyt triglyseridejä, keramideja ja diasyyliglyseroleja. ‘Geneettisessä NAFLD:ssa’ maksaan oli niinikään väkevöitynyt triglyseridejä, mutta ei diasyyliglyseroleja tai keramideja. Tämän eron maksan rasvakoostumuksessa todettiin selittävän hyvin tautiriskien irtikytkeytyminen rasvamaksataudin eri alatyypeissä. Maksan rasva voi olla peräisin ruokavalion rasvasta, rasvakudoksen pilkkoutumisesta tai rasvan uudismuodostuksesta. Osatyössä IV tutkittiin ruokavalion makroravinnekoostumuksen (tyydyttynyt tai tyydyttymätön rasva, tai sokeri) vaikutusta ylensyönnin (1000 kcal/päivä) aiheuttamaan maksan rasvapitoisuuden ja insuliiniresistenssin muutokseen 38 tutkimushenkilöllä. Tutkimusmenetelminä käytettiin magneettiresonanssikuvantamista, stabiili isotooppi-infuusioita ja hyperinsulineemista-euglykeemista clamp-menetelmää sekä plasman lipidomiikka-, rasvakudoksen transkriptomiikka- ja suoliston mikrobianalyyseja. Tyydyttyneen rasvan ylensyönti lisäsi kolmen viikon aikana maksan rasvapitoisuutta enemmän kuin tyydyttymätön rasva lisäämällä rasvakudoksen pilkkoutumista. Tyydyttynyt rasva lisäsi myös insuliiniresistenssiä, verenkierron keramideja sekä suoliston gram-negatiivisia bakteereita. Sokerin ylensyönti lisäsi maksan rasvapitoisuutta kiihdyttämällä rasvan uudismuodostusta maksassa. Ruokavalioilla oli hyvin erilaiset vaikutukset rasvakudoksen transkriptomiikkaprofiiliin. Ylensyömisen aiheuttamien vaikutusten maksan rasvapitoisuuteen ja insuliiniresistenssiin sekä niiden taustalla oleviin mekanismeihin todettiin riippuvan ruokavalion makroravinnekoostumuksesta. Tyydyttyneen rasvan ylensyönti oli haitallisempaa maksan aineenvaihdunnalle verrattuna tyydyttymättömään rasvaan tai sokeriin. Yhteenvetona väitöskirjatutkimuksessa todettiin, että alkoholiin liittymätön rasvamaksatauti on monimuotoinen sairaus. Sekä yleiset geenimuutokset että ravinnon makroravinnekoostumus vaikuttavat maksan rasva-aineenvaihduntaan ja taudin ilmiasuun.

We describe two Finnish siblings in whom an incidentally detected elevated creatine kinase activity eventually led to a diagnosis of limb-girdle muscular dystrophy-dystroglycanopathy (Type C12; MDDGC12). When diagnosed at age 10 and 13 years, they were mildly affected with a slow or non-progressive disease course. The main symptoms comprised infrequent hip cramps triggered by flexion, neck cramps triggered by yawning, transient growing pains, calf hypertrophy and mild proximal muscle weakness. Their cognitive and motor developments were unremarkable and they were physically active. Whole-exome sequencing revealed compound heterozygous mutations, both of which were novel, in the protein O-mannosyl kinase (POMK) gene in both siblings; a missense mutation, p.Pro322Leu (c.965C > T), and a nonsense mutation, p.Arg46Ter (c.136C > T). The results were confirmed by Sanger sequencing, showing that the parents were heterozygous carriers of one mutation each. Parkinson’s disease (PD) is the most common movement disorder, affecting approximately 1% of the population aged over 60 years. According to current knowledge, there are both genetic and environmental risk factors for this disease. High plasma uric acid level has been shown to reduce the risk of PD and also to decelerate the progression of motor symptoms in patients with PD. In this study, 40 PD patients and 28 spouse controls were recruited to examine whether patients have low plasma and daily urine uric acid levels relative to the controls. Also neuropsychological testing was performed on patients both at baseline and after three years of follow-up to determine whether cognition is associated with the uric acid levels and whether low levels of uric acid at baseline predict future cognitive decline. An electroencephalogram with evoked response potentials (EEG-ERP) was given to a subsample of patients and controls to examine neurophysiological differences between the groups and to investigate whether cognitive alterations are reflected in patients’ EEG-ERP measurements. PD patients had lower levels of plasma uric acid than controls. Low plasma and urine uric acid levels were associated with poor achievement in neuropsychological tests measuring visuospatial and visuoconstructive abilities, sustaining attention and executive control. After three years of follow-up, only minor cognitive decline was noted in patients. Cognition did not seem to be associated with past or present uric acid levels. On the other hand, the stability of the patient sample made it difficult to assess the effect of uric acid on the prognosis of cognitive decline in PD. The EEG-ERP study revealed that PD patients had longer latency and poorer habituation of the evoked response potential N100 than healthy controls. In the patient group, the neurophysiological changes were associated with poor achievement in a neuropsychological test measuring visual working memory, but not with uric acid levels. Uraatti Parkinsonin-taudissa Uraattia esiintyy ihmisten verenkierrossa korkeina pitoisuuksina, koska ihminen on jossain lajinkehityksen vaiheessa menettänyt uraattia hajottavan urikaasi-entsyymin. Tämä entsyymipuutos on ilmeisesti ollut etu evoluutiossa sillä korkea uraatti-pitoisuus suojaa elimistöä vapaan raudan ja happiradikaalien solutuhoa aiheuttavilta vaikutuksilta ja saattaa jopa olla osasyy ihmisten pitkään elinikään. Verenkierron uraatti-pitoisuus kuitenkin vaihtelee eri ihmisten välillä sekä perinnöllisistä syistä että ruokavaliosta riippuen. Hyvin korkea uraatti altistaa kivuliaalle niveltulehdukselle eli kihdille ja matala puolestaan Parkinsonin-taudille. Parkinsonin-tauti on etenevä neurologinen sairaus, johon sairastutaan keskimäärin 62-vuoden iässä. Sairauden oireina esiintyy vapinaa, jäykkyyttä, liikkeiden hitautta, univaikeuksia, ummetusta, mielialan laskua ja myöhemmässä vaiheessa osalle potilaista kehittyy myös muistisairauden oireita. Parkinsonin-tautiin tunnetaan edelleenkin ainoastaan oireisiin vaikuttavaa hoitoa eikä sairautta voida parantaa. Parkinsonin-taudin syy on tuntematon mutta nykykäsityksen mukaan sairaus syntyy geenien ja ympäristön yhteisvaikutuksesta. Elimistön uraatti-pitoisuus määräytyy perinnöllisten tekijöiden, ruokavalion, painon ja lääkityksen perusteella. Myös sukupuoli vaikuttaa niin että miesten pitoisuus on keskimäärin korkeampi kuin naisten. Väitöstutkimuksessa osoitettiin että Parkinsonin-tautia sairastavilla on matalampi seerumin uraatti-pitoisuus kuin samaa ruokavaliota noudattavilla puoliso-verrokeilla. Matalan uraatti-pitoisuuden on todettu altistavan Parkinsonin-taudin motoristen oireiden etenemiselle erityisesti taudin alkuvaiheessa. Tutkimuksessa saatiin myös viitteitä siitä että matala uraatti liittyisi Parkinsonin-taudin muistisairauden oireiden esiintymiseen. Tutkimuksen potilasmäärä oli kuitenkin liian pieni uraatin ennustevaikutuksen arvioimiseksi eli ei tiedetä altistaako matala uraatti myös muistisairauden oireiden etenemiselle.

Status epilepticus (SE), i.e. prolonged epileptic seizure, is a life-threatening medical emergency, which is associated with high mortality and morbidity. International guidelines suggest early and efficient treatment. Thus, long duration of SE is one of the main predictors of poor prognosis and the only prognostic factor that can be affected by shortening the delays in the treatment. However, studies on delays, implementation of treatment guidelines and the effect of delays on outcome are scarce. The aim of this thesis was to systematically investigate delays in the treatment of SE and factors related to the delays along the whole treatment chain. We also aimed at clarifying the effect of delays on the outcome and at identifying the significant delays related to outcome in order to propose evidence-based targets for streamlining the SE treatment protocol. The material of this retrospective study consists of 82 consecutive SE patients treated in a tertiary hospital emergency department over two years. Delays, patient characteristics and parameters related to treatment chain were identified and their relations, correlations and effects were investigated. The results of this thesis reveal that the delays in the treatment of SE are unacceptably long and exceed markedly the suggested time frames in the guidelines. Fulfilment of the suggested SE treatment algorithm is frequently hampered by failing recognition of SE at onset, also by professionals, which may increase the delays in consecutive parts of the treatment chain. Delays seem to be more significant determinants of SE duration than previously established outcome predictors. Additionally, various long delays in the treatment (second- and third-stage medication, diagnostic and tertiary hospital delays) increase the risk of mortality and poor functional outcome at hospital discharge and since the predictive cut-off point of these delays lies under 2,5 hours, the focus of protocol streamlining should be in the pre-hospital phase of the treatment. However, none of the delays are independent risk factors for poor outcome, which reflects the dynamism of SE, but also demonstrates that every step of the treatment chain needs to be optimized. In conclusion, we propose that generation of simplified criteria for suspicion of an imminent SE and streamlining pre-hospital treatment chain are advocated. We suggest amendments to the protocol, such as triaging suspected SE patients with highest priority, recruiting physician-based EMS units upon primary alarm, administration of second-stage medication out-of-hospital and transportation of SE patients exclusively to hospitals with neurological expertise. Also improvement of diagnostic possibilities on emergency site should be considered. Kohtauksen pitkä kesto on yksi merkittävimmistä huonoon ennusteeseen liittyvistä tekijöistä. Kansainväliset hoitosuositukset suosittavat nopeaa ja tehokasta hoitoa SE:n lopettamiseksi. Tutkimustieto hoidon viiveistä, hoitosuositusten toteutumisesta ja näiden vaikutuksista ennusteeseen on kuitenkin erittäin vähäistä. Väitöstutkimuksen tavoitteena oli selvittää status epilepticuksen hoitoon liittyviä viiveitä ja viiveisiin vaikuttavia tekijöitä hoitoketjun eri vaiheissa. Lisäksi tavoitteena oli tutkia viiveiden vaikutusta SE-potilaiden ennusteeseen ja tunnistaa sen kannalta merkittävimmät viiveet ja tekijät, jotta hoitoketjun toiminnan tehostaminen tulevaisuudessa olisi mahdollista. Tavoitteiden saavuttamiseksi tarkasteltiin Helsingin Yliopistollisen Keskussairaalan päivystyksessä kahden vuoden aikana hoidettujen aikuisten SE-potilaiden asiakirjamerkinnät. Tutkimustulosten perusteella viiveet status epilepticuksen hoidossa ovat pitkiä ja ylittävät kestoltaan selvästi kansainvälisten hoitosuositusten viiterajat. Hoidon viiveet näyttävät vaikuttavan SE:n kestoon enemmän kuin muut aiemmin tunnistetut ennustetekijät. Lääkehoitoihin, diagnostiikkaan ja hoitopaikan valintaan liittyvillä viiveillä on merkittävä vaikutus SE-potilaiden kuolleisuuteen ja toipumisennusteeseen: mitä lyhyemmät viiveet, sitä parempi ennuste akuuttivaiheen sairaalahoidon päättyessä. Useat eri viiveet, erityisesti hoitoketjun alkupäässä, vaikuttavat merkittävästi ennusteeseen, mikä korostaa hoitoketjun jokaisen vaiheen mahdollisimman hyvän onnistumisen tärkeyttä ennusteen parantamiseksi. Status epilepticuksen hoidon viiveet eivät tutkimuksen perusteella olleet hyväksyttävällä tasolla ja niiden lyhentämiseksi tulisi pyrkiä hoitoketjun virtaviivaistamiseen. Epäiltäessä potilaalla SE:tä lääkäriyksikön tai koulutetun hoitoyksikön hälyttäminen olisi suositeltavaa heti ensivaiheessa, jotta viiveiden lyhentämiseksi kaikki olennaiset lääkkeet ja hoitotoimenpiteet olisi mahdollista toteuttaa jo ennen sairaalaan saapumista. Erityisesti toisen linjan lääkehoidon annostelun mahdollistaminen ensihoidossa olisi suositeltavaa. SE-potilaat tulisi kuljettaa jatkohoitoon sellaiseen sairaalaan, jossa neurologinen erityisosaaminen ja riittävät diagnostiset tutkimusmahdollisuudet ovat ympärivuorokautisessa käytössä.

Facial fractures are a common problem worldwide. They are often caused by falls, falls from heights, motor vehicle accidents, and violence. In Studies I-IV, we retrieved all 2413 emergency room physicians’ multidetector computed tomography (MDCT) requests for facial injury during a 62-month time period. These were categorized by etiology: 155 cases of fall-from-height accidents, 500 cases of falling accidents, 727 cases due to violence, and 374 cases following a motor vehicle accident. The fractures were divided into groups: nasal bone, orbital, naso-orbito-ethmoid, zygomatic complex, zygomatic arch, Le Fort I, II, and III, frontal bone, maxillary, mandibular, and skull base fractures. These were further classified into different subtypes. Paranasal sinus effusions were noted as well. Further, the age, gender, and a more precise injury mechanism were registered. When taking into account the patients with a fracture, males predominated in all our studies (I-IV). In falls, the difference between genders was the smallest and most distinctive in falls from heights. In falls from heights, nasal bone and zygomatic arch fractures were seldom solitary fractures, but were indicators of a more severe fracture. There was a slight increase in the number and severity of fractures with increasing height. However, there was considerable overlap in heights, and thus, falling height cannot be the only predictor of injury probability. In falls, the most common fracture was the zygomatic complex fracture, whereas in violence-related fractures nasal and orbital fractures predominated. In motor vehicle accidents, Le Fort, frontal bone, and zygomatic arch fractures were always accompanied by other fractures. We also found that a clear sinus sign is a valuable aid in detecting facial fractures, but it might be less reliable than previously thought. Le Fort fractures were often asymmetric or unilateral. Postoperative maxillofacial imaging has earlier been somewhat challenging due to the artifacts caused by metal. In Study V, we compared different devices and protocols by scanning a phantom with a 64-slice computed tomography, cone-beam computed tomography, and a high-definition multislice computed tomography with dual-energy scan and iterative reconstruction methods. For postoperative maxillofacial imaging, cone-beam computed tomography was not optimal. Multidetector computed tomography with adaptive statistical iterative reconstruction offers fast image volume reconstruction with good image quality. Kasvomurtumat ovat maailmanlaajuisesti yleisiä vammoja. Ne syntyvät usein kaatumisen, putoamisen, moottoriajoneuvo-onnettomuuden tai väkivallan seurauksena. Osatöitä I-IV varten kävimme läpi 62 kuukauden ajalta Töölön sairaalassa suoritettujen kasvojen monileiketietokonetomografiakuvausten lähetetekstit, ja tapaukset lajiteltiin vammamekanismin perusteella. Kuvauksista 155 tehtiin putoamisten, 500 kaatumisten, 727 väkivallan ja 374 moottoriajoneuvo-onnettomuuksien herättämän kasvomurtumaepäilyn vuoksi. Digitaaliseen kuva-arkistoon tallennetut kuvat tulkittiin murtumien havaitsemiseksi. Murtumat jaoteltiin ryhmiin sijainnin perusteella ja luokittelua jatkettiin vielä murtumien eri alatyyppeihin. Nenän sivuonteloiden effuusiot kirjattiin. Lisäksi potilaiden ikä, sukupuoli sekä vammautumisen tarkempi mekanismi huomioitiin. Kaikissa tutkimissamme ryhmissä murtumapotilaista enemmistö oli miehiä, ero naisiin oli pienin kaatumis- ja suurin putoamistapaturmissa. Putoamisten yhteydessä todettiin, että nenämurtumat ja zygomakaaren murtumat olivat harvoin yksittäisiä löydöksiä, vaan ne viittasivat usein vakavampaan murtumaan. Putoamiskorkeus ei myöskään yksinään määrittänyt vammojen todennäköisyyttä. Kaatumisten yhteydessä yleisin murtumatyyppi oli zygomakompleksin murtumat, kun taas väkivallan seurauksena yleisimmät vammat olivat nenän ja orbitan murtumat. Moottoriajoneuvo-onnettomuuksissa Le Fort -murtumat, frontaaliluun murtumat ja zygomakaaren murtumat eivät koskaan esiintyneet yksittäisinä murtumina. Tutkimuksissa havaitsimme, että Le Fort -murtumat olivat usein epäsymmetrisiä tai toispuolisia. Lisäksi totesimme, että vaikka nenän sivuonteloiden effuusioiden huomioiminen on hyvä apu murtumien havaitsemisessa, se saattaa kuitenkin olla aiemmin ajateltua epäluotettavampi apukeino. Kasvomurtumien leikkauksen jälkeinen kuvantaminen on ollut aiemmin melko haastavaa metallin aiheuttaman artefaktan vuoksi. Osatyössä V vertasimme eri tietokonetomografialaitteita ja protokollia kuvaamalla fantomia. Kartiokeilatietokonetomografia ei ollut optimaalinen kasvomurtumien postoperatiiviseen kuvantamiseen. Sen sijaan monileiketietokonetomografia ASiR:lla (adaptive statistical iterative reconstruction) tuottaa nopeasti hyvälaatuista kuvaa ja soveltuu hyvin kasvovammojen postoperatiiviseen kuvantamiseen.

The aim of the study was to characterize third molars that have remained disease-free in a representative sample of the Finnish population aged 30 years and older. Two-staged stratified cluster sampling (N = 8028) was used, and 6005 subjects participated in a clinical oral examination and panoramic radiography. Disease-free third molars were characterized as follows: no dentinal caries, no filling, periodontal pocket depths of neighboring second molars less than 4 mm, and no radiographic pathological findings. Logistic regression analyses served for assessment of the strength of characteristics of third molars for disease-free status. Of the subjects, 2653 (44%) had at least one third molar. Of them, the majority (62%) had only diseased third molars, 16% had only disease-free ones, and 22% had both. Participants had in total 5665 third molars; 29% of them were disease-free. Background: Preterm birth (before 37 weeks of gestation) is a major cause of infant mortality and morbidity worldwide, and preventing its burden is a health care priority. Even in adulthood, individuals who were born preterm perform, on average, worse on tests of cognitive functioning than term-born peers do, and may have more mental health problems. Early growth failure is common among preterm individuals, and some studies have suggested that those preterm individuals who grow poorly in infancy have more neurodevelopmental problems later on in childhood. It is unclear whether these associations persist into adulthood and whether they extend beyond the smallest and most immature of preterm infants, to the majority of preterm infants who are born late preterm (at 34-36 completed weeks of gestation). It also remains unknown whether early postnatal growth patterns predict mental health outcomes - some partly conflicting evidence suggests that intrauterine growth restriction at least associates with mental health problems. The mechanisms explaining the associations between growth and neurodevelopment are also unclear. Early growth reflects a number of intertwined early-life environmental factors and individual characteristics, and while altering neonatal nutrition can affect growth, it is not known whether changes in nutrition can improve long-term neurodevelopmental outcomes. It has even been suggested that faster growth and higher nutritional intakes during the early postnatal period can present a trade-off between improved neurodevelopment and increased cardiovascular risk. Tämä tutkimus käsittelee kallon saumojen ennenaikaisen sulkeutumisen (kraniosynostoosi) leikkaushoitoa. Perinteiseen leikkausmetodiin avoimeen kallon uudelleen kokoamiseen (kranioplastia) liittyy useita haasteita: leikkauksessa syntyvät luupuutokset jäävät joskus luutumatta,kestävän luiden välisen liitoksen saavuttaminen on haastavaa, toimenpide on hyvin invasiivinen henkeä uhkaavine komplikaatioineen. Uuteen vähemmän invasiiviseen hoitomuotoon kallon venytykseen liittyy myös tällä hetkellä usein ongelmia kuten luun venyttimien hajoamista tai irtoamista. Tutkimuksessa etsittiin ratkaisuja näihin ongelmiin. 18:sta kraniosynostoosipotilaalle avoimen kranioplastian aikana luupuutosten peitoksi oli laitettu demineralisoitu luulevy (DML). Nämä luupuutokset osoittivat parempaa luutumista (74%) kuin vastaavat tyhjät luupuutokset (54%) saman potilaan kallossa. Vertailu tehtiin viikko ja vuosi leikkauksen jälkeen otetuista tietokonetomografia kuvista. DML osoittautui turvalliseksi ja hyödylliseksi materiaaliksi luuputosten peitoksi. Aalto-yliopiston kanssa yhteistössä kehitettiin metodi, jotta kallon venytyshoidossa syntyvät voimatpystytään määrittämään. Mittauksissa todettiin venytystä vastustavan voiman kasvavan jyrkästi venytyksen aikana ja laskevan tämän jälkeen merkittävästi (keskimäärin 69.9%). Keskimääräinen venytyksen aloitusvoima oli 20.4 N ja lopetusvoima 57.6 N. Tutkimuksesta voidaan päätellä, että kallon venytyshoidon komplikaatioita voidaan vähentää jakamalla venytys useampiin lyhyempiin venytys kertoihin. Aalto-yliopiston kanssa yhteistyössä tehdyssä laboratoriokokeissa todettiin, ettei kahdella erilaisellabiohajoavalla luun kiinnitysmekanismilla, perinteisellä ruuvilla ja ultraäänellä aktivoitavalla pinnillä, ole kliinisti merkittävää kestävyys eroa. Toisessa samantyyppisessä laboratoriokokeessa testattiin kolmen erilaisen biohajoavan kiinnitysmekanismin soveltuvuutta kallon luun venyttimen kiinnittämiseen. Tutkimuksessa todettiin, että luun venytin voidaan kiinnittää kalloon 8 biohajovallaruuvilla tai 12:sta biohajoavalla lämmöllä aktivoitavalla pinnillä. Biohajoavalla kiinnityksellä voidaan mahdollisesti vähentää hoitoon liittyviä komplikaatioita ja vähentää luun venyttimen poistoleikkauksen laajuutta.

Despite advances in acute treatment of stroke, over third of the patients suffer from a disability, most often upper-limb paresis, still five years after stroke. To improve rehabil- itation, further understanding of stroke-induced plasticity is required. During the plastic period, cortical excitability increases, likely promoting cortical reorganization. Afferent input modulates the rolandic 20-Hz rhythm. The modulation, reflecting motor- cortex excitability, is observable as the activation-associated suppression and inhibition- associated rebound of the 20-Hz rhythm. In this Thesis, motor-cortex excitability was monitored by applying two different afferent inputs while recording the 20-Hz rhythm with magnetoencephalography (MEG), first in healthy controls and then in stroke patients in a one-year longitudinal study. Study I, comprising 22 healthy controls, focused on the modulation of the 20-Hz rhythm to tactile stimulation and passive movement as proprioceptive stimulation. The suppres- sion of the rhythm was similar to both stimuli whereas the rebound was stronger to pas- sive movement. Thus, passive movement could better serve in studying motor-cortex excitability changes. In Studies II and III, modulation of the 20-Hz rhythm to afferent input was measured in 23 patients having their first-ever stroke in the territory of the middle cerebral artery and related upper-limb paresis. Passive movement of the index finger (Study II) and tactile stimulation (Study III) were applied during MEG recordings in the acute (T 0 ; 1–7 days), subacute (T 1 ; one month) and chronic (T 2 ; 12 months) phases after stroke onset in conjunction with clinical testing of hand motor performance. The results showed that in the acute phase, the rebound was strongly diminished to both stimuli compared to the controls and increased significantly during the first month. During the follow-up period, the rebound strengths to both stimuli correlated with motor performance of the impaired hand. The bilateral weakness of the rebounds in the acute phase indicate hyperexcitability of both hemispheres after stroke. The subsequent increase in the rebound strength during the first month, reflecting an increase in motor-cortex inhibition, is in line with earlier studies in animals and humans suggesting a sensitive and motor-recovery-related plastic period immediately after stroke. The rebound strength to impaired-hand stimulation correlatedwith hand motor performance across the follow-up indicating that adequate integration of afferent input with motor functions is important for motor recovery. During the follow- up, the 20-Hz rebound to both tactile and passive-movement stimuli increased similarly. However, the rebounds to tactile stimuli recovered to the level of the controls whereas those to proprioceptive stimuli did not. This might indicate that proprioception did not recover fully in our patients. It would be most important to be able to predict and evaluate the progress of an individual patient during recovery from stroke to intensify and tailor rehabilitation for individual needs. Though, the efficacy of rehabilitation may be evaluated with different neuroimag- ing methods and clinical tests in a group level, so far there are no objective biomarkers to evaluate rehabilitation in an individual level. The results of this Thesis indicate that the 20-Hz rebound magnitude strongly reflects motor-cortex excitability and thus could serve as a robust noninvasive marker of stroke-induced neurophysiological processes that are relevant for motor recovery. Such a biomarker may enable to assess the efficacy of new therapeutical methods in stroke rehabilitation in both group and individual levels. Aivoinfarktin akuuttihoidon kehittymisestä huolimatta yli kolmannes potilaista kärsii ai- voinfarktin aiheuttamista vammoista, yleisimmin yläraajahalvauksesta, vielä viisi vuot- ta infarktin jälkeen. Kuntoutuksen tehostamiseksi aivoinfarktin jälkeistä aivojen muo- vautumiskykyä eli plastisiteettia tulisi ymmärtää nykyistä paremmin. Plastisen ajanjak- son aikana aivokuoren aktivaatiotila on lisääntynyt, mikä todennäköisesti mahdollistaa aivokuoren uudelleenjärjestäytymisen. Aistiärsykkeet moduloivat liikeaivokuoren rolandisen alueen 20 hertsin rytmiä. Rytmin modulaatio kuvastaa liikeaivokuoren aktivaatiotilaa, mikä voidaan havaita aktivaatioon liittyvänä rytmin vaimenemisena sekä inhibitioon liittyvänä rytmin elpymisenä ns. purs- keena. Tässä väitöskirjatutkimuksessa liikeaivokuoren aktivaatiotilaa tutkittiin rekisteröi- mällä magnetoenkefalografiamenetelmällä (MEG) 20 hertsin rytmiä käyttäen kahta eri- laista aistiärsykettä ensin terveillä verrokeilla ja sen jälkeen potilailla vuoden kestävässä pitkittäistutkimuksessa. Osatutkimuksessa I mittasimme 20 hertsin rytmin muutoksia 22 terveellä verrokilla käyt- täen kosketusärsykettä ja proprioseptisena ärsykkeenä passiiviliikettä. Rytmin vaimene- minen oli samankaltaista kummankin ärsykkeen vaikutuksesta, kun taas passiiviliike ai- heutti voimakkaamman rytmin purskeen kuin kosketusärsyke. Siten passiiviliike voisi toimia paremmin tutkittaessa liikeaivokuoren aktivaatiotilan muutoksia. Osatöissä II ja III mittasimme 20 hertsin rytmin modulaatiota 23 potilaalla, jotka olivat sairastuneet elämänsä ensimmäiseen aivoinfarktiin keskimmäisen aivovaltimon verisuonit- tamalla alueella ja siihen liittyen yläraajan halvaukseen. Aistiärsykkeinä käytimme etu- sormen passiiviliikettä (osatutkimus II) sekä kosketusärsykettä (osatutkimus III) MEG- mittauksen aikana akuutissa (1–7 päivää), subakuutissa (yksi kuukausi) ja kroonisessa (12 kuukautta) vaiheessa infarktiin sairastumisen jälkeen. Lisäksi kunkin mittauksen yhtey- dessä arvioitiin käden motorinen toiminta kliinisin testein. Tulokset osoittavat, että stimu- laation jälkeinen rytmin purske oli akuutissa vaiheessa voimakkaasti heikentynyt molem- mille ärsykkeille verrattuna verrokkien arvoihin, mutta ensimmäisen kuukauden aikana purske voimistui merkittävästi. Rytmin purskeen voimakkuus korreloi sairaan käden mo- torisen suorituksen kanssa koko seurantatutkimuksen ajan. Rytmin purskeen heikkous akuuttivaihessa viittaa molempien aivopuoliskojen liikeai-vokuorien hyperaktiivisuuteen. Purskeen voimistuminen ensimmäisen kuukauden aikana kuvastaa lisääntynyttä liikeaivokuoren inhibitiota, jota on havaittu aiemmissa aivoinfark- tin jälkeistä herkkää ja lyhyttä sekä motoriseen toipumiseen liittyvää plastisiteettijaksoa osoittavissa eläin- ja ihmistutkimuksissa. Rytmin purskeen voimakkuus korreloi sairaan käden motorisen toiminnan parantumisen kanssa koko seurantajakson ajan viitaten sii- hen, että tuntoaistin ja proprioseptiivisen informaation integraatio motorisen aivokuoren toimintaan ovat tärkeitä motoriselle toipumiselle. Seurantajakson aikana 20 hertsin rytmi voimistui samankaltaisesti kummankin aistiärsykkeen, niin kosketusärsykkeen kuin pro- prioseptiivisen ärsykkeen vaikutuksesta. Purskeen voimakkuus kosketusärsykkeen vaiku- tuksesta kuitenkin saavutti terveiden verrokkien tason, kun taas proprioseptiiviselle ärsyk- keelle se ei toipunut samalle tasolle kuin verrokeilla. Tämä saattaisi viitata siihen, että potilaidemme proprioseptiikka ei toipunut täysin. Olisi tärkeää pystyä ennustamaan potilaan yksilöllistä toipumista aivoinfarktin jälkeen, jotta kuntoutusta voitaisiin tehostaa ja räätälöidä kunkin potilaan tarpeiden mukaan. Vaik- ka erilaisilla kuvantamistutkimuksilla ja kliinisillä mittareilla voidaankin arvioida kun- touksen edistymistä ryhmätasolla, toistaiseksi objektiivista yksilöllistä mittaria ei ole ole- massa. Tämän väitöskirjatutkimuksen tulokset viittaavat siihen, että 20 hertsin rytmin purskeen suuruus kuvastaa voimakkaasti liikeaivokuoren aktiviteettimuutoksia ja voisi siten toimia luotettavana kajoamattomana mittarina tutkittaessa aivoinfarktin aiheuttamia ja motoriselle toipumiselle tarpeellisia liikeaivokuoren neurofysiologisisa muutoksia. Täl- lainen biomarkkeri mahdollistaisi uusien aivoinfarktin jälkeisten kuntoutusmenetelmien tehon arvioimisen sekä ryhmä- että yksilötasolla.

Developmental language disorders in children are common and constitute a common reason for support in both health care and school system. Specific language impairment (SLI) is diagnosed in Finland in accordance with the ICD (International Classification of Diseases) with either F80.1 or F80.2 diagnoses. The terminology related to SLI is not internationally unambiguous. Studies indicate that SLI may entail more extensive difficulties than those related purely to language and, also, that SLI in childhood persists into adulthood. SLI has been studied very little within the Finnish service system. The present study explored SLI in the primary health care of one Finnish town. The children participating in the study belonged to the multidisciplinary SLI in Vantaa study group. SLI in Vantaa consisted of all the Finnish speaking children born in 1998 and 1999 who had been diagnosed with the diagnosis F80.1 or F80.2 in the secondary health care, and their matched controls. The first data set assessed the test use of the speech and language therapists (SLTs). In the second data set, the test performance of the children diagnosed with SLI was compared to that of their matched controls. The third study analysed the prevalence of the diagnosed SLI in the SLTs’ statistics over a period of eleven years. The fourth study compared the home activities of the children with SLI with those of their matched controls. The tests that were considered to be the best in separating the children with SLI from their peers were the same ones that the SLTs had most confidence in and that they most frequently used. On the other hand, the SLTs used some tests to assess also other constructs of language than those for which these tests were originally devised. The prevalence of SLI remained lower than the internationally reported level, even though it did increase during the study period. The low prevalence raised the possibility of under-diagnosing of SLI. When comparing the peer groups in home activities, similarities outnumbered differences. The existing differences seemed to be related to something else than language difficulties. Unifying and developing the assessment procedures of SLI is needed in Finland. Organising the future studies in collaboration between the researchers and the clinicians may benefit the children with language disorders in the best possible way. Lasten kielenkehityksen häiriöt ovat yleinen syy tukitoimiin terveyspalveluissa ja kouluissa. Kielenkehityksen häiriöistä kielellinen erityisvaikeus diagnosoidaan Suomessa ICD -luokituksen mukaisesti joko F80.1 tai F80.2 – diagnooseilla, mutta diagnosointikriteerit ja terminologia eivät ole kansainvälisesti yksiselitteisiä. Kielellisestä erityisvaikeudesta on Suomen palvelujärjestelmissä kerätty vain vähän tietoa, vaikka tutkimukset osoittavat, että lapsuuden kielellinen erityisvaikeus vaikuttaa aikuisiässäkin. Tutkimusten mukaan kielelliseen erityisvaikeuteen voi liittyä myös laajempia kuin pelkästään kieleen liittyviä vaikeuksia. Tutkimuksessa tarkasteltiin kielellistä erityisvaikeutta yhden kaupungin perusterveydenhuollon puheterapeuttien työssä. Tutkimukseen osallistuneet lapset olivat osa monialaista Kielellinen erityisvaikeus Vantaalla tutkimusta. Kielellinen erityisvaikeus Vantaalla tutkimukseen kuuluivat kaikki suomea äidinkielenään puhuvat vantaalaiset vuosina 1998 ja 1999 syntyneet lapset, jotka olivat erikoissairaanhoidossa saaneet diagnoosin F80.1 tai F80.2 ja heidän vertaisparinsa. Tutkimus koostui neljästä eri aineistosta. Ensimmäisessä aineistossa tarkasteltiin puheterapeuttien testikäyttöä. Toisessa aineistossa verrattiin kielellinen erityisvaikeus -diagnoosin saaneiden lasten testisuoriutumista vertaisparien suoriutumiseen. Kolmannessa aineistossa tarkasteltiin kielellisen erityisvaikeuden esiintyvyyttä puheterapeuttien tilastoissa yhdentoista vuoden seurannan aikana. Neljännessä aineistossa verrattiin tutkimuslasten ja vertaisparien kotiajankäyttöä viikon ajan. Testit, jotka erottelivat kielihäiriöiset lapset verrokeista parhaiten, olivat pääosin samoja, joihin puheterapeutit luottivat eniten ja joita he käyttivät eniten. Puheterapeutit käyttivät testejä arvioimaan myös muita kielellisiä osa-alueita kuin mihin ne alun perin on tarkoitettu. Kielellisen erityisvaikeuden esiintyvyys jäi kauaksi kansainvälisesti raportoidusta esiintyvyydestä, vaikka se tarkastelujaksolla lisääntyikin. Alhainen esiintyvyys viittaa kielellisen erityisvaikeuden alidiagnosoinnin mahdollisuuteen. Lapsiryhmien kotiajankäyttö oli enemmän samanlaista kuin erilaista. Havaitut erot näyttävät liittyvät enemmän toiminnan ohjaamisen vaikeuteen kuin kielelliseen vaikeuteen. Kielellisen erityisvaikeuden tutkimuskäytänteiden yhdenmukaistaminen ja niiden kehittäminen ovat tarpeen. Tutkijoiden ja kliinistä työtä tekevien puheterapeuttien osaamisen yhdistäminen saattaa hyödyttää kielihäiriöistä lasta parhaiten.

The incidence and complications of peptic ulcer disease (PUD) have declined during the last decades in Western countries. The Helicobacter pylori (H.pylori) infection and the use of non-steroidal anti-inflammatory drugs are the two main risk factors for peptic ulcer disease. Despite the use of H.pylori eradication therapy and gastroprotective agents, mortality associated with PUD has not decreased as assumed. The aims of this thesis were to evaluate time trends in the incidence of PUD and its complications in hospitalised patients, their short- and long-term mortality, risk factors for mortality. Moreover, the prevalence of PUD among patients suffering from acute gastrointestinal bleeding (GIB) symptoms in the beginning of the 21st century in the capital area of Finland was studied. In the retrospective epidemiological cohort study, data on adult patients hospitalised and diagnosed with PUD was collected from the Hospital District of Helsinki and Uusimaa patient register during 2000-2008. The data was linked with Prescription Register of the Finnish Social Insurance Institution allowed detailed individual medicine purchase data and all patients were followed in the National Cause of Death Register until the end of 2009. In the prospective observational cohort study, data on hospitalised patients referred for acute upper endoscopy during 2012-2014 in Meilahti and Jorvi hospitals were collected. The incidence of peptic ulcers decreased significantly from 121/100000 in 2000-2002 to 79/100000 in 2006-2008. The rate of severe complications was also reduced, mainly due to a reduction in bleedings. The one-year cumulative incidence of recurrent peptic ulcers was 13%. The use of several drugs indicating comorbidity was associated with an increased risk for recurrence. No change in standardized mortality ratio was seen; 3.7% of patients died within 30 days, and 11.8% within 1 year in the retrospective cohort. Both the short- and long-term survival was significantly impaired in women presenting with perforated duodenal ulcer. The main causes of death at one year were malignancies and cardiovascular diseases. Interestingly, previous use of statins was associated with significant reduction in all-cause mortality. Of all patients admitted for acute upper endoscopy during 2012-2014, 23% had PUD. Other specific diagnoses in acute upper endoscopy were oesophagitis (12%), oesophageal or gastric varices (10%), 10% gastroduodenitis (10%), Mallory-Weiss lesion (8%), and malignancy (5%). Of the PUD patients, 31% were H.pylori positive. The 30-day mortality was very low 0.7%. This is most probably explained by the exclusion of unstable or otherwise fragile patients who were not able to give written informed consent at the admission. The one-year mortality was 12.9% which is in line with results of the retrospective study. Comorbidities were associated with decreased survival (p=0.029) and obesity (BMI>=30) was associated with better survival (p=0.023). Of all individual patients referred for endoscopy, the source for acute bleeding or severe anaemia was not evident in 19% of patients. In further examinations, the diverticular disease of the colon was regarded as the most probable source of bleeding in 39% of patients of these patients. In conclusion, both the incidence and complications of PUD have markedly decreased in Finland. However, the one-year mortality associated with PUD remained high with no change. PUD is still the most common cause of acute gastro-intestinal bleeding. Peptisen haavataudin (PUD) ilmaantuvuus ja sen komplikaatiot ovat vähentyneet länsimaissa viime vuosikymmenten aikana. Haavauman synnyn yleisimmät riskitekijät ovat helikobakteeritulehdus ja tulehduskipulääkkeiden runsas käyttö. Peptiseen haavatautiin liittyvä kuolleisuus ei ole kuitenkaan vähentynyt samassa suhteessa kuin haavataudin ilmaantuvuus. Väitöskirjatutkimukseni tarkoitus oli tutkia, mikä oli PUDn ilmaantuvuus ja taudin komplikaatioiden yleisyys pääkaupunkiseudulla 2000-luvun alussa. Samassa yhteydessä selvitettiin tautiin liittyvää kuolleisuutta ja sen riskitekijöitä. Lisäksi tutkittiin peptisen haavan osuutta mahalaukun ja pohjukaissuolen päivystyksellisissä tähystyksissä (gastroskopioissa). Tutkimusaineisto muodostettiin HUS-alueen sairaalarekisteristä PUDn vuoksi sairaalahoitoon vuosien 2000-2008 aikana joutuneista aikuispotilaista. Aineistoon yhdistettiin Kelan reseptirekisterin ja Tilastokeskuksen kuolinsyyrekisterin tietoja. Toinen aineisto muodostui päivystykselliseen gastroskopiaan Meilahden ja Jorvin sairaaloissa vuosina 2012-2014 ohjatuista aikuispotilaista. PUDn ilmaantuvuus pieneni merkitsevästi vuosien 2000-2008 aikana. Myöskin komplikaatiot vähenivät johtuen pääosin haavaumavuotojen vähenemisestä. Uuden haavan kehitti vuoden aikana 13 % potilaista ja uusiutuminen oli yhteydessä potilaan käyttämään monilääkitykseen. Kuolleisuudessa ei havaittu merkitsevää muutosta suhteessa seurantajaksoon; vuosien 2000-2008 aineiston potilaista 3.7 % kuoli kuukauden ja 11.8 % vuoden kuluessa haavauman toteamisesta. Puhjennutta pohjukaissuolen haavaumaa sairastavilla naisilla oli huonompi ennuste kuin miehillä. Vuoden aikana menehtyneiden potilaiden yleisimmät peruskuolinsyyt olivat pahanlaatuiset taudit sekä sydän- ja verisuonisairaudet. Rasva-aineenvaihduntaan vaikuttavia statiinilääkkeitä ennen haavan toteamista käyttäneillä potilailla kuolleisuus oli merkitsevästi muita tutkittuja pienempi. Vuosina 2012-2014 päivystykselliseen gastroskopiaan joutuneista potilaista 23 %:lla todettiin peptinen haavauma. Muita todettuja sairauksia olivat ruokatorvitulehdus (12%), ruokatorven/mahalaukun suonikohjut (10%), maha/pohjukaissuolen tulehdus (10%), mahan ja ruokatorven Mallory-Weiss haavauma- ja verenvuoto -oireyhtymä (8%), ja pahanlaatuinen muutos (5%). Tähystyksessä todettuun PUD liittyi ainoastaan 31 %:ssa helikobakteeritulehdus. Potilaista vain 0.7 % kuoli kuukauden aikana. Potilailta edellytettiin kirjallinen suostumus potilastietojen tarkasteluun, mikä todennäköisesti johti huonokuntoisten ja hauraiden potilaiden poisjääntiin verrattuna aiempaan osatutkimukseen. Vuonna 2012-2014 gastroskopiassa todetuista PUD sairastavista potilaista vuoden aikana kuoli 12.9%, mikä on samaa tasoa kuin aiemmassa aineistossa. Monisairaiden potilaiden ennuste oli huonompi, mutta ylipaino (BMI>=30) oli yhteydessä parempaan ennusteeseen. Päivystyksellisen gastroskopian potilaista 19 % jäi toimenpiteessä ilman selitystä äkillisille vuoto-oireille tai anemialle. Jatkotutkimuksissa näistä 39 % selittyi paksusuolen divertikkelivuodolla. Yhteenvetona on pääteltävissä PUDn ilmaantuvuuden ja komplikaatioiden vähenevän Suomessa. Silti PUD on edelleen yleisin äkillistä ylämahasuolikanavan verenvuotoa aiheuttava sairaus ja siihen liittyy yhä merkittävä kuolleisuus. PUD vuoksi sairaalahoitoon joutuneiden potilaiden lyhyt- ja pitkäaikaisennuste on huonompi kuin ikä- ja sukupuolivakioidun verrokkiväestön. Uusi havainto oli, että statiinilääkkeiden käyttö ennen haavaumaa näyttää olevan yhteydessä parempaan ennusteeseen.

Head and neck cancers constitute the seventh most common cancer group worldwide. Their incidence has been declining in the Western world, along with the decrease in tobacco smoking. The incidence of oropharyngeal squamous cell carcinoma (OPSCC) has been rising over the last two to three decades in many Western countries. This trend is attributed to human papillomavirus (HPV), which is responsible for the majority of OPSCC cases, whereas, the incidence of HPV-unrelated OPSCC has decreased. HPV-positive OPSCC differs from the HPV-negative form in various aspects: Patients are younger, and they typically have less tobacco and alcohol consumption. They tend to have smaller primary tumors but more advanced disease in the neck. Patients with HPV-positive tumors have better odds of survival, but their post-treatment lifetime may be harmed by major treatment-related morbidity. Many prospective randomized trials delivering de-intensified treatment for this OPSCC patient group are currently ongoing. Validated predictive biomarkers could aid in the treatment individualization of OPSCC. The only well-validated prognostic biomarkers in OPSCC are HPV and protein p16, reflecting HPV involvement. Toll-like receptors (TLRs) are receptors, which initiate immunological cascades. In HPV-associated cervical carcinoma, alteration in the expression of TLRs has been observed. These receptors are also expressed in an altered pattern when HPV infection persists in the cervix. Therefore, these receptors could have a role in HPV-associated OPSCC. The first part of the present study analyzed the treatment and outcome in two patient series treated over a ten-year period. Study I included all OPSCC patients treated at the Finnish university hospitals, and Study II focused on the management of the neck in cN+ disease of patients treated at the Helsinki University Hospital. The cohort included 674 patients, and during the study period, the incidence of this disease increased, which mainly occurred in the group of patients carrying a HPV-associated tumor. The outcome of lateral-wall OPSCC (tonsil) remained similar compared to an earlier Finnish nationwide report, but the outcome of anterior-wall disease (base of tongue) slightly improved. Study II revealed that the relative rate of neck dissections had decreased while the delivery of definitive oncological treatment had increased. However, the regional control rate had not worsened. Regional failures occurred in patients with class cN2b or higher and they often developed on the contralateral side of the neck, including in patients with an ipsilateral primary tumor. The second part of this thesis study (Studies III and IV) evaluated the role of TLRs in OPSCC. As a pilot study, we analyzed TLR 2, 3, 4, 5, 7, and 9 expression, p16 expression, and HPV status in 35 OPSCC samples. TLR 5, 7, and 9 expression varied according to p16 and HPV status. Based on these results, we evaluated the association of TLR 5, 7, and 9 with clinicopathological and outcome data in a cohort of 202 OPSCC patients. The findings indicated that high TLR 5 expression and low TLR 7 expression were related to poor disease-specific survival in the group of HPV-positive OPSCC patients. Pään ja kaulan syövät ovat maailman seitsemänneksi yleisin syöpätyyppi. Niiden esiintyvyys länsimaissa on laskenut tupakoinnin vähentyessä. Suunielusyöpä on kuitenkin yleistynyt monessa länsimaassa viimeisen muutaman vuosikymmenen aikana. Muutoksen taustalla on ihmisen papilloomaviruksen (HPV) aiheuttamien suunielusyöpätapausten merkittävä lisääntyminen. HPV-positiivinen suunielusyöpä eroaa monin tavoin HPV-negatiivisesta suunielusyövästä. Potilaat, jotka sairastuvat taudin HPV-positiiviseen muotoon ovat nuorempia, tyypillisesti tupakoivat vähemmän ja käyttävät vähemmän alkoholia. Lisäksi emokasvain on yleensä pienempi, mutta kaulalle on usein kehittynyt enemmän etäpesäkkeitä. HPV-positiivista tautia sairastavat potilaat ovat usein nuorempia ja heillä on suurempi todennäköisyys välttyä taudin uusiutumalta, joten osa heistä joutuu kärsimään pitkään hoitojen jälkeisistä haittavaikutuksista. Hoitojen keventäminen HPV-positiivista tautia sairastavien kohdalla saattaa tarjota avun tähän ongelmaan. Kliiniseen käyttöön sopivien ennusteellisten biomerkkiaineiden löytäminen voisi mahdollistaa suunielusyöpäpotilaiden yksilöllisempien hoitojen suunnittelun. Ainoat validoidut ennusteelliset merkkiaineet ovat HPV ja proteiini p16, joka on HPV-assosiaation osoittamiseen käytettävä sijaismerkkiaine. Tollin kaltaiset reseptorit (TLR) aloittavat tulehdusreaktiota, mutta niillä on myös todettu olevan yhteys syövän kehittymiseen. HPV-infektion pitkittyessä kohdunkaulassa ja myös kohdunkaulan syövässä on todettu, että usean TLR:n ilmentyminen on poikkeavaa. Tällä perusteella voidaan olettaa, että TLR:llä voisi olla keskeinen merkitys myös suunielusyövässä. Tämän tutkimuksen ensimmäisessä osakokonaisuudessa analysoitiin vuosien 2000-2009 aikana annettuja hoitoja ja hoitotuloksia kahdessa potilassarjassa. Ensimmäiseen osatyöhön oli kerätty kaikki Suomen viidessä yliopistosairaalassa hoidetut suunielusyöpäpotilaat. Toisessa osatyössä analysoitiin kaulan hoitoa HYKS:ssä potilailla, joilla oli diagnoosivaiheessa kaulalla etäpesäkkeet. Tutkimusajanjakson aikana hoidettiin yhteensä 674 potilasta. Tauti, ja erityisesti sen HPV:hen liittyvä muoto lisääntyi tutkimusajanjakson aikana. Aikaisempaan kansalliseen suunielusyöpätutkimukseen verrattuna nielun sivuseinämän (nielurisan) taudin ennuste oli pysynyt muuttumattomana, mutta nielun etuseinämän (kielen tyven) taudin ennuste oli hieman parantunut. Toisen osatyön tulosten mukaan kauladissektioiden suhteellinen määrä väheni samalla kun definitiivinen kemosädehoito yleistyi. Taudin uusiutumismäärät kaulalla eivät kuitenkaan lisääntyneet. Kaulan alueen uusiutumat kehittyivät potilaille, joiden kaulan kliininen luokitus oli N2b tai korkeampi ja ne kehittyivät usein emokasvaimen vastakkaiselle puolelle. Tutkimuksen toisessa osakokonaisuudessa (osatyöt III-IV) tutkimme TLR:n merkitystä suunielusyövässä. Analysoimme ensin TLR 2, 3, 4, 5, 7 ja 9 sekä p16 ilmentymisen ja HPV-statuksen 35:sta suunielusyövästä. TLR 5, 7 ja 9 ilmentyminen erosi p16-positiivisten ja –negatiivisten sekä HPV-positiivisten ja –negatiivisten suunielusyöpien välillä. Näiden tulosten perusteella selvitimme TLR 5, 7, ja 9 merkitystä laajemmassa 202 potilaan aineistossa verraten näiden proteiinien ilmentymistä potilaisiin ja tautiin liittyviin tekijöihin sekä taudin ennusteeseen. Tulokset vahvistivat aiempia löydöksiä. Sen lisäksi havaitsimme, että voimakas TLR 5 ja matala TLR 7 ilmentyminen olivat yhteydessä korkeaan tautikuolleisuuteen HPV-positiivista tautimuotoa sairastavilla potilailla.

Chronic obstructive pulmonary disease (COPD) is a common, preventable and treatable chronic airway disease (CAD) and is the fourth leading cause of illness and death worldwide. CAD represents a group of diseases with different clinical phenotypes, airway inflammation, obstruction and destruction of the lung parenchyma. COPD is a heterogeneous disease, which is characterized by persistent respiratory symptoms and airflow limitation and its prevalence varies considerably across populations. Asthma-COPD overlap syndrome (ACOS) had been reported in 2009 and recognised by Global Strategy for Asthma Management and Prevention (GINA) and Global Initiative for Chronic Obstructive Lung Disease (GOLD) in 2014. ACOS is identified by the features that it shares with both asthma and COPD. Inhaling tobacco smoke is the main risk for CAD, thus smoking cessation is not only the key step for prevention and maintenance therapy for COPD (GOLD 2017), but also it is encouraged in the patient with asthma and their families (GINA 2017). Smoking cessation, pulmonary rehabilitation, pharmacologic therapy and the other therapies for COPD may improve symptoms, health status and exercise ability and reduce frequency of exacerbations (GOLD 2017). At present, COPD is generally diagnosed by lung function measurement using spirometry, but not all individuals presenting with airflow limitation follow the paradigm. On the individual level spirometry may poorly correlate with symptoms, risk of exacerbation, prognosis and response to treatment in COPD. Due to disease heterogeneity, limitations of spirometry and lack of predictive molecular markers, the mechanisms underlying different COPD phenotypes are still unclear. Therefore, identification of the similarities and differences of COPD clinical phenotypes is important, as these phenotypes require different therapeutic approaches and have distinct clinical outcomes. We hypothesised that the potential biomarkers may play an important role and even predict the development of COPD. Accordingly, patients with asthma, COPD and ACOS would exhibit different biomarker profiles. Furthermore, distinguishing COPD phenotypes via distinct molecular and cellular pathways may facilitate the development of individualised diagnosis and precision medicine. The first goal of this study was to identify COPD-specific proteomic changes involved in disease onset and severity. The second goal was to identify novel biomarkers for the clinical COPD phenotypes in smokers and in patients with COPD, asthma and ACOS in cross-sectional and longitudinal studies. Those studies included the 6-year longitudinal cohort of adult smokers for early diagnosis of COPD studies, and the Finnish discovery cohort and Japanese validation cohort for ACOS studies. The third goal was to investigate whether the levels of these markers are associated with variables such as pack-years, lung function and sputum cell profiles. Finally, we aimed to evaluate the association of the biomarker levels with a longitudinal decline of lung function. We selected biomarkers identified in our non-hypothesis-based proteomic approaches for early COPD diagnosis, such as transglutaminase 2 (TGM2), bactericidal/permeability-increasing (BPI) fold-containing protein B1 (BPIFB1) and soluble receptor for advanced glycation end-products (sRAGE). Based on our previous results and non-hypothesis-based proteomic approaches, we developed two panels of potential biomarkers for ACOS: COPD-related biomarkers and asthma-related biomarkers. The four COPD-related biomarkers are neutrophil gelatinase-associated lipocalin (NGAL), myeloperoxidase (MPO), surfactant protein A (SP-A) and sRAGE. The three asthma-related biomarkers are chitinase-like protein (YKL-40), interleukin-6 (IL-6) and interleukin IL-13 (IL-13). For the early COPD diagnosis studies, induced sputum and plasma samples were categorised into 3 groups: non-smokers, smokers without COPD and smokers with COPD (stable stage I and stage II-III). Lung tissue samples were obtained from non-smokers, smokers without COPD, smokers with COPD and patients with idiopathic pulmonary fibrosis (IPF). For the ACOS studies, induced sputum and plasma samples were categorised into 5 groups: non-smokers, healthy smokers, patients with asthma, COPD and ACOS. Sputum cell counts were evaluated from sputum cytospins. TGM2 and BPIFB1 were assessed by Western blot analysis and by cysteine-specific two-dimensional difference gel electrophoresis (2D-DIGE) coupled with mass spectrometry (MS). BPIFB1 was further detected by immunohistochemistry and functional enzyme-linked immunosorbent assay (ELISA). TGM2, sRAGE, NGAL, MPO, SP-A, YKL-40, IL-6 and IL-13 levels were measured by commercial ELISA or magnetic human high sensitivity luminex assay (luminex assay). Our results showed that TGM2 levels in sputum and plasma were elevated in mild-moderate COPD and associated with lung function. Sputum BPIFB1 levels were elevated in smokers with COPD, whereas plasma sRAGE levels were decreased in smokers without COPD and smokers with COPD. Sputum BPIFB1 and plasma sRAGE levels were significantly associated with reduced airflow limitation over time in smokers with COPD. In the Finnish discovery cohort of the ACOS study, sputum MPO and plasma SP-A levels were significantly elevated in ACOS, whereas plasma sRAGE levels were decreased in ACOS. Sputum IL-13 levels were increased in ACOS when compared with controls. Sputum YKL-40 and IL-6 differentiated ACOS from COPD and asthma based on receiver operating characteristic (ROC) curve analysis. However, only sputum NGAL results could be repeated from the Finnish discovery cohort to Japanese validation cohort. Importantly, NGAL differentiated ACOS from COPD and independently correlated with forced expiratory volume in one second % (FEV1%-predict) in both cohorts. In conclusion, our findings support the hypothesis that monitoring of these biomarkers may be useful for differential diagnosis between the clinical phenotypes of COPD. TGM2 may be a potential diagnostic and therapeutic marker in COPD, which may relate to COPD onset and severity. BPIFB1 and sRAGE play vital roles in the pathogenesis of smoking-related COPD. High concentrations of sRAGE might be involved in the protective role of the airway. Patients with COPD and ACOS exhibit different NGAL profiles in sputum. These findings suggest that these potential biomarkers could be a novel diagnostic and therapeutic target for COPD. On the other hand, sputum biomarkers reflect both airway inflammation and tissue remodelling, and monitoring these biomarkers may be useful for differential diagnosis between asthma, COPD and ACOS. Keuhkoahtaumatauti (COPD) on yleinen estettävissä ja hoidettavissa oleva keuhkosairaus, jonka taudinkuva vaihtelee huomattavasti eri potilaiden välillä. Taudille tyypillisiä yhdistäviä tekijöitä ovat pitkittyneet hengitystieoireet sekä ilmateiden pysyvä ahtauma. Tupakasta vieroituksella, liikuntaharjoitteluun perustuvalla kuntoutuksella ja lääkehoidolla voidaan helpottaa oireita, parantaa potilaan yleistä terveyden tilaa sekä vähentää pahenemisvaiheita. Keuhkoahtaumataudin diagnoosi perustuu nykyisin spirometriatutkimuksiin, joilla mitataan potilaan hengityskapasiteettia. Menetelmällä saadut tulokset kuitenkin poikkeavat viitearvoista vasta siinä vaiheessa, kun tauti on jo edennyt pitkälle. Spirometrialla ei myöskään saada tietoa taudinkuvasta ja ennusteesta. Tämän vuoksi olisi tärkeää löytää ennustavia tekijöitä keuhkoahtaumataudin varhaisempaan toteamiseen ja sen erilaisten taudinmuotojen toteamiseen. Yksi näistä on sekamuotoinen astma-keuhkoahtaumatauti (ACOS), jonka taudinkuva on huonompi kuin pelkän keuhkoahtaumataudin. Hypoteesimme mukaan verestä tai ysköksestä mitattavia yhdisteitä (biomarkkereita) voidaan hyödyntää taudin diagnosoinnissa, seurannassa ja taudinkulun ennustamisessa. Tämän tutkimuksen ensimmäisen osatyön tavoitteena oli löytää proteiiniprofiilien seulontamenetelmän (proteomiikka) avulla keuhkoahtaumataudin puhkeamiseen ja taudin vaikeusasteeseen liittyviä täysin uusia biomarkkereita. Seuraavissa osatöissä hyödynnettiin aiemmin löydettyjä biomarkkereita keuhkoahtaumataudin erilaisten alamuotojen tunnistamisessa käyttäen tupakoitsijoilta sekä astma-, keuhkoahtaumatauti- ja ACOS-potilailta kerättyjä plasma- ja yskösnäytteitä. Lisäksi tavoitteena oli selvittää löydettyjen biomarkkerien ja taudin ilmiasuun liittyvien kliinisten ominaisuuksien välisiä yhteyksiä. Proteomiikan ja kirjallisuushakujen perusteella valitsimme TGM2, BPIFB1 ja sRAGE proteiinit biomarkkereiksi ja selvitimme niiden toimivuutta keuhkoahtaumataudin varhaisessa tunnistamisessa. Lisäksi selvitimme, voidaanko keuhkoahtaumatautiin (NGAL, MPO, SP-A and sRAGE) ja astmaan aiemmin yhdistettyjä biomarkkereita (YKL-40, IL-6 and IL-13) käyttää ACOS:n tunnistamiseen. Tulostemme perusteella yskös- ja plasmanäytteiden TGM2:n pitoisuudet olivat koholla keuhkoahtaumataudissa ja liittyivät taudin vaikeusasteeseen. Yskösten BPIFB1:n pitoisuudet olivat nousseet tupakoivilla keuhkoahtaumatautipotilailla, kun taas plasman sRAGE-pitoisuudet olivat vähentyneet tupakoitsijoilla ja tupakoivilla keuhkoahtaumatautipotilailla. Molempien biomarkkerien pitoisuudet olivat yhteydessä ajan myötä heikkenevään ilmateiden ahtaumaan tupakoivilla keuhkoahtaumatautipotilailla. Yskösnäytteiden NGAL:n pitoisuuksien perusteella voitiin erotella ACOS-potilaat keuhkoahtaumatautipotilaista ja NGAL:n pitoisuudet myös liittyivät ilmateiden ahtauman vaikeusasteeseen (FEV1% viitearvo). Yhteenvetona TGM2 on mahdollinen biomarkkeri keuhkoahtaumataudin vaikeusasteeseen liittyvään diagnostiikkaan. BPIFB1- ja sRAGE -proteiineilla on tärkeä merkitys tupakoinnista aiheutuvassa keuhkoahtaumataudissa. Keuhkoahtaumatauti- ja ACOS-potilailla on erilaiset yskösten NGAL:n pitoisuudet. Tutkimamme biomarkkerit ovat lupaavia uusia biomarkkereita keuhkoahtaumataudin diagnostiikkaa ja hoitoa varten. Lisäksi niistä voisi olla hyötyä astman, keuhkoahtaumataudin ja ACOS:n välisessä erotusdiagnostiikassa.

A high proportion of patients with mental disorders experience concurrent anxiety symptoms and substance abuse, which impacts the course and outcome of principal psychiatric disorders and increases the risk of physical morbidity and suicide. Another prominent problem that worsens outcome and increases healthcare costs, is poor adherence to psychiatric treatment. As a consequence of severe course and poor treatment adherence, mental disorders are highly disabling. Because of methodological variations in the studies on anxiety and substance use comorbidity, adherence, and functioning, it remains unclear whether such conditions share similar characteristics across schizophrenia spectrum and mood disorders. The Helsinki University Psychiatric Consortium Study was performed in the metropolitan area of Helsinki between 12.01.2011 and 20.12.2012, covering specialized psychiatric care units. For this study, patients were divided into three subgroups: schizophrenia or schizoaffective disorder (SSA, n=113), bipolar disorder (BD, n=99), and depressive disorder (DD, n=188). Anxiety symptoms were measured with the Overall Anxiety Severity and Impairment Scale; substance use was assessed with recorded substance use disorder diagnoses, Alcohol Use Disorders Identification Test, and original questionnaires; treatment adherence was assessed with patients´ self-reports; subjective level of functioning was assessed with the Sheehan Disability Scale; and data on objective work status were gathered from medical records. Nearly half of all patients felt severe anxiety frequently, but SSA patients less often than mood disorders peers. High neuroticism, symptoms of depression and borderline personality disorder were associated with co-occurring anxiety within all diagnostic groups. Almost half of the patients reported hazardous alcohol use or were daily smokers. Symptoms of anxiety and borderline personality disorder were associated with self-reported alcohol consumption. Outpatients were significantly more adherent than current inpatients. Non-adherence to outpatient visits was strongly associated with hospital setting and substance use disorder. Nearly one-third of mood disorder patients were employed, while in SSA patients this proportion was only 5.3%. Severe course of disease and current depressive symptoms were likely to affect work status and perceived functional impairment, respectively. Careful detection and treatment of harmful substance use and affective symptoms are necessary to enhance treatment adherence, and functional level of patients with mood or schizophrenia spectrum disorders. Ahdistusoireet ja päihteiden ongelmakäyttö ovat yleisiä mielenterveyspotilailla ja vaikeuttavat taudinkulkua ja ennustetta, ja ovat yhteydessä lisääntyneeseen somaattiseen sairastuvuuteen ja itsemurhariskiin. Puutteellinen hoitoon sitoutuminen on toinen merkittävä ongelma, jolla on kielteisiä vaikutuksia taudin ennusteeseen ja hoitokustannuksiin. Vakavan taudinkulun ja puutteellisen hoitoon sitoutumisen seurauksena, mielenterveyshäiriöiden aiheuttama työ- ja toimintakyvyn lasku korostuu. Mielenterveydenhäiriöistä kärsivien potilaiden ahdistusoireita, päihteidenkäyttöä, hoitoon sitoutumista ja toiminnallista profiilia käsittelevät tutkimukset ovat menetelmiltään vaihtelevia. Toistaiseksi epäselvää on, eroavatko niiden ilmiöiden taustatekijät mieliala- ja skitsofreniaryhmän häiriöistä kärsivillä potilailla. Helsinki University Psychiatric Consortium Study toteutettiin pääkaupunkiseudulla 12.01.2011 – 20.12.2012 välisenä aikana psykiatrian erikoissairaanhoidon yksiköissä. Potilaat jakautuivat kolmeen ryhmään: skitsofrenia tai skitsoaffektiivinen häiriö (SSA, n=113), kaksisuuntainen mielialahäiriö (BD, n=99) ja depressio (DD, n=188). Ahdistusoireita arvioitiin Overall Anxiety Severity and Impairment Scale – kyselyllä; päihteidenkäyttöä sairauskertomusten päihdehäiriödiagnooseja tutkimalla ja Alcohol Use Disorders Identification Test – kyselyllä. Sitoutumista hoitoon arvioitiin potilaiden kyselyllä. Subjektiivista toimintakykyä arvioitiin Sheehan Disability Scale – kyselyllä ja tieto ajankohtaisesta työkyvystä kerättiin sairauskertomuksesta. Tutkimus osoittaa, että ahdistusoireet ja päihteiden ongelmakäyttö ovat yhteydessä toisiinsa ja ovat yleisiä kaikissa kolmessa tutkitussa potilasryhmässä. Noin puolet potilaista oli kärsinyt vakavasta ahdistuksesta, mutta SSA-ryhmän potilaat harvemmin kuin mielialahäiriöpotilaat. Noin puolet potilaista raportoi haitallista alkoholinkäyttöä tai päivittäistä tupakointia. Ahdistusoireet ja päihteidenkäyttö yhdistyivät tunne-elämältään epävakaan persoonallisuuden piirteisiin sekä luonteenpiirteistä neuroottisuuteen ja tunnollisuuteen. Potilaiden avohoitoon sitoutumattomuus oli yhteydessä ajankohtaiseen sairaalahoitoon ja päihdeongelmaan. Noin kolmasosa mielialahäiriöpotilaista oli työelämässä, kun taas vain 5.3% SSA-ryhmän potilaista kävi työssä. Vaikeampi taudinkulku todennäköisesti alentaa työkykyä ja ajankohtaiset masennusoireet liittyvät koettuun toimintakyvyttömyyteen. Ahdistusoireiden ja päihdeongelmien huolellinen tunnistaminen ja asianmukainen hoito ovat tärkeitä sekä hoitoon sitoutumisen vahvistamiseksi, että potilaiden toiminta- ja työkyvyn parantamiseksi.

Electrocardiography (ECG) is a routine diagnostic method for all young ischemic stroke (IS) patients, although the relevance of its findings is as yet poorly known. A diagnostic work-up to reveal etiology in a young IS patient includes many cardiac diagnostic methods, as finding a high-risk source of cardioembolism (HRCE) will influence the secondary prevention after IS, and also provides a marker for high risk of recurrent events and mortality. IS per se is a disastrous event, and recurrent cardiovascular events may worsen the situation further. Identifying patients at a high risk of recurrent events is therefore important. The Helsinki Young Stroke Registry (HYSR) includes all 15- to 49-yearold IS patients treated at the Helsinki University Hospital between 1994 and 2007. Blinded to other current clinical data, we analyzed 12-lead resting ECGs obtained 1 to 14 days after IS in 690 patients. We used logistic regression, adjusted for demographic and clinical confounders, to investigate, in 78 patients, what ECG findings are related to an etiology of HRCE (Study I). We investigated what ECG findings bear an increased risk of new cardiovascular events (Study II) and mortality (Study III), using Cox regression adjusted for demographic confounders and comorbidities. We also collected a cohort of stroke-free control subjects in order to study the association of ECG markers with IS at young age, with stratified analysis according to IS subtype (Study IV). Of our IS patient cohort (median age 41 years, 63% male), 35% showed some ECG abnormality. The most common abnormalities were T-wave inversions (16%), left ventricular hypertrophy (LVH) (14%), prolonged P-waves (13%), and prolonged corrected QT interval (QTc) (12%). Of all IS patients, 3% had atrial fibrillation (AF), and 4% P-terminal force in lead V1 (PTF). A longer QRS complex duration, a longer QTc, and wider QRS-T-angle were independently associated with HRCE. Interestingly, PTF had the strongest independent association with HRCE (hazard ratio 43.18). After a median follow-up of 8.8 years, 26.4% of patients experienced some cardiovascular event. 14.6% suffered a recurrent stroke, and 16.1% died; 9.1% died from cardiovascular causes. ECG parameters associated with recurrent cardiovascular events were bundle branch blocks, PTF, LVH, and a broader QRS complex. No ECG parameter was associated with stroke recurrence. A higher heart rate, a shorter P-wave and longer QTc were associated with increased all-cause mortality. Only a higher heart rate was associated with death from cardiovascular causes. In the case-control study, abnormal P-waves, PTF, interatrial block – and combinations of these P-wave abnormalities with LVH – were associated with cardioembolic IS. Abnormal P-wave and IAB were also associated with cryptogenic IS; and LVH was associated with small-vessel disease (SVD) subtype. In conclusion, ECG in young IS patients provides information on IS etiology, risk of recurrent events, and mortality. P-wave abnormalities and ECG markers of LVH are more frequent in young IS patients than in stroke-free controls, suggesting they may be markers of increased IS risk, which is mostly explained by the HRCE subgroup. Sydänfilmi (EKG) on rutiininomainen diagnostinen toimenpide kaikilla nuorilla aivoinfarktipotilailla, mutta sen löydösten merkitys on edelleen huonosti tunnettu. Infarktin syyn paljastamiseksi tehtävä diagnostisten toimenpiteiden polku sisältää useita sydändiagnostisia menetelmiä, koska korkean riskin sydänperäisen embolian lähteen löytäminen vaikuttaa infarktinjälkeiseen hoitoon, ja on myös merkki suuresta vaarasta uusintatapahtumille ja kuolleisuudelle. Helsinki Young Stroke Registry (HYSR) sisältää kaikki 15-49 vuotiaat aivoinfarktipotilaat, jotka hoidettiin Helsingin Yliopistollisessa Keskussairaalassa vuosina 1994-2007. Analysoimme 690 potilaassa 12-kytkentäisen leposydänfilmin, joka oli otettu 1-14 päivää aivoinfarktin jälkeen. Tutkimme, mitkä sydänfilmilöydökset ovat yhteydessä korkean riskin sydänperäisen embolian lähteelle (78 potilasta) (Tutkimus I), mitkä sydänfilmilöydökset ovat yhteydessä kohonneeseen uusien sydän- ja verisuonitapahtumien riskiin (Tutkimus II), sekä kuolleisuuteen (Tutkimus III). Keräsimme myös joukon verrokkihenkilöitä ilman aivoinfarktia, tutkiaksemme sydänfilmilöydösten yhteyttä aivohalvaukseen nuorella iällä, ja ositimme analyysin aivohalvauksen alatyypin mukaan (Tutkimus IV). Pidempi QRS-kompleksin kesto, pidempi QTc ja leveämpi QRS-T-kulma olivat itsenäisesti yhteydessä korkean riskin sydänperäisen embolian lähteisiin. PTF:llä oli vahvin itsenäinen yhteys korkean riskin sydänperäisen embolian lähteisiin (vaarakerroin 43,18). Haarakatkokset, PTF, vasemman kammion liikakasvu ja leveämpi QRS-kompleksi olivat yhteydessä uusiin sydän- ja verisuonitautitapahtumiin. Mikään sydänfilmilöydös ei ollut yhteydessä aivohalvauksen uusiutumiseen. Korkeampi syke, lyhyempi P-aalto ja pidempi QTc olivat yhteydessä lisääntyneeseen kokonaiskuolleisuuteen. Tapaus-verrokkitutkimuksessa poikkeavat P-aallot, PTF, eteistenvälinen katkos, sekä yhdistelmät näiden P-aaltopoikkeavuuksien ja vasemman kammion liikakasvun välillä, olivat yhteydessä sydänperäiseen aivoinfarktiin. Poikkeavat P-aallot ja eteistenvälinen katkos olivat myös yhteydessä salasyntyiseen aivoinfarktiin, ja vasemman kammion liikakasvu oli yhteydessä pienten suonten taudin alaryhmään. Sydänfilmi siis antaa nuorilla aivoinfarktipotilailla tietoa aivoinfarktin syystä, uusintatapahtumien riskistä sekä kuolleisuudesta.

Type 1 diabetes (T1D) is an immune-mediated disease that affects ~0.7% of children in Finland. Its incidence is the highest in Finland worldwide. The proposed etiology of T1D is both genetic and environmental. Many of these etiological factors are shared between other autoimmune diseases (AIDs) and, accordingly, these diseases co-occur in patients with T1D and their relatives. This thesis aims at characterizing the frequency of additional autoimmunity in Finnish children under the age of 15 years with newly diagnosed T1D and in their extended family members, as well as characterizing the effects of this additional autoimmunity on clinical, metabolic, and genetic markers, and T1D autoantibodies. The subjects for this thesis are participants of the Finnish Pediatric Diabetes Register and Sample Repository. Participating families fill in questionnaires on the clinical parameters, family characteristics, and diabetes and other diseases of the index child and family members. This thesis analysed data from the time of diagnosis on 2245 children diagnosed with T1D between 2002 and 2009. The mean age at diagnosis of T1D for the study population was 7.9 years and the majority were boys (57.1%). For a subset of the cases, follow-up data was available. Additional AIDs were reported by 1.6% children at diagnosis and by 3.2% after a median eight years of follow-up. More than 20% of the families reported first- and/or second-degree relatives with T1D, and over a third, relatives with other AIDs. Fathers were more often affected by T1D compared to mothers (6 vs. 3%), whereas mothers were more often affected by other AIDs (10 vs. 4%). Girls had more often T1D affected paternal and boys T1D affected maternal second-degree relatives. Transient anti-tTG not developing to CD seemed more common among children with T1D than among their relatives. The HLA-DR3-DQ2 haplotype was associated with CD autoimmunity and the HLA-DR4-DQ8 haplotype with familial T1D. Also, non-HLA loci were shown to contribute to the clustering of AIDs in children with multiple AIDs and in autoimmune families. Familial T1D, even with only second-degree relatives affected, leads to less severe onset of T1D in the index child. This thesis provides current estimates of the frequency of additional autoimmunity in Finnish children with newly diagnosed T1D and in their relatives. These figures are in line with those reported previously internationally and in Finland. Novel discoveries were the milder clinical onset of T1D in familial T1D even if only second-degree relatives were affected (readily explained by the increased awareness of the disease in these families), and the gender difference of girls having paternal and boys maternal second-degree relatives affected by T1D. This gender difference, transient anti-tTG among children with T1D at diagnosis, and the reported candidate non-HLA SNPs for clustered autoimmunity require validation by further studies. Tyypin 1 diabetes on immuunivälitteinen sairaus, jota sairastaa n. 0,7% suomalaisista lapsista. Suomessa tautiin sairastuvuus on korkeinta koko maailmassa. Tyypin 1 diabeteksen syntymissyyt ajatellaan liittyvän sekä genetiikkaan että ympäristötekijöihin. Monet näistä tekijöistä ovat yhteisiä myös muille autoimmuunisairauksille, kuten esimerkiksi keliakialle ja reumasairauksille. Siksi nämä taudit esiintyvät usein yhdessä. Tässä väitöskirjatutkimuksessa selvitettiin Suomessa vuosina 2002-2009 tyypin 1 diabetekseen sairastuneiden alle 15-vuotiaiden lasten omaa ja perhehistoriaa muiden autoimmuunisairauksien osalta diagnoosihetkellä sekä osittain myös n. 8 vuoden seurannan aikana. Lisäksi vertailtiin diagnoosihetken kliinistä tilannetta, autovasta-aineita sekä genetiikkaa autoimmuunisairauksien esiintymisen suhteen. Mukana työssä oli yhteensä 2245 tyypin 1 diabetekseen sairastunutta lasta valtakunnallisesta lasten diabetesrekisteristä sekä heidän perheenjäsenensä. Tiedot perheen autoimmuunisairauksista saatiin pääasiassa rekisteriin kuuluvalta kyselylomakkeelta, mutta osittain käytettiin myös Kelan rekistereitä ja keliakiaan liittyviä vasta-aineita seulottiin osalta osallistujista. Tutkimuksen lapsista 1,6%:lla oli jokin muu autoimmuunisairaus jo diabeteksen diagnoosihetkellä ja luku nousi yli 3%:iin seurannassa. Joka viidennellä oli sukulaisia, jotka sairastavat tyypin 1 diabetesta ja kolmasosalla sukulaisia, joilla on jokin muu autoimmuunisairaus. Lasten HLA-genetiikassa oli eroa riippuen autoimmuunitaustasta: esimerkiksi tyypin 1 diabeteksen ja keliakian yhdessä esiintyminen liittyi HLA-DR3-DQ2 genotyyppiin, kun taas lapsilla, joilla on perheessä tyypin 1 diabetesta, oli useammin HLA-DR4-DQ8 genotyyppi. Muutkin geenialueet kuin HLA vaikuttivat autoimmuunisairauksien esiintymiseen yhdessä. Metabolinen tilanne oli diagnoosihetkellä rauhallisempi, esim. verensokeritasot matalammat, paino ehtinyt laskea vähemmän ja diabeettista ketoasidoosia vähemmän, jos perheessä oli muita tyypin 1 diabetesta sairastavia, vaikka he olisivatkin kaukaisempia sukulaisia (esim. setä, täti, isovanhempi). Vaikka näissä perheissä diabetesoireet tunnistetaan helpommin, potentiaalisesti hengenvaarallista ketoasidoosia esiintyi silti lähes 10%:lla. Parantamisen varaa varhaisessa diagnosoimisessa siis edelleen on. Tutkimuksella saatiin ajankohtaista tietoa diabetesta sairastavien lasten ja heidän perheensä sairastavuudesta muiden autoimmuunisairauksien osalta. Nämä osuudet pitkälti vastaavat kansainvälistä tasoa. Eroavaisuuksia genetiikassa ja kliinisessä tilanteessa oli nähtävissä riippuen autoimmuunitaustasta. Osa tuloksista, kuten esimerkiksi tiettyjen HLA:n ulkopuolisten geenien merkitys, vaativat lisätutkimuksia.

Breast cancer is the most common cancer among women in the Western world. In Finland, approximately 5000 women are diagnosed with breast cancer each year. Due to advances in treatments, disease prognosis has improved markedly. Pain after breast cancer treatments is a common adverse symptom. The purpose of this prospective study was to identify factors associated with the pain experience in women treated for breast cancer, and to uncover clinically feasible factors associated with acute and persistent pain to develop an easy-to-use screening tool to identify women at the highest risk for persistent pain. The whole cohort included 1000 patients (18-75 years) recruited at the Helsinki University Hospital. Preoperatively, they filled in questionnaires about medical history, overall health, and pain symptoms, depressive symptoms, anxiety, and anger regulation. Experimental pain tests (cold 4°C and heat 48°C) were performed preoperatively. Anesthesia protocol and perioperative pain treatment were recorded. Patients documented pain on the first postoperative week (days 1-7) three times daily in the area of surgery. In the follow-ups (1 month, 6 months, 1, 2, and 3 years) patients completed again the same questionnaires about pain, depressive symptoms, and anxiety. The range of pain sensitivity between women treated for breast cancer was high. Of the women treated for breast cancer 13.5% had developed clinically significant persistent pain at the one-year follow-up. The best predictors of pain of any kind; experimental, acute clinical, or persistent pain, were found to be quite similar. Pain (other chronic pain condition, pain in the area of surgery, or intensity of acute pain), more invasive surgery (axillary clearance), and psychological distress (mainly anxiety) were found to be consistent predictors of heightened pain experience. In addition to these, pain expectation and higher need for oxycodone to achieve satisfactory pain relief after surgery were associated with higher pain intensity during the first postoperative week. Obesity was associated with persistent pain at six months and one year after surgery. The adjuvant treatments added to the risk for persistent pain at one year. Screening tools for preoperative and acute phase use to identify women at risk for persistent pain at six months and at one year after surgery were developed. The one-year prediction tool was validated in two other prospective patient cohorts. The average levels of psychological burden were quite low. However, there was a group of women whose distress remained quite stable during the first year. Anger regulation had only a modest association with pain, and was influenced by age and mood. However, anger inhibition was associated with higher depressive symptoms throughout the three-year follow-up. COMT rs4680 was associated with anger-out. Rintasyöpä on naisten yleisin syöpätyyppi. Vuosittain Suomessa rintasyöpään sairastuu noin 5000 naista. Kipu rintasyöpään liittyvien hoitojen jälkeen on merkittävä ongelma. Hoitojen kehittymisen myötä eliniän ennuste sairastumisen jälkeen on noussut huomattavasti, jonka vuoksi elämänlaatu hoitojen jälkeen on tärkeää. Tutkimuksessa selvitettiin tekijöitä, jotka vaikuttavat kivun kokemiseen rintasyövän vuoksi hoidetuilla naisilla. Selvitimme tekijöitä, joita on mahdollista seuloa kliinisessä työssä, liittyen sekä akuuttiin kokeelliseen että kliiniseen kipuun ja pitkittyneeseen kipuun. Aineisto koostui HYKS sairaalassa leikatuista 18-75 vuotiaista naisista. Potilaat täyttivät ennen leikkausta laajasti terveyshistoriaa, masennusoireita, tilanne- ja yleistä ahdistustaipumusta ja suuttumuksen ilmaisutyyliä kartoittavia kyselylomakkeita. Potilaille tehtiin leikkausta ennen kuuma- ja kylmäkipua mittaavat kokeellisen kivun testit. Leikkauksen anestesia, sekä akuutin kivun hoito kirjattiin. Potilas kirjasi ensimmäisen leikkauksen jälkeisen viikon ajan kipua leikkausalueella. Kipu ja osa mielialaan liittyvistä kyselylomakkeista lähetettiin 1kk, 6kk, 1v, 2v ja 3v leikkauksen jälkeen. Potilaat olivat keskimäärin psyykkisesti melko hyvinvoivia, mutta osalla psyykkinen kuormittuneisuus pysyi melko muuttumattomana vuoden seurannassa. Sisäänpäin suuntautuvan suuttumuksen ilmaisun huomattiin olevan yhteydessä voimakkaampiin masennusoireisiin kolmen vuoden seurannassa. Kipuherkkyydessä oli suuria eroja naisten välillä. Potilaista 13.5 % ilmoitti kipua leikatulla alueella vuoden seurannassa. Akuuttiin ja pitkittyneeseen kipuun vaikuttavat tekijät olivat melko samanlaisia. Aikaisempi kipu, muu krooninen tai kipu leikattavalla alueella olivat yhteydessä sekä akuuttiin että pitkittyneeseen kipuun. Voimakkaampi akuutti kipu leikkauksen jälkeisinä päivinä ennusti pitkittynyttä kipua vuoden seurannassa. Psyykkinen kuormittuneisuus sekä voimakkaan leikkauksen jälkeisen kivun odotus vaikuttivat ensimmäisen viikon kivun määrään. Ahdistus oli yhteydessä sekä akuuttiin että pitkittyneeseen kipuun. Leikkaustyyppi (kainalon tyhjennys) lisäsi riskiä sekä akuuttiin että pitkittyneeseen kipuun. Pitkittynyttä kipua vuoden kohdalla ennustivat myös sädehoito ja voimakas ylipaino (BMI>31). Tulosten perusteella kehitettiin ja validoitiin seula, jolla voidaan löytää ennen leikkausta sekä ensimmäisen leikkauksen jälkeisen viikon aikana kerätyn tiedon avulla ne naiset, jotka voivat olla riskissä kehittää pitkittynyt kipu.

Non-alcoholic fatty liver disease (NAFLD) may result from obesity accompanied by insulin resistance and adipose tissue inflammation or from the common genetic variants in PNPLA3 (rs738409, C>G/I148M), TM6SF2 (rs58542926, C>T/E167K) and MBOAT7 (rs641738, C>T). These variants increase the liver fat content and the severity of NAFLD without features of insulin resistance. This thesis aimed to determine the following: i) whether NAFLD predicts type 2 diabetes independent of obesity and other known risk factors; ii) whether adipose tissue is inflamed in subjects homozygous for the PNPLA3 I148M variant; iii) whether obesity and insulin resistance rather than liver fat content increase coagulation factor activities and expression; and iv) which factors predict NAFLD and liver stiffness during an 11-year follow-up period. The present thesis includes one systematic review, two cross-sectional studies and one longitudinal study. Study subjects comprised Finnish adult men and women. Liver fat content was measured by proton magnetic resonance spectroscopy (studies II–IV). The gene expression of inflammatory markers in adipose tissue and that of coagulation factors in the liver were measured using qPCR (studies II and III). We conducted a systematic review of prospective longitudinal studies to determine if NAFLD predicts type 2 diabetes for aim i). Based on these studies ultrasound-diagnosed NAFLD and liver enzymes predict type 2 diabetes independent of confounders such as age and obesity. We found no studies indicating that NAFLD associated with genetic risk variants predicting future risk of type 2 diabetes. A group of 82 subjects were divided into two groups based on body mass index and PNPLA3 genotype for aim ii). The liver fat content was similarly increased in obese/insulin-resistant subjects and in carriers of the I148M variant compared to non-obese subjects and non-carriers of this variant. In obese subjects, the adipose tissue expression of pro-inflammatory chemokine MCP-1 was increased and anti-inflammatory ADIPOQ and TWIST1 were decreased compared with non-obese subjects, while these were comparable between carriers and non-carriers of the I148M variant. In study III, 92 non-diabetic subjects were divided into two groups based on insulin sensitivity (HOMA-IR) and PNPLA3 genotype. Coagulation factor activities (FVIII, FIX, FXIII, fibrinogen and VWF:RCo) were increased, and the prothrombin time and activated partial thromboplastin time were shortened in insulin-resistant subjects when compared to insulin-sensitive subjects; yet, these factors were similar in carriers and non-carriers of the I148M variant. The hepatic gene expression of FVIII, FIX and fibrinogen gamma-chain were higher in insulin-resistant subjects with NAFLD (n=13) compared with equally obese insulin-sensitive subjects without NAFLD (n=13). In study IV, 97 subjects were examined twice over an interval of 11 years. The baseline liver fat content independently predicted NAFLD and an increased liver stiffness (measured using transient elastography) at 11.3 years more accurately than routinely available clinical and biochemical parameters. Conclusions: Obesity, adipose tissue inflammation and insulin resistance rather than excess hepatic fat per se are related to an increased risk of type 2 diabetes and a pro-coagulant plasma profile observed often in NAFLD. However, the liver fat content emerges as more important predictor of advanced liver fibrosis than the associated metabolic abnormalities. These data support the view that NAFLD is heterogeneous disease. Alkoholiin liittymätön rasvamaksatauti (non-alcoholic fatty liver disease, NAFLD) on yleisin krooninen maksasairaus länsimaissa. Maksan rasvoittuminen on tiiviisti yhteydessä lihavuuteen, rasvakudoksen matala-asteiseen tulehdustilaan, insuliiniherkkyyden heikentymiseen, metaboliseen oireyhtymään, tyypin 2 diabetekseen sekä sydän- ja verisuonisairauksiin ('metabolinen rasvamaksatauti'). Viime vuosina on löydetty yleisiä rasvamaksalle altistavia geenivariantteja, kuten PNPLA3–geenin I148M-variantti, joiden aiheuttaman rasvamaksataudin ('geneettisen rasvamaksataudin') yhteys insuliiniherkkyyteen ja rasvakudoksen tulehdustilaan on epäselvä. Maksan rasvoittuminen voi johtaa maksatulehdukseen (NASH), sidekudoksen muodostumiseen ja kirroosiin. Tämän väitöskirjatutkimuksen tavoitteena oli lisätä ymmärrystä rasvamaksataudin riskitekijöistä, etenemisestä ja yhteydestä aineenvaihdunnallisiin sairauksiin kuten tyypin 2 diabetekseen. Systemaattisen kirjallisuuskatsauksen avulla selvitettiin, että rasvamaksatauti ennustaa tyypin 2 diabetekseen sairastumista riippumatta muista tunnetuista riskitekijöistä. Kirjallisuudesta ei löytynyt seurantatutkimuksia, joissa olisi arvioitu geneettisestä alttiudesta aiheutuneen rasvamaksataudin lisäävän riskiä diabetekseen sairastumiselle. Rasvakudoksen tulehdustilan on ajateltu altistavan rasvamaksataudin kehittymiselle. Toisessa osatyössä määritettiin 82 tutkimushenkilön ihonalaisen rasvakudoksen tulehdustilaa tulehdusta lisäävien ja hillitsevien geenien ilmentymistä mittaamalla. Maksan rasvapitoisuus mitattiin magneettiresonanssispektroskopialla (1H-MRS). Tutkittavat jaettiin kahteen ryhmään painoindeksin sekä PNPLA3-genotyypin perusteella. Ylipainoisilla maksan rasvapitoisuus oli korkeampi kuin hoikemmilla tutkittavilla. Lisäksi heillä esiintyi rasvakudoksen tulehdusta. PNPLA3-variantin kantajilla ei havaittu rasvakudoksen tulehdusta huolimatta yhtälailla korkeammasta maksan rasvapitoisuudesta verrattuna henkilöihin ilman riskivarianttia. Kolmannessa osatyössä tutkittiin hyytymistekijöiden aktiivisuutta 92 tutkimushenkilöllä, jotka jaettiin kahteen ryhmään insuliiniherkkyyttä kuvastavan HOMA-IR -arvon sekä PNPLA3-genotyypin perusteella. Hyytymistekijöiden tuotantoa mitattiin 26 henkilön maksakudoksesta. Veren hyytymisaktiivisuuden sekä hyytymistekijöiden tuotannon havaittiin olevan koholla 'metabolisessa rasvamaksataudissa'. Hyytymistekijöiden aktiivisuuden ei havaittu nousseen PNPLA3-variantin kantajilla verrattuna henkilöihin ilman riskivarianttia. Neljännessä osatyössä selvitettiin, mitkä tekijät ennustavat rasvamaksatautia ja sidekudoksen esiintymistä. Mitä enemmän maksaan kertyy sidekudosta, sitä jäykemmäksi maksa muuttuu. Seurantatutkimus toteutettiin 97 henkilölle, joilta tutkittiin lähtötilanteessa kehonkoostumus, maksan rasvapitoisuus (H-MRS) sekä insuliiniherkkyyteen ja maksasairauksiin liittyviä laboratoriotutkimuksia. Nämä mittaukset toistettiin 11 vuoden päästä yhdessä maksan jäykkyysmittauksen kanssa. Lähtötilanteen maksan rasvapitoisuus ennustaa rasvamaksatautia ja merkittävää sidekudoksen muodostumista. Väitöskirjatutkimus osoittaa, että rasvamaksataudin syyt pikemmin kuin itse rasvoittuminen selittävät, liittyykö tautiin lisääntynyt riski sairastua tyypin 2 diabetekseen tai liitännäishäiriöitä kuten rasvakudoksen tulehdustila tai hyytymistekijöiden lisääntynyt aktiivisuus. Maksan rasvoittuminen syystä riippumatta ennustaa rasvamaksataudin ja merkittävän sidekudosmäärän esiintymistä 11 vuoden päästä.

Tibia plateau fractures are relatively uncommon, but they are among the most challenging intra-articular fractures to treat. These fractures can lead to early posttraumatic osteoarthritis (OA) and cause disability and constant pain. Currently, the most common treatment is open reduction and stable internal fixation (ORIF), allowing early mobilization of the knee. Tibial plateau fractures can be associated with several concomitant soft tissue injuries of the knee. Historically, the operativetreatment with internal fixation has also been related to an increased risk for serious wound complications. The purpose of this study was to investigate the current management and outcome of proximal tibia fractures. The study population consisted of various groups of patients with proximal tibia fractures treated between 2002 and 2013 at our level I trauma center. The study aimed to determine factors predicting the development of posttraumatic OA following tibial lateral or medial plateau fractures. Another focus was on the incidence of concomitant injuries after the most common lateral plateau fracture type and the need for MRI as a diagnostic tool when treating these fractures. Finally, the predictors for deep surgical site infection after plate fixation of proximal tibia fracture were examined. The results showed that relatively good functional outcome can be predicted after internal fixation of lateral and medial tibial plateau fractures. However, patients with lateral plateau fractures with residual depression of the articular surface >2 mm or valgus deformity >5° had significantly more severe (Kellgren-Lawrence grade 3–4) posttraumatic OA. The most significant predictor of posttraumatic OA after medial plateau fracture was the amount of initial depression of the articular surface measured from the preoperative computer tomography, while the quality of reduction was not found to predict OA. MRI had low sensitivity and specificity in the diagnosis of concomitant injuries in the lateral tibial plateau fracture setting. Also nearly all of the clinically relevant concomitant injuries could be treated through the same lateral arthrotomy at the time of ORIF without the need for additional arthroscopy. There is high morbidity associated with deep SSI in plated proximal tibial fractures. Patient’s age ≥50 years, obesity, history of alcohol abuse, and AO type C fracture are independent risk factors for infection. Performing a fasciotomy also increases the risk of deep infection and should be done with meticulous technique only when deemed necessary. Sääriluun yläosan kondyylimurtumat ovat suhteellisen harvinaisia vammoja, mutta toisaalta yleensä varsin vaativia ja haasteellisia hoitaa. Koska murtumiin liittyy polven kantavan nivelpinnan rikkoutuminen, ne voivat helposti johtaa ennenaikaisen nivelrikon kehittymiseen ja pysyvään toiminnan alenemiseen ja kipuun. Operatiivisen hoidon tarkoituksena on palauttaa nivelpinnan kongruenssi ja saavuttaa stabiili fiksaatio, joka sallii polven varhaisen mobilisaation. Luisen murtuman lisäksi kondyylimurtumiin liittyy riski pehmytkudoksiin kohdistuvista liitännäisvammoista, kuten nivelkierukkavaurioista ja nivelsidevammoista. Avoimeen reduktioon ja sisäiseen fiksaatioon on perinteisesti liittynyt myös suurentunut vakavien haavakomplikaatioiden riski. Tutkimuksen tarkoituksena oli selvittää kondyylimurtumien nykyhoitoa ja hoidon tuloksia. Tutkimusaineisto koostui eri ryhmistä, joille oli tehty kondyylimurtuman vuoksi leikkaus Töölön sairaalassa vuosina 2002–2013. Tutkimuksen tavoitteena oli selvittää ennenaikaisen nivelrikon kehittymisen riskitekijöitä sääriluun sisemmän ja ulomman nivelnastan murtuman jälkeen. Lisäksi tarkasteltiin pehmytkudosliitännäisvammojen insidenssiä sekä magneettikuvauksen tarpeellisuutta yleisimmän murtumatyypin eli ulomman nivelnastan murtuman diagnostiikassa. Tutkimuksessa selvitettiin myös riskitekijöitä syvälle haavainfektiolle sääriluun yläosan murtuman levytyshoitoon liittyen. Tutkimus osoitti että nykyisellä levytyshoidolla on saavutettavissa keskimäärin verrattain hyvä funktionaalinen lopputulos. Ulomman nivelnastan murtumien hoidossa nivelpinnalle jäänyt >2 mm painuma tai >5° valgiteetti johtivat tilastollisesti todennäköisemmin hankalan (Kellgren-Lawrence 3–4 asteen) nivelrikon kehittymiseen. Sisemmän nivelnastan murtumien hoidossa murtuman primaaridislokaation määrällä näytti olevan selvästi merkitystä posttraumaattisen nivelrikon kehittymiselle, kun taas saavutetun reduktion laadulla ei ollut selvää merkittävyyttä. Tutkimuksessa kävi ilmi että magneettikuvauksen herkkyys ja tarkkuus on verrattain alhainen liitännäisvammojen diagnostiikassa kun kyseessä on ulomman nivelnastan murtuman käsittävä polvi. Lähes kaikki kliinisesti merkittävät liitännäisvammat voitiin hoitaa saman artrotomian kautta, jota käytettiin murtuman reduktioon ja näin ollen polvinivelen tähystyksen merkitys jäi vähäiseksi. Osoitimme että sääriluun yläosan levytyksen jälkeiseen haavainfektioon liittyy merkittävä morbiditeetti. Erityisesti yli 50-vuotiaat, ylipainoiset ja alkoholin suurkuluttajat ovat riskissä saada syvä haavainfektio levytyshoidon jälkeen. Myös hankalampi, molemmat kondyylit käsittävä murtuma altistaa infektiolle. Aitiopainesyndrooman vuoksi tehdyt faskiotomiat lisäävät selvästi infektioriskiä ja ne tulisi suorittaa huolellista kirurgista tekniikka noudattaen silloin kun ne on arvioitu tarpeellisiksi.

Non-alcoholic fatty liver disease (NAFLD) is a heterogeneous condition and at least two different forms exist. ‘Obese/Metabolic NAFLD’ characterizes subjects with metabolic syndrome and insulin resistance, whereas NAFLD associated with the common PNPLA3 I148M variant (‘PNPLA3 NAFLD’) is not accompanied by insulin resistance. This thesis was undertaken to elucidate the pathogenetic mechanisms of NAFLD (Studies I and III) and to estimate the prevalence of non-alcoholic steatohepatitis in Finnish subjects (Study II). The study groups consisted of 417 (I) and 372 (III) subjects, as well as 296 bariatric surgery patients from Finland and 2849 subjects from a population-based D2D-sample (II). In Study II, 380 non-bariatric surgery patients who had undergone a liver biopsy for suspected NAFLD from Italy were used as an external validation cohort. In Study I liver fat content was measured by proton magnetic resonance spectroscopy (1H-MRS), in Study II by either biopsy or 1H-MRS, and in Study III in 75% by 1H-MRS and in 25% by biopsy. Clinical characteristics (I, II, III), plasma apoC3 concentrations (I), and serum cytokeratin 18 fragments (II) were determined. In Study III, lipidomic analyses were performed using ultra-performance liquid chromatography coupled to time-of-flight mass spectrometry (UPLC-MS). Individuals were genotyped for rs2854116 and rs2854117 in APOC3 (I) and the known rs738409 in PNPLA3 (I, II, III). In Study II, we developed and validated a ‘NASH score’ in the Finnish and Italian biopsy cohorts, which was then used to predict NASH prevalence in the population-based D2D study. In Study III, the subjects were divided into groups based on PNPLA3 genotype or obesity, and the absolute and relative triacylglyceride concentrations were compared between the subgroups. No difference existed between the APOC3 variant allele (T-455C or C-482T or both) carriers and non-carriers in liver fat or apoC3 concentrations, whereas those with the PNPLA3 GG genotype at rs738409 had a 2.7-fold higher liver fat content than those with the CC genotype (I). The ‘NASH score’ included PNPLA3 genotype, aspartate aminotransferase (AST), and fasting insulin. The area under the ROC for this score was 0.774 (95% CI 0.709-0.839) in Finns and 0.759 (95% CI 0.711-0.807) in Italians (NS) (II). The prevalence of NASH based on this score in the D2D study was 6.0% (95% CI 5.0-6.9%) (II). Sensitivity analysis was performed by a Bayesian model, which gave a NASH population prevalence of 3.6% (95% CI 0.2-7.7%) using the ‘NASH Score’ (II). Absolute and relative deficiency of distinct circulating TAGs was observed in the PNPLA3148MM/148MI as compared with the PNPLA3148II group (III). Genotypes in the obese and ‘non-obese’ groups were similar but the obese subjects were insulin-resistant (III). Liver fat was increased in both obese and PNPLA3148MM/148MI groups (III). Multiple changes in the relative TAG concentrations were observed between obese and ‘non-obese’ groups (III). These closely resembled those between obese subjects with ‘obese NAFLD’ versus the ‘PNPLA3 NAFLD’ (III). Variation in liver fat cannot be explained by genetic variants in APOC3 (I). The population-based prevalence of NASH in Finnish subjects is ~5% (II). The circulating TAG profile depends on the aetiology of NAFLD (III). In ‘PNPLA3 NAFLD’ a relative deficiency of TAGs is observed, supporting the idea that the I148M variant prevents lipolysis rather than stimulates TAG synthesis in the liver (III). Maksan rasvoittuminen ei-alkoholiperäisestä syystä (non-alcoholic fatty liver disease - NAFLD) on todettu olevan vahvasti yhteydessä metaboliseen oireyhtymään, insuliiniresistenssiin, tyyppi 2 diabetekseen ja lihavuuteen. Rasvamaksaan voi myös kehittyä solunekroosia ja tulehdusta (non-alcoholic steatohepatitis, NASH), mikä pahimmassa tapauksessa etenee kirroosiin. Koska rasvamaksa ei aina selity tunnetuilla tekijöillä, kuten obesiteetilla tai matalalla HDL-kolesterolilla, on geenien merkitystä maksan rasvoittumisen tutkittu laajasti. Etenkin yhden, PNPLA3 geenin polymorfioiden, on todettu olevan vahvasti yhteydessä maksan rasvoittumiseen. Henkilöillä, joilla on PNPLA3 geenin GG variantti kohdassa rs739409 on keskimäärin 80% suurempi maksan rasvapitoisuus verrattuna henkilöihin, joilla on CC variantti tässä kohdassa. Lisäksi PNPLA3 variantilla selittyvällä rasvamaksapotilailla on todettu olevan erilainen metabolinen profiili, kuin obesiteetilla selittyvällä rasvamaksapotilailla. Toisin kuin obesiteettiin liittyvässä maksan rasvottumisessa, PNPLA3 varianttiin ei liity insuliini resistenssiä, kohonneita veren sokeriarvoja, hypertriglyseridemiaa, tai matalaa HDL-kolesterolia. Tämän väitöskirjatutkimuksen tarkoituksena oli laajentaa ymmärrystämme maksan rasvoittumisen patogeneesiin ja arvioida rasvamaksatulehduksen väestöpohjaista esiintyvyyttä. Ensimmäisessä osatyössä selvitettiin PNPLA3 geenin ja apoCIII geenin variaatioiden yhteyttä rasvamaksaan. Potilaamme genotyypattiin APOC3 geeni kohdista rs2854116 ja rs2854117 ja PNPLA3 kohdasta rs739409. Maksan rasvaprosentti määriteltiin protonispektroskopialla (1H-MRS) ja plasman apoC3 konsentraatiot mitattiin entsymaattisesti ELISA:lla. PNPLA3 geenivariantti liittyi vahvasti maksan rasvoittumiseen, kun taas APOCIII geenillä ei ollut yhteyttä maksan rasvapitoisuuteen. Pelkkä rasvamaksa voi edetä NASH:ksi ja maksa voi fibrotisoitua, jonka on todettu olevan paras maksasairauden etenemisen ja maksansiirron tarpeen ennustaja. Koska NASH:n diagnoosi perustuu biopsiaan, on NASH:n esiintyvyyttä väestössä ollut vaikeata arvioida. Toisessa osatyössä 296 potilaan maksabiopsia-aineiston avulla kehitettiin NASH-riskiä arvioiva yhtälö "NASH-pisteytys", joka validoitiin italialaisessa 380 muun kuin bariatrisen potilaan maksabiopsia-aineistossa. Pisteytys sisälsi PNPLA3-genotyypin, sekä ASAT- ja paastoinsuliinipitoisuudet, ja ennusti NASH:n 72 %:n herkkyydellä ja 74 %:n spesifisyydellä. Yhtälön avulla arvioitiin NASH:n esiintyvyys 2 849 henkilön väestöpohjaisessa D2D-aineistossa. Suomessa NASH:n esiintyvyys 45 - 74-vuotiailla oli noin 5 %. Koska alkoholiin liittymättömän rasvamaksan etiologia on monisyinen, selvitimme, miten eri lipidi (triglyseridi) pitoisuudet suhteessa maksan rasvaan eroavat henkilöillä, joilla on rasvamaksa PNPLA3 geenivariantista johtuen ('PNPLA3 NAFLD') verrattuna niihin, joilla NAFLD on obesiteettiin liityvä ('Obeesi/ metabolinen NAFLD'). Lipidomiikkaprofiilien määritykset tehtiin ultra-performance -nestekromatografia ja time-of-flight massaspektrometri (UPLC-MS) -menetelmällä. Tutkimuksessa havaittiin, että NAFLD etiologialla on vaikutus veren rasvapitoisuuksiin. 'PNPLA3 NAFLD':ssa seerumin absoluuttiset triglyseridipitoisuudet ovat madaltuneet, siinä missä 'Obeesi/metabolinen NAFLD':ssa ne ovat vastaavasti nousseet. Löydökset heijastavat lihavuuden ja PNPLA3-I148M geenivariantin assosiaatiota seerumin triglyseridiprofiiliin potilailla, joilla on sama maksan rasvapitoisuus. Tutkimus osoittaa, että maksan rasvoittumisen etiologia vaikuttaa rasvamaksan syntyyn ja kehittymiseen.

Adult-type granulosa cell tumors (AGCTs) belong to the sex cord-stromal group of ovarian tumors and account for 3-5% of ovarian neoplasms. Etiological factors of AGCTs remain mostly unknown, although studies have found a somatic missense mutation in the transcription factor FOXL2. AGCTs are usually diagnosed at an early stage and have a favorable prognosis compared with the more common epithelial ovarian cancer. However, tumor recurrence develops in up to 35% of patients, even in early-stage disease - often unpredictably and several years or even decades after the primary diagnosis. The aims of this study were to determine the incidence and epidemiological background of AGCTs in a large, multinational Nordic cohort and to estimate the incidence of other, especially endocrine-related primary malignancies among patients with AGCT. Furthermore, the objective was to describe the clinical characteristics and prognostic factors linked to AGCT–related recurrence and survival, and to introduce an optimal follow-up strategy for these patients. Our epidemiological registry study on AGCT incidence utilized the Finnish, Icelandic, Norwegian, and Swedish Cancer Registry data on AGCTs over several decades. We showed that the age-adjusted incidence rates were quite constant in 1953-2012: about 0.6-0.8 per 100,000 for most of the study period. The age-specific incidence was highest at 50-64 years of age, and there were no occupations with significantly increased risk of AGCT. The conclusions drawn from these results point to AGCT as a primarily sporadic, not exposure-related cancer, typically occurring in peri- or postmenopause. We estimated the incidence of other primary malignancies among AGCT patients using data from the Finnish Cancer Registry in 1968-2013. After AGCT, we found increased risks for cancers of the soft tissue and thyroid as well as leukemia, which likely indicate shared risk factors and therapy-induced side effects. The incidence of AGCT was significantly increased among women with previous breast cancer, suggesting shared hormonal etiology or treatment-induced effects. To evaluate the clinical and prognostic factors, all AGCT patients diagnosed at Helsinki University Hospital during nearly six decades were included in the clinical study cohort (n=240). After a histological review, we analyzed the clinical data for association with both AGCT-related and overall survival and tumor relapse. Of the original study cohort, the diagnosis was histologically confirmed in 78% of patients and molecularly confirmed for the FOXL2 mutation in 68% of patients. In multivariate analysis, stage was the only independent prognostic factor related to AGCT-specific survival. Spontaneous or iatrogenic tumor rupture was independently associated with tumor recurrence. By utilizing the extensive cancer registry data together with the internationally unique, large and carefully validated single-institute patient cohort, these studies reveal the diagnostic challenges of AGCTs, and provide novel epidemiological data and evidence-based tools to develop follow-up strategies for this rare cancer. Aikuistyypin munasarjan granuloosasolukasvain (AGSK) kuuluu sukupienakasvaimiin ja käsittää 3-5 % kaikista munasarjasyövistä. Tyypillisesti AGSK erittää hormoneja, kuten estrogeenia ja inhibiinejä. Kasvaimen synty tunnetaan huonosti, mutta sille on osoitettu pistemutaatio transkriptiotekijä FOXL2:a koodaavassa geenissä. Tämän tutkimuksen tarkoituksena oli tutkia AGSK:n kansainvälistä esiintyvyyttä ja epidemiologista taustaa laajassa pohjoismaisessa aineistossa, sekä selvittää muiden, erityisesti hormoniriippuvaisten syöpien esiintyvyyttä kyseisillä potilailla. Lisäksi pyrkimyksenä oli määrittää AGSK:n taudinkuvaa ja ennustetekijöitä. Tulosten avulla pyrimme muodostamaan näille potilaille optimaalisen seurantastrategian kasvaimen toteamisen jälkeen. Rekisteritutkimuksemme hyödynsi Suomen, Ruotsin, Norjan ja Islannin syöpärekisteritietoja usean vuosikymmenen ajalta. Ikäkorjattu ilmaantuvuus pysyi vuosina 1953-2012 melko tasaisena, noin 0,6-0,8/100 000. Ilmaantuvuus oli korkeimmillaan 50-64-vuotiaiden ryhmissä, eikä erityisiä ammattialtisteita löydetty eri ammattiryhmiä tutkittaessa. Johtopäätöksenä tuloksista voidaan todeta, että AGSK on pikemmin satunnaisesti ilmaantuva kuin altistesidonnainen syöpä, joka ilmenee tyypillisesti vaihdevuosi-iässä tai tämän jälkeen. AGSK-potilailla ilmeneviä muita syöpiä tutkittiin käyttämällä Suomen Syöpärekisterin aineistoa vuosilta 1968-2013. AGSK:n jälkeen todettiin merkitsevästi enemmän pehmytkudos- ja kilpirauhassyöpää sekä leukemiaa, mikä todennäköisesti viittaa yhteisiin riskitekijöihin sekä syöpähoitojen myöhäisiin haittavaikutuksiin. Rintasyövän sairastaneilla naisilla esiintyi merkitsevästi enemmän AGSK:ta, mikä saattaa viitata näille kasvaimille yhteiseen hormonaaliseen taustaan tai rintasyövän hoitojen vaikutukseen. Kliininen aineistotutkimus kattoi HYKS Naistenklinikalla hoidetut AGSK-potilaat lähes kuuden vuosikymmenen ajalta (N=240). Diagnoosi varmistettiin ensin sekä patologin tutkimuksessa että myöhemmin FOXL2-mutaatin osalta, ja aineisto analysoitiin sekä kokonais- ja tautikohtaisen kuolleisuuden että kasvaimen uusiutumisen suhteen. Kasvaimen levinneisyys toteamishetkellä oli ainoa itsenäinen ennustetekijä tautikohtaisen kuolleisuuden suhteen, mutta kasvaimen puhkeaminen joko spontaanisti tai leikkauksen aikana ennusti itsenäisesti AGSK:n uusiutumista. Nämä kansainvälisesti ainutlaatuisen laajan ja tarkkaan vahvistetun potilasaineiston sekä kattavan syöpärekisteriaineiston avulla tehdyt tutkimukset osoittavat AGSK:n diagnostiset haasteet ja tuovat tämän harvinaisen syövän osalta sekä uutta epidemiologista tietoa että välineitä kehittää näyttöön perustuvia seurantastrategioita.
