The impact of set-up and dose variables upon organs at risk (OAR) dose in DIBH RT was investigated.Methods and materials: All patients with left-breast cancer referred between 2011 and 2014 – of all disease stages, set-up variations, and dose prescriptions – were included. Radiographers used simple screening criteria at CT simulation, to systematically assess patients for obvious DIBH benefit and capability. Selected patients received forward-planned intensity-modulated RT (IMRT) based on a DIBH CT scan. A 3D-surface monitoring system with visual feedback assured reproducible DIBH positioning during gated radiation delivery. Patient, target set-up, and OAR dose information were collected at treatment.Results: Of 272 patients who were screened, 4 withdrew, 56 showed no obvious advantage, and 56 showed benefit but had suitability issues; 156 patients were selected and successfully completed DIBH treatment. The technique was compatible with complex set-up and optimal target coverage was maintained. Comparison of free-breathing (FB) and DIBH treatment plans in the first five patients enrolled confirmed DIBH reduced heart radiation by ~80% (p = 0.032). Low OAR doses were achieved overall: the mean (95% confidence interval [CI]) heart dose was 1.17 (1.12–1.22) Gy, and the mean ipsilateral lung dose was 5.26 (5.01–5.52) Gy. Patients who underwent a standard radiation schedule (40 Gy/15#) after breast-conserving surgery had the lowest OAR doses: post-mastectomy treatment, simultaneous supraclavicular (SCV) node coverage, and alternative dose schedule (50 Gy/25#) were interrelated variables associated with increased OAR risk and compromised ipsilateral lung dose constraints.Conclusion: The DIBH technique was successfully implemented and resulted in optimally low heart radiation. All patients who demonstrate sufficient DIBH technique at planning CT are now offered DIBH RT at our clinic. Patients with more advanced disease, particularly those with additional pulmonary risk factors, warrant additional focus to improve lung sparing. 3. A method for generating coherent spatially explicit maps of seasonal palaeoclimates from site-based reconstructions

Abstract

We describe a method for reconstructing spatially explicit maps of seasonal palaeoclimate variables from site-based reconstructions. Using a 3D-Variational technique, the method finds the best statistically unbiased, and spatially continuous, estimate of the palaeoclimate anomalies through combining the site-based reconstructions and a prior estimate of the palaeoclimate state. By assuming a set of correlations in the error of the prior, the resulting climate is smoothed both from month to month and from grid cell to grid cell. The amount of smoothing can be controlled through the choice of two length-scale values. The method is applied to a set of reconstructions of the climate of the Last Glacial Maximum (ca. 21,000 years ago, yr BP) for southern Europe derived from pollen data with a prior derived from results from the third phase of the Palaeoclimate Intercomparison Project (PMIP3). We demonstrate how to choose suitable values for the smoothing length scales from the datasets used in the reconstruction.

30. Reconstructing ice–age climates: quantifying low-CO2 effects on plants

Abstract

We present a novel method to quantify the ecophysiological effects of changes in CO2 concentration during the reconstruction of climate changes from fossil pollen assemblages. The method does not depend on any particular vegetation model. Instead, it makes use of general equations from ecophysiology and hydrology that link moisture index (MI) to transpiration and the ratio of leaf-internal to ambient CO2 (χ). Statistically reconstructed MI values are corrected post facto for effects of CO2 concentration. The correction is based on the principle that e, the rate of water loss per unit carbon gain, should be inversely related to effective moisture availability as sensed by plants. The method involves solving a non-linear equation that relates e to MI, temperature and CO2 concentration via the Fu-Zhang relation between evapotranspiration and MI, Monteith’s empirical relationship between vapour pressure deficit and evapotranspiration, and recently developed theory that predicts the response of χ to vapour pressure deficit and temperature. The solution to this equation provides a correction term for MI. The numerical value of the correction depends on the reconstructed MI. It is slightly sensitive to temperature, but primarily sensitive to CO2 concentration. Under low LGM CO2 concentration the correction is always positive, implying that LGM climate was wetter than it would seem from vegetation composition. A statistical reconstruction of last glacial maximum (LGM, 21±1kyr BP) palaeoclimates, based on a new compilation of modern and LGM pollen assemblage data from Australia, is used to illustrate the method in practice. Applying the correction brings pollen-reconstructed LGM moisture availability in southeastern Australia better into line with palaeohydrological estimates of LGM climate.

Abstract

Background: Effective anticancer therapy is thought to involve induction of tumour cell death through apoptosis and/or necrosis. [18F]ICMT-11, an isatin sulfonamide caspase-3/7-specific radiotracer, has been developed for PET imaging and shown to have favourable dosimetry, safety, and biodistribution. We report the translation of [18F]ICMT-11 PET to measure chemotherapy-induced caspase-3/7 activation in breast and lung cancer patients receiving first-line therapy.Results: Breast tumour SUVmax of [18F]ICMT-11 was low at baseline and unchanged following therapy. Measurement of M30/M60 cytokeratin-18 cleavage products showed that therapy was predominantly not apoptosis in nature. While increases in caspase-3 staining on breast histology were seen, post-treatment caspase-3 positivity values were only approximately 1%; this low level of caspase-3 could have limited sensitive detection by [18F]ICMT-11-PET. Fourteen out of 15 breast cancer patients responded to first-line chemotherapy (complete or partial response); one patient had stable disease. Four patients showed increases in regions of high tumour [18F]ICMT-11 intensity on voxel-wise analysis of tumour data (classed as PADS); response was not exclusive to patients with this phenotype. In patients with lung cancer, multi-parametric [18F]ICMT-11 PET and MRI (diffusion-weighted- and dynamic contrast enhanced-MRI) showed that PET changes were concordant with cell death in the absence of significant perfusion changes.Conclusion: This study highlights the potential use of [18F]ICMT-11 PET as a promising candidate for non-invasive imaging of caspase3/7 activation, and the difficulties encountered in assessing early-treatment responses. We summarize that tumour response could occur in the absence of predominant chemotherapy-induced caspase-3/7 activation measured non-invasively across entire tumour lesions in patients with breast and lung cancer.
