We describe the case of a 79-year-old woman with mitral insufficiency and a double-orifice mitral valve (DOMV), discovered by echocardiography. Transthoracic echocardiography showed two insufficiency jets. Transesophageal echocardiography revealed a DOMV. Each orifice was provided with a subvalvular apparatus. No associated congenital abnormalities were present. Our case demonstrates that even in elderly patients with a double regurgitant jet, DOMV should be suspected and assessed by transesophageal echocardiography. [ABSTRACT FROM AUTHOR]

Purpose: As the global population aged 60+ grows, ensuring mobility and independence for older adults is a critical public health goal. This paper examines barriers to life-space mobility in older adults and explores wearable lower limb exoskeletons (LLEs) and green exercise as innovative solutions., Methods: Literature search and interdisciplinary expert input were utilized., Results: Life-space mobility, the physical space individuals move through daily, is often restricted by physical and environmental barriers, leading to increased dependency, depression, and reduced quality of life. LLEs offer promising support by compensating for decreased intrinsic capacities, thus facilitating outdoor mobility. Green exercise (i.e., physical activity in nature) provides mental and physical health benefits, further promoting mobility and well-being., Conclusion: Combining LLEs with green exercise may create powerful rehabilitation modalities, addressing both physical and mental aspects of aging. Integrating life-space mobility research into exoskeleton development and rehabilitation programs ensures these solutions meet real-world needs, supporting Healthy Aging by maintaining functional ability. Future research should focus on user-centered development of optimized exoskeleton designs for outdoor use, developing tailored rehabilitation programs, and educating healthcare professionals and caregivers to maximize benefits. This integrated approach holds potential to significantly improve life-space mobility and quality of life for the aging population.

The global aging population faces significant health challenges, including an increasing vulnerability to disability due to natural aging processes. Wearable lower limb exoskeletons (LLEs) have emerged as a promising solution to enhance physical function in older individuals. This systematic review synthesizes the use of LLEs in alignment with the WHO's healthy aging vision, examining their impact on intrinsic capacities and functional abilities. We conducted a comprehensive literature search in six databases, yielding 36 relevant articles covering older adults (65+) with various health conditions, including sarcopenia, stroke, Parkinson's Disease, osteoarthritis, and more. The interventions, spanning one to forty sessions, utilized a range of LLE technologies such as Ekso ® , HAL ® , Stride Management Assist ® , Honda Walking Assist ® , Lokomat ® , Walkbot ® , Healbot ® , Keeogo Rehab ® , EX1 ® , overground wearable exoskeletons, Eksoband ® , powered ankle-foot orthoses, HAL ® lumbar type, Human Body Posturizer ® , Gait Enhancing and Motivation System ® , soft robotic suits, and active pelvis orthoses. The findings revealed substantial positive outcomes across diverse health conditions. LLE training led to improvements in key performance indicators, such as the 10 Meter Walk Test, Five Times Sit-to-Stand test, Timed Up and Go test, and more. Additionally, enhancements were observed in gait quality, joint mobility, muscle strength, and balance. These improvements were accompanied by reductions in sedentary behavior, pain perception, muscle exertion, and metabolic cost while walking. While longer intervention durations can aid in the rehabilitation of intrinsic capacities, even the instantaneous augmentation of functional abilities can be observed in a single session. In summary, this review demonstrates consistent and significant enhancements in critical parameters across a broad spectrum of health conditions following LLE interventions in older adults. These findings underscore the potential of LLE in promoting healthy aging and enhancing the well-being of older adults.

Gas gangrene (GG) remains a life-threatening and deadly disease. Early recognition together with daily surgical debridement remains the mainstay of therapy. We sought to describe a fatal case of necrotizing soft tissue infection, which was a gas gagrene in this case. This case was remarkable as two main sites were infected simultaneously in geographical zones very far from each other making dissemination between both sites almost impossible. Circulating myoglobin is recognized as an early and sensitive marker of acute coronary diseases. Long turnaround time of myoglobin assays jeopardize their clinical utility. We evaluated the analytical performance of the Stratus CS fluorometric enzyme immunoassay based on dendrimer technology, and claimed to achieve a fast and reliable determination of plasma myoglobin concentrations. Precision complied with the recommended analytical performance criteria. Method comparison and recovery experiments indicated, that despite good between-method correlations, the Stratus CS method overestimated myoglobin concentrations in comparison with values obtained on Cobas Integra 400 and BN A. However, since the manufacturers' cut-off for elevated plasma myoglobin levels was higher for Stratus CS than for other techniques, few discrepant results were observed between methods. Elevated levels of hemoglobin, triglycerides and rheumatoid factors did not interfere in the Stratus CS method but hyperbilirubinemia caused a positive difference.

Objective: To determine the relationship between plasma procalcitonin (PCT) levels, C-reactive protein (CRP), white blood cell count (WBC), ionized calcium (Ca2+), and patient outcome; and to compare the diagnostic and prognostic information provided by PCT and by CRP., Design: Prospective, observational study., Setting: Intensive care unit., Patients: Fifty-three patients with septic shock, consecutively diagnosed according to consensus guidelines., Interventions: None., Measurements and Main Results: Blood was sampled at diagnosis and 24 and 48 hrs later and in a subgroup (n = 23) after 120 hrs. PCT was measured with LUMItest and CRP with Vitros slides. Ca2+ was calculated according to McLean-Hastings from calcium and protein levels on Vitros. In all 53 patients, PCT and CRP were elevated (>0.5 ng/mL and >10 mg/L, respectively) within 24 hrs after diagnosis. Nonsurvivors (n = 25) were older (p <.001) and had higher Acute Physiology and Chronic Health Evaluation (APACHE) II scores (p =.02) at diagnosis but did not differ in sepsis etiology, medical history, sex ratio, levels of PCT, CRP, and Ca2+, or WBC count at any time point. Using logistic regression, initial PCT levels were correlated with CRP values (p =.001) and APACHE II score (p <.05), but not with age, gender, Ca2+ levels, survival, or type of pathogen. Within 48 hrs, however, PCT levels decreased more frequently from baseline in survivors than in nonsurvivors (80% vs. 41%, p <.05). Likewise, CRP levels decreased more often in survivors (100% vs. 64%, p <.05) but only at 120 hrs., Conclusions: PCT levels were correlated with the severity of disease at onset (APACHE II) and inflammation (CRP) but not with Ca2+ levels. Inaugural PCT or CRP levels per se poorly predicted outcome but decreasing levels were associated with a higher probability of survival. In this respect, PCT was found to be an earlier marker than CRP.

Gastrocnemical insufficiency is a common syndrome (in modern society mainly affecting young females who have been taking the pill for several years). Often it is the starting point for superficial varicosis. The clinical diagnosis is easy, provided the doctor is informed of its symptoms. Phlebography or Duplex scan are compulsory, deciding surgical treatment. Varicose muscular veins have two different causes: avalvulation at the popliteal vein or compression by the Post. Tib. nerve. Persistent pain, deterioration of the superficial network or neuromyositis are the main reasons for deciding upon surgical treatment. The results are gratifying and lasting.
