La osteogénesis imperfecta es una enfermedad rara de herencia autosómica dominante en la cual existen anomalías en la síntesis o degradación del colágeno que afecta al tejido conectivo ocasionando múltiples fracturas. Neonato femenino, a término 38 semanas, nacido por cesárea, procedente de Pichari, ingresó por cuadro de dificultad respiratoria instalada durante atención inmediata. Al examen físico se palpa crepitaciones en articulaciones. La paciente fue diagnosticada de osteogénesis imperfecta en base a los hallazgos clínicos y de imágenes, al carecer de evaluaciones genéticas.

Este artigo explora um contexto de utilização de VANTs como parte da infraestrutura de comunicação, através da utilização de IA, sob uma infraestrutura de redes de 6º Geração (6G). Além disso, esse artigo introduz a função de rede virtualizada NWDAF como elemento habilitador para IA em redes de comunicação.

Osteogenesis imperfecta is a rare autosome dominant disease in which there are anomalies in the synthesis of collagen affecting the connective tissue leading to multiple fractures. We present the case of a 38-week newborn from Pichari born of a cesarean section who presented respiratory failure. The patient presented crackles on the joints and was diagnosed on clinical grounds of osteogenesis imperfecta. There is limited information regarding epidemiological data, disease characteristics, variability of clinical care, and in-hospital mortality of neurocritically ill patients worldwide. We addressed these issues in the Point PRevalence In Neurocritical CarE (PRINCE) study, a prospective, cross-sectional, observational study. METHODS: We recruited patients from various intensive care units (ICUs) admitted on a pre-specified date, and the investigators recorded specific clinical care activities they performed on the subjects during their first 7 days of admission or discharge (whichever came first) from their ICUs and at hospital discharge. In this manuscript, we analyzed the final data set of the study that included patient admission characteristics, disease type and severity, ICU resources, ICU and hospital length of stay, and in-hospital mortality. We present descriptive statistics to summarize data from the case report form. We tested differences between geographically grouped data using parametric and nonparametric testing as appropriate. We used a multivariable logistic regression model to evaluate factors associated with in-hospital mortality. RESULTS: We analyzed data from 1545 patients admitted to 147 participating sites from 31 countries of which most were from North America (69%, N = 1063). Globally, there was variability in patient characteristics, admission diagnosis, ICU treatment team and resource allocation, and in-hospital mortality. Seventy-three percent of the participating centers were academic, and the most common admitting diagnosis was subarachnoid hemorrhage (13%). The majority of patients were male (59%), a half of whom had at least two comorbidities, and median Glasgow Coma Scale (GCS) of 13. Factors associated with in-hospital mortality included age (OR 1.03; 95% CI, 1.02 to 1.04); lower GCS (OR 1.20; 95% CI, 1.14 to 1.16 for every point reduction in GCS); pupillary reactivity (OR 1.8; 95% CI, 1.09 to 3.23 for bilateral unreactive pupils); admission source (emergency room versus direct admission [OR 2.2; 95% CI, 1.3 to 3.75]; admission from a general ward versus direct admission [OR 5.85; 95% CI, 2.75 to 12.45; and admission from another ICU versus direct admission [OR 3.34; 95% CI, 1.27 to 8.8]); and the absence of a dedicated neurocritical care unit (NCCU) (OR 1.7; 95% CI, 1.04 to 2.47). CONCLUSION: PRINCE is the first study to evaluate care patterns of neurocritical patients worldwide. The data suggest that there is a wide variability in clinical care resources and patient characteristics. Neurological severity of illness and the absence of a dedicated NCCU are independent predictors of in-patient mortality.

Introduction Neurocritical care focuses on the care of critically ill patients with an acute neurologic disorder and has grown significantly in the past few years. However, there is a lack of data that describe the scope of practice of neurointensivists and epidemiological data on the types of patients and treatments used in neurocritical care units worldwide. To address these issues, we designed a multicenter, international, point-prevalence, cross-sectional, prospective, observational, non-interventional study in the setting of neurocritical care (PRINCE Study). Methods In this manuscript, we analyzed data from the initial phase of the study that included registration, hospital, and intensive care unit (ICU) organizations. We present here descriptive statistics to summarize data from the registration case report form. We performed the Kruskal–Wallis test followed by the Dunn procedure to test for differences in practices among world regions. Results We analyzed information submitted by 257 participating sites from 47 countries. The majority of those sites, 119 (46.3%), were in North America, 44 (17.2%) in Europe, 34 (13.3%) in Asia, 9 (3.5%) in the Middle East, 34 (13.3%) in Latin America, and 14 (5.5%) in Oceania. Most ICUs are from academic institutions (73.4%) located in large urban centers (44% > 1 million inhabitants). We found significant differences in hospital and ICU organization, resource allocation, and use of patient management protocols. The highest nursing/patient ratio was in Oceania (100% 1:1). Dedicated Advanced Practiced Providers are mostly present in North America (73.7%) and are uncommon in Oceania (7.7%) and the Middle East (0%). The presence of dedicated respiratory therapist is common in North America (85%), Middle East (85%), and Latin America (84%) but less common in Europe (26%) and Oceania (7.7%). The presence of dedicated pharmacist is highest in North America (89%) and Oceania (85%) and least common in Latin America (38%). The majority of respondents reported having a dedicated neuro-ICU (67% overall; highest in North America: 82%; and lowest in Oceania: 14%). Conclusion The PRINCE Study results suggest that there is significant variability in the delivery of neurocritical care. The study also shows it is feasible to undertake international collaborations to gather global data about the practice of neurocritical care. Monitoring of magnetic and electric fields in high voltage transmission lines (HVTL) of power plants and substations (SEs) is contemplated by Brazilian Regulatory Standard 616/2014. The measurement procedures of the magnetic and electric fields in SEs must follow a methodology in case of continuous field monitoring of the power equipment where it is common lightning stroke incidences. These monitoring procedures must be carried out in such a way that the recording of electromagnetic fields is done without necessity of exposuring the technical team to irradiation from the equipment and machines generating electrical power. This paper describes a prototype which is able to produce data at a specified safe distance from the irradiating area. The experimental recorded data was acquired, processed, compared and analyzed in areas of more intense radiation levels, acceptable levels and safe levels. This work aimed to establish basis of a technological innovation for the continuous recording of electromagnetic data, trying to cover the surroundings of transmission lines in urban environments, close to the generating units and substation installations wherever the highest levels of magnetic field could be found.

This paper deals with the implementation of the Bachelor's Degree in Electrical Engineering in Western Amazonia, as well as it presents an analysis of the academic performance of students in the Basic and Professional Core. The Federal University of Rondônia Foundation (UNIR), located in Porto Velho, Brazil, started the activities of the course in 2007 and continues to invest in the infrastructure and qualification of human resources in order to stimulate technological development in the region. Analyzing the performance data of the academics regarding the degree of difficulty established from the curricular offer, it is possible to infer some answers. In the course of the analysis, the databases emerged by taking note, such as students who only enroll and do not attend the course, among others. The disciplines diverged into two segments, Basic Core (NB) and Professional Core (NP). The data refers to the grades obtained by the students in all the disciplines studied and offered by the Academic Electrical Engineering Department (DAEE) in the period from 2007/1 to 2016/2. Along the analysis, two graphs demonstrated the evaluation. The analysis of the academic performance of students in the subjects during the NB and NP reinforces the result obtained in the Program for International Student Assessment (PISA), since Brazil has not obtained progress in science, reading, and mathematics averages. The results obtained indicate the necessity of effective measures in primary and secondary education since the deficit propagated to superior levels.

A nitrosamine photooxidation reaction is shown to generate a peroxy intermediate by experimental physical-organic methods. The irradiation of phenyl and methyl-substituted nitrosamines in the presence of isotopically labeled 18-oxygen revealed that an O atom was trapped from a peroxy intermediate to trimethylphosphite or triphenylphosphine, or by nitrosamine itself, forming two moles of nitramine. The unstable peroxy intermediate can be trapped at low temperature in postphotolyzed solution in the dark. Chemiluminescence was also observed upon thermal decomposition of the peroxy intermediate, that is, when a postphotolysis low-temperature solution is brought up to room temperature. A DFT study provides tentative information for cyclic nitrogen peroxide species on the reaction surface.

Nitric oxide (NOx) availability in biological systems is associated with either favorable or unfavorable outcomes. In this sense, several studies bring about evidence that unbalanced NOx production may be underlying to the pathophysiology of vascular disorders. Our study investigated the possible association of clinical, biochemical and inflammatory variables with total circulating levels of NOx in elderly patients devoid of major inflammatory conditions. Clinical (demographics, lifestyle, anthropometry, pressoric traits) and biochemical characteristics (lipemic, glycemic and hormonal profiles) were assessed from 168 geriatrics outpatients eligible for primary care for age-related disorders. Furthermore, circulating levels of 10 inflammatory mediators and of NOx were measured. Correlation tests analyzed categorical or continuous traits according to serum NOx and found no association between NOx and any of the clinical or laboratory data but a negative correlation between plasma NOx concentrations and levels of the immune mediator IL17a (r = -0.236; P = 0.004). Evidence for a correlation between circulating NOx and IL17 is already present in the literature, mostly from studies conducted under inflammatory conditions. Our hypothesis is that such negative correlation can be attributed to an endogenous homeostatic system that IL17 production by the constitutively produced NOx from the vascular endothelium.

Triathletes typically wear a wetsuit during the swim portion of an event, but it is not clear if muscle activity is influenced by wearing a wetsuit. The purpose of this study was to investigate if shoulder muscle activity was influenced by wearing a full sleeved wetsuit vs. no wetsuit during simulated dryland swimming. Subjects (n=10 males; 179.07±13.18cm; 91.20±7.25 kg; 45.6±10.52 years) completed two dry land simulated swimming conditions (VASA Ergometer Trainer): No Wetsuit (NWS) and with Wetsuit (WS). Electromyography (EMG) of four upper extremity muscles was recorded (Noraxon telemetry EMG, sample rate = 1000 Hz) for each condition: Trapezius (TRAP), Triceps (TRI), Anterior Deltoid (AD) and Posterior Deltoid (PD). Each condition lasted 90 seconds with data collected during the last 60 seconds. Resistance setting was self-selected and remained constant for both conditions. Stroke rate was controlled at 60 strokes per minute pace controlled by metronome. Average (AVG) and Root Mean Square (RMS) EMG were calculated over 45 seconds and each were compared between conditions using a paired t-test (_=0.05). PD and AD AVG EMG and RMS EMG were each greater during WS vs. NWS (p0.05). The greater PD and AD muscle activity while wearing a wetsuit might affect swimming performance and /or stroke technique on long distance event.

Objetivo: Evaluar el efecto de la inflamación en yeyuno inducida por LPS de E. coli sobre la expresión molecular de PβD-2 en lechones posdestete. Materiales y métodos: el trabajo de campo se realizó en el Centro de Producción San Pablo de la Universidad Nacional de Colombia, sede Medellín. El estudio se realizó con 32 cerdos destetados a los 21 días de edad, los cuales fueron alimentados durante 10 días con una dieta basal que tuvo como componentes leche y algunos de sus derivados, y que además cumplía con todos los mínimos nutricionales. Los cerdos fueron sacrificados escalonadamente los días 1, 5, 7 y 10 posdestete y se les extrajo completamente el yeyuno. Para inducir la inflamación intestinal los animales fueron alimentados con la dieta basal adicionada con 0.3 µg de LPS/mg de alimento. Los cerdos fueron sacrificados escalonadamente los días 1, 5, 7 y 10 posdestete y se les extrajo completamente el yeyuno. Para inducir la inflamación intestinal los animales fueron alimentados con una dieta basal, adicionada con 0.3 µg de LPS/mg de alimento). El diseño estadístico utilizado fue de bloques al azar en arreglo factorial de 2X4. Resultados: Se observó un aumento (P and lt;0.01) en la expresión de PβD-2 en los animales que consumieron la dieta con mayor nivel de inclusión de LPS. Conclusiones: El LPS de E. coli aumenta la expresión molecular de PβD-2 secretada para la defensa intestinal; esto lleva al desequilibrio en la diferenciación celular a nivel intestinal. Este desequilibrio celular podría provocar la presentación de cambios morfológicos intestinales caracterizados por la disminución en la altura de las vellosidades e incremento en la profundidad de las criptas, reducción en la actividad enzimática y absortiva de nutrientes, y finalmente, diarrea.
