Abstract

Context. The recent estimates of the 3D shape of the M/Xe-type triple asteroid system (216) Kleopatra indicated a density of 5 g.cm$^{-3}$. Such a high density implies a high metal content and a low porosity which is not easy to reconcile with its peculiar dumbbell shape. Aims. Given the unprecedented angular resolution of the VLT/SPHERE/ZIMPOL camera, we aim to constrain the mass and the shape of Kleopatra with high accuracy, hence its density. Methods. We combined our new VLT/SPHERE observations of Kleopatra recorded in 2017 and 2018 with archival data, as well as lightcurve, occultation, and delay-Doppler images, to derive its 3D shape model using two different algorithms (ADAM, MPCD). Furthermore, an N-body dynamical model allowed us to retrieve the orbital elements of the two moons as explained in the accompanying paper. Results. The shape of Kleopatra is very close to an equilibrium dumbbell figure with two lobes and a thick neck. Its volume equivalent diameter (118.75$\pm$1.40) km and mass (2.97$\pm$0.32) 10$^{18}$ kg imply a bulk density of (3.38$\pm$0.50) g cm$^{-3}$. Such a low density for a supposedly metal-rich body indicates a substantial porosity within the primary. This porous structure along with its near-equilibrium shape is compatible with a formation scenario including a giant impact followed by reaccumulation. Kleopatra's current rotation period and dumbbell shape imply that it is in a critically rotating state. The low effective gravity along the equator of the body, together with the equatorial orbits of the moons and possibly rubble-pile structure, opens the possibility that the moons formed via mass shedding. Conclusions. Kleopatra is a puzzling multiple system due to the unique characteristics of the primary. It deserves particular attention in the future, with the Extremely Large Telescopes and possibly a dedicated space mission., Comment: Accepted for publication in Astronomy and Astrophysics

Abstract

To interpret adaptive-optics observations of (216) Kleopatra, we need to describe an evolution of multiple moons, orbiting an extremely irregular body and including their mutual interactions. Such orbits are generally non-Keplerian and orbital elements are not constants. Consequently, we use a modified $N$-body integrator, which was significantly extended to include the multipole expansion of the gravitational field up to the order $\ell = 10$. Its convergence was verified against the `brute-force' algorithm. We computed the coefficients $C_{\ell m},S_{\!\ell m}$ for Kleopatra's shape, assuming a~constant bulk density. For solar-system applications, it was also necessary to implement a variable distance and geometry of observations. Our $\chi^2$ metric then accounts for the absolute astrometry, the relative astrometry (2nd moon with respect to 1st), angular velocities, and also silhouettes, constraining the pole orientation. This allowed us to derive the orbital elements of Kleopatra's two moons. Using both archival astrometric data and new VLT/SPHERE observations (ESO LP 199.C-0074), we were able to identify the true periods of the moons, $P_1 = (1.822359\pm0.004156)\,{\rm d}$, $P_2 = (2.745820\pm0.004820)\,{\rm d}$. They orbit very close to the 3:2 mean-motion resonance, but their osculating eccentricities are too small compared to other perturbations (multipole, mutual), so that regular librations of the critical argument are not present. The resulting mass of Kleopatra, $m_1 = (1.49\pm0.16)\cdot10^{-12}\,M_\odot$ or $2.97\cdot10^{18}\,{\rm kg}$, is significantly lower than previously thought. Abstract

Dynamical models of Solar System evolution have suggested that P-/D-type volatile-rich asteroids formed in the outer Solar System and may be genetically related to the Jupiter Trojans, the comets and small KBOs. Indeed, their spectral properties resemble that of anhydrous cometary dust. High-angular-resolution images of P-type asteroid (87) Sylvia with VLT/SPHERE were used to reconstruct its 3D shape, and to study the dynamics of its two satellites. We also model Sylvia's thermal evolution. The shape of Sylvia appears flattened and elongated. We derive a volume-equivalent diameter of 271 +/- 5 km, and a low density of 1378 +/- 45 kg.m-3. The two satellites orbit Sylvia on circular, equatorial orbits. The oblateness of Sylvia should imply a detectable nodal precession which contrasts with the fully-Keplerian dynamics of the satellites. This reveals an inhomogeneous internal structure, suggesting that Sylvia is differentiated. Sylvia's low density and differentiated interior can be explained by partial melting and mass redistribution through water percolation. The outer shell would be composed of material similar to interplanetary dust particles (IDPs) and the core similar to aqueously altered IDPs or carbonaceous chondrite meteorites such as the Tagish Lake meteorite. Numerical simulations of the thermal evolution of Sylvia show that for a body of such size, partial melting was unavoidable due to the decay of long-lived radionuclides. In addition, we show that bodies as small as 130-150 km in diameter should have followed a similar thermal evolution, while smaller objects, such as comets and the KBO Arrokoth, must have remained pristine, in agreement with in situ observations of these bodies. Abstract

Context. The electrostatic potential of a spacecraft, VS, is important for the capabilities of in situ plasma measurements. Rosetta has been found to be negatively charged during most of the comet mission and even more so in denser plasmas. Aims. Our goal is to investigate how the negative VS correlates with electron density and temperature and to understand the physics of the observed correlation. Methods. We applied full mission comparative statistics of VS, electron temperature, and electron density to establish VS dependence on cold and warm plasma density and electron temperature. We also used Spacecraft-Plasma Interaction System (SPIS) simulations and an analytical vacuum model to investigate if positively biased elements covering a fraction of the solar array surface can explain the observed correlations. Results. Here, the VS was found to depend more on electron density, particularly with regard to the cold part of the electrons, and less on electron temperature than was expected for the high flux of thermal (cometary) ionospheric electrons. This behaviour was reproduced by an analytical model which is consistent with numerical simulations. Conclusions. Rosetta is negatively driven mainly by positively biased elements on the borders of the front side of the solar panels as these can efficiently collect cold plasma electrons. Biased elements distributed elsewhere on the front side of the panels are less efficient at collecting electrons apart from locally produced electrons (photoelectrons). To avoid significant charging, future spacecraft may minimise the area of exposed bias conductors or use a positive ground power system., Comment: 12 pages, 17 figures, Accepted for publication in A&A

Abstract

Asteroid (31) Euphrosyne is one of the biggest objects in the asteroid main belt and the Euphrosyne family occupies a highly inclined region in the outer main belt and contains a remarkably large number of members, which is interpreted as an outcome of a disruptive cratering event. The goals of this adaptive-optics imaging study were threefold: to characterize the shape of Euphrosyne, to constrain its density, and to search for the large craters that may be associated with the family formation event. We obtained disk-resolved images of Euphrosyne using SPHERE/ZIMPOL at ESO's 8.2-m VLT as part of our large program (ID: 199.C-0074, PI: Vernazza). We reconstructed its 3D-shape using the adam shape modeling algorithm based on the SPHERE images and the available lightcurves of this asteroid. We analyzed the dynamics of the satellite with the genoid meta-heuristic algorithm. Finally, we studied the shape of Euphrosyne using hydrostatic equilibrium models. Our SPHERE observations show that Euphrosyne has a nearly spherical shape with the sphericity index of 0.9888 and its surface lacks large impact craters. Euphrosyne's diameter is 268+/-6 km, making it one of the top 10 largest main belt asteroids. We detected a satellite of Euphrosyne -- S/2019 (31) 1-- that is about 4 km across, on an circular orbit. The mass determined from the orbit of the satellite together with the volume computed from the shape model imply a density of 1665+/-242 kg/m^3, suggesting that Euphrosyne probably contain a large fraction of water ice in its interior. We find that the spherical shape of Euphrosyne is a result of the reaccumulation process following the impact, as in the case of (10) Hygiea. However, our shape analysis reveals that, contrary to Hygiea, the axis ratios of Euphrosyne significantly differ from the ones suggested by fluid hydrostatic equilibrium following reaccumulation., Comment: 12 pages, 6 figures, accepted to be published in A&A

Abstract

In this work the potential mineral identification of meteorites is analysed for the mid-infrared range, to evaluate observational possibilities for future missions targeting small body surfaces. Three carbonaceous and three ordinary chondrite meteorites are examined by a diffuse reflection (DRIFT) instrument, and the presence of principal minerals is confirmed by a powder diffraction method as well. The possibilities and constraints of mineral identifications in the mid-infrared are simulated by artificially degrading the spectral resolution. Our research shows that for the definite identification of principal mineral bands, a spectral resolution $\leq 10$~cm$^{-1}$ ($\leq 0.15 \mu$m) is needed. At 20-100~cm$^{-1}$ (0.3 - 1.5~$\mu$m) resolution the identification of these minerals is uncertain, and with a resolution $>100$~cm$^{-1}$, it is almost impossible., Comment: Accepted in MNRAS for publication on May 22nd, 2020; 7 pages, 3 figures

Abstract

With an estimated diameter in the 320 to 350 km range, (704) Interamnia is the fifth largest main belt asteroid and one of the few bodies that fills the gap in size between the four largest bodies with $D$ > 400 km (Ceres, Vesta, Pallas and Hygiea) and the numerous smaller bodies with $D$ $\lesssim$ 200 km. However, despite its large size, little is known about the shape and spin state of Interamnia and, therefore, about its bulk composition and past collisional evolution. We aimed to test at what size and mass the shape of a small body departs from a nearly ellipsoidal equilibrium shape (as observed in the case of the four largest asteroids) to an irregular shape as routinely observed in the case of smaller ($D$ $\lesssim$ 200 km) bodies. We observed Interamnia as part of our ESO VLT/SPHERE large program (ID: 199.C-0074) at thirteen different epochs. In addition, several new optical lightcurves were recorded. These data, along with stellar occultation data from the literature, were fed to the All-Data Asteroid Modeling (ADAM) algorithm to reconstruct the 3D-shape model of Interamnia and to determine its spin state. Interamnia's volume-equivalent diameter of 332 $\pm$ 6 km implies a bulk density of $\rho$=1.98 $\pm$ 0.68 gcm$^{-3}$ , which suggests that Interamnia - like Ceres and Hygiea - contains a high fraction of water ice, consistent with the paucity of apparent craters. Our observations reveal a shape that can be well approximated by an ellipsoid, and that is compatible with a fluid hydrostatic equilibrium at the 2 $\sigma$ level. The rather regular shape of Interamnia implies that the size and mass limit, under which the shapes of minor bodies with a high amount of water ice in the subsurface become irregular, has to be searched among smaller ($D$ $\lesssim$ 300km) less massive ($m$ $\lesssim$ 3x10$^{19}$ kg) bodies., Comment: Accepted for publication in Astronomy and Astrophysics

The disease burden includes physical limitations, psychosocial discomfort, and a reduced quality of life (HRQoL). This study presents the results of a parent-reported survey on the psychosocial impact of AD on Italian pre-adolescent children (6– 11 years old), with a specific focus on bullying, self-isolation, absenteeism, and presenteeism.Methods: An online questionnaire was sent to 3067 random recipients and 160 matched the inclusion criteria for age, self-reported AD diagnosis, localizations (according to ISAAC), and disease severity (POEM ≥ 8). 100 children, with comparable ages, not matching the inclusion criteria for AD, were recruited as a control group.Results: Children with AD and their caregivers had a significantly lower quality of sleep (QoS) compared to the control group. The presence of AD was directly responsible for many restless nights, both in children and caregivers (58.9 and 55.4 respectively). Children with AD and their parents also experienced significantly more daytime drowsiness (43.6 and 54.6 days, respectively). Children with AD were more frequently victims of bullying at school (20.0% vs 9.0%; p≤ 0.05) or in other social environments (16.9% vs 3.0%; p≤ 0.05). AD caused 17.7 days of absenteeism and 20.1 days of presenteeism per student over the previous 12 months, accounting for 37.8 days of study impairment overall. Severe/very severe AD had a significantly greater impact on presenteeism than moderate AD (25.1 vs 17.5 days; p≤ 0.05). Presenteeism, which was more pronounced among bullied students, was positively correlated with absenteeism only in the AD cohort.Conclusion: AD has a detrimental impact on the HRQoL of pediatric patients, causing stigmatization and social isolation. Functional distress was also reported by caregivers. Abstract

Asteroid (7) Iris is an ideal target for disk-resolved imaging owing to its brightness (V$\sim$7-8) and large angular size of 0.33 arcsec during its apparitions. Iris is believed to belong to the category of large unfragmented asteroids that avoided internal differentiation, implying that its current shape and topography may record the first few 100 Myr of the solar system's collisional evolution. We recovered information about the shape and surface topography of Iris from disk-resolved VLT/SPHERE/ZIMPOL images acquired in the frame of our ESO large program. We used the All-Data Asteroid Modeling (ADAM) shape reconstruction algorithm to model the 3D shape of Iris, using optical disk-integrated data and disk-resolved images from SPHERE as inputs. We analyzed the SPHERE images to infer the asteroid's global shape and the morphology of its main craters. We present the volume-equivalent diameter D$_{{\rm eq}}$=214$\pm$5 km, and bulk density $\rho$=2.7$\pm$0.3 g cm$^{-3}$ of Iris. Its shape appears to be consistent with that of an oblate spheroid with a large equatorial excavation. We identified eight putative surface features 20--40 km in diameter detected at several epochs, which we interpret as impact craters. Craters on Iris have depth-to-diameter ratios that are similar to those of analogous 10 km craters on Vesta. The bulk density of Iris is consistent with that of its meteoritic analog, namely LL ordinary chondrites. Considering the absence of a collisional family related to Iris and the number of large craters on its surface, we suggest that its equatorial depression may be the remnant of an ancient (at least 3 Gyr) impact. Iris's shape further opens the possibility that large planetesimals formed as almost perfect oblate spheroids. Abstract

Context. Over the past decades, several interplanetary missions have studied small bodies in situ, leading to major advances in our understanding of their geological and geophysical properties. These missions, however, have had a limited number of targets. Among them, the NASA Dawn mission has characterised in detail the topography and albedo variegation across the surface of asteroid (4) Vesta down to a spatial resolution of approximately 20 m/pixel scale. Aims. Here our aim was to determine how much topographic and albedo information can be retrieved from the ground with VLT/SPHERE in the case of Vesta, having a former space mission (Dawn) providing us with the ground truth that can be used as a benchmark. Methods. We observed Vesta with VLT/SPHERE/ZIMPOL as part of our ESO large programme (ID 199.C-0074) at six different epochs, and deconvolved the collected images with a parametric point spread function (PSF). We then compared our images with synthetic views of Vesta generated from the 3D shape model of the Dawn mission, on which we projected Vesta's albedo information. Results. We show that the deconvolution of the VLT/SPHERE images with a parametric PSF allows the retrieval of the main topographic and albedo features present across the surface of Vesta down to a spatial resolution of approximately 20-30 km. Contour extraction shows an accuracy of approximately 1 pixel (3.6 mas). The present study provides the very first quantitative estimate of the accuracy of ground-based adaptive-optics imaging observations of asteroid surfaces. Conclusions. In the case of Vesta, the upcoming generation of 30-40m telescopes (ELT, TMT, GMT) should in principle be able to resolve all of the main features present across its surface, including the troughs and the north-south crater dichotomy, provided that they operate at the diffraction limit.

Abstract

Context. CM-like asteroids (Ch and Cgh classes) are a major population within the broader C-complex, encompassing about 10% of the mass of the main asteroid belt. Their internal structure has been predicted to be homogeneous, based on their compositional similarity as inferred from spectroscopy (Vernazza et al., 2016, AJ 152, 154) and numerical modeling of their early thermal evolution (Bland & Travis, 2017, Sci. Adv. 3, e1602514). Aims. Here we aim to test this hypothesis by deriving the density of the CM-like asteroid (41) Daphne from detailed modeling of its shape and the orbit of its small satellite. Methods. We observed Daphne and its satellite within our imaging survey with the Very Large Telescope extreme adaptive-optics SPHERE/ZIMPOL camera (ID 199.C-0074, PI P. Vernazza) and complemented this data set with earlier Keck/NIRC2 and VLT/NACO observations. We analyzed the dynamics of the satellite with our Genoid meta-heuristic algorithm. Combining our high-angular resolution images with optical lightcurves and stellar occultations, we determine the spin period, orientation, and 3-D shape, using our ADAM shape modeling algorithm. Results. The satellite orbits Daphne on an equatorial, quasi-circular, prograde orbit, like the satellites of many other large main-belt asteroids. The shape model of Daphne reveals several large flat areas that could be large impact craters. The mass determined from this orbit combined with the volume computed from the shape model implies a density for Daphne of 1.77+/-0.26 g/cm3 (3 {\sigma}). This density is consistent with a primordial CM-like homogeneous internal structure with some level of macroporosity (~17%). Conclusions. Based on our analysis of the density of Daphne and 75 other Ch/Cgh-type asteroids gathered from the literature, we conclude that the primordial internal structure of the CM parent bodies was homogeneous.

Abstract

Asteroid (16) Psyche is the target of the NASA Psyche mission. It is considered one of the few main-belt bodies that could be an exposed proto-planetary metallic core and that would thus be related to iron meteorites. Such an association is however challenged by both its near- and mid-infrared spectral properties and the reported estimates of its density. Here, we aim to refine the density of (16) Psyche to set further constraints on its bulk composition and determine its potential meteoritic analog. We observed (16) Psyche with ESO VLT/SPHERE/ZIMPOL as part of our large program (ID 199.C-0074). We used the high angular resolution of these observations to refine Psyche's three-dimensional (3D) shape model and subsequently its density when combined with the most recent mass estimates. In addition, we searched for potential companions around the asteroid. We derived a bulk density of 3.99\,$\pm$\,0.26\,g$\cdot$cm$^{-3}$ for Psyche. While such density is incompatible at the 3-sigma level with any iron meteorites ($\sim$7.8\,g$\cdot$cm$^{-3}$), it appears fully consistent with that of stony-iron meteorites such as mesosiderites (density $\sim$4.25\,$\cdot$cm$^{-3}$). In addition, we found no satellite in our images and set an upper limit on the diameter of any non-detected satellite of 1460\,$\pm$\,200}\,m at 150\,km from Psyche (0.2\%\,$\times$\,R$_{Hill}$, the Hill radius) and 800\,$\pm$\,200\,m at 2,000\,km (3\%\,$\times$\,$R_{Hill}$). Considering that the visible and near-infrared spectral properties of mesosiderites are similar to those of Psyche, there is merit to a long-published initial hypothesis that Psyche could be a plausible candidate parent body for mesosiderites., Comment: 16 pages

Abstract

Purpose: Single large hepatocellular carcinoma >5cm (SLHCC) traditionally requires a major liver resection. Minor resections are often performed with the goal to reduce morbidity and mortality. Aim of the study was to establish if a major resection should be considered the best treatment for SLHCC or a more limited resection should be preferred. Methods: A multicenter retrospective analysis of the HE.RC.O.LE.S. Group register was performed. All collected patients with surgically treated SLHCC were divided in 5 groups of treatment (major hepatectomy, sectorectomy, left lateral sectionectomy, segmentectomy, non-anatomical resection) and compared for baseline characteristics, short and long-term results. A propensity-score weighted analysis was performed. Results: 535 patients were enrolled in the study. Major resection was associated with significantly increased major complications compared to left lateral sectionanectomy, segmentectomy and non-anatomical resection (all p<0.05) and borderline significant increased major complications compared to sectorectomy (p=0.08). Left lateral sectionectomy showed better overall survival compared to major resection (p=0.02), while other groups of treatment resulted similar to major hepatectomy group for the same item. Absence of oncological benefit after major resection and similar outcomes among the 5 groups of treatment was confirmed even in the sub-population excluding patients with macrovascular invasion. Conclusion: Major resection was associated to increased major post-operative morbidity without long-term survival benefit; when technically feasible and oncologically adequate, minor resections should be preferred for the surgical treatment of SLHCC.

We describe disease social stigmatization by measuring bullying and self-isolation in students and professional discrimination in workers. Overall loss of productivity, either at school and at the workplace, was quantified as the sum of absenteeism (number of days AD sick leave) and presenteeism (number of days with decreased focus and functionality).Methods: An on-line web survey was sent to 3235 random recipients and 401 met the inclusion criteria (self-reporting AD and ≥ 12 yo). The survey domains included daily limitations, QoL, feelings and relationships, together with specific questions about bullying, discrimination and loss of productivity.Results: AD negatively affected QoL in 51.6% of respondents, whereas 68.8% considered AD as a real limit to daily routine. More in detail, 39.3% of students were victims of bullying and 33.9% of workers felt discriminated because of AD. On average, absenteeism in students was for 17.1 days/year (presenteeism: 19.5 days/year), whereas in workers, the estimate was 10.9 days/year (presenteeism: 13.1 days/year). Absenteeism and presenteeism were more pronounced in bullied/discriminated subjects.Conclusion: AD multidimensional implications deeply affect and undermine personal and professional fulfillments. Abstract

This article presents the first study of the interaction between the Jupiter Icy Moons Explorer (JUICE) spacecraft and the solar wind environment at 1 AU. The state‐of‐the‐art software Spacecraft Plasma Interaction Software was used to simulate the surface charging of the spacecraft and the altered particle environment around the spacecraft. The simulations show that for a typical solar wind environment the spacecraft will charge to around 6 V, with the different dielectric parts of the spacecraft charging to potentials from around −36 to 8 V. For the studied extreme solar wind environment, similar to the environment found in the sheath region inside the shock front of an Interplanetary Coronal Mass Ejection, the surface potential of the spacecraft is lower due to the increased accumulation of electrons. The spacecraft will charge to around 3 V, with the different dielectric surfaces charging from around −45 to 9 V. We also show how the interaction between the spacecraft and its environment alters the ion and electron particle environment around the spacecraft. This study is the first step toward developing correction techniques for the impact that the interaction between the JUICE spacecraft and its environment has on the JUICE charged particle and field measurements. We construct a family of spectral triples for the Sierpinski Gasket $K$. For suitable values of the parameters, we determine the dimensional spectrum and recover the Hausdorff measure of $K$ in terms of the residue of the volume functional $a\to$ tr$(a\,|D|^{-s})$ at its abscissa of convergence $d_D$, which coincides with the Hausdorff dimension $d_H$ of the fractal. We determine the associated Connes' distance showing that it is bi-Lipschitz equivalent to the distance on $K$ induced by the Euclidean metric of the plane, and show that the pairing of the associated Fredholm module with (odd) $K$-theory is non-trivial. Abstract

Objective: To evaluate the effect of a liver transplantation (LT) program on the outcomes of resectable hepatocellular carcinoma (HCC). Background: Surgical treatment of HCC includes both hepatic resection (HR) and LT. However, the presence of cirrhosis and the possibility of recurrence make the management of this disease complex and probably different according to the presence of a LT program. Methods: Patients undergoing HR for HCC between January 2005 and December 2019 were identified from a national database of HCC. The main study outcomes were major surgical complications according to the Comprehensive Complication Index, posthepatectomy liver failure (PHLF), 90-day mortality, overall survival, and disease-free survival. Secondary outcomes were salvage liver transplantation (SLT) and postrecurrence survival. Results: A total of 3202 patients were included from 25 hospitals over the study period. Three of 25 (12%) had an LT program. The presence of an LT program within a center was associated with a reduced probability of PHLF (odds ratio=0.38) but not with overall survival and disease-free survival. There was an increased probability of SLT when HR was performed in a transplant hospital (odds ratio=12.05). Among transplant-eligible patients, those who underwent LT had a significantly longer postrecurrence survival. Conclusions: This study showed that the presence of a LT program was associated with decreased PHLF rates and an increased probability to receive SLT in case of recurrence.

Abstract

Background: Contemporary management of patients with synchronous colorectal cancer and liver metastases is complex. The aim of this project was to provide a practical framework for care of patients with synchronous colorectal cancer and liver metastases, with a focus on terminology, diagnosis, and management. Methods: This project was a multiorganizational, multidisciplinary consensus. The consensus group produced statements which focused on terminology, diagnosis, and management. Statements were refined during an online Delphi process, and those with 70 per cent agreement or above were reviewed at a final meeting. Iterations of the report were shared by electronic mail to arrive at a final agreed document comprising 12 key statements. Results: Synchronous liver metastases are those detected at the time of presentation of the primary tumour. The term 'early metachronous metastases' applies to those absent at presentation but detected within 12 months of diagnosis of the primary tumour, the term 'late metachronous metastases' applies to those detected after 12 months. 'Disappearing metastases' applies to lesions that are no longer detectable on MRI after systemic chemotherapy. Guidance was provided on the recommended composition of tumour boards, and clinical assessment in emergency and elective settings. The consensus focused on treatment pathways, including systemic chemotherapy, synchronous surgery, and the staged approach with either colorectal or liver-directed surgery as first step. Management of pulmonary metastases and the role of minimally invasive surgery was discussed. Conclusion: The recommendations of this contemporary consensus provide information of practical value to clinicians managing patients with synchronous colorectal cancer and liver metastases.

Abstract

Background: Despite second-line transplant(SLT) for recurrent hepatocellular carcinoma(rHCC) leads to the longest survival after recurrence(SAR), its real applicability has never been reported. The aim was to compare the SAR of SLT versus repeated hepatectomy and thermoablation(CUR group). Methods: Patients were enrolled from the Italian register HE.RC.O.LE.S. between 2008 and 2021. Two groups were created: CUR versus SLT. A propensity score matching (PSM) was run to balance the groups. Results: 743 patients were enrolled, CUR = 611 and SLT = 132. Median age at recurrence was 71(IQR 6575) years old and 60(IQR 53-64, p < 0.001) for CUR and SLT respectively. After PSM, median SAR for CUR was 43 months(95%CI = 37 – 93) and not reached for SLT(p < 0.001). SLT patients gained a survival benefit of 9.4 months if compared with CUR. MilanCriteria(MC)-In patients were 82.7% of the CUR group. SLT(HR 0.386, 95%CI = 0.23 – 0.63, p < 0.001) and the MELD score(HR 1.169, 95%CI = 1.07 – 1.27, p < 0.001) were the only predictors of mortality. In case of MC-Out, the only predictor of mortality was the number of nodules at recurrence(HR 1.45, 95%CI= 1.09 – 1.93, p = 0.011). Conclusion: It emerged an important transplant under referral in favour of repeated hepatectomy or thermoablation. In patients with MC-Out relapse, the benefit of SLT over CUR was not observed.

Abstract

Prion diseases, also known as Transmissible Spongiform Encephalopathies (TSEs), are fatal conditions which affect humans and animals. The hallmark of Transmissible Spongiform Encephalopathies is the accumulation in the brain of PrPSc, which is an abnormal isoform of the cellular protein PrPc. It has been proposed that PrPSc is able to impose its own conformation on PrP , but the molecular mechanism by which PrP is converted into PrPSc is unclear. PrPc and PrPSc share the same primary sequence but they are different in their biochemical properties. PrP is rich in alpha helices, detergent soluble and Protease K sensitive on the other hand, PrPSc is mainly composed of beta sheets, detergent insoluble and Protease K resistant. To better understand PrPSc generation, a genetic screen to identify proteins that preferentially interact with a misfolded version of PrP has been employed. This novel protein-protein interaction system takes place in the yeast cytoplasm where PrP has been reported to adopt a beta sheet-rich conformation characterised by increased Protease K resistance. Initial analysis revealed that one candidate molecule, PrP Interacting Protein 7 (PIP7), encoded a transmembrane protein present on the cell surface, suggesting a potential function as a novel PrP receptor. The interaction between PIP7 and PrP has been confirmed in vitro by GST pull down experiments and in vivo by co- immunoprecipitation studies. A panel of monoclonal antibodies directed against the C- terminus of PIP7, which contains the PrP binding region, has been generated. FACS analysis using these antibodies confirmed the presence of PIP7 on the cell surface of N2a cells, commonly used to study PrPSc propagation, and PIP7 immunohistochemistry has revealed an intense staining in all brain regions including the cortex, hippocampus and cerebellum. Overexpression of PrP in N2a cells resulted in an accumulation of PIP7 surface levels. Furthermore, PIP7 levels appear to be reduced in the brains of PrP knockout mice. The direct correlation between the levels of PIP7 and PrP suggests a potential role for PIP7 as a regulator in prion biology.

Abstract

Background: We aimed to evaluate, in a large Western cohort, perioperative and long-term oncological outcomes of salvage hepatectomy (SH) for recurrent hepatocellular carcinoma (rHCC) after primary hepatectomy (PH) or locoregional treatments. Methods: Data were collected from the Hepatocarcinoma Recurrence on the Liver Study Group (He.RC.O.Le.S.) Italian Registry. After 1:1 propensity score-matched analysis (PSM), two groups were compared: the PH group (patients submitted to resection for a first HCC) and the SH group (patients resected for intrahepatic rHCC after previous HCC-related treatments). Results: 2689 patients were enrolled. PH included 2339 patients, SH 350. After PSM, 263 patients were selected in each group with major resected nodule median size, intraoperative blood loss and minimally invasive approach significantly lower in the SH group. Long-term outcomes were compared, with no difference in OS and DFS. Univariate and multivariate analyses revealed only microvascular invasion as an independent prognostic factor for OS. Conclusion: SH proved to be equivalent to PH in terms of safety, feasibility and long-term outcomes, consistent with data gathered from East Asia. In the awaiting of reliable treatment-allocating algorithms for rHCC, SH appears to be a suitable alternative in patients fit for surgery, regardless of the previous therapeutic modality implemented.

Abstract

Background and aims: We investigated the clinical impact of the newly defined metabolic-associated fatty liver disease (MAFLD) in patients undergoing hepatectomy for HCC (MAFLD-HCC) comparing the characteristics and outcomes of patients with MAFLD-HCC to viral- and alcoholic-related HCC (HCV-HCC, HBV-HCC, A-HCC). Methods: A retrospective analysis of patients included in the He.RC.O.Le.S. Group registry was performed. The characteristics, short- and long-term outcomes of 1315 patients included were compared according to the study group before and after an exact propensity score match (PSM). Results: Among the whole study population, 264 (20.1%) had MAFLD-HCC, 205 (15.6%) had HBV-HCC, 671 (51.0%) had HCV-HCC and 175 (13.3%) had A-HCC. MAFLD-HCC patients had higher BMI (p < 0.001), Charlson Comorbidities Index (p < 0.001), size of tumour (p < 0.001), and presence of cirrhosis (p < 0.001). After PSM, the 90-day mortality and severe morbidity rates were 5.9% and 7.1% in MAFLD-HCC, 2.3% and 7.1% in HBV-HCC, 3.5% and 11.7% in HCV-HCC, and 1.2% and 8.2% in A-HCC (p = 0.061 and p = 0.447, respectively). The 5-year OS and RFS rates were 54.4% and 37.1% in MAFLD-HCC, 64.9% and 32.2% in HBV-HCC, 53.4% and 24.7% in HCV-HCC and 62.0% and 37.8% in A-HCC (p = 0.345 and p = 0.389, respectively). Cirrhosis, multiple tumours, size and satellitosis seems to be the independent predictors of OS. Conclusion: Hepatectomy for MAFLD-HCC seems to have a higher but acceptable operative risk. However, long-term outcomes seems to be related to clinical and pathological factors rather than aetiological risk factors.

Abstract

Background: The benefits of pure laparoscopic and robot-assisted liver resections (LLR and RALR) are known in comparison to open surgery. The aim of the present retrospective comparative study is to investigate the role of RALR and LLR according to different levels of difficulty. Methods: The institutional databases of six high-volume hepatobiliary centers were retrospectively reviewed. The study population was divided in two groups: LLR and RALR. The procedures were stratified for difficulty levels accordingly to three classifications. A propensity score matching was implemented to mitigate selection bias. Short-term outcomes were the object of comparison. Results: Nine hundred and thirty-six LLR and 403 RALR were collected. RALR exhibited fewer cases of intraoperative blood loss, lower transfusion and conversion rates (especially for oncological radicality) than LLR in the setting of highly difficult operations, whereas LLR had lower postoperative morbidity and fewer low-grade complications. For intermediate and low-difficulty resections, the intraoperative advantages of RALR gradually decreased to nonsignificant results and LLR remained associated with lower postoperative morbidity. Conclusion: Robot-assisted liver resections do not show operative nor clinically significant benefits over LLR for low- and intermediate-difficulty resections. By reducing conversion rates, RALR can favour the operative feasibility of difficult resections possibly extending the indications of minimally invasive approaches for liver resection.

Abstract

Importance: Clear indications on how to select retreatments for recurrent hepatocellular carcinoma (HCC) are still lacking. Objective: To create a machine learning predictive model of survival after HCC recurrence to allocate patients to their best potential treatment. Design, Setting, and Participants: Real-life data were obtained from an Italian registry of hepatocellular carcinoma between January 2008 and December 2019 after a median (IQR) follow-up of 27 (12-51) months. External validation was made on data derived by another Italian cohort and a Japanese cohort. Patients who experienced a recurrent HCC after a first surgical approach were included. Patients were profiled, and factors predicting survival after recurrence under different treatments that acted also as treatment effect modifiers were assessed. The model was then fitted individually to identify the best potential treatment. Analysis took place between January and April 2021. Exposures: Patients were enrolled if treated by reoperative hepatectomy or thermoablation, chemoembolization, or sorafenib. Main Outcomes and Measures: Survival after recurrence was the end point. Results: A total of 701 patients with recurrent HCC were enrolled (mean [SD] age, 71 [9] years; 151 [21.5%] female). Of those, 293 patients (41.8%) received reoperative hepatectomy or thermoablation, 188 (26.8%) received sorafenib, and 220 (31.4%) received chemoembolization. Treatment, age, cirrhosis, number, size, and lobar localization of the recurrent nodules, extrahepatic spread, and time to recurrence were all treatment effect modifiers and survival after recurrence predictors. The area under the receiver operating characteristic curve of the predictive model was 78.5% (95% CI, 71.7%-85.3%) at 5 years after recurrence. Abstract

Objective: To evaluate the effect of a liver transplantation (LT) program on the outcomes of resectable hepatocellular carcinoma (HCC). Background: Surgical treatment of HCC includes both hepatic resection (HR) and LT. However, the presence of cirrhosis and the possibility of recurrence make the management of this disease complex and probably different according to the presence of a LT program. Methods: Patients undergoing HR for HCC between January 2005 and December 2019 were identified from a national database of HCC. The main study outcomes were major surgical complications according to the Comprehensive Complication Index, posthepatectomy liver failure (PHLF), 90-day mortality, overall survival, and disease-free survival. Secondary outcomes were salvage liver transplantation (SLT) and postrecurrence survival. Results: A total of 3202 patients were included from 25 hospitals over the study period. Three of 25 (12%) had an LT program. The presence of an LT program within a center was associated with a reduced probability of PHLF (odds ratio=0.38) but not with overall survival and disease-free survival. There was an increased probability of SLT when HR was performed in a transplant hospital (odds ratio=12.05). Among transplant-eligible patients, those who underwent LT had a significantly longer postrecurrence survival. Conclusions: This study showed that the presence of a LT program was associated with decreased PHLF rates and an increased probability to receive SLT in case of recurrence.

Abstract

Background: Minimal invasive liver resections are a safe alternative to open surgery. Different scoring systems considering different risks factors have been developed to predict the risks associated with these procedures, especially challenging major liver resections (MLR). However, the impact of neoadjuvant chemotherapy (NAT) on the difficulty of minimally invasive MLRs remains poorly investigated. Methods: Patients who underwent laparoscopic and robotic MLRs for colorectal liver metastases (CRLM) performed across 57 centers between January 2005 to December 2021 were included in this analysis. Patients who did or did not receive NAT were matched based on 1:1 coarsened exact and 1:2 propensity-score matching. Pre- and post-matching comparisons were performed. Results: In total, the data of 5189 patients were reviewed. Of these, 1411 procedures were performed for CRLM, and 1061 cases met the inclusion criteria. After excluding 27 cases with missing data on NAT, 1034 patients (NAT: n = 641; non-NAT: n = 393) were included. Before matching, baseline characteristics were vastly different. Before matching, the morbidity rate was significantly higher in the NAT-group (33.2% vs. 27.2%, p-value = 0.043). No significant differences were seen in perioperative outcomes after the coarsened exact matching. After the propensity-score matching, statistically significant higher blood loss (mean, 300 (SD 128–596) vs. 250 (SD 100–400) ml, p-value = 0.047) but shorter hospital stay (mean, 6 [4-8] vs. 6 [5-9] days, p-value = 0.043) were found in the NAT-group. Conclusion: The current study demonstrated that NAT had minimal impact on the difficulty and outcomes of minimally-invasive MLR for CRLM.

Abstract

Objective: To establish global benchmark outcomes indicators after laparoscopic liver resections (L-LR). Background: There is limited published data to date on the best achievable outcomes after L-LR. Methods: This is a post hoc analysis of a multicenter database of 11,983 patients undergoing L-LR in 45 international centers in 4 continents between 2015 and 2020. Three specific procedures: left lateral sectionectomy (LLS), left hepatectomy (LH), and right hepatectomy (RH) were selected to represent the 3 difficulty levels of L-LR. Fifteen outcome indicators were selected to establish benchmark cutoffs. Results: There were 3519 L-LR (LLS, LH, RH) of which 1258 L-LR (40.6%) cases performed in 34 benchmark expert centers qualified as low-risk benchmark cases. These included 659 LLS (52.4%), 306 LH (24.3%), and 293 RH (23.3%). The benchmark outcomes established for operation time, open conversion rate, blood loss ≥500 mL, blood transfusion rate, postoperative morbidity, major morbidity, and 90-day mortality after LLS, LH, and RH were 209.5, 302, and 426 minutes; 2.1%, 13.4%, and 13.0%; 3.2%, 20%, and 47.1%; 0%, 7.1%, and 10.5%; 11.1%, 20%, and 50%; 0%, 7.1%, and 20%; and 0%, 0%, and 0%, respectively. Conclusions: This study established the first global benchmark outcomes for L-LR in a large-scale international patient cohort. It provides an up-to-date reference regarding the "best achievable" results for L-LR for which centers adopting L-LR can use as a comparison to enable an objective assessment of performance gaps and learning curves.

Abstract

Importance: Clear indications on how to select retreatments for recurrent hepatocellular carcinoma (HCC) are still lacking. Objective: To create a machine learning predictive model of survival after HCC recurrence to allocate patients to their best potential treatment. Design, Setting, and Participants: Real-life data were obtained from an Italian registry of hepatocellular carcinoma between January 2008 and December 2019 after a median (IQR) follow-up of 27 (12-51) months. External validation was made on data derived by another Italian cohort and a Japanese cohort. Patients who experienced a recurrent HCC after a first surgical approach were included. Patients were profiled, and factors predicting survival after recurrence under different treatments that acted also as treatment effect modifiers were assessed. The model was then fitted individually to identify the best potential treatment. Analysis took place between January and April 2021. Exposures: Patients were enrolled if treated by reoperative hepatectomy or thermoablation, chemoembolization, or sorafenib. Main Outcomes and Measures: Survival after recurrence was the end point. Results: A total of 701 patients with recurrent HCC were enrolled (mean [SD] age, 71 [9] years; 151 [21.5%] female). Of those, 293 patients (41.8%) received reoperative hepatectomy or thermoablation, 188 (26.8%) received sorafenib, and 220 (31.4%) received chemoembolization. Treatment, age, cirrhosis, number, size, and lobar localization of the recurrent nodules, extrahepatic spread, and time to recurrence were all treatment effect modifiers and survival after recurrence predictors. The area under the receiver operating characteristic curve of the predictive model was 78.5% (95% CI, 71.7%-85.3%) at 5 years after recurrence. Abstract

Background: Presently, according to different difficulty scoring systems, there is no difference in complexity estimation of laparoscopic liver resection (LLR) of segments 7 and 8. However, there is no published data supporting this assumption. To date, no studies have compared the outcomes of laparoscopic parenchyma-sparing resection of the liver segments 7 and 8. Methods: A post hoc analysis of patients undergoing LLR of segments 7 and 8 in 46 centers between 2004 and 2020 was performed. 1:1 Propensity score matching (PSM) was used to compare isolated LLR of segments 7 and 8. Subset analyses were also performed to compare atypical resections and segmentectomies of 7 and 8. Results: A total of 2411 patients were identified, and 1691 patients met the inclusion criteria. Comparison after PSM between the entire cohort of segment 7 and segment 8 resections revealed inferior results for segment 7 resection in terms of increased blood loss, blood transfusions, and conversions to open surgery. Subset analyses of only atypical resections similarly demonstrated poorer outcomes for segment 7 in terms of increased blood loss, operation time, blood transfusions, and conversions to open surgery. Conversely, a subgroup analysis of segmentectomies after PSM found better outcomes for segment 7 in terms of a shorter operation time and hospital stay. Conclusion: Differences in the outcomes of segments 7 and 8 resections suggest a greater difficulty of laparoscopic atypical resection of segment 7 compared to segment 8, and greater difficulty of segmentectomy 8 compared to segmentectomy 7.
