Summary Introduction: To investigate the sociodemographic and morbidity profile of advanced dementia patients and sociodemographic data of their primary caregivers. Method: Data was obtained from 67 elderly recruited for an observational prospective study, through interviews performed with primary caregivers. For statistical analysis, the Statistical Package for the Social Sciences (SPSS(r)) for Windowsr, version 19.0. was employed. Results: Advanced dementia patients were mostly women, widows, and non-institutionalized, with low-income. An elevated rate of stroke, sarcopenia, and pressure ulcers in this population is noted. Caregivers were mostly women, married, children of the studied patients, and unemployed. Only one third of caregivers were hired for the task. Conclusion: Patients with advanced dementia present a high morbidity profile, low income, and depend on the care given by family members, mostly unemployed daughters. Resumo Objetivo: investigar o perfil sociodemográfico e de morbidade de pacientes com demência avançada e dados sociodemográficos de seus respectivos cuidadores primários. Método: foram obtidos dados de 67 idosos recrutados para um estudo observacional, prospectivo, não randomizado e não cego de pacientes em fase avançada de demência, por meio de entrevistas realizadas com os cuidadores primários. Para as análises estatísticas, foi utilizado o Statistical Package for the Social Sciences (SPSSr) para Windowsr, na versão 19.0. Resultados: a maioria dos pacientes com demência avançada é do sexo feminino, viúva, não institucionalizada e de baixa renda. Destacam-se as elevadas taxas de acidente vascular encefálico, sarcopenia e úlceras por pressão. A maioria dos cuidadores são do sexo feminino, casados, filhos do paciente e desempregados. Somente um terço dos pacientes apresenta cuidador contratado. Conclusão: os pacientes com demência avançada apresentam elevado grau de morbidades, baixa renda e são dependentes de cuidados de familiares, geralmente filhas desempregadas.

Abstract

Background: Post Covid‐19 syndrome can cause cognitive impairment and may be associated with a higher risk of developing dementia in older people. Method: This is a cohort study that is evaluating over 12 months the functional status and cognition of elderly people after hospital discharge. One group is composed of elderly people admitted to the hospital for Covid‐19 and the other group is composed of elderly people hospitalized for other clinical reasons. The groups are evaluated with The Mattis (DRS), AD8, ADCS‐ADL, PCL‐C, GDS‐15, FIM, COPM, CFS, FSS, mMRC and SARC‐calf. We will describe the preliminary results of the first 31 patients included in the COVID‐19 group on functional status, cognition, mood and quality of life. Result: Thirty‐one patients with COVID‐19 were evaluated three months after hospital discharge. One was excluded due to previous mental disorder and another due to a request to be excluded from the study. We found a mean age of 69.55 ± 7.72 years, schooling 7.14 ± 4.48 years, 51.7% female, Charlson Comorbidity Index 2.21 ± 1.84, 48.3% with smoking history and 62.9% had incomplete vaccine protection for COVID‐19. Considering cognition, 13.9% of the sample had no impairment, 55.1% had mild cognitive impairment (MCI) (17.2% amnestic MCI and 37.9% non‐amnesic MCI) and 31% had dementia. 24.1% declared their health status and quality of life as fair or poor. Mean CFS was 4.45 ± 1.18, GDS‐15 4.28 ± 3.38, FSS 30.76 ± 19.24, PCL‐c 29.34 ± 10.41, SARC‐calf 4.03 ± 4.61. All had mMRC dyspnea classified between 1 and 4. Conclusion: There was a high prevalence of mild cognitive impairment and dementia among patients after three months of discharge, with a predominance of mild frailty due to CFS and the presence of fatigue. Abstract

INTRODUCTION: Care of the elderly with dementia in advanced stages has important knowledge gaps, especially in those with oropharyngeal dysphagia in whom tube feeding, rather than oral feeding, is used as an alternative route. There are controversies about the systematic use of gastrostomy or enteral feeding to feed this population. OBJECTIVES: To verify, through literature overview, the effectiveness of tube feeding in relation to survival, pneumonia incidence, prevalence and healing of pressure ulcers, nutritional status and quality of life. METHODOLOGY: A database search in MEDLINE, LILACS and SciELO using as descriptors the Portuguese words for dementia, gastrostomy and enteral nutrition and their equivalents in English for the whole database existence until December 2012, limited to English, Portuguese and Spanish languages. We excluded editorials, non-systematic overviews and letters. Among the articles found, only prospective studies, or retrospective cohorts comparing elderly with alternative route feeding and patients with exclusive oral route, were selected for this overview. RESULTS: Three hundred sixty four articles were found. Of these, only nine met all the selection criteria. The studies selected were not able to demonstrate benefits of gastrostomy and enteral nutrition in any of the variables studied in the elderly in an advanced stage of dementia. CONCLUSION: There is no evidence in the literature that alternative routes of feeding improve survival outcomes, aspiration pneumonia, pressure ulcers, nutritional status and quality of life in patients with advanced stage dementia.

Abstract

INTRODUCTION: The Healthcare Program for Alzheimer's Disease Carriers established in 2002 that anticholinesterasic drugs should be provided on a free basis in Brazil. OBJECTIVES: To evaluate the estimates for the program coverage based on data regarding the delivery of anticholinesterasics in several regions and states of Brazil. METHODOLOGY: It was estimated the number of elderly individuals with dementia per state based on Brazilian epidemiological studies (7.1%). It was predicted that 60% of these individuals had probable or possible Alzheimer's disease (AD), and the delivery of anticholinesterasics (IChEs) to 50% of this population was established as the major goal by the Brazilian program. Brazilian Health System as well as the Superintendence of Pharmaceutical Aid of Minas Gerais State Secretary of Health and IBGE provided data on outpatient care. RESULTS: The prevalence of dementia symptoms was of 1,332,034 cases. Of these, 799,220 patients had AD. In 2008, the Ministry of Health provided IChEs to 47,886 patients at a yearly cost of R$ 99.747.356,53. The program coverage reached only 12% of the patients who were eligible for receiving IChEs. This rate varied from 1.2% in Amazon state to 22.3% in São Paulo state. The most prescribed anticholinesterasic drug was Rivastigmine (71.4%). CONCLUSION: The program coverage was found to be low and extremely variable in 2008, possibly because of both poor diagnosis of AD and regional differences in the access to the program.

Objective: To compare the results of neuropsychological tests, evoked potentials N200 and P300 and polymorphisms of ApoE and BDNF rs6265 between patients with normal cognition and those with mild cognitive impairment (MCI) and Alzheimer's dementia (AD).Methods: This is a cross-sectional study of elderly individuals with normal cognition and those with MCI and AD, who were submitted to evoked potential tests (N200 and P300) by means of hearing stimuli based on the auditory oddball paradigm. Genotyping was obtained by using the real-time PCR technique.Results: Sixty-five patients were evaluated as follows: 14 controls, 34 with MCI and 17 with AD. N200 latency and P300 latency and amplitude were not associated with MCI and AD diagnosis. Patients with cognitive impairment (MCI or AD) showed increase in the latencies of P300 and N200. BNDF gene was not associated with cognitive impairment.Conclusion: Latencies of N200 and P300 increased in cognitively impaired patients with the presence of ApoE ε-4 allele. [ABSTRACT FROM AUTHOR]

Mild cognitive impairment is a clinical condition more frequent in patients with Parkinson's disease than in general population. The nonamnestic presentations, usually characterized by executive dysfunction, are most prevalent. We present a case report of a Parkinson’s disease patient diagnosed with nonamnestic mild cognitive impairment that showed complete remission of cognitive symptoms after one year. We discuss the possible causes for the remission, focusing on the treatment of medical conditions such as a major depressive episode and vitamin B12 deficiency, in addition to the change of pharmacological treatment. In a third assessment, cognitive performance remained normal. The case report highlights the importance of controlling clinical comorbidities on the assessment and followup of mild cognitive impairment, especially on Parkinson’s disease.
