Colorectal cancer is the third most common cancer in Turkey. The current guidelines do not provide sufficient information to cover all aspects of the management of rectal cancer. Although treatment has been standardized in terms of the basic principles of neoadjuvant, surgical, and adjuvant therapy, uncertainties in the management of rectal cancer may lead to significant differences in clinical practice. In order to clarify these uncertainties, a consensus program was constructed with the participation of the physicians from the Acıbadem Mehmet Ali Aydınlar and Koç Universities. This program included the physicians from the departments of general surgery, gastroenterology, pathology, radiology, nuclear medicine, medical oncology, radiation oncology, and medical genetics. The gray zones in the management of rectal cancer were determined by reviewing the evidence-based data and current guidelines before the meeting. Topics to be discussed consisted of diagnosis, staging, surgical treatment for the primary disease, use of neoadjuvant and adjuvant treatment, management of recurrent disease, screening, follow-up, and genetic counseling. All those topics were discussed under supervision of a presenter and a chair with active participation of related physicians. The consensus text was structured by centralizing the decisions based on the existing data.

Although there are still some controversies on the choice of ideal stent as well as balloon-expandable versus self-expanding stents for aorto-iliac disease; iliac stenting is widely accepted procedure for focal atherosclerotic iliac artery stenosis. Herein, we present an unusual unilateral iliac stenting complication caused by disintegration struts of balloon-expandable stent which was successfully handled by means of kissing stents. 62-year-old woman presented with complaints of severe claudication of the right lower extremity. Short segment stenosis of the proximal right common iliac artery was detected. Balloon-expandable stent was deployed and stent delivery catheter was then exchanged uneventfully for the pigtail catheter followed by a control arteriogram which demonstrated the fractured and elongated stent into the distal aorta centrally. Then, two self-expandable nitinol stents were deployed in bilateral common iliac artery in order to compress the fractured stent struts against the aortic wall. Control arteriogram was obtained showing excellent result with no residual stenosis. However, stent fractures involving balloon-expandable stents in the iliac arteries are extremely rare, this unusual stent fracture type should be considered before implantation of a stent in the iliac artery., İlyak stentleme, fokal aterosklerotik ilyak arter stenoz tedavisinde kabul edilen tedavi yöntemi olmakla birlikte stent seçimi önemli rol oynamaktadır. Bu olguda nadir bir ilyak stent komplikasyonu olan balon ile genişleyebilen stent strutlarının bozunmasının kissing stent tekniği ile tedavisini sunuyoruz. Sağ bacağında şiddetli kladikasyo şikayeti olan 62 yaşındaki hastanın yapılan tetkiklerinde proksimal sağ ana ilyak arterde kısa segment darlık saptandı. Sorunsuz yerleştirilen balon ile genişleyebilen stent sonrası elde olunan kontrol anjiografilerde stentin strutlarının dağılarak stent boyunun uzadığı distal aortaya doğru yer değiştirdiği görüldü. Bunun üzerine strutları damar duvarına yaslamak için kissing tekniği ile bilateral kendiliğinden genişleyen nitinol stent yerleştirildi. Kontrol anjiografide rezidüel stenoz saptanmadı. Sonuç olarak ilyak arter stentlemede bu tip nadir komplikasyonlar göz önünde bulundurulmalıdır

We describe a complex congenital pelvic AVM with multiple feeding arteries arising from the side branches of the right internal iliac artery and a single draining vein in a male patient. Concomitant transarterial and transvenous embolization with a new liquid embolic agent Squid-12 and metallic coils enabled a complete embolization at a single session. Squid-12 is composed of ethylene vinyl alcohol copolymers and its lower viscosity makes it a promising agent for the treatment of AVMs. The patient showed prompt resolution of the symptoms and complete devascularization of the AVM lesion was persisted on the 1-month control angiography. The patient was asymptomatic on the 6th month follow-up.
