Aim: Hypotheses about idiopathic sudden sensorineural hearing loss (ISSNHL) have focused on inflammation and vascular etiopathogenesis. We evaluated the relationship between ISSNHL and choroidal thickness (CT), ocular pulse amplitude (OPA), and intraocular pressure (IOP) to reveal the generality of inflammation and vascular pathology.Materials and Methods: This study was conducted on 34 ISSNHL patients who were admitted within 5 days and were considered idiopathic with no etiological cause and 34 healthy controls. The ISSNHL group and the control group were assessed in terms of CT, OPA and IOP and ISSNHL groups on both affected (ipsilateral) and non-affected (contralateral) sides. Mild-moderate hearing loss group (n=20) and severe-profound hearing loss group (n=14) were also investigated and ocular parameters were compared.Results: CT values were increased statistically significantly in the ISSHNL group (p=0.03), but there was no significant difference between groups in terms of OPA and IOP. When the ipsilateral and contralateral sides of ISSHNL patients were compared, no statistical difference was found between the CT, OPA and IOP values (p>0.05). Choroid thickness was found to be higher in severe-profound group rather than the mild-moderate group, but it was not statistically significant.Conclusion: Increase in CT values of the ISSNHL group supports the relationship between sudden hearing loss and inflammatory processes. The bilateral nature of the findings support that systemic inflammation rather than a local inflammatory response., Amaç: Ani idiyopatik sensörinöral işitme kaybı (AİSNİK) ile ilgili hipotezler, inflamasyon ve vasküler etiyopatogenez üzerine odaklanmıştır. Biz bu çalışmada AİSNİK ile koroid kalınlığı (KK), oküler pulsamplitüdü (OPA) ve göz içi basıncı (GİB) arasındaki ilişkiyi değerlendirerek, inflamasyon ve vasküler patolojilerin hastalıktaki rolünü ortaya koymayı amaçladık. Gereç ve Yöntemler: Ani işitme kaybı şikayetiyle beş gün içinde başvuran ve etiyolojik bir neden bulunma yarak idiyopatik kabul edilen 34 AİSNİK hastası ve 34 sağlıklı gönüllü çalışmaya dahil edildi. AİSNİK grubu ile kontrol grubu ve kendi içinde AİSNİK grubunun etkilenen (ipsilateral) ve etkilenmeyen (kontralateral) tarafları KK, OPA ve GİB açısından karşılaştırıldı. Ayrıca hafif-orta işitme kayıplı grup (n=20) ve ileri-çok ileri işitme kayıplı gruplar (n=14) arasındaki oküler parametreler karşılaştırıldı. Bulgular: Koroid kalınlığı, AİSNİK grubunda istatistiksel olarak anlamlı düzeyde artmış bulundu (p=0,03). Ancak OPA ve GİB açısından gruplar arasında anlamlı bir fark görülmedi. AİSNİK hastalarının, aynı taraf ve karşı tarafları karşılaştırıldığında KK, OPA ve GİB değerleri arasında istatistiksel olarak fark bulunmadı (p>0.05). Koroid kalınlığı, ileri-çok ileri işitme kaybı izlenen grupta, hafif-orta işitme kaybı izlenen gruba göre, artmıştı ancak istatistiksel olarak anlamlı değildi. Sonuç: AİSNİK grubunun KK değerlerindeki artış, ani işitme kaybı ile inflamatuar süreçler arasındaki ilişkiyi desteklemektedir. Bulguların bilateral olması, lokal bir inflamatuar yanıttan çok sistemik inflamasyonu desteklemektedir.

Ani işitme kaybı AİK KBB ve baş boyun cerrahisinin acil tedavi gerektiren bir hastalığıdır. Tedavinin bir an önce başlaması ile düzelme doğru orantılıdır. Bu nedenle hastalığın tanısının konması ve tedavisinin başlanılması çok önemlidir. Etiyolojisi kesin belli olmayan ani işitme kaybında tedavi seçenekleri de faklıdır. Ocak 1998 ile Aralık 2002 yılları arasında SSK Tepecik Eğitim Hastanesi KBB polikliniği veya acil servise başvurmuş olan yaşları 14-92 arasında değişen 97’si erkek, 95’i kadın, toplam 192 olgu geriye dönüşümlü olarak incelendi. Tedavide 10 gün süre ile 150 cc serum fizyolojik içinde: Liquemin 1 cc, deksametazon 2 cc, adrenalinsiz xylocain 6 cc, pentoksifilin 10 cc 2x1 ve ağız yoluyla gingko biloba 3x1 , diazepam 10 mg 1x1 ve nörotonik ilaçlar; B1, B6, 2x1 verildi. Olgulara tedavi öncesi, 7. gün ve bir ay sonrası odiyogramları alındı. Olgularımızda %53.7’lik bir iyileşme görüldü. Erken başvurunun iyileşme üzerinde olumlu etkileri saptanmıştır, yani ilk 10 gün içinde tedavi ile düzelme şansı artmaktadır. Ani işitme kaybında işitme kaybı ile birlikte vertigo, koroner arter hastalığı, diabet ve hipertansiyonun varlığı prognoza kötü etki yapmaktadır. Bu bulguların ışığı altında; uygulanan kokteyl tedavisi ile olumlu sonuçlar alınmaktadır. Hastalarda tedaviye erken başlanılmalıdır. Ancak 6 aya kadar geç başvuranlarda da tedavi denenmelidir, Sudden deafness is a disease of E.N.T. which must be cured immediately. The cure rate is directly in proportion with the unset of the therapy date. Because of this, it is important to diagnose and begin the therapy immediately. The therapy options vary because of the unknown ethiology. 192 cases were analyzed retrospectively who applied to the E.N.T. Clinic or the Emergency Department of Tepecik Educational Hospital of Social Security Council, İzmir, during January 1998 and December 2002. Of these were 97 men, 95 women with the ages of 14-92; with the mean of 45.6. We treated the patients with a cocktail given İ.V. for ten days. 1 cc Liquemine, 2 cc dexamethasone, 6 cc xylocaine simple and 10 cc pentoxyphyline in 150 cc of physiologic serum were applied two times daily. We also perscribed oral gingkho biloba three times, diazepam once a day and neurotonic B complex vitamins. The pure tone audiograms were realised before and at the 7 th day and a month after the therapy. The cure rate was 53.7 %. The positive affect of the early therapy was found meaning that if the case apply within the first 10 days of the onset, the cure rate is to be high. If there is vertigo, coroner artery disease, diabetes, and hypertension with sudden deafness, the prognosis is very poor. With these findings, we can say that good results can be attained with our cocktail therapy. The therapy must begin immediately. We must also use this cocktail at late cases even with an onset of 6 months. Our goal was to introduce the therapy modality of our clinic and to discuss it with the ones of the literature
