Five patients with reticular erythematous mucinosis were studied by skin biopsy and phototesting. Light microscopy showed a dermal mononuclear cell infiltrate (composed largely of lymphocytes), separation of collagen bundles and fragmentation of elastic fibres. Histochemical stains showed increased dermal mucin with a profile consistent with hyaluronic acid. In one case, the ultrastructural distribution of mucin was demonstrated using colloidal iron staining, electron microscopy and X-ray microanalysis. In two cases, virus-like tubular aggregates were identified in the cytoplasm of dermal endothelial cells and pericytes. Twenty scalp hairs were plucked from each of 131 Caucasoid women, and measurements of diameter, at 1 mm intervals, starting just above the root, were made for each hair. There was little variation in diameter along normal anagen hairs in any of the subjects. The mean anagen hair diameter in 58 women with diffuse hair thinning, of whom 44 were without detectable endocrine abnormality and 14 were clinically hypothyroid, significantly decreased with age. This appeared to be an accentuation of a normal trend, although this could not be statistically established in the control group of 73 women. At any age the mean thickness of hair in the subjects with diffuse thinning was less than In normal subjects of the same age. Normal subjects showed a symmetrical distribution of hair diameter, with a marked peak at 0.08 mm. In contrast, subjects with diffuse alopecia without detectable endue crine abnormality showed a wide spread of diameters, with 2 equal peaks at 0.04 mm and 0.06 mm. The hypothyroid group showed a similar distribution, except that there was a larger proportion of hairs of the smaller diameters. The results suggest that, in the development of diffuse thinning, not all follicles behave identically, and that there may be more than one type of follicle In the human scalp. The fractured ends of exclamation mark hairs were found to be quite distinct from those of normal hairs broken by traction when examined with the scanning electron microscope. The cuticular scales of exclamation mark hairs, telogen hairs from around lesions, hairs from unaffected parts of the scalp, and hairs from normal subjects were found to possess smooth edges when first produced. The pathogenesis of alopecia areata was studied by plucking hairs from each of a series of concentric zones, using felt patterns placed over lesions. It was possible to group lesions according to the zone with the greatest proportion of club hairs. The results are logically interpreted to mean that a wave of hair follicle damage or arrest moves centrifugally from a focal point beyond the area of alopecia. It is suggested that follicles may react in 1 of 3 ways. Firstly, severe damage may weaken the hair in the keratogenous zone, this being followed by breakage when the weak zone reaches the plane of the scalp; at the same time the follicle is precipitated into catagen and the hair is extruded as an exclamation mark. Secondly, the follicle may be precipitated into catagen with loss of the club in a normal manner, followed by replacement with a dystrophic anagen hair. The article presents information on a study on patients who have within recent years attended the Dermatology Department of the Sheffield Royal Infirmary complaining of falling or thinned hair. Post-partum and post-febrile hair loss is caused by a sudden shedding of club hairs, known as telogen effluvium and is usually temporary. Relatively little is known about the dynamics of other types. When hair loss is sufficiently severe to cause obvious thinning, two main patterns are evident. Either there is frontal recession with some central thinning similar to that seen in hereditary baldness of the male, or there is diffuse thinning over the vertex with retention of the normal hairline.

A case of ocular pemphigus which has on two occasions in eight years developed a widespread bullous eruption is reported. Histological examination of the skin showed a subepidermal blister with no aeantholysis, thus supporting the view that ocular pemphigus is a separate entity from pemphigus vulgaris. The patient responded favourably to treatment with cortictropin and delta-cortisone, but it was not found possible to control all the mucosal lesions. [ABSTRACT FROM AUTHOR]

The article discusses a cutaneous syndrome associated with Islet-cell carcinoma of the pancreas. Carcinoma of the pancreas accounts for 1 to 2% of all cancers, yet when the body and tail of the gland are involved the diagnosis can be one of the most puzzling problems. However, carcinoma of the pancreas is seldom recorded in association with skin eruptions. In 42 cases of acanthosis nigricans with cancer, 3 cases of pancreatic carcinoma were found. In a review of 92 eases of dermatomyositis associated with cancer R.C. Williams did not find a single instance of carcinoma of the pancreas although it is an important cause of death in the age group studied.
