Abstract Background Sporadic colorectal cancers (CRC) are multifactorial diseases resulting from the combined effects of numerous genetic, environmental and behavioral risk factors. Genetic association studies have suggested low-penetrance alleles of extremely varied genes to be involved in susceptibility to CRC in Caucasian populations. Methods Through a large genetic association study based on 1023 patients with sporadic CRC and 1121 controls, we tested a panel of these low-penetrance alleles to find out whether they could determine "genotypic profiles" at risk for CRC among individuals of the French population. We examined 52 polymorphisms of 35 genes – drawn from inflammation, xenobiotic detoxification, one-carbon, insulin signaling, and DNA repair pathways – for their possible contribution to colorectal carcinogenesis. The risk of cancer associated with these polymorphisms was assessed by calculation of odds ratios (OR) using multivariate analyses and logistic regression. Results Whereas all these polymorphisms had previously been found to be associated with CRC risk, especially in Caucasian populations, we were able to replicate the association for only five of them. Three SNPs were shown to increase CRC risk: PTGS1 c.639C>A (p.Gly213Gly), IL8 c.-352T>A, and MTHFR c.1286A>C (p.Ala429Glu). On the contrary, two other SNPs, PLA2G2A c.435+230C>T and PPARG c.1431C>T (p.His477His), were associated with a decrease in CRC risk. Further analyses highlighted genotypic combinations having a greater predisposing effect on CRC (OR 1.97, 95%CI 1.31–2.97, p = 0.0009) than the allelic variants that were examined separately. Conclusion The identification of CRC-predisposing combinations, composed of alleles PTGS1 c.639A, PLA2G2A c.435+230C, PPARG c.1431C, IL8 c.-352A, and MTHFR c.1286C, highlights the importance of inflammatory processes in susceptibility to sporadic CRC, as well as a possible crosstalk between inflammation and one-carbon pathways.

Regional citrate anticoagulation use in intermittent hemodialysis is limited by the increased risk of metabolic complications due to faster solute exchanges than with continuous renal replacement therapies. Several simplifications have been proposed. The objective of this study was to validate a mathematical model of hemodialysis anticoagulated with citrate that was then used to evaluate different prescription scenarios on anticoagulant effectiveness (free calcium concentration in dialysis filter) and calcium balance. A study was conducted in hemodialyzed patients with a citrate infusion into the arterial line and a 1.25 mmol/L calcium dialysate. Calcium and citrate concentrations were measured upstream and downstream of the citrate infusion site and in the venous line. The values measured in the venous lines were compared with those predicted by the model using Bland and Altman diagrams. The model was then used with 22 patients to make simulations. The model can predict the concentration of free calcium, bound to citrate or albumin, accurately. Irrespective of the prescription scenario a decrease in free calcium below 0.4 mmol/L was obtained only in a fraction of the dialysis filter. Calcium has two important roles in haemodialysis. It participates in the activation of blood coagulation and calcium intakes have a major impact on patient mineral and bone metabolism. The aim of this article is to propose a mathematical model for calcium ions concentration in a dialyzer during haemodialysis using a citrate dialysate. The model is composed of two elements. The first describes the flows of blood and dialysate in a dialyzer fibre. It was obtained by asymptotic analysis and takes into account the anisotropy of the fibres forming a dialyzer. Newtonian and non-Newtonian blood rheologies were tested. The second part of the model predicts the evolution of the concentration of five chemical species present in these fluids. The fluid velocity field drives the convective part of a convection-reaction-diffusion system that models the exchange of free and complexed calcium. We performed several numerical experiments to calculate the free calcium concentration in the blood in a dialyzer using dialysates with or without citrate. The choice of blood rheology had little effect on the fluid velocity field. Our model predicts that only a citrate based dialysate without calcium can decrease free calcium concentration at the blood membrane interface low enough to inhibit blood coagulation. Moreover for a given calcium dialysate concentration, adding citrate to the dialysate decreases total calcium concentration in the blood at the dialyzer outlet. This decrease of the calcium concentration can be compensated by infusing in the dialyzed blood a quantity of calcium computed from the model.

The hypothesis made is that thermal resistance of sorghum and miscanthus stem pieces taken at well-defined positions of the stem is simply related to their biochemical composition. For miscanthus, two different genotypes and two internode levels were selected. For each region, the stem was divided into three radial layers. For sorghum, two different genotypes were selected and the stem was divided into the same three radial layers. The results show that the thermal analysis is only sensitive to very large variations of compositions. But aside of such large composition differences, it is impossible to correlate thermal effects to biochemical composition even on very small size, well-identified pieces of plant materials. The interplay between sugar-based components, lignin and minerals is totally blurring the thermal response. A new machine has been developed to measure heat dissipated in the tissues especially by convection linked to the blood flow. Its use is simple and without any danger as far as application to the pregnant cervix goes. New discoveries are presented. A new concept of the micro-circulation is considered together with the consequences. The practitioner may be able to draw conclusions as to the prognosis, as to whether a small-for-dates fetus together with or without premature labour is likely. This depends on the tracings obtained which register the intrinsic activity of the periarteriolar muscles.

The use of heparin to interfere with the appearance of fetal maldevelopment (or to correct it) is logical but the results are difficult to prove because it is difficult to be certain of early diagnosis at the time when therapy has the best chances of being efficacious. The authors compare two groups of pregnant women in whom the muscular vascular tone of the cervix is abolished. The second group was treated with heparin. The results are statistically significant and prove that treatment with heparin is well based in cases of underdevelopment of the fetus when it is due to abnormalities in the maternal microscopic circulation (acute atherosis). They confirm that the enzyme, hormone and ultrasound parameters return to normal from the beginning of treatment with heparin.
