Mycobacterial infection-related morbidity and mortality in patients following cardiopulmonary bypass surgery is high and there is a growing need for a consensus-based expert opinion to provide international guidance for diagnosing, preventing and treating in these patients. 6. Shallow turbulent flows by video imaging method

Shallow turbulent flows were produced in a tank of small thickness to study the friction effects on large-scale turbulent motion of small depth. The tank was constructed of two parallel walls. The space between the parallel walls (4.4, 1.57, and 0.59 cm) was small compared with the height (107 cm) and the width (212 cm) of the tank, and was varied during the experiments for different friction effects. Turbulent flows were produced by the injection of water in the form of starting jets into the tank filled with water. The large-scale turbulent flow in the small space between the walls of the tank is confined to essentially two-dimensional motion, and the motion is retarded by the force of friction. Dye was injected with the source fluid as the tracer for the highly unsteady and quasitwo-dimensional turbulent motion. From the initiation of the turbulent motion at the source to the final interaction of the jets with the tank bottom, the entire sequence of events was recorded by a pair of video cameras. The depth-averaged concentration of the dye was analyzed using the recorded video images. Numerical computations are conducted of the flow across a turbulent mixing layer in an open channel of small depth using three turbulence models: a single-length-scale model, a modified single-length-scale model, and a two-length-scale model. The performance of these turbulence models is evaluated by comparing the numerical results with the available experimental data. The single-length-scale turbulence model is shown to be biased toward the small-scale turbulence. The coefficients of this single-length-scale turbulence model have been modified in an attempt to improve the performance of the model. The results of the modified single-length-scale model are in agreement with the laboratory data but not with the field data. The two-length-scale turbulence model, by treating the small-scale and the large-scale turbulence as separate components, has produced results in better agreement with the experimental data than those obtained by the single-length-scale model treating both components together as a whole.

The recirculating flow produced by a sudden widening of flow in a shallow open channel is computed using three turbulence models including (1) a constant-eddy-viscosity model; (2) a depth-averaged k-[Epsilon] model; and (3) a two-length-scale model. The computational results demonstrate the existence of a friction-dominant regime. In this regime, all turbulence models give approximately the same recirculating length and the same recirculating flow rate despite the different levels of eddy viscosity determined by the models. The length of the recirculating zone obtained by the models is approximately equal to one frictional-length scale, and the recirculating flow rate equal to 2.5% of the frictional flow rate; these numerical results are in agreement with the experimental observations. The implication on the computation of the turbulent flow in natural waterways is discussed in light of these numerical results.

- Abstract
Objectives Left-sided methicillin-susceptible Staphylococcus aureus (MSSA) endocarditis treated with cloxacillin has a poorer prognosis when the vancomycin minimum inhibitory concentration (MIC) is >=1.5 mg/L. We aimed to validate this using the International Collaboration on Endocarditis cohort and to analyse whether specific genetic characteristics were associated with a high vancomycin MIC (>=1.5 mg/L) phenotype. Methods All patients with left-sided MSSA infective endocarditis treated with antistaphylococcal beta-lactam antibiotics between 2000 and 2006 with available isolates were included. Vancomycin MIC was determined by Etest as either high (>=1.5 mg/L) or low (<1.5 mg/L). Isolates underwent spa typing to infer clonal complexes and multiplex PCR for identifying virulence genes. Univariate analysis was performed to evaluate the association between in-hospital and 1-year mortality, and vancomycin MIC phenotype. Results Sixty-two cases met the inclusion criteria. Vancomycin MIC was low in 28 cases (45%) and high in 34 cases (55%). No significant differences in patient demographic data or characteristics of infection were observed between patients with infective endocarditis due to high and low vancomycin MIC isolates. Isolates with high and low vancomycin MIC had similar distributions of virulence genes and clonal lineages. In-hospital and 1-year mortality did not differ significantly between the two groups (32% (9/28) vs. 27% (9/34), p 0.780; and 43% (12/28) vs. 29% (10/34), p 0.298, for low and high vancomycin MIC respectively). Conclusions In this international cohort of patients with left-sided MSSA endocarditis treated with antistaphylococcal beta-lactams, vancomycin MIC phenotype was not associated with patient demographics, clinical outcome or virulence gene repertoire.Copyright © 2017 European Society of Clinical Microbiology and Infectious Diseases

- Abstract
Background. The impact of early valve surgery (EVS) on the outcome of Staphylococcus aureus (SA) prosthetic valve infective endocarditis (PVIE) is unresolved. The objective of this study was to evaluate the association between EVS, performed within the first 60 days of hospitalization, and outcome of SA PVIE within the International Collaboration on Endocarditis-Prospective Cohort Study. Methods. Participants were enrolled between June 2000 and December 2006. Cox proportional hazards modeling that included surgery as a time-dependent covariate and propensity adjustment for likelihood to receive cardiac surgery was used to evaluate the impact of EVS and 1-year all-cause mortality on patients with definite left-sided S. aureus PVIE and no history of injection drug use. Results. EVS was performed in 74 of the 168 (44.3%) patients. One-year mortality was significantly higher among patients with S. aureus PVIE than in patients with non-S. aureus PVIE (48.2% vs 32.9%; P = .003). Staphylococcus aureus PVIE patients who underwent EVS had a significantly lower 1-year mortality rate (33.8% vs 59.1%; P = .001). In multivariate, propensity-adjusted models, EVS was not associated with 1-year mortality (risk ratio, 0.67 [95% confidence interval, .39-1.15]; P = .15). Conclusions. In this prospective, multinational cohort of patients with S. aureus PVIE, EVS was not associated with reduced 1-year mortality. The decision to pursue EVS should be individualized for each patient, based upon infection-specific characteristics rather than solely upon the microbiology of the infection causing PVIE.Copyright © The Author 2014.

- Abstract
Candida infective endocarditis is a rare disease with a high mortality rate. Our understanding of this infection is derived from case series, case reports, and small prospective cohorts. The purpose of this study was to evaluate the clinical features and use of different antifungal treatment regimens for Candida infective endocarditis. This prospective cohort study was based on 70 cases of Candida infective endocarditis from the International Collaboration on Endocarditis (ICE)-Prospective Cohort Study and ICE-Plus databases collected between 2000 and 2010. The majority of infections were acquired nosocomially (67%). Congestive heart failure (24%), prosthetic heart valve (46%), and previous infective endocarditis (26%) were common comorbidities. Overall mortality was high, with 36% mortality in the hospital and 59% at 1 year. On univariate analysis, older age, heart failure at baseline, persistent candidemia, nosocomial acquisition, heart failure as a complication, and intracardiac abscess were associated with higher mortality. Mortality was not affected by use of surgical therapy or choice of antifungal agent. A subgroup analysis was performed on 33 patients for whom specific antifungal therapy information was available. In this subgroup, 11 patients received amphotericin B-based therapy and 14 received echinocandin-based therapy. Despite a higher percentage of older patients and nosocomial infection in the echinocandin group, mortality rates were similar between the two groups. In conclusion, Candida infective endocarditis is associated with a high mortality rate that was not impacted by choice of antifungal therapy or by adjunctive surgical intervention. Objective: To quantify the change in prescribing of antibiotic prophylaxis before invasive dental procedures for patients at risk of infective endocarditis, and any concurrent change in the incidence of infective endocarditis, following introduction of a clinical guideline from the National Institute for Health and Clinical Excellence (NICE) in March 2008 recommending the cessation of antibiotic prophylaxis in the United Kingdom.\ud Design: Before and after study.\ud Setting: England.\ud Population: All patients admitted to hospital in England with a primary or secondary discharge diagnosis of acute or subacute infective endocarditis.\ud Main outcome measures: Monthly number of prescriptions for antibiotic prophylaxis consisting of a single 3 g oral dose of amoxicillin or a single 600 mg oral dose of clindamycin, and monthly number of cases of infective endocarditis, infective endocarditis related deaths in hospital, or cases of infective endocarditis with a possible oral origin for streptococci.\ud Results: After the introduction of the NICE guideline there was a highly significant 78.6% reduction (P

Background— The impact of early surgery on mortality in patients with native valve endocarditis (NVE) is unresolved. This study sought to evaluate valve surgery compared with medical therapy for NVE and to identify characteristics of patients who are most likely to benefit from early surgery. Methods and Results— Using a prospective, multinational cohort of patients with definite NVE, the effect of early surgery on in-hospital mortality was assessed by propensity-based matching adjustment for survivor bias and by instrumental variable analysis. Patients were stratified by propensity quintile, paravalvular complications, valve perforation, systemic embolization, stroke, Staphylococcus aureus infection, and congestive heart failure. Of the 1552 patients with NVE, 720 (46%) underwent early surgery and 832 (54%) were treated with medical therapy. Compared with medical therapy, early surgery was associated with a significant reduction in mortality in the overall cohort (12.1% [87/720] versus 20.7% [172/832]) and after propensity-based matching and adjustment for survivor bias (absolute risk reduction [ARR] −5.9%, P P P =0.002) and those with paravalvular complications (ARR −17.3%, P P =0.002), S aureus NVE (ARR −20.1%, P P =0.02) but not those with valve perforation or congestive heart failure. Conclusions— Early surgery for NVE is associated with an in-hospital mortality benefit compared with medical therapy alone.

- Abstract
Methods and results Among 5591 patients included in the International Collaboration on Endocarditis Prospective Cohort Study, 1467 patients with definite IE were operated on during the active phase and had a biological (37%) or mechanical (63%) valve replacement. Patients who received bioprostheses were older (62 vs 54 years), more often had a history of cancer (9% vs 6%), and had moderate or severe renal disease (9% vs 4%); proportion of health care-associated IE was higher (26% vs 17%); intracardiac abscesses were more frequent (30% vs 23%). In-hospital and 1-year death rates were higher in the bioprosthesis group, 20.5% vs 14.0% (p = 0.0009) and 25.3% vs 16.6% (p <.0001), respectively. In multivariable analysis, mechanical prostheses were less commonly implanted in older patients (odds ratio: 0.64 for every 10 years), and in patients with a history of cancer (0.72), but were more commonly implanted in mitral position (1.60). Bioprosthesis was independently associated with 1-year mortality (hazard ratio: 1.298). Conclusions Patients with IE who receive a biological valve replacement have significant differences in clinical characteristics compared to patients who receive a mechanical prosthesis. Biological valve replacement is independently associated with a higher in-hospital and 1-year mortality, a result which is possibly related to patient characteristics rather than valve dysfunction. Background Nearly half of patients require cardiac surgery during the acute phase of infective endocarditis (IE). Staphylococcus lugdunensis native valve endocarditis (NVE) is rare and associated with an aggressive clinical course. Our current understanding of this disease is based on case reports and reviews of the literature. The purpose of this investigation is to compare the clinical features of S. lugdunensis and S. epidermidis NVE. The International Collaboration on Endocarditis – Prospective Cohort Study (ICE-PCS) is a cohort of >3000 patients with IE from 61 centers in 28 countries. The ICE – microbiology repository comprises a subset of bacterial isolates from ICE-PCS contributed by 18 centers in 12 countries. From the ICE-microbiology repository, there were 23 S. epidermidis and 4 S. lugdunensis NVE isolates available for comparison. All S. lugdunensis isolates were susceptible to penicillin compared to 4 (17%) S. epidermidis isolates. Patients with S. lugdunensis NVE were younger than patients with S. epidermidis NVE (median age 52 vs 73 years, respectively). Three of the 4 cases of S. lugdunensis NVE cases were community-acquired, while 65% of S. epidermidis cases were acquired in the healthcare setting. Healthcare-associated risks for staphylococcal infection were absent for patients with S. lugdunensis NVE. In contrast, hemodialysis dependence (17%), cardiac devices (17%), and receipt of invasive procedures (39%) were prominent characteristics of patients with S. epidermidis NVE. Complications occurred in a larger proportion of patients with S. lugdunensis NVE [systemic embolization 2 (50%) ; heart failure 3 (75%)] than S. epidermidis NVE [systemic embolization 3 (13%) ; heart failure 11 (48%)]. All 4 (100%) patients with S. lugdunensis NVE underwent surgery compared to 14 (61%) S. epidermidis NVE patients. The rate of death was similar in S. lugdunensis and S. epidermidis NVE [1 (25%) vs 6 (26%)]. Given the small sample size, p-values were not significant for all analyses. S. lugdunensis NVE is associated with an aggressive clinical course and a high rate of surgery. Clinicians should be suspicious for S. lugdunensis in patients who present with coagulase-negative staphylococcal NVE from the community, without healthcare associated risks.

- Abstract
Background. The timing of cardiac surgery after stroke in infective endocarditis (IE) remains controversial. We examined the relationship between the timing of surgery after stroke and the incidence of in-hospital and 1-year mortalities. Methods. Data were obtained from the International Collaboration on Endocarditis-Prospective Cohort Study of 4794 patients with definite IE who were admitted to 64 centers from June 2000 through December 2006. Multivariate logistic regression and Cox regression analyses were performed to estimate the impact of early surgery on hospital and 1-year mortality after adjustments for other significant covariates. Results. Of the 857 patients with IE complicated by ischemic stroke syndromes, 198 who underwent valve replacement surgery poststroke were available for analysis. Overall, 58 (29.3%) patients underwent early surgical treatment vs 140 (70.7%) patients who underwent late surgical treatment. After adjustment for other risk factors, early surgery was not significantly associated with increased in-hospital mortality rates (odds ratio, 2.308; 95% confidence interval[CI], .942-5.652). Overall, probability of death after 1-year follow-up did not differ between 2 treatment groups (27.1% in early surgery and 19.2% in late surgery group, P = .328; adjusted hazard ratio, 1.138; 95% CI, .802-1.650). Conclusions. There is no apparent survival benefit in delaying surgery when indicated in IE patients after ischemic stroke. Further observational analyses that include detailed pre- and postoperative clinical neurologic findings and advanced imaging data (eg, ischemic stroke size), may allow for more refined recommendations on the optimal timing of valvular surgery in patients with IE and recent stroke syndromes.Copyright © The Author 2012.

- Abstract
The HACEK organisms (Haemophilus species, Aggregatibacter species, Cardiobacterium hominis, Eikenella corrodens, and Kingella species) are rare causes of infective endocarditis (IE). The objective of this study is to describe the clinical characteristics and outcomes of patients with HACEK endocarditis (HE) in a large multi-national cohort. Patients hospitalized with definite or possible infective endocarditis by the International Collaboration on Endocarditis Prospective Cohort Study in 64 hospitals from 28 countries were included and characteristics of HE patients compared with IE due to other pathogens. Of 5591 patients enrolled, 77 (1.4%) had HE. HE was associated with a younger age (47 vs. 61 years; p<0.001), a higher prevalence of immunologic/vascular manifestations (32% vs. 20%; p<0.008) and stroke (25% vs. 17% p = 0.05) but a lower prevalence of congestive heart failure (15% vs. 30%; p = 0.004), death in-hospital (4% vs. 18%; p = 0.001) or after 1 year follow-up (6% vs. 20%; p = 0.01) than IE due to other pathogens (n = 5514). On multivariable analysis, stroke was associated with mitral valve vegetations (OR 3.60; CI 1.34-9.65; p<0.01) and younger age (OR 0.62; CI 0.49-0.90; p<0.01). The overall outcome of HE was excellent with the in-hospital mortality (4%) significantly better than for non-HE (18%; p<0.001). Prosthetic valve endocarditis was more common in HE (35%) than non-HE (24%). The outcome of prosthetic valve and native valve HE was excellent whether treated medically or with surgery. Current treatment is very successful for the management of both native valve prosthetic valve HE but further studies are needed to determine why HE has a predilection for younger people and to cause stroke. The small number of patients and observational design limit inferences on treatment strategies. Self selection of study sites limits epidemiological inferences. © 2013 Chambers et al.

- Abstract
Background. Using multinational collections of methicillin-susceptible Staphylococcus aureus (MSSA) isolates from infective endocarditis (IE) and soft tissue infections (STIs), we sought to (1) validate the finding that S. aureus in clonal complex (CC) 30 is associated with hematogenous complications and (2) test the hypothesis that specific genetic characteristics in S. aureus are associated with infection severity. Methods. IE and STI isolates from 2 cohorts were frequency matched by geographic origin. Isolates underwent spa typing to infer CC and multiplex polymerase chain reaction for presence of virulence genes. Results. 114 isolate pairs were genotyped. IE isolates were more likely to be CC30 (19.5% vs 6.2%; P = .005) and to contain 3 adhesins (clfB, cna, map/eap; P <.0001 for all) and 5 enterotoxins (tst, sea, sed, see, and sei; P <=.005 for all). CC30 isolates were more likely to contain cna, tst, sea, see, seg, and chp (P < .05 for all). Conclusions. MSSA IE isolates were significantly more likely to be CC30 and to possess a distinct repertoire of virulence genes than MSSA STI isolates from the same region. The genetic basis of this association requires further study. © The Author 2011.

The present study deals with the noninvasive measurement of concentration in the intermediate shallow turbulent wake region using a video-imaging technique. The flow depths considered in the present study are small compared to the width of the channel and the generated wakes are categorized as shallow. On the basis of the observed behavior, the wakes are classified as deep-shallow wakes and shallow-shallow wakes. The topology of the dye concentration distribution in the near and intermediate wake region indicates that the vortex structure tends to be preserved when the flow depth is relatively high and the dominant eddy structures are similar to that noticed in conventional two-dimensional wakes. In shallow-shallow wakes, the conventional Karman vortex street appears to be annihilated or intermittent. The lateral concentration distribution at several axial stations covering the first thirty body widths are considered for analysis. The instantaneous concentrations are observed to be several times higher than the corresponding mean values. Attempts are also made to determine the paths traversed by the vortex cores and the vortex core convection velocity. The axial variation of the wake half-width with depth of flow is also examined. A model is developed to predict the spread of the wake with downstream distance from the test body. A friction length scale is introduced in the model to account for the influence of depth and bed friction on the development of the wake.

The scalar concentration and velocity field of an axisymmetric turbulent jet in a coflow are measured using laser-induced fluorescence and laser Doppler anemometry techniques. Both the time-averaged and turbulence data suggest the use of the top-hat average velocity as the characteristic velocity in this shear flow. The visual boundary and the 25% maximum concentration (0.25[C.sub.m]) contour are found to correspond closely to the edge of the top-hat profile, where r = B = [-square root of 2] [b.sub.g] (B and [b.sub.g] are the nominal half-width of the top-hat and Gaussian profiles, respectively). This jet edge, defined by the 25%[C.sub.m] contour, also matches with the 50% contour of the intermittency factor. An integral model of the coflowing jet has been formulated using a Lagrangian jet spreading hypothesis for turbulent closure. The model predictions are in good agreement with the present concentration, velocity data obtained in the near field (strong jet), and the velocity data of a recent study in the far field (weak jet).

- Abstract
Background: Although cardiac devices have been found to reduce symptoms and mortality rates in appropriate patient populations, the implications of certain important risks, such as infection, are incompletely understood. The purpose of this study was to use a large population-based database to define the population that is at risk for cardiac device infections, determine the prevalence of device infections, and study changes in the rates of cardiac device implantation and infection in the past decade. Methods: Patients with cardiac device implantations and infections were identified with claims files from the Health Care Finance Administration for Medicare beneficiaries from January 1, 1990, through December 31, 1999. Rates of implantation of cardiac devices were determined. Time trend analyses were performed to determine the significance of the observed change in rates. Results: Cardiac device implantation rates increased from 3.26 implantations per 1000 beneficiaries in 1990 to 4.64 implantations per 1000 beneficiaries in 1999, which represents an increase of 42% in 10 years (P for trend <.001). Cardiac device infections showed a larger increase, from 0.94 device infections per 1000 beneficiaries in 1990 to 2.11 device infections per 1000 beneficiaries in 1999, which represents an increase of 124% during the study period (P for trend <.001). Conclusions: During the previous decade, there was a significant increase in both cardiac device implantations and infections in elderly patients, although the increase in the rates of device infections was substantially higher. Additional studies are needed to better understand the relationship and timing between cardiac device implantation and infection.

International audience; BACKGROUND: We sought to provide a contemporary picture of the presentation, etiology, and outcome of infective endocarditis (IE) in a large patient cohort from multiple locations worldwide. METHODS: Prospective cohort study of 2781 adults with definite IE who were admitted to 58 hospitals in 25 countries from June 1, 2000, through September 1, 2005. RESULTS: The median age of the cohort was 57.9 (interquartile range, 43.2-71.8) years, and 72.1% had native valve IE. Most patients (77.0%) presented early in the disease (
