Abstract

El objetivo de la presente investigación ha sido determinar la relación que existe entre el marketing y el posicionamiento de la marca de útiles escolares David en el distrito de Los Olivos. La metodología empleada está basada en un tipo de investigación descriptiva y correlacional. Se trabajó con una muestra de 383 padres de familia cuyos hijos estudian en los distintos colegios del distrito de Los Olivos, provincia de Lima, región Lima. Para la recopilación de datos se empleó el cuestionario de marketing, el cuestionario de posicionamiento y el análisis estadístico para comprobar las hipótesis utilizando la “rho” de Spearman. Los resultados obtenidos (r=0.886) indican que existe correlación significativa entre ambas variables, por lo podemos concluir que existe una relación positiva entre el Marketing y el Posicionamiento de los útiles escolares de la marca David en Lima Metropolitana durante el primer trimestre del año 2017. 8. Communicating with dying children and their siblings: a retrospective analysis

Abstract

Recognition that children over age 4 with fatal illnesses generally anticipate their death has prompted more open communication between them and the adults caring for them. We investigated factors influencing the extent and success of such communication between parents, dying children, and their siblings, using data provided by 77 mothers who are members of the Compassionate Friends organization. We found that those mothers who talked more freely with dying children also did so with the siblings and that communication was more open with older than with younger children. This dialogue was most helpful for both the dying children and their siblings if the former were mostly at home immediately before death, if there was extensive and specific discussion about death and dying, when a parent was the major discussant, and if the family's religious faith was also a significant source of support. Following such discussions the emotional state of the dying children and the siblings contrasted markedly, with the latter showing significantly more sadness, anger, denial, and fear. We found that most mothers had apparently recovered well from their bereavement and that this recovery correlated with having been helped by their religious faith, having been the main discussants with the children about death, and with having the child away from home the greater part of the terminal phase. Recovery from bereavement was also better among those mothers whose dying children had been better adjusted emotionally. We recommend that health professionals use all appropriate resources to encourage early, full, and candid discussions between dying children, their siblings, and their families.

Abstract

Axillary lymph node dissection is commonly performed as part of the primary management of breast carcinoma. Its value in patient management, however, has recently been questioned. Few studies exist that document long term complications. Four hundred thirty-two patients with Stage I or II breast carcinoma who were free of recurrence 2-5 years after surgery were identified. A cross-sectional survey was conducted to determine the prevalence of long term symptoms and complications as perceived by the patient, and patient and treatment factors that may have predicted complications were determined. Three hundred thirty of the 432 (76%) completed a mailed, self-administered questionnaire. In addition, the medical records of the 330 patients were reviewed. Patient and treatment factors were analyzed with logistic regression. Numbness was reported by 35% of patients at the time of the survey. Pain was noted in 30%, arm swelling in 15%, and limitation of arm movement in 8%. Eight percent reported episodes of infection or inflammation at some point since the diagnosis of breast carcinoma. The majority of symptoms were mild and interfered minimally with daily activities. Younger age (P = 0.001) was associated with more frequent reporting of pain. Numbness was more common in younger patients (P = 0.004) as well as in those with a history of smoking (P = 0.012). There was a positive association of limitation of arm motion with adjuvant tamoxifen therapy (P = 0.016). Arm swelling was associated with both younger age (P = 0.004) and greater body surface area (P = 0.008). Radiation therapy was associated with a higher frequency of infection or inflammation in the arm and/or breast (P = 0.001). Mild symptoms, especially pain and numbness, are common 2-5 years after axillary lymph node dissection. The frequency of inflammation or infection in patients treated with radiation to the breast or chest wall after an axillary lymph node dissection may be greater than previously appreciated. Severe complications or symptoms that have a major impact on daily activities are uncommon. These findings should help health care providers and their patients with breast carcinoma weigh the pros and cons of axillary lymph node dissection. Abstract

Purpose: Thalidomide and its more potent immunomodulatory derivative lenalidomide enhance rituximab-mediated antibody-dependent cell-mediated cytotoxicity. We therefore evaluated lenalidomide and rituximab in symptomatic Waldenstrom's macroglobulinemia (WM) patients naive to either agent., Experimental Design: Intended therapy consisted of 48 weeks of lenalidomide (25 mg/d for 3 weeks and then 1 week off) along with rituximab (375 mg/m(2)/wk) dosed on weeks 2 to 5 and 13 to 16. Sixteen patients were enrolled, 12 of whom were previously untreated., Results: Unexpectedly, we observed an acute decrease in hematocrit in 13 of 16 patients (median hematocrit decrease, 4.8%), which was attributable to lenalidomide patients and which led to cessation of further enrollment on this study. Lenalidomide-related anemia was observed even at doses as low as 5 mg/d and occurred in the absence of hemolysis or other cytopenias. The overall response and major response (<50% decrease in serum IgM) rates were 50% and 25%, respectively, on an intent-to-treat basis. With a median follow-up of 31.3 months, 4 of 8 responding patients have progressed with a median time to progression of 18.9 months., Conclusion: Lenalidomide produces unexpected but clinically significant acute anemia in patients with WM. In comparison with our previous study with thalidomide and rituximab in an analogous patient population, the responses achieved in WM patients with lenalidomide and rituximab appear less favorable.
