Today, the vast majority of medical treatments are for diseases of a chronic nature, in particular chronic pain. The treatment of chronic pain is costly not only in the United States but countries around the world adding up to billions of dollars for treatment costs as well as disability costs. There is an urgent need at this time to explore effective alternative treatments for chronic pain. A relatively unknown technique initially developed in Germany during the early 1900s known as neural therapy (NT) is emerging as a simple and effective treatment for chronic pain. NT is now gradually being adopted by medical communities throughout the world, and cases being described report many remarkable results. Neural therapy technique primarily involves the injection of local anesthetic into scars, trigger points, tendon and ligament insertions, peripheral nerves, autonomic ganglia, epidural space, and tissues. The mechanism of action calls upon the concept whereby each cell within the autonomic nervous system (ANS) is controlled via ubiquitous synapses occurring within the intracellular fluid (also known as the 'matrix'). Chronic pain results from long-term disruption/irritation of this complex system. NT generally uses non-anesthetic properties of local anesthetics to re-establish homeostasis throughout the autonomic nervous system. NT in its simplest form is intradermal injection of those scars, tattoos, or piercings which are believed to be causing an interference field or disruption of the autonomic nervous system. The local anesthetic is injected in such a way as to produce a linear wheal over the interference field of approx. 0.7 cc of solution per cm of scar. The location of the interference field can be in the vicinity of the patient's pain or in an entirely different location, and the therapeutic benefits range from gradual improvement after repeated treatments versus immediate complete relief of symptoms. Our center has witnessed numerous cases where this technique has led to dramatic improvement and often complete cure of a patient's long-term pain. Objective: To conduct a quality improvement evaluation of the Empower Veterans Program (EVP), an interdisciplinary pain rehabilitation/functional restoration program option for functional restoration for high-impact chronic pain, offered in a large metro-area Veterans Health Administration (VHA) system. Data Sources: VHA Corporate Data Warehouse electronic medical record data for patients treated by EVP between 2015 and 2019. Evaluation Design: This retrospective design first compared EVP patients considered engaged or not engaged in completing treatment in terms of demographic characteristics and post-treatment changes in clinical measures related to opioid use and mental health. We then compared mortality risk between matched groups of treated and untreated patients with chronic pain and concurrent opioid prescriptions using propensity score matching and Cox proportional hazards methods. 'Treated' in the matched groups was defined as any level of EVP participation (i.e., both engaged and not engaged). Data Collection/Extraction Methods: We first identified 1053 EVP patients with 1 year of pre-and post-treatment follow-time and determined their engagement level. From those with chronic pain and prescription opioids, we matched 237 EVP patients to 375 untreated patients. Principal Findings: Engaged patients (57.4% of treated patients), were somewhat older than the non-engaged (mean age 57.1 vs. 53.7, Cohen's D = 0.30), and achieved lower mean PHQ9 depression scores in the post-treatment year (9.2 vs. 10.6, Cohen's D = 0.20). Participation in EVP was associated with a 65% lower mortality risk among Veterans with chronic pain and opioid use when compared to the untreated patients: (HR: 0.35, 95% CI: 0.17, 0.75). Conclusions: EVP was associated with a large reduction in mortality risk for Veterans with both chronic pain and opioid use. This result could inform the decision process in a VA station or region when considering providing or expanding access to an interdisciplinary rehabilitation/functional restoration program for chronic pain. Chronic musculoskeletal pain is prevalent, challenging to treat, and often disabling. Objectives. Transcutaneous electrical nerve stimulation (TENS) is a nonpharmacological intervention that provides an electrical current through the skin to produce analgesia. The primary purpose of this study is to examine if the addition of TENS to routine physical therapy improves movement-evoked pain in individuals with fibromyalgia in a physical therapy clinical setting. Methods. Fibromyalgia TENS in Physical Therapy Study is a phase III embedded pragmatic clinical trial funded through the National Institutes of Health Helping to End Addiction Long-Term Initiative. This trial will utilize a randomized cluster design that includes more than 110 physical therapists in 24 to 30 physical therapy clinics within 6 health care systems and 7 states. Clinics will be randomized to TENS or No-TENS, stratified by health care system and clinic size. The plan is to enroll 600 participants, with all participants completing physical therapy as prescribed by their physical therapist. Participants at TENS clinics will utilize TENS for a minimum of 2-hour per day while at the physical therapy clinic and at home when active. The primary outcome is reduction in movement-evoked pain from baseline to day 60 on an 11-point numeric rating scale when participants sit and stand 5 times (Sit and Stand Test). Secondary outcomes include resting pain and fatigue, pain interference, fibromyalgia disease activity, movement-evoked fatigue, multidimensional assessment of fatigue, rapid assessment of physical activity, patient global impression of change, and common data elements shared across studies supported through the Helping to End Addiction Long-Term Initiative. Impact. The findings from this study will provide effectiveness data on TENS for individuals with fibromyalgia for health care policymakers, clinicians, and insurers. Data from this study will also inform future pragmatic trials for nonpharmacological interventions and chronic musculoskeletal pain conditions.
