1. Laryngeal Mask Airway Versus Endotracheal Intubation during Lacrimal Duct Stenosis Surgery in Children—A Retrospective Analysis

Abstract: Background: The use of laryngeal masks in the surgical treatment of infantile lacrimal duct stenosis is controversial due to the potential risk of aspiration. Aims: This study investigates airway procedures in children aged Methods: After institutional approval, airway procedures, duration of anesthesiological measures, and airway-related complications were retrospectively analyzed. Patients were divided into two groups according to the airway procedures used (endotracheal tube [ET] vs. laryngeal mask [LMA] airway). Associations were calculated using the Chi-square test or Mann-Whitney U-test. Results: Clinical data of 84 patients (ET n = 36 [42.9%] vs. LMA n = 48 [57.1%]) were analyzed. There were no significant differences in surgical treatment, age distribution, and pre-existing conditions between the groups. None of the patients showed evidence of tracheal aspiration or changes in measured oxygen saturation. LMA airway shortened time for anesthesia induction (p = 0.006) and time for recovery/emergence period (p = 0.03). In contrast, the time to discharge from the recovery room was significantly prolonged using LMA (p = 0.001). A total of 7 adverse events were recorded. Five of these were directly or indirectly related to ET (laryngo-/bronchospasm; muscle relaxant residual). Conclusions: LMA airway for infantile lacrimal duct stenosis seems to be a safe procedure and should be used in appropriate pediatric patients due to its lower invasiveness, low complication rate, and time savings.

Abstract: Background and Objectives: Pediatric extracorporeal membrane oxygenation (ECMO) support is often the ultimate therapy for neonatal and pediatric patients with congenital heart defects after cardiac surgery. The impact of lactate clearance in pediatric patients during ECMO therapy on outcomes has been analyzed. Materials andMethods: We retrospectively analyzed data from 41 pediatric vaECMO patients between January 2006 and December 2016. Blood lactate and lactate clearance have been recorded prior to ECMO implantation and 3, 6, 9 and 12 h after ECMO start. Receiver operating characteristic (ROC) analysis was used to identify cut-off levels for lactate clearance. Results: Lactate levels prior to ECMO therapy (9.8 mmol/L vs. 13.5 mmol/L; p = 0.07) and peak lactate levels during ECMO support (10.4 mmol/L vs. 14.7 mmol/L; p = 0.07) were similar between survivors and nonsurvivors. Areas under the curve (AUC) of lactate clearance at 3, 9 h and 12 h after ECMO start were significantly predictive for mortality (p = 0.017, p = 0.049 and p = 0.006, respectively). Cut-off values of lactate clearance were 3.8%, 51% and 56%. Duration of ECMO support and respiratory ventilation was significantly longer in survivors than in nonsurvivors (p = 0.01 and p < 0.001, respectively). Conclusions: Dynamic recording of lactate clearance after ECMO start is a valuable tool to assess outcomes and effectiveness of ECMO application. Poor lactate clearance during ECMO therapy in pediatric patients is a significant marker for higher mortality.

Abstract: Background and study aims The optimal approach to small subepithelial tumors (SETs) of the upper gastrointestinal tract remains inconclusive. The aim of this study was to evaluate endoscopic ultrasound-guided fine needle biopsy (EUS-FNB) for less invasive tissue sampling of small SETs of the upper gastrointestinal tract. Patients and methods In this prospective observational study patients with small ( ≤ 3 cm) SETs of the upper gastrointestinal tract were eligible and underwent EUS-FNB with a 22-gauge core biopsy needle. The main outcome measure was the diagnostic yield. The number of obtained core biopsies was also assessed. Results Twenty patients were included. The mean SET size was 16 mm (range 10 – 27 mm). EUS-FNB was technically feasible in all cases and no complications were observed. The diagnostic yield was 75 %. Core biopsy specimens were obtained in only 25 % of cases. Conclusion EUS-FNB with a 22-gauge core biopsy needle of small SETs can achieve a definite diagnosis in the majority of cases. However, because core samples cannot regularly be obtained, EUS-FNB seems not to be convincingly superior to standard EUS-FNA in this setting

Abstract: Background: Hypertrophic pyloric stenosis in otherwise healthy neonates frequently requires urgent surgical procedure but anaesthesia care may result in respiratory complications, such as hypoxaemia, pulmonary aspiration of gastric contents, and postoperative apnoea. The primary aim was to study whether or not the incidence of difficult airway management and of hypoxaemia in neonates undergoing pyloric stenosis repair was higher than that in neonates undergoing other surgeries. Methods: Data on neonates and infants undergoing anaesthesia and surgery for pyloric stenosis were extracted from the NEonate and Children audiT of Anesthesia pRactice In Europe (NECTARINE) database, for secondary analysis. Results: We identified 310 infants who had anaesthesia for surgery for pyloric stenosis. Difficult airway management (more than two attempts at laryngoscopy) was higher in children with pyloric stenosis when compared with the entire NECTARINE cohort (7.9% [95% confidence interval {CI}, 5.22–11.53] vs 4.4% [95% CI, 1.99–6.58]; relative risk [RR]=1.81 [95% CI, 1.21–2.69]; P=0.004), whereas transient hypoxaemia with oxygen saturation

5. Human metapneumovirus infection in the cardiac paediatric ICU before and during COVID-19 pandemic: a retrospective cohort analysis

Abstract: Introduction: This study investigates the hygiene standards in the context of the COVID-19 pandemic and their impact on the perioperative incidence of human metapneumovirus as well as the typical symptom burden of human metapneumovirus-infected children with CHDs. Materials and methods: Between March 2018 and July 2021, all patients of a cardiac paediatric ICU of a German university hospital were included in this retrospective cohort analysis. Results: A total of 589 patients with CHD were included in the analysis. Three hundred and fifty-two patients (148 females and 204 males) were admitted before the introduction of social distancing and face masks between March 2018 and 15 April 2020 (cohort A). Two hundred and thirty-seven patients (118 females and 119 males) were admitted after the introduction between April 16 and July 2021 (cohort B). In cohort A, human metapneumovirus was detected in 11 out of 352 patients (3.1%) during their stay at cardiac paediatric ICU. In cohort B, one patient out of 237 (0.4%) tested positive for human metapneumovirus. Patients who tested positive for human metapneumovirus stayed in cardiac paediatric ICU for a median of 17.5 days (range, 2–45 days). Patients without a detected human metapneumovirus infection stayed in the cardiac paediatric ICU for a median of 4 days (range, 0.5–114 days). Nine out of 12 (75%) human metapneumovirus-positive patients showed atelectasis. Conclusion: Perioperative human metapneumovirus infections prolong cardiac paediatric ICU stay in children with CHD. In affected patients, pulmonary impairment with typical symptoms appears. Under certain circumstances, a complication-rich perioperative infection with human metapneumovirus could be prevented in paediatric cardiac high-risk patients by prophylactic hygiene intervention.

6. Dexmedetomidine as a sole sedative for procedural sedation in preterm and neonate infants: A retrospective analysis

Abstract: Many different sedation concepts for magnetic resonance imaging have been described for prematurely and term-born infants, ranging from "no sedation" to general anesthesia. Dexmedetomidine is an alpha-2 receptor agonist that is frequently used to sedate older children, because the anesthesiologist can easily adjust sedation depth, the patient maintains spontaneous breathing, and awakens rapidly afterwards.The present study evaluates whether dexmedetomidine could safely be used as the sole sedative for prematurely and term-born infants less than 60 weeks postconceptional age undergoing diagnostic procedures.We performed a retrospective monocentric analysis of n = 39 prematurely and term-born infants (60 weeks postconceptional age or a body weight5 kg) who were sedated with dexmedetomidine for an MRI at a German university hospital from August 2016 to November 2018.Successful imaging was achieved in all cases. The median initial bolus of dexmedetomidine administered over 10 min was 1.39 μg kgThese results indicate that dexmedetomidine can be safely used for procedural sedation in the high-risk cohort of prematurely and term-born infants less than 60 weeks postconceptional age. Apnea during procedural sedation and subsequent stay in the recovery room is avoided, but bradycardia remains a relevant risk that may require treatment.

Abstract: Background: Neonates and infants are susceptible to hypoxaemia in the perioperative period. The aim of this study was to analyse interventions related to anaesthesia tracheal intubations in this European cohort and identify their clinical consequences. Methods: We performed a secondary analysis of tracheal intubations of the European multicentre observational trial (NEonate and Children audiT of Anaesthesia pRactice IN Europe [NECTARINE]) in neonates and small infants with difficult tracheal intubation. The primary endpoint was the incidence of difficult intubation and the related complications. The secondary endpoints were the risk factors for severe hypoxaemia attributed to difficult airway management, and 30 and 90 day outcomes. Results: Tracheal intubation was planned in 4683 procedures. Difficult tracheal intubation, defined as two failed attempts of direct laryngoscopy, occurred in 266 children (271 procedures) with an incidence (95% confidence interval [CI]) of 5.8% (95% CI, 5.1e6.5). Abstract: BACKGROUND Little is known about current clinical practice concerning peri-operative red blood cell transfusion in neonates and small infants. Guidelines suggest transfusions based on haemoglobin thresholds ranging from 8.5 to 12��g��dl-1, distinguishing between children from birth to day 7 (week 1), from day 8 to day 14 (week 2) or from day 15 (��week 3) onwards. OBJECTIVE To observe peri-operative red blood cell transfusion practice according to guidelines in relation to patient outcome. DESIGN A multicentre observational study. SETTING The NEonate-Children sTudy of Anaesthesia pRactice IN Europe (NECTARINE) trial recruited patients up to 60 weeks' postmenstrual age undergoing anaesthesia for surgical or diagnostic procedures from 165 centres in 31 European countries between March 2016 and January 2017. PATIENTS The data included 5609 patients undergoing 6542 procedures. Inclusion criteria was a peri-operative red blood cell transfusion. MAIN OUTCOME MEASURES The primary endpoint was the haemoglobin level triggering a transfusion for neonates in week 1, week 2 and week 3. Secondary endpoints were transfusion volumes, 'delta haemoglobin' (preprocedure - transfusion-triggering) and 30-day and 90-day morbidity and mortality. RESULTS Peri-operative red blood cell transfusions were recorded during 447 procedures (6.9%). The median haemoglobin levels triggering a transfusion were 9.6 [IQR 8.7 to 10.9] g��dl-1 for neonates in week 1, 9.6 [7.7 to 10.4] g��dl-1 in week 2 and 8.0 [7.3 to 9.0] g��dl-1 in week 3. The median transfusion volume was 17.1 [11.1 to 26.4] ml��kg-1 with a median delta haemoglobin of 1.8 [0.0 to 3.6] g��dl-1. Thirty-day morbidity was 47.8% with an overall mortality of 11.3%. CONCLUSIONS Results indicate lower transfusion-triggering haemoglobin thresholds in clinical practice than suggested by current guidelines. The high morbidity and mortality of this NECTARINE sub-cohort calls for investigative action and evidence-based guidelines addressing peri-operative red blood cell transfusions strategies. Abstract: Background/aimsPerioperative pain in children is often inadequately treated, and emergence agitation is common. The purpose of this analysis was to determine whether nalbuphine is suitable for perioperative eye pain and to analyse if it influences the occurrence of emergence delirium/agitation (EDA) in children undergoing ophthalmic surgery in general anaesthesia.MethodsRetrospective cohort analysis of 50 children in preschool age undergoing general anaesthesia for ophthalmic surgery receiving nalbuphine as a postoperative analgesic in a German university hospital from June 2020 to February 2021.Scores and values for pain and EDA were routinely recorded after awakening and during the stay in the recovery room. Data were evaluated retrospectively from the medical records.ResultsA total of 50 children (17 girls and 33 boys) underwent general anaesthesia for ophthalmic surgery. The median age of the children included was 20.5 months (range, 1–68 months), the median body weight was 12.25 kg (range, 2.9–29 kg). All patients received ibuprofen (10 mg/kg1) during induction of anaesthesia and nalbuphine (0.1 mg/kg) at the end of surgery. All patients had an Paediatric-Anaesthesia-Emergence-Delirium-I-score (PAED-ED-I Score) of less than 6 and acceptable Face-Legs-Activity-Cry-Consolability-scores (FLACC less than 3) on waking and on leaving the recovery room.ConclusionNalbuphine shows a sufficient analgesic effect for pain therapy following ophthalmic surgery in preschool children. Nalbuphine seems to reduce the incidence of EDA in children undergoing ophthalmic surgery.

Abstract: We demonstrate an optical phased-array equipped with a 3D-printed facet-attached element for shaping and deflection of the emitted beam. The beam shaper combines freeform refractive surfaces with total-internal-reflection mirrors and is in-situ printed to edge-emitting waveguide facets using high-resolution multi-photon lithography, thereby ensuring precise alignment with respect to on-chip waveguide structures. In a proof-of-concept experiment, we achieve a grating-lobe free steering range of ± 30 ∘ and a full-width-half-maximum beam divergence of approximately 2 ∘ . The concept opens an attractive alternative to currently used grating structures and is applicable to a wide range of integration platforms.

Abstract: Background: Neonates and infants requiring anaesthesia are at risk of physiological instability and complications, but triggers for peri-anaesthetic interventions and associations with subsequent outcome are unknown. Methods: This prospective, observational study recruited patients up to 60 weeks' postmenstrual age undergoing anaesthesia for surgical or diagnostic procedures from 165 centres in 31 European countries between March 2016 and January 2017. The primary aim was to identify thresholds of pre-determined physiological variables that triggered a medical intervention. The secondary aims were to evaluate morbidities, mortality at 30 and 90 days, or both, and associations with critical events. Results: Infants (n=5609) born at mean (standard deviation [SD]) 36.2 (4.4) weeks postmenstrual age (35.7% preterm) underwent 6542 procedures within 63 (48) days of birth. Abstract: Background:Congenital heart disease is the most frequent malformation in newborns. The postoperative mortality of these patients can be assessed with the Risk Adjustment in Congenital Heart Surgery-1 (RACHS-1) score. This study evaluates whether the RACHS-1 score can also be used as a predictor for the length of postoperative ventilation and what is the influence of age.Material and Methods:In a retrospective study over the period from 2007 to 2013, all patient records were evaluated: 598 children with congenital heart disease and cardiac surgery were identified and 39 patients have been excluded because of additional comorbidities. For evaluation of mortality, 559 patients could be analysed, after exclusion of 39 deceased patients, 520 cases remained for analysis of postoperative ventilation.Results:Overall mortality was 7% with a dependency on RACHS-1 categories. The median length of postoperative ventilation rose according to the RACHS-1 categories: RACHS-1 category 1: 9 hours (interquartile range (IQR) 7–13 hours), category 2: 30 hours (IQR 12–85 hours), category 4: 58 hours (IQR 13–135 hours), category 4: 71 hours (IQR 29–165 hours), and category 6: 189 hours (IQR 127–277 hours). Some of the RACHS-1 subgroups differed significantly from the categories, especially the repair of tetralogy of Fallot with a longer ventilation time and strong variability. Younger age was an independent factor for longer postoperative ventilation.Conclusion:RACHS-1 is a good predictor for the length of postoperative ventilation after cardiac surgery with the exception of some subgroups. Younger age is another independent factor for longer postoperative ventilation. These data provide better insight into ventilation times and allow better planning of operations in terms of available intensive care beds.

Abstract: We report two cases of neonates with complex congenital heart disease and volume, catecholamine, and corticosteroid refractory shock treated with arginine–vasopressin. Arginine–vasopressin was able to reverse critical hypotension, but both patients developed severe hyponatremia, which recovered after discontinuation of arginine–vasopressin. Close control and prompt substitution of serum sodium is required in neonates with advanced heart failure on high-dose vasopressin therapy.

Abstract: Objectives: Despite the increasing use of near-infrared spectroscopy across pediatric cardiac ICUs, there is significant variability and equipoise with no universally accepted management algorithms. We aimed to explore the use of near-infrared spectroscopy in pediatric cardiac ICUs in the United Kingdom, Ireland, Italy, and Germany. Design: A cross-sectional multicenter, multinational electronic survey of one consultant in each pediatric cardiac ICU. Setting: Pediatric cardiac ICUs in the United Kingdom and Ireland (n = 13), Italy (n = 12), and Germany (n = 33). Interventions: Questionnaire targeted to establish use, targets, protocols/thresholds for intervention, and perceived usefulness of near-infrared spectroscopy monitoring. Results: Overall, 42 of 58 pediatric cardiac ICUs (72%) responded: United Kingdom and Ireland, 11 of 13 (84.6%); Italy, 12 of 12 (100%); and Germany, 19 of 33 (57%, included all major centers). Near-infrared spectroscopy usage varied with 35% (15/42) reporting that near-infrared spectroscopy was not used at all (7/42) or occasionally (8/42); near-infrared spectroscopy use was much less common in the United Kingdom (46%) when compared with 78% in Germany and all (100%) in Italy. Only four units had a near-infrared spectroscopy protocol, and 18 specifically used near-infrared spectroscopy in high-risk patients; 37 respondents believed that near-infrared spectroscopy added value to standard monitoring and 23 believed that it gave an earlier indication of deterioration, but only 19 would respond based on near-infrared spectroscopy data alone. Targets for absolute values and critical thresholds for intervention varied widely between units. The reasons cited for not or occasionally using near-infrared spectroscopy were expense (n = 6), limited evidence and uncertainty on how it guides management (n = 4), difficulty in interpretation, and unreliability of data (n = 3). Amongst the regular or occasional near-infrared spectroscopy users (n = 35), 28 (66%) agreed that a multicenter study is warranted to ascertain its use. Conclusions: Although most responding units used near-infrared spectroscopy for high-risk patients, the majority (31/35 [88%]) did not have any protocols or guidelines for intervention. Target thresholds and intervention algorithms are needed to support the use of near-infrared spectroscopy in pediatric cardiac ICUs; an international multicenter study is warranted. Abstract: This paper presents a case study on integrating a new model extension describing the production of the enzyme soluble hydrogenase (SH) into an already existing model by means of on-line optimal experimental design (OED). These investigations were done on the autotrophic cultivation system and process model of Ralstonia eutropha, i.e., a cultivation where aeration by air is replaced by a gas consisting of H2/O2/CO2. Prior to the experiment, three different structures of the new model extension are postulated and coarsely identified. Off-line OED with the best model yields initial feeding trajectories. During the experiment, a repetitive model refinement is performed. This consists of identifying the parameters of the new model extension for all three models, selecting the best model and recalculating optimal feeding trajectories. It is shown that on-line OED in the early modelling stage followed by a manual modelling step results in an acceptable quality of the description of SH production.

Abstract: Plasmonic nanorings provide the unique advantage of a pronounced plasmonic field enhancement inside their core. If filled with a polarizable medium, it may significantly enhance its optical effects. Here, we demonstrate this proposition by filling gold nanorings with lithium niobate. The generated second harmonic signal is compared to the signal originating from an unpatterned lithium niobate surface. Measurements and simulation confirm an enhancement of about 20. Applications requiring nanoscopic localized light sources like fluorescence spectroscopy or quantum communication will benefit from our findings.

Abstract: In this work, we show local laser-induced heating in fluids with gold nanodot arrays prepared by electron-beam lithography that cover resonances in the near infrared spectral range from 750 nm to 880 nm. We utilize two approaches to demonstrate thermal effects, solvent evaporation and flow stop, with a thermosensitive polymer solution. We show that with fluences as low as 4 μJ cm−2, significant heating of the nanostructures occurs in their immediate vicinity. We perform power and wavelength dependent measurements to determine the threshold of the thermal effects. Using wavelengths about 20 nm away from the plasmonic resonance peak, the heating drops drastically, and 30 to 40 nm away, there is mostly no thermal effect. Therefore, working close to the threshold laser power offers the possibility of multiplexed reactions or sensing without cross-talk even though a typical full width at half maximum of a plasmonic resonance spectrum can be as broad as 200 nm. Abstract: An optical metamaterial, operating in between the MID/IR and the visible domain, which uses the effect of extreme coupling between closely spaced nanostructured gold metal plates and a planar gold substrate was realized by means of electron beam lithography and lift-off technique. This regime is only accessible by using an ultrathin oxide layer between metal plates and substrate, where the oxide layerisrealized by atomic layer deposition here. The involved fabrication is discussed in detail. The metamaterial shows the predicted effect of perfect absorption with multiple resonances in the spectral range from 100 THz to 600 THz. The plasmonic based resonance frequencies are very sensitive to the plate size of the grating and the spacer thickness.

Abstract: The high potential of bottom-up fabrication strategies for realizing sophisticated optical sensors combining the high sensitivity of a surface plasmon resonance with the exceptional properties of stimuli-responsive hydrogel is demonstrated. The sensor is composed of a periodic hole array in a gold film whose holes are filled with gold-capped poly(N-isoproyl-acrylamide) (polyNIPAM) microspheres. The production of this sensor relies on a pure chemical approach enabling simple, time-efficient, and cost-efficient preparation of sensor platforms covering areas of cm(2). The transmission spectrum of this plasmonic sensor shows a strong interaction between propagating surface plasmon polaritons at the metal film surface and localized surface plasmon resonance of the gold cap on top of the polyNIPAM microspheres. Computer simulations support this experimental observation. These interactions lead to distinct changes in the transmission spectrum, which allow for the simultaneous, sensitive optical detection of refractive index changes in the surrounding medium and the swelling state of the embedded polyNIPAM microsphere under the gold cap. The volume of the polyNIPAM microsphere located underneath the gold cap can be changed by certain stimuli such as temperature, pH, ionic strength, and distinct molecules bound to the hydrogel matrix facilitating the detection of analytes which do not change the refractive index of the surrounding medium significantly.

Abstract: Transition metal dichalcogenides (TMDs) are class of materials, which have recently attracted a high degree of scientific attention. In these materials, the adjacent crystal layers are mutually attached only by weak Van-der-Waals forces, enabling the preparation of TMD monolayers with thicknesses below one nanometer. Interestingly, TMDs, which are indirect semiconductors in their bulk form, become direct semiconductors in the monolayer limit. These monolayers show pronounced excitonic emission lines and a strong second-order nonlinearity, making TMDs very interesting for a variety of optical applications.

The proposed formalism can be applied to any material system, arbitrarily shaped metaatoms, at any frequency, and with arbitrary subwavelength periods. Circumventing the introduction of any kind of effective parameters we directly use the $S$ parameters of the individual metasurfaces to calculate the response of an arbitrary stack. In fact, the $S$ parameters are the complex parameters of choice fully characterizing the homogeneous metasurfaces, in particular with respect to its polarization manipulating properties. Just as effective material parameters like the permittivity and the permeability or wave parameters like the propagation constant and the impedance, the stacking based upon $S$ matrices can be applied as long as the individual layers are decoupled with respect to their near fields. This requirement eventually sets the limits for using the optical properties of the individual layers to calculate the response of the stacked system---this being the conceptual aim for any homogeneous metasurface or metamaterial layer and therefore the essence of what is eventually possible with homogeneous metasurfaces. As simple and appealing as this approach is, it is as powerful as well: Combining structured metasurfaces with each other as well as with isotropic, anisotropic, or chiral homogeneous layers is possible by simple semianalytical $S$-matrix multiplication. Hence, complex stacks and resonators can be set up, accurately treated and optimized with respect to their dispersive polarization sensitive optical functionality without the need for further rigorous full-wave simulations.

Abstract: We report on the investigation of an advanced circular plasmonic nanoantenna under ultrafast excitation using nonlinear photoemission electron microscopy (PEEM) under near-normal incidence. The circular nanoantenna is enhanced in its performance by a supporting grating and milled out from a gold film. The considered antenna shows a sophisticated physical resonance behavior that is ideal to demonstrate the possibilities of PEEM for the experimental investigations of plasmonic effects on the nanoscale. Field profiles of the antenna resonance for both possible linear polarizations of the incident field are measured with high spatial resolution. In addition, outward propagating Hankel plasmons, which are also excited by the structure, are measured and analyzed. We compare our findings to measurements of an isolated plasmonic nanodisc resonator and scanning near-field optical microscopy (SNOM) measurements of both structures. All results are in very good agreement with numerical simulations as well as analytial models that are also discussed in our paper.

Abstract: We determine the rigorously calculated scattered field of meta-atoms and decompose it into spherical harmonics with complex amplitudes. Transforming these spherical harmonics into a Cartesian base reveals all multipole moments in this coordinate system, i.e. all electric and magnetic dipole, quadrupole and any higher order moments can be completely determined. We show that these multipole moments provide a deeper understanding on how light interacts with the considered meta-atoms. After sketching the analytical framework, we investigate exemplarily two well-established meta-atoms and show the applicability of our approach.

Abstract: Atmospheric pressure matrix-assisted laser desorption/ionization (AP-MALDI) has proven a convenient and rapid method for ion production in the mass spectrometric (MS) analysis of biomolecules. AP-MALDI and electrospray ionization (ESI) sources are easily interchangeable in most mass spectrometers. However, AP-MALDI suffers from less-than-optimal sensitivity due to ion losses during transport from the atmosphere into the vacuum of the mass spectrometer. Here, we study the signal-to-noise ratio (S/N) gains observed when an on-chip dynamic pre-concentration/focusing approach is coupled to AP-MALDI for the MS analysis of neuropeptides and protein digests. It was found that, in comparison with conventional AP-MALDI targets, focusing targets showed (1) a sensitivity enhancement of approximately two orders of magnitude with S/N gains of 200-900 for hydrophobic substrates, and 150-400 for weak cation-exchange (WCX) substrates; (2) improved detection limits as low as 5 fmol/microL for standard peptides; (3) significantly reduced matrix background; and (4) higher inter-day reproducibility. The improved sensitivity allowed successful tandem mass spectrometric (MS/MS) sequencing of dilute solutions of a derivatized tryptic digest of a protein standard, and enabled the first reported AP-MALDI MS detection of neuropeptides from Aedes aegypti mosquito heads.

Analysis of these 10 candidates by tandem mass spectrometry enabled the determination of seven candidates as fragments from the precursors (I) ferritin heavy chain subunit, (II) HLA class II histocompatibility antigen, (III) vimentin, (IV) indoleamine 2,3-dioxygenase, (V and VI) pre-B cell enhancing factor (PBEF), and (VII) Interleukin-8 (CXCL8). Abstract: The excitation of surface plasmon polaritons in metallic nanostructures significantly enhances light-matter interactions at sub-wavelength scales. This enables novel optical effects that rely on artificial materials, so-called metamaterials. Metamaterials are made from densely packed and sufficiently small nanostructured unit cells. The purpose of metamaterials is to act comparable to bulk materials but with effective properties that can be tailored by the geometry of the unit cells. However, recently it became obvious that many appealing applications are already in reach by employing an ultrathin layer of metamaterial. Exploiting the metamaterials’ primary optical properties in the form of their dispersive complex reflection and transmission coefficients, single functional layers instead of bulk metamaterials are already sufficient for achieving sophisticated device properties. In particular, if the unit cells change across the functional layer, a new class of devices can be perceived that shape the light in the far-field according to predefined patterns. These metamaterial layers, usually referred to as metasurfaces, possess major advantages when compared to traditional optical elements. Here, we provide an overview of the burgeoning field of research that explores metasurfaces to affect an incident field spatially and spectrally in a deterministic way to enable functional diffractive optical elements.

Abstract: Native peptides and proteins are of increasing interest in biomedical research because they hold promise to represent a large number of useful diagnostic and therapeutic biomarkers. Discovery attempts from patient samples have to deal with the complexity of biology from a disease perspective as well as with a high individual variability. High throughput screening of samples is therefore the strategy of choice to detect relevant peptidic biomarkers, and requires a high order of automation particularly in the detection process. In this contribution, a novel technical approach employing a fully automated MALDI-TOF/TOF mass spectrometer is described. This approach combines high throughput biomarker discovery with the identification of corresponding endogenous peptides in one instrument and from the same set of samples. The degree of automation allows the analysis of thousands of chromatographic fractions corresponding to up to one hundred patient samples per day. The applied relative quantification via Differential Peptide Display((R)) is performed in a label-free way and shows a dynamic range of up to four orders of magnitude in the accessible peptide concentrations. The typical limit of detection is in the mid- to low-picomolar range for body fluids such as blood plasma, urine and cerebrospinal fluid. Sequence assignment via MALDI-TOF/TOF mass spectrometry is carried out either in an overview approach, characterizing rapidly the peptide composition e.g. of a novel sample, or in a directed approach, analyzing a list of biomarker candidates deduced from statistically significant abundance differences from the biomarker discovery process.

Abstract: Human blood plasma is a useful source of proteins associated with both health and disease. Analysis of human blood plasma is a challenge due to the large number of peptides and proteins present and the very wide range of concentrations. In order to identify as many proteins as possible for subsequent comparative studies, we developed an industrial-scale (2.5 liter) approach involving sample pooling for the analysis of smaller proteins (M(r) generally < ca. 40 000 and some fragments of very large proteins). Plasma from healthy males was depleted of abundant proteins (albumin and IgG), then smaller proteins and polypeptides were separated into 12 960 fractions by chromatographic techniques. Analysis of proteins and polypeptides was performed by mass spectrometry prior to and after enzymatic digestion. Thousands of peptide identifications were made, permitting the identification of 502 different proteins and polypeptides from a single pool, 405 of which are listed here. The numbers refer to chromatographically separable polypeptide entities present prior to digestion. Combining results from studies with other plasma pools we have identified over 700 different proteins and polypeptides in plasma. Relatively low abundance proteins such as leptin and ghrelin and peptides such as bradykinin, all invisible to two-dimensional gel technology, were clearly identified. Proteins of interest were synthesized by chemical methods for bioassays. We believe that this is the first time that the small proteins in human blood plasma have been separated and analyzed so extensively.

Abstract: The fundamental work and current knowledge on the mechanisms of matrix-assisted laser desorption ionization with pulsed infrared lasers is reviewed and discussed, with a special emphasis on the work performed in the laboratory of the authors in the recent past. The role of the different relevant irradiation parameters (laser wavelength, laser pulse duration, irradiated area and laser fluence) and matrix properties (absorption, state of aggregation) in the MALDI process is summarized as well as results on the dynamic parameters of the expanding desorption plume. New results include an extension of former wavelength studies to the 10 μm wavelength range. Furthermore, a detailed analysis of matrix ion signals generated at infrared laser wavelengths around 3 and 10 μm is presented and compared to those generated by UV-MALDI, revealing a complex picture of the hitherto hardly understood IR-MALDI ionization mechanisms. Preliminary results from a photoacoustic study of the desorption process point to a prolonged material evaporation in case of liquid matrices and pulsed Er:YAG laser excitation. The combined description of the various fundamental work promotes the understanding of the IR-MALDI desorption and ionization processes and allows an assessment of possible solutions for current mass spectrometric limitations of the IR-MALDI method.

Abstract: Carbon comes in many forms, ranging from sp-bonded metastable carbene chains, sp-bonded graphite, graphene, single-wall and multiwall nanotubes, fullerenes, and amorphous soot to sp-bonded diamond. In particular, carbon nanotubes (CNTs) exhibit unique electrical, magnetic, thermal, chemical, mechanical, and optical properties that stem from their tubular topology and reduced dimensionality. Topologically, a single-wall nanotube (SWNT) can be viewed as one sp-bonded graphene sheet rolled into a seamless tube. A sibling of the SWNT is the multiwalled nanotube (MWNT), made of a few to tens of concentric shells with random chiral registries, spaced by 0.34 nm, and coupled through van der Waals interactions as in graphite. In the past few decades, CNTs have been extensively studied from fundamental research and technological points of view.With a nm scale diameter and a large aspect ratio, CNTs are one of the closest material realizations of a 1-D system, offering unprecedented opportunities to study 1-D physics. Furthermore, CNTs are prime candidates for molecular electronics, one of the most promising directions of nanotechnology. The electrical properties of CNTs are sensitive not only to intrinsic parameters such as the number of shells, tube diameter, and chirality but also to extrinsic parameters such as defects, dopants, and the environment in which they reside [1]. For example, devices based on macroscopic forms of CNTs such as arrays, yarns, and films are gaining intensive interest. In these devices, the primary factor governing the macroscopic electrical transport is the micromorphology, comprised of three basic aspects: (1) the crystal chemistry of an individual CNT such as chirality, defects, and dopants, (2) the topology of the interconnected CNTs, and (3) the density and nature of inter-tube junctions. It is often the case that aspects (2) and (3) matter more than (1) in determining the electrical properties of a macroscopic CNT network. The control of these intrinsic and extrinsic parameters remains challenging and relies on techniques capable of tailoring CNT structures on the atomic, nm, and mm scales.

Abstract: The role of the laser pulse duration in matrix-assisted laser desorption/ionization mass spectrometry with infrared lasers (IR-MALDI-MS) emitting in the 3 microm wavelength range has been evaluated. Mass spectrometric performance and characteristics of the IR-MALDI process were examined by comparing a wavelength-tuneable mid-infrared optical parametric oscillator (OPO) laser of 6 ns pulse duration, tuned to wavelengths of 2.79 and 2.94 microm, with an Er:YAG laser (lambda = 2.94 microm) with two pulse durations of 100 and 185 ns, and an Er:YSGG laser (lambda = 2.79 microm) with a pulse duration of 75 ns. Threshold fluences for the desorption of cytochrome C ions were determined as a function of the laser pulse duration for various common IR-MALDI matrices. For the majority of these matrices a reduction in threshold fluence by a factor of 1.2-1.9 was found by going from the 75-100 ns long pulses of the Erbium lasers to the short 6 ns OPO pulse. Within the experimental accuracy threshold fluences were equal for the 100 and the 185 ns pulse duration of the Er:YAG laser. Some pronounced pulse duration effects related to the ion formation from a glycerol matrix were also observed. The effect of the laser pulse length on the duration of ion emission was furthermore investigated.

Abstract: We systematically study both experimentally and theoretically the links between the lattice symmetries of metasurfaces and their optical properties at both normal and oblique illumination. We attribute different symmetry elements to number of polarization phenomena. In particular, we predict analytically and verify experimentally the influence of rotational axes, mirror planes and inversion centres on optical activity, circular dichroism and asymmetric transmission. We fabricate and test nanostructured optical metasurfaces with four different inner structures: square and hexagonal lattices, quasicrystalline layout, and amorphous arrangement. We demonstrate the ability to enhance/suppress particular optical response by appropriate choice of the metasurface’s symmetry.

Abstract: The mechanisms of energy deposition in matrix-assisted laser desorption/ionization mass spectrometry with infrared lasers (IR-MALDI-MS) have been evaluated in a series of experiments. In a first part, the threshold fluences for the generation of IR-MALDI spectra were determined between 2.7 and 4.0 μm wavelength with an optical parametric oscillator as a tunable laser source for nine solid state and two liquid matrices of different chemical structure and compared to the infrared absorption spectra of the compounds. Preliminary spectra of IR-MALDI in the wavelength range of 1.45–1.75 μm are also presented using the overtone vibrations of a glycerol matrix. Matrices were chosen with regard to their IR-MALDI performance and to allow conclusions on the IR-absorption mechanisms. Whereas the wavelength dependence of the threshold fluence for non–hydrogen-bound C–H vibrations essentially follows the absorption spectrum of this functional group, strong discrepancies between the spectral dependence of threshold fluences and IR-absorption spectra were found for the vibrations of O–H and N–H groups around 3-μm wavelength that form strong intermolecular and intramolecular hydrogen bonds. In a second part, experiments are described that interrogate the two most probable mechanisms for the observed deviation of the threshold fluence behavior from the wavelength course of the IR-absorption spectra, that is, absorption by either free or weakened O–H and N–H stretching modes or by residual free water. All investigations were performed with glycerol and succinic acid as examples for common liquid and solid state matrices for IR-MALDI. For glycerol, a fluence-dependent, dynamic change in absorption during the laser pulse was revealed by laser transmission measurements on thin glycerol layers. This effect, characterized by a significant blue shift of the O–H stretch absorption, can be attributed to a weakening of intermolecular hydrogen bonds caused by the transient laser heating of the sample. Taking this effect into account, a good correspondence of the wavelength dependence of the threshold fluence with the infrared absorption under IR-MALDI conditions is derived for glycerol. For succinic acid, in contrast, the identification of the predominant absorption mechanism in the 3-μm wavelength range appears more difficult. A fluence-dependent absorption was not detected in laser transmission experiments with succinic acid single crystals. A change in analyte-to-matrix ratio, with the intention of inducing free absorbers near crystal defects, also did not influence the wavelength dependence of the threshold fluences. However, an influence of the surface-to-volume ratio on the wavelength-dependent threshold fluences was found by a comparison of three different preparation techniques for succinic acid, indicating a putative influence of weakly hydrogen-bound surface absorbers. In combination with the detailed analysis of the wavelength dependence of the threshold fluence given from the first part, a determination of the IR-MALDI process for succinic acid based on the absorption by weakly hydrogen-bound hydroxyl groups is suggested. No evidence for a significant contribution of residual free water absorption to the low-threshold fluences around 3-μm wavelength was found by monitoring a possible change in threshold fluence at the phase transition from water to ice and by reducing the analyte hydration and varying the water content in glycerol preparations. Also, in preparations with frozen hydrated proteins without organic matrices, the wavelength dependence of the threshold fluence did not reflect the spectral absorption of ice, supporting the assumption of a rather minor role of the absorption by residual water in IR-MALDI.

Abstract: The dependence of the signal intensity of analyte and matrix ions on laser fluence was investigated for infrared matrix-assisted laser desorption/ionization (IR-MALDI) mass spectrometry using a flat-top laser beam profile. The beam of an Er : YAG laser (wavelength, 2.94 microm; pulse width, 90 ns) was coupled into a sapphire fiber and the homogeneously illuminated end surface of the fiber imaged on to the sample by a telescope. Three different laser spot sizes of 175, 350 and 700 microm diameter were realized. Threshold fluences of common IR matrices were determined to range from about 1000 to a few thousand J m(-2), depending on the matrix and the size of the irradiated area. In the MALDI-typical fluence range, above the detection threshold ion signals increase strongly with fluence for all matrices, with a dependence similar to that for UV-MALDI. Despite the strongly different absorption coefficients of the tested matrices, varying by more than an order of magnitude at the excitation laser wavelength, threshold fluences for equal spot sizes were found to be comparable within a factor of two. With the additional dependence of fluence on spot size, the deposited energy per volume of matrix at threshold fluence ranged from about 1 kJ mol(-1) for succinic acid to about 100 kJ mol(-1) for glycerol.

Abstract: Background: Bacteriorhodopsin (bR) from Halobacterium salinarum is a proton pump that converts the energy of light into a proton gradient that drives ATP synthesis. The protein comprises seven transmembrane helices and in vivo is organized into purple patches, in which bR and lipids form a crystalline two-dimensional array. Upon absorption of a photon, retinal, which is covalently bound to Lys216 via a Schiff base, is isomerized to a 13- cis ,15- anti configuration. This initiates a sequence of events – the photocycle – during which a proton is transferred from the Schiff base to Asp85, followed by proton release into the extracellular medium and reprotonation from the cytoplasmic side. Results: The structure of bR in the ground state was solved to 1.9 A resolution from non-twinned crystals grown in a lipidic cubic phase. The structure reveals eight well-ordered water molecules in the extracellular half of the putative proton translocation pathway. The water molecules form a continuous hydrogen-bond network from the Schiff-base nitrogen (Lys216) to Glu194 and Glu204 and includes residues Asp85, Asp212 and Arg82. This network is involved both in proton translocation occurring during the photocycle, as well as in stabilizing the structure of the ground state. Nine lipid phytanyl moieties could be modeled into the electron-density maps. Matrix-assisted laser desorption/ionization mass spectrometry (MALDI-MS) analysis of single crystals demonstrated the presence of four different charged lipid species. Conclusions: The structure of protein, lipid and water molecules in the crystals represents the functional entity of bR in the purple membrane of the bacteria at atomic resolution. Proton translocation from the Schiff base to the extracellular medium is mediated by a hydrogen-bond network that involves charged residues and water molecules. Abstract: Understanding the impact of order and disorder is of fundamental importance to perceive and to appreciate the functionality of modern photonic metasurfaces. Metasurfaces with disordered and amorphous inner arrangements promise to mitigate problems that arise for their counterparts with strictly periodic lattices of elementary unit cells such as, e.g., spatial dispersion and allows the use of fabrication techniques that are suitable for large scale and cheap fabrication of metasurfaces. In this study, we analytically, numerically and experimentally investigate metasurfaces with different lattice arrangements and uncover the influence of lattice disorder on their electromagnetic properties. The considered metasurfaces are composed of metal-dielectric-metal elements that sustain both electric and magnetic resonances. Emphasis is placed on understanding the effect of the transition of the lattice symmetry from a periodic to an amorphous state and on studying oblique illumination. For this scenario, we develop a powerful analytical model that yields, for the first time, an adequate description of the scattering properties of amorphous metasurfaces, paving the way for their integration into future applications.
