Concerns were raised that domestic violence might increase because of the enacted infection control measures. Previous findings on this issue have been contradictory. Since existing studies mainly rely on official reports, administrative data, helpline calls, or retrospective measures, their findings are likely to prove unreliable. Few population-based surveys include pre-pandemic data, limiting their ability to test for causality regarding increasing violence. Therefore, the aim of this study was to compare findings from population-representative surveys on the prevalence of intimate partner violence (IPV) and violence against children (VAC) before and during the COVID-19 pandemic. Based on the data of N = 3,639 individuals living with a romantic partner and N = 1,313 parents living with at least one of their children from three German representative population surveys, we estimated average marginal effects for the temporal trends (i.e., pre vs. post infection control measures) of domestic violence separately for males and females. To minimize bias across survey waves, inverse probability weighting was used. Results show no statistically significant increase in either physical or psychological forms of IPV or VAC as a result of the implementation of COVID measures. On the contrary, the 1-year prevalence was decreasing for certain forms of violence. Our findings suggest that the assessment of the consequences of infection control measures needs an empirical basis. Further research should be conducted using high-quality data sources. Therefore, the present study should be considered a stepping stone for ongoing research efforts to examine the consequences of pandemic-related infection control measures on the general population.

ZusammenfassungNach einer Gewalttat entstehen bei Betroffenen nicht nur psychische und körperliche Schäden, sondern auch eine Beeinträchtigung in der beruflichen, sozialen und gesellschaftlichen Teilhabe. Bislang fehlen allerdings Studien zur Ausprägung der Teilhabebeeinträchtigung und prognostisch relevanter Merkmale. Anhand eines Kategoriensystems wurden aus Akten soziodemographische Angaben, trauma-, tat- und gesundheitsspezifische Merkmale erwachsener Opfer von Gewalttaten, deren Anspruch auf Leistungen nach dem Opferentschädigungsgesetz (OEG) anerkannt waren, erfasst. Basierend auf psychiatrischen Gutachten zur Schädigungsfolge wurde ferner die Teilhabebeeinträchtigung orientiert an der International Classification of Functioning, Disability and Health (ICF) post-hoc beurteilt. Die Daten beruhen auf 99 Akten; 72 der Betroffenen waren weiblich. Das durchschnittliche Alter der Opfer bei der Tat betrug 25 Jahre. 53 waren Opfer von Gewalttaten gegen die sexuelle Selbstbestimmung, 24 Opfer von Formen von Körperverletzung und 45 Opfer anderer Gewalttaten. 29 wurden Opfer mehrerer Arten von Gewalttaten. Bei allen Personen lag mindestens eine psychische Störung mit Krankheitswert vor. Zwischen dem ersten und zweiten Gutachten vergingen im Durchschnitt vier Jahre. Während 85 der Betroffenen angegeben hatten, vor der Tat berufstätig gewesen zu sein, waren dies nach der Tat noch 53. Die ausgeprägteste Beeinträchtigung der Teilhabefähigkeit zeigte sich in der beruflichen Teilhabe. Mithilfe einer multiplen Regressionsanalyse wurde deutlich, dass eine höhere Teilhabebeeinträchtigung mit einer fehlenden Berufstätigkeit und einem höheren Grad der Schädigung nach der Tat sowie der persönlichen Bekanntheit des Opfers mit dem Täter assoziiert war; nicht aber mit dem Vorliegen einer psychischen Störung oder das Fehlen vertrauensvoller Beziehungen des Opfers. Zukünftig sollte die Teilhabebeeinträchtigung regulär während der psychiatrischen Begutachtung ICF-basiert eingeschätzt werden. Die Ergebnisse sprechen dafür, dass insbesondere Leistungen zur beruflichen Teilhabe im Rahmen des OEGs zu gewähren sind.

Objective Anorexia nervosa (AN) and bulimia nervosa (BN) impose a significant financial burden and immense sufferings on affected individuals. Yet little is known about the differences between how each disorder affects males and females, respectively. Method We performed a retrospective cost-development analysis of anonymized claims data from the German statutory health-insurance system. Insured persons who suffered from an onset of AN (F50.0; N = 1,242 females and 71 males) or BN (F50.2; N = 1,104 females and 64 males) were analyzed for cost-of-illness over a 5-year period, beginning 2 years before the index diagnosis. Results In total, all groups incurred similar distributions of total costs over the 5-year observation period, with roughly 14,000-20,000 EUR median costs. About two-thirds of the total costs for females and males with AN are associated with mental illness, whereas for females and males with BN, this applies to approximately half the total costs. Analyses revealed differences between disorders and genders for single outcomes. AN is associated with a stronger increase in costs within a short period following onset and higher inpatient treatment costs, whereas BN entails more instances of incapacity to work before and after onset. Compared to females, males incurred lower costs in outpatient treatments. Discussion Our study adds evidence as to the disparities in health-care utilizations and costs over the course of illness, in outcome ratios, and between genders, for both AN and BN.

The quality of socio-medical expert opinions, which are prepared for the German Pension Insurance (GPI) in the context of applications for reduced earning capacity pension due to mental disorder, has hardly been investigated so far. The aim of the present study was to investigate to what extent expert medical reports on post-traumatic stress disorder (PTSD) comply with the guideline of the GPI on socio-medical reports for mental and behavioral disorders.To examine the quality of expert opinions, 52 socio-medical expert opinions in which a diagnosis of post-traumatic stress disorder (PTSD) was assigned and a performance capacity of less than three hours was determined were analyzed. The quality of the expert opinions was assessed both quantitatively by calculating quality points and qualitatively by analyzing the content. Percent agreement and Cohen's kappa were calculated for two raters to examine rater agreement based on a subsample (n=11).The analyzed appraisals scored an average of 36.1 (SD=7.4) out of a possible 92 quality points (range 22/56). In most of the expert opinions, the descriptions of participation limitations, epicrisis, and sociomedical conclusion were unsatisfactory.The inadequate presentation of the functional and participation limitations in the expert opinions may not do justice to the actual facts of the case, so that the evidence of a health disorder with the corresponding participation limitations cannot be provided beyond doubt. Accordingly, measures to improve the quality of expert opinions such as training and regular quality reviews are necessary.ZIEL: Die Qualität von sozialmedizinischen Gutachten, die im Rahmen von Anträgen auf Erwerbsminderungsrente wegen psychischer Störung für die Deutsche Rentenversicherung (DRV) angefertigt werden, ist bislang kaum untersucht worden. In der vorliegenden Studie sollte untersucht werden, inwieweit fachärztliche Gutachten zur Posttraumatischen Belastungsstörung (PTBS) der Leitlinie der DRV zu sozialmedizinischen Gutachten bei psychischen und Verhaltensstörungen entsprechen.Zur Überprüfung der Gutachtenqualität wurden 52 sozialmedizinische Gutachten, in denen die Diagnose einer Posttraumatischen Belastungsstörung (PTBS) vergeben und ein Leistungsvermögen von unter drei Stunden festgestellt wurde, analysiert. Die Gutachtenqualität wurde sowohl quantitativ mit der Berechnung von Qualitätspunkten, als auch qualitativ durch inhaltliche Analysen erfasst. Zur Überprüfung der Beurteiler_innenübereinstimmung wurde die prozentuale Übereinstimmung und Cohens Kappa für zwei Rater_innen anhand einer Teilstichprobe (n=11) berechnet.Die analysierten Gutachten erreichten durchschnittlich 36,1 (SD=7,4) von 92 möglichen Qualitätspunkten (Range 22/56). In den meisten Gutachten waren die Beschreibungen der Teilhabeeinschränkungen, der Epikrise und der sozialmedizinischen Schlussbildung unbefriedigend. Die unzureichende Darstellung der Funktions- und Teilhabeeinschränkungen in den Gutachten wird möglicherweise dem tatsächlichen Sachverhalt nicht gerecht, so dass der Beweis einer Gesundheitsstörung mit den entsprechenden Leistungseinschränkungen nicht zweifelsfrei erbracht werden kann. Dementsprechend sind Maßnahmen zur Verbesserung der Gutachtenqualität wie Schulungen und regelmäßige Qualitätsüberprüfungen notwendig.

Return-to-work self-efficacy (RTW-SE) is an important predictor of the duration until employees return to work after a sickness-related absence. The aim of the present validation study was to investigate the psychometric properties of the German RTW-SE scale. Data were obtained from three independent samples of employees who were in outpatient care due to mental disorders or musculoskeletal diseases (n1 = 301, n2 = 103, n3 = 104). Confirmatory factor analyses showed an inadequate fit for a one-factor solution and an acceptable fit for a two-factor model that distinguished by item-wording direction. To test whether the two factors represent substantively different dimensions of the construct or rather a statistical item-wording effect, two subscales were formed based on item-wording direction. As the subscales were not differentially associated with external measures, the one-factor solution may be considered appropriate. The scale showed good to excellent internal consistency values over time and across samples, had low retest reliability indices, and indicated construct validity based on moderate to high associations with cognitive and disease-related variables. The results further demonstrated the scale’s sensitivity to change. The RTW-SE baseline score predicted physical performance and pain-related psychological impairment after orthopedic rehabilitation. In multiple regression analysis, RTW-SE remained a significant predictor of pain-related psychological impairment but not physical performance, partially demonstrating the predictive validity of the scale. The German version of the RTW-SE scale demonstrated satisfactory results regarding its validity and reliability.

ZusammenfassungZwar besteht Übereinstimmung darüber, dass die gestörte Emotionsregulation ein zentrales Merkmal der Borderline-Persönlichkeitsstörung (BPS) ist, doch gibt es anhaltend weiterhin konkurrierende Ansichten zur dimensionalen Struktur der Störung. Die „Skala zur Erfassung der Impulsivität und emotionalen Dysregulation“ (IES-27) wurde bisher erfolgreich bei PatientInnen mit BPS eingesetzt. Das Ziel der vorliegenden Studie war Hinweise auf die zugrundeliegende dimensionale Struktur mithilfe konfirmatorischer Faktorenanalysen zu gewinnen. Eine heterogene Stichprobe von PatientInnen mit BPS (n=484), Aufmerksamkeits-Defizits-(Hyperaktivitäts-)Störung (n=191) und Essstörungen (n=197) einer psychosomatischen Klinik erhielt mittels computergestützter Testung verschiedene Selbstbeurteilungsinstrumente. Es zeigte sich wie vermutet ein Bi-Faktor-Modell mit einem generellen Faktor und den 3 spezifischen Faktoren „Emotionale Dysregulation“, „Beziehungsschwierigkeiten“ und „Suizidales und selbstverletzendes Verhalten“ überlegen (ΔCFI=0,032). Die interne Konsistenz der Gesamtstichprobe nahm Werte zwischen ω=0,83−0,93 an. Es zeigten sich erwartungsgemäße Zusammenhänge zu konstruktnahen Subskalen. Die psychometrischen Gütekriterien der IES-27 konnten damit bestätigt werden. Die Bildung von Subskalen erlaubt eine differenzierte Darstellung und damit eine spezifischere Behandlungsplanung.

ZusammenfassungBei der weiblichen Genitalverstümmelung (englisch female genital mutilation, FGM) werden die äußeren Genitalien ohne medizinischen Grund und ohne Mitspracherecht der betroffenen Mädchen beschädigt oder entfernt. Häufig geht diese Prozedur mit erheblichen Konsequenzen für die physische und psychische Gesundheit einher. Rekonstruktive chirurgische Maßnahmen können die Beschwerden der Betroffenen lindern. Anhand einer explorativen Untersuchung von N=97 Frauen, die an einem medizinischen Beratungsgespräch zur rekonstruktiven Operation teilnahmen, wurden die besonderen Charakteristika dieser Stichprobe deskriptiv analysiert, Gründe der Inanspruchnahme einer rekonstruktiven Operation erfragt, sowie das Wissen der Patientinnen über ihren Beschneidungstyp mit der medizinischen Diagnose verglichen. Bei 56,7% (n=55) der Untersuchten lag der Verdacht einer PTBS vor. Keine Schmerzen mehr zu haben war der am häufigsten genannte Grund für eine Operation (45,4%; n=44). Die Übereinstimmung zwischen dem Wissen der Patientinnen über den Beschneidungstyp und dem ärztlichen Urteil fiel gering aus (κ=0,09). Bei physischer und psychischer Belastung sollten Frauen nach FGM neben chirurgischen Maßnahmen auch psychotherapeutisch betreut werden. Psychotherapeutische Interventionen im Kontext von FGM müssen evaluiert werden. Längsschnittstudien sind hierfür unabdingbar.

Introduction: The long-term impact and cost-effectiveness of weight management interventions depends on post-treatment weight maintenance. There is growing evidence that interventions based on third-wave cognitive behavioural therapy (3wCBT), specifically Acceptance and Commitment Therapy (ACT), could improve long-term weight management. However these interventions are typically delivered face-to-face by clinical psychologists, which limits their scalability. We used an evidence-, theory-and person-based approach to develop an ACT-based intervention for weight-loss maintenance that uses digital technology and non-specialists to minimise resources needed for delivery at scale. Methods: Intervention development was guided by the Medical Research Council framework for the development of complex interventions in healthcare, Intervention Mapping Protocol, and the person-based approach for enhancing the acceptability and feasibility of interventions. Two phases of work were conducted: phase one consisted of collating and analysing existing and new primary evidence, and phase two consisted of theoretical modelling and intervention development, including development of guiding principles, a logic model and the intervention design and content. Target user and stakeholder panels were established to inform each phase of development. Results: This process resulted in a guided self-help intervention called SWiM (Supporting Weight Management). SWiM is a 4-month programme, consisting of weekly web-based sessions for 13 consecutive weeks, followed by a 4-week break for participants to reflect and practice their new skills, and a final session at week 17. Each session consists of psycho-educational content, reflective exercises, and behavioural experiments. SWiM includes specific sessions on key determinants of weightloss maintenance, including developing skills to identify and manage high-risk situations for lapses, creating new helpful habits, breaking old unhelpful habits, and learning to manage interpersonal relationships and their impact on weight-management. A trained, non-specialist coach supports participants through the programme with four scheduled 30-minute telephone calls and three further optional calls, which are guided by scripts. Conclusions: This comprehensive and iterative approach facilitated the development of an intervention that is based on scientific theory and evidence to support people with weight loss maintenance, and is grounded in the experiences of the target users and the context in which the intervention is intended to be delivered. The development of SWiM is prudent given the novel coronavirus pandemic, and the increased need for remote delivery of weight management interventions. The intervention will be refined based on findings of a planned pilot randomised controlled trial.

Sexual contact patterns determine the spread of sexually transmitted infections and are a central input parameter for mathematical models in this field. We evaluated the importance of country-specific sexual contact pattern parametrization for high-income countries with similar cultural backgrounds by comparing data from two independent studies (HaBIDS and SBG) in Germany, a country without systematic sexual contact pattern data, with data from the National Survey of Sexual Attitudes and Lifestyles (Natsal) in the UK, and the National Survey of Family Growth (NSFG) in the US, the two longest running sexual contact studies in high-income countries. We investigated differences in the distribution of the reported number of opposite-sex partners, same-sex partners and both-sex partners using weighted negative binomial regression adjusted for age and sex (as well as stratified by age). In our analyses, UK and US participants reported a substantially higher number of lifetime opposite-sex sexual partners compared to both German studies. The difference in lifetime partners was caused by a higher proportion of individuals with many partners in the young age group (

Around 200 million women and girls worldwide are affected by female genital mutilation/cutting (FGM/C). FGM/C is a procedure that harms or alters the external female genitals for non-medical reasons, and is usually performed on children. Often, this procedure leads to severe consequences for the women’s physical and mental health. In a cross-sectional sample of 112 women seeking medical counseling, physical and mental health characteristics associated with FGM/C were examined and possible predictors for the development of post-traumatic stress disorder (PTSD) in women affected by FGM/C were identified. A total of 55.4% (n = 66) of the women reported symptom levels of probable PTSD. Predictors for higher PTSD symptomology were an older age at the time of the FGM/C procedure, feelings of guilt and the centrality of the event in the woman’s life. Thus, cognitive–emotional processing was found to play an important role in the emergence of PTSD in women suffering from FGM/C. However, interventions taking into consideration these characteristics are mostly lacking and need to be investigated further in the context of FGM/C.

Objective The aims of this study were to estimate the value a population-representative sample places on electroconvulsive therapy (ECT) through willingness to pay (WTP) and to assess the effects of individual characteristics on WTP for ECT. Methods A German population-representative sample of 518 was presented with a hypothetical health loss scenario of depression and was asked to indicate WTP for ECT. Probit and quantile regression were used to estimate the effects of individual characteristics on the probability of stating a positive WTP and on the amount of money respondents were willing to pay. Results Two thirds of respondents stated that they had no knowledge about ECT. Most (56.5%) respondents indicated zero WTP for health gains through ECT treatment. Mean WTP was €5201 ($5612); median WTP was €1000 ($1079). Respondents' monthly household income had a significant effect on the probability of stating a positive WTP. Assessing WTP above zero, income showed a significant positive effect, whereas a higher score of depressive complaints showed a significant negative effect on the amount respondents were willing to pay. Conclusion Knowledge about ECT treatment is particularly low in the German public.

Background Post-traumatic stress disorders (PTSD) pose a high burden for individuals and societies. Although prevalence rates are rather low, high co-occurrence rates and overall impairments cause deleterious suffering and significant costs. Still, no long-term data on costs and trends in cost developments are available. Methods Claims data from a German research database were analysed regarding direct and indirect costs occurring for individuals with incident diagnoses of PTSD. Results were compared to non-exposed average insurants matched on age and gender. Costs were analysed over a 5-year period from 2 years preceding until 3 years following an incident diagnosis of PTSD. Results Overall costs for PTSD account for approximately 43,000 EUR per individual, which is three times higher than costs for non-exposed controls. Of these costs, 59% are caused by mental disorders, 18% specifically by PTSD. In the control group, costs for mental disorders account for 19% of total costs. Costs increase by 142% in the year after an incident diagnosis of PTSD but return to the initial level 2 years later. Still, costs are at least twice as high in every year as in those for the comparison group. Conclusions Individuals with PTSD seem to suffer from far more impairments in their general health conditions and incur many more costs than average insurants. Most of these seem to be caused by co-occurring mental disorders and show their maximum in the index year. Nevertheless, as costs decrease to their initial level, treatments seem to have counterbalanced the impairments due to PTSD. Thus, treatments for PTSD can be considered as beneficial and their cost-effectiveness should be further investigated.

ZusammenfassungBislang existiert kein deutschsprachiges Instrument zur Ermittlung von einsatzbezogener Belastung bei Mitgliedern der Freiwilligen Feuerwehr (MFF). Das Ziel der vorliegenden Studie war die Evaluation des Sources of Occupational Stress in Firefighters & Paramedics (SOOS-11). In Zusammenarbeit mit der Feuerwehr-Unfallkasse Niedersachsen wurden MFF zu 3 verschiedenen Messzeitpunkten (MZP) befragt. Beim ersten MZP (November 2016–Februar 2017) handelte es sich um eine Fragebogenerhebung, die diverse Selbstbeurteilungsinstrumente umfasste (N=475). Am zweiten MZP (Mai 2017 – November 2017) nahmen N = 117 MFF (davon n=55 vom ersten MZP) an einem Interview teil, das die Module des Strukturierten Klinischen Interviews für DSM-IV (SKID-I; Wittchen, Wunderlich, Gruschwitz & Zaudig, 1997) zur Erfassung einer Depression und Posttraumatischen Belastungsstörung beinhaltete. Im Zuge des dritten MZP (November 2017–Mai 2018) wurden von einer Teilstichprobe des zweiten MZP (n=58) verschiedene Selbstbeurteilungsinstrumente erneut beantwortet. Es konnten für den SOOS-11 zufriedenstellende Werte zur Internen Konsistenz (α=0,73–0,76) geschätzt werden. Die 6-Monats-Test-Retest-Reliabilität betrug r=0,60. Eine exploratorische sowie eine konfirmatorische Faktorenanalyse ergaben für eine ein – und auch eine 2-faktorielle Struktur des Fragebogens einen adäquaten Modell-Fit. Hoch signifikante Korrelationen ergaben sich zwischen der Ausprägung des Wohlbefindens, dem Organisationsklima und dem SOOS-11. Somit stellt der SOOS-11 eine gute Möglichkeit dar, einsatzbezogene Belastung von MFF zu messen. Der Fragebogen sollte noch an einer weiteren Stichprobe, wie bspw. der Berufsfeuerwehr, erprobt werden.

ZusammenfassungDie kognitive Verhaltenstherapie einschließlich Exposition und Reaktionsmanagement gilt bei Zwangsstörungen als Psychotherapie der ersten Wahl. In dieser Studie wurde der Therapieerfolg von 799 Patienten im Rahmen einer stationären kognitiv-behavioralen Psychotherapie untersucht. Als zentrales Erfolgsmaß wurde die Yale-Brown Obsessive Compulsive Scale (Y-BOCS) eingesetzt. Der Therapieerfolg wurde in Effektstärken (ES) sowie in Remissions- und Responseraten ausgedrückt. Im Prä-Post-Vergleich der Y-BOCS betrug die ES 1,32. Response wurde nach 2 verschiedenen Definitionen klassifiziert: dem Reliabel Change Index (RCI) und der prozentualen Symptomverbesserung (PSV). Die Responseraten betrugen 75,5% (RCI) bzw. 37% (PSV). Remission erreichten 39,3% nach RCI und 32,2% nach PSV. Von den eingeschlossenen Patienten beendeten 10,4% die Therapie vorzeitig. Das Abbruchrisiko war bei Männern sowie bei Patienten, die zusätzlich mit Neuroleptika behandelt wurden, erhöht. Der negative Einfluss von Neuroleptika auf den Therapieverlauf legt nahe, dass eine entsprechende Medikation im Rahmen der Expositionsbehandlung kritisch überprüft werden sollte.

Abstract. Spousal forgiveness is positively linked to aspects of relationship satisfaction, as well as to the physical and psychological health of both partners. Oftentimes, the respective values for men and women are compared under the untested assumption of measurement invariance of the assessment tool. In order to investigate the factorial invariance (configural, metric, scalar, and residual) of the German version of the Marital Offence-Specific Forgiveness Scale (MOFS; Paleari et al., 2009 ), we conducted several multigroup confirmatory factor analyses. In the context of comparing increasingly stringent models, tests of measurement invariance indicated at least partial measurement invariance. These findings suggest that the MOFS operates in a similar fashion among men and women, thus supporting the assessment of spousal forgiveness with the German MOFS.

Religiositat gilt in der Delinquenzforschung als ein protektiver Faktor. Jedoch fanden kurzlich erschienene Studien widerspruchliche Ergebnisse bezuglich des Einflusses von Religiositat auf deutsche Muslime, sodass unklar bleibt, ob muslimische Religiositat mit verminderter oder erhohter Delinquenz assoziiert ist. Es stellt sich die Frage, inwiefern individuelle religiose Praktiken, soziale religiose Praktiken, religioser Fundamentalismus und wahrgenommene religiose Diskriminierung delinquentes Verhalten von Muslimen pradizieren. Zur Beantwortung dieser Frage wurden Daten von 761 muslimischen Schulern, bereitgestellt vom Institut fur Interdisziplinare Konflikt- und Gewaltforschung, per Korrelations- und Regressionsanalysen ausgewertet. Es zeigt sich, dass religioser Fundamentalismus und wahrgenommene religiose Diskriminierung hohere Delinquenzraten pradizieren, dass soziale religiose Praktiken nicht signifikant mit Delinquenz zusammenhangen, und dass individuelle religiose Praktiken niedrigere Delinquenzraten vorhersagen. Somit weisen verschiedene Aspekte muslimischer Religiositat differenzielle Zusammenhange zu delinquentem Verhalten auf. Eine mogliche Erklarung fur die widerspruchlichen Ergebnisse vorangegangener Studien ist, dass Religiositat, je nachdem welcher der oben genannten Aspekt uberwiegt, protektive oder kriminogene Effekte ausuben kann. Die Berucksichtigung dieser differenziellen Rolle der Religiositat kann zur Verbesserung der Kriminalpravention und -prognose, insbesondere bei Muslimen, beitragen.

There is a high prevalence of posttraumatic stress disorder (PTSD) in the refugee population. In order to identify affected individuals and offer targeted help, there is an urgent need for easily understandable, reliable, valid, and efficient screening measures. The aim of the present study was to compare the diagnostic efficiency of the Process of Recognition and Orientation of Torture Victims in European Countries to Facilitate Care and Treatment (PROTECT) questionnaire (PQ) to that of the eight-item short-form Posttraumatic Diagnostic Scale (PDS-8) and the Patient Health Questionnaire (PHQ-9). Using structured clinical interviews, the prevalence rates of PTSD and major depression episode (MDE) were assessed in a refugee sample (N = 118), and receiver operating characteristic analyses were determined and compared. Of participants in the sample, 29.7%, 95% CI [22.0%, 38.5%], were diagnosed with PTSD and 33.1%, 95% CI [24.4%, 41.9%], were diagnosed with MDE. The area under the curve (AUC) for all measures was moderate, AUCs = 0.79-0.86; hence, measures did not differ in terms of their discriminatory abilities. Using the favored cutoff points, sensitivity and specificity were 80-97% and 60-70%, respectively. In terms of their discriminatory abilities, none of the investigated measures can be favored more than the others. Thus, for detection of these two disorders, the shorter PQ could be more efficient. Because the high co-occurrence of PTSD and MDE might limit the explanatory power of results in the present study, the findings should be cross-validated in the future.

Purpose According to estimations of the World Health Organization, depressive disorders, and cardiovascular disease will be the leading causes for global burden of disease in 2030. The aim of the present study was to estimate the value a representative sample of the German population places on quality-adjusted life years (QALYs) for depressive disorders compared to heart disease. Methods A representative sample of N = 967 of the German general public was randomly presented with one of two hypothetical health-loss scenarios: One version of the questionnaire presented respondents with health loss due to depression, while the other version dealt with health loss due to experiencing a heart disease. Respondents were asked to indicate their willingness to pay (WTP) for four hypothetical health-gain scenarios with different treatment options. Results In the depression questionnaire median WTP values ranged from 1000 to 1500 EUR; in the heart disease questionnaire from 1000 to 2000 EUR. Results of the Mann–Whitney U-Test and Median Test indicate higher WTP values for heart disease compared to depressive disorders when QALY gains were minor and stretched over a long period of time, and when treatment with bypass operation (rather than treatment with ECT) was offered. Zero WTP was significantly higher in all scenarios of the depression questionnaire in comparison to the hearth disease questionnaire. Conclusion Results indicate that respondents valued the necessity of paying for treatment higher when presented with heart disease compared to depression.

There is a growing consensus that emotion dysregulation is the defining feature of borderline personality disorder, but there are also persisting competing views on the pathology and dimensional structure of the disorder. The "Impulsivity and Emotion Dysregulation Scale" (IED-27) is already being used for assessing patients with borderline personality disorder. The particular aim of this study was to gain evidence of the dimensional structure of the disorder by confirmatory factorial analysis. A heterogeneous sample of patients with borderline personality disorder (n=484), attention deficit (hyperactivity) disorder (n=191) and eating disorders (n=197) of a psychosomatic clinic were given general and specific self-assessment inventories via computer-assisted testing system, including the IED-27. Results showed a bifactor model with a general factor and three specific factors: "emotional dysregulation", "relationship difficulties" and "self-injuries and suicidal behaviours", to be the model with the best fit (ΔCFI=0,032). The internal consistency of the sample seems to be good (ω=0,83-0,93) and expected correlations to similar subscales could be proved. Good psychometric criteria of the IED-27 were replicated in this study. Prospectively, the subscales allow a more detailed representation and specific treatment planning.Zwar besteht Übereinstimmung darüber, dass die gestörte Emotionsregulation ein zentrales Merkmal der Borderline-Persönlichkeitsstörung (BPS) ist, doch gibt es anhaltend weiterhin konkurrierende Ansichten zur dimensionalen Struktur der Störung. Die „Skala zur Erfassung der Impulsivität und emotionalen Dysregulation“ (IES-27) wurde bisher erfolgreich bei PatientInnen mit BPS eingesetzt. Das Ziel der vorliegenden Studie war Hinweise auf die zugrundeliegende dimensionale Struktur mithilfe konfirmatorischer Faktorenanalysen zu gewinnen. Eine heterogene Stichprobe von PatientInnen mit BPS (n=484), Aufmerksamkeits-Defizits-(Hyperaktivitäts-)Störung (n=191) und Essstörungen (n=197) einer psychosomatischen Klinik erhielt mittels computergestützter Testung verschiedene Selbstbeurteilungsinstrumente. Es zeigte sich wie vermutet ein Bi-Faktor-Modell mit einem generellen Faktor und den 3 spezifischen Faktoren „Emotionale Dysregulation“, „Beziehungsschwierigkeiten“ und „Suizidales und selbstverletzendes Verhalten“ überlegen (ΔCFI=0,032). Die interne Konsistenz der Gesamtstichprobe nahm Werte zwischen ω=0,83−0,93 an. Es zeigten sich erwartungsgemäße Zusammenhänge zu konstruktnahen Subskalen. Die psychometrischen Gütekriterien der IES-27 konnten damit bestätigt werden. Die Bildung von Subskalen erlaubt eine differenzierte Darstellung und damit eine spezifischere Behandlungsplanung.

Female genital mutilation (FGM) is a procedure that injures or removes parts of the external female genitals for nonmedical reasons and without the consent of the affected girls. Often this procedure leads to severe consequences for the women's physical and mental health. Reconstructive surgery can reduce these symptoms. In an exploratory study of 97 women seeking medical counselling, characteristics of the sample were analysed descriptively, reasons for reconstructive surgery were examined and agreement between the women's knowledge with the results of the medical examination was assessed. 56.7% (n=55) of the women examined were suspected to suffer from PTSD. Relief of pain was the reason most frequently mentioned for reconstructive surgery (45.4%; n=44). Accordance between women's knowledge of anatomic structures and the degree of mutilation with the results of the medical examination was low (κ=0.09). In addition to surgical measures, women suffering from FGM should be offered psychotherapeutic assistance. To improve the understanding on the effect of psychotherapy after FGM, sound longitudinal studies are indispensable.Bei der weiblichen Genitalverstümmelung (englisch female genital mutilation, FGM) werden die äußeren Genitalien ohne medizinischen Grund und ohne Mitspracherecht der betroffenen Mädchen beschädigt oder entfernt. Häufig geht diese Prozedur mit erheblichen Konsequenzen für die physische und psychische Gesundheit einher. Rekonstruktive chirurgische Maßnahmen können die Beschwerden der Betroffenen lindern. Anhand einer explorativen Untersuchung von N=97 Frauen, die an einem medizinischen Beratungsgespräch zur rekonstruktiven Operation teilnahmen, wurden die besonderen Charakteristika dieser Stichprobe deskriptiv analysiert, Gründe der Inanspruchnahme einer rekonstruktiven Operation erfragt, sowie das Wissen der Patientinnen über ihren Beschneidungstyp mit der medizinischen Diagnose verglichen. Bei 56,7% (n=55) der Untersuchten lag der Verdacht einer PTBS vor. Keine Schmerzen mehr zu haben war der am häufigsten genannte Grund für eine Operation (45,4%; n=44). Die Übereinstimmung zwischen dem Wissen der Patientinnen über den Beschneidungstyp und dem ärztlichen Urteil fiel gering aus (κ=0,09). Bei physischer und psychischer Belastung sollten Frauen nach FGM neben chirurgischen Maßnahmen auch psychotherapeutisch betreut werden. Psychotherapeutische Interventionen im Kontext von FGM müssen evaluiert werden. Längsschnittstudien sind hierfür unabdingbar.

Bariatric surgery is a life-changing treatment, but knowledge of its influence on changes in relationship satisfaction (RS) is still limited. The present study examines whether a patient’s RS changes after having bariatric surgery, in response to the surgery-induced weight loss. The predictive value of further variables such as age and sex are also investigated. A total of 145 participants scheduled for bariatric surgery were recruited between September 2015 and June 2018. RS was assessed with a reliable and valid short form of a standardized self-report measure, which was completed before, 6 months and 12 months after surgery. Loss of excess weight at 6 and 12 months post-surgery was 49% and 66.3%, respectively. The aim of this study was to assess whether epidemics of sexually transmitted infections caused by poorly transmittable agents corresponded to self-reported sexual activity in a distinct population. To exemplify this, a model was used to investigate whether HIV infection incidences corresponded to the extent of sexual activity as assessed by a questionnaire-based study. The model suggested between 97 and 486 sexual contacts per German individual during a sexually active lifetime based on the annual HIV incidence of 680 among the heterosexual population reported by the German National Health Authority. This is in line with the estimated 296 sexual contacts during one&rsquo, s lifetime, which was indicated by questionnaire respondents. The model confirms the correspondence of self-reported sexual activity with HIV incidence as reported by the German National Health Authority. Accordingly, HIV incidence- and prevalence-based modeling of sexual activity in a population provides crude estimations in situations where a range of uncertainty is acceptable. The model&rsquo, s veracity is limited by a number of assumptions necessitated by the paucity of data. Nevertheless, the model may be suitable in settings where severe reporting bias has to be expected for legal or socio-cultural reasons.

So far, there is no German instrument for determining occupational stress in members of volunteer firefighting (MVF). The aim of the present study was to evaluate the Sources of Occupational Stress in FirefightersParamedics (SOOS-11). MVF were interviewed in cooperation with the fire department accident fund of Lower Saxony at three different measurement times (MT). The first MT was a questionnaire survey (November 2016-February 2017) which contained various self-assessment questionnaires (N=475). At the second MT (May 2017-November 2017) N = 117 MVF (n=55 from the first MT) participated in an interview that contained the modules of the structural clinical interview for DSM-IV (SKID-I; Wittchen, Wunderlich, GruschwitzZaudig, 1997) for diagnosing a depression and posttraumatic stress disorder. During the course of the third MT (November 2017-May 2018) a part of the second sample (n=58) answered various assessment questionnaires again. It was possible to estimate satisfactory values for the internal consistence of the SOOS-11 (α=0.73-0.76). The 6-month-test-retest-reliability was r=0.60. An explorative as well as a confirmatory factor analysis suggested an adequate model-fit for a one-factorial and a 2-factorial structure of the questionnaire. High significant correlations were found between the manifestations of the well-being, the organizational climate and the SOOS-11. Therefore, the SOOS-11 represents a good possibility for measuring occupational stress in MVF. The questionnaire should be tested with a further sample, for example members of the professional fire brigade.Bislang existiert kein deutschsprachiges Instrument zur Ermittlung von einsatzbezogener Belastung bei Mitgliedern der Freiwilligen Feuerwehr (MFF). Das Ziel der vorliegenden Studie war die Evaluation des Sources of Occupational Stress in FirefightersParamedics (SOOS-11). In Zusammenarbeit mit der Feuerwehr-Unfallkasse Niedersachsen wurden MFF zu 3 verschiedenen Messzeitpunkten (MZP) befragt. Beim ersten MZP (November 2016–Februar 2017) handelte es sich um eine Fragebogenerhebung, die diverse Selbstbeurteilungsinstrumente umfasste (N=475). Am zweiten MZP (Mai 2017 – November 2017) nahmen N = 117 MFF (davon n=55 vom ersten MZP) an einem Interview teil, das die Module des Strukturierten Klinischen Interviews für DSM-IV (SKID-I; Wittchen, Wunderlich, GruschwitzZaudig, 1997) zur Erfassung einer Depression und Posttraumatischen Belastungsstörung beinhaltete. Im Zuge des dritten MZP (November 2017–Mai 2018) wurden von einer Teilstichprobe des zweiten MZP (n=58) verschiedene Selbstbeurteilungsinstrumente erneut beantwortet. Es konnten für den SOOS-11 zufriedenstellende Werte zur Internen Konsistenz (α=0,73–0,76) geschätzt werden. Die 6-Monats-Test-Retest-Reliabilität betrug r=0,60. Eine exploratorische sowie eine konfirmatorische Faktorenanalyse ergaben für eine ein – und auch eine 2-faktorielle Struktur des Fragebogens einen adäquaten Modell-Fit. Hoch signifikante Korrelationen ergaben sich zwischen der Ausprägung des Wohlbefindens, dem Organisationsklima und dem SOOS-11. Somit stellt der SOOS-11 eine gute Möglichkeit dar, einsatzbezogene Belastung von MFF zu messen. Der Fragebogen sollte noch an einer weiteren Stichprobe, wie bspw. der Berufsfeuerwehr, erprobt werden.

Borderline personality disorder (BPD) is a severe mental disorder which is accompanied by impaired functioning, complex psychosocial problems and incurs large costs. However, it rarely has been examined whether BPD symptoms form a hierarchical (Mokken) scale, which would imply that symptoms are ordered along a latent dimension of borderline severity. In this study the data from 1,198 inpatients of a psychosomatic clinic were examined. A non-parametric Mokken analysis was employed to investigate if the BPD symptoms measured by the impulsivity and emotion dysregulation scale (IES-27) formed a hierarchical scale. 16 of the 27 items, mainly regarding emotion dysregulation, formed a monotonous Mokken scale with a very strong hierarchy of BPD symptoms. These results imply that single BPD symptoms provide information about BPD severity, suggesting several important consequences regarding escalation of BPD, diagnosis and treatment, e.g. how to optimize treatment planning.

Purpose We examined changes in the burden of depressive symptoms between 2006 and 2014 in 18 European countries across different age groups. Methods We used population-based data drawn from the European Social Survey (N = 64.683, 54% female, age 14–90 years) covering 18 countries (Austria, Belgium, Denmark, Estonia, Finland, France, Germany, Great Britain, Hungary, Ireland, The Netherlands, Norway, Poland, Portugal, Slovenia, Spain, Sweden, Switzerland) from 2006 to 2014. Depressive symptoms were measured via the CES-D 8. Generalized additive models, multilevel regression, and linear regression analyses were conducted. Results We found a general decline in CES-D 8 scale scores in 2014 as compared with 2006, with only few exceptions in some countries. This decline was most strongly pronounced in older adults, less strongly in middle-aged adults, and least in young adults. Including education, health and income partially explained the decline in older but not younger or middle-aged adults. Conclusions Burden of depressive symptoms decreased in most European countries between 2006 and 2014. However, the decline in depressive symptoms differed across age groups and was most strongly pronounced in older adults and least in younger adults. Future studies should investigate the mechanisms that contribute to these overall and differential changes over time in depressive symptoms.

Background The perception that e‑cigarettes are less harmful than traditional tobacco products can influence the consumption of e‑cigarettes. Objectives Three questions were examined: (1) How do different population groups perceive health risks of e‑cigarettes? (2) Do sociodemographic variables explain differences in the risk assessment of e‑cigarettes? (3) Does the perception of health risks predict the use of e‑cigarettes for smoking cessation? Methods Data came from the 2015 Epidemiological Survey of Substance Abuse (ESA) with a sample size of n = 9204 participants, aged 18 to 64 years (response rate 52.2%). Data were collected by telephone, online, or by written questionnaires. Assessments of risk perception of e‑cigarettes and conventional cigarettes (more harmful, just as harmful, less harmful, do not know) were compared. Descriptive statistics and logistic regressions were performed. Results Individuals with lower education rated e‑cigarettes as more harmful. Older people and women perceived e‑cigarettes as just as harmful. Smokers considered e‑cigarettes to be more harmful than or just as harmful as conventional tobacco products. The likelihood of using e‑cigarettes for smoking cessation was higher if people thought they were less harmful than conventional cigarettes. Conclusions Only one-third of the population knows that e‑cigarettes are less harmful to health than conventional cigarettes. The perception of health risks is related to the usage of e‑cigarettes for smoking cessation.

Zusammenfassung. Ein traumafokussiertes kognitiv-behaviorales Behandlungsprogramm mit spielerischen Therapiebausteinen wurde für Kinder mit Traumafolgestörungen und ihre Bezugspersonen entwickelt. Untersucht wurde, ob eine Verminderung der posttraumatischen und phobischen Symptomatik sowie weiterer Verhaltensauffälligkeiten erreicht werden konnte. Die Daten von 47 Kindern im Alter von 5 bis 13 Jahren (61.7 % männlich) wurden vor und nach der Therapie sowie sechs Monate im Anschluss erhoben. Nach der Behandlung waren die selbstbeurteilten posttraumatischen und fremdbeurteilten phobischen Symptome, sowie die fremdbeurteilten Auffälligkeiten signifikant verbessert. Die Effektstärken fielen erwartungsgemäß moderat bzw. hoch aus; die Responseraten für die posttraumatischen und phobischen Symptome betrugen 70.4 % bzw. 19.2 %. Alle Kinder wiesen am Behandlungsende eine Remission der posttraumatischen Symptome auf. Das Behandlungsprogramm sollte zukünftig in randomisiert-kontrollierten Studien untersucht werden.

ZusammenfassungDie New Sexual Satisfaction Scale (NSSS) ist ein international etablierter Fragebogen zur Erfassung der sexuellen Zufriedenheit mit ursprünglich 2 Subskalen (selbstfokussierte bzw. partner-/aktivitätenfokussierte sexuelle Zufriedenheit). Ziel der vorliegenden Studie war es, die deutsche Kurzversion des Fragebogens (NSSS-SD) im Rahmen einer Repräsentativerhebung in Deutschland (N=2524) zu überprüfen. Zusätzlich wurden Zusammenhänge zwischen sexueller Zufriedenheit und soziodemografischen Faktoren (Alter, Geschlecht, Bildungsstand) sowie Aspekten der Partnerschaft und Sexualität (Beziehungszufriedenheit, Koitusfrequenz, Anzahl der Sexualpartner) untersucht. Die interne Konsistenz der NSSS-SD lag bei 0,96. Die 2-dimensionale Struktur der Langversion konnte für die Kurzversion nicht bestätigt werden. Es konnte ein Faktor extrahiert werden, welcher 68,94% der Varianz aufklärt. Varianzanalysen ergaben signifikante Unterschiede in der sexuellen Zufriedenheit bezüglich Alter, Bildungsstand, Beziehungszufriedenheit und Koitusfrequenz, nicht aber bezogen auf Geschlecht oder Anzahl der Sexualpartner. Die NSSS-SD ermöglicht die Messung der sexuellen Zufriedenheit für sexuell aktive Personen. Für sexuell inaktive Personen sollte eine Änderung der Instruktion in Betracht gezogen oder eine visuelle Analogskala angeboten werden.

Aim of the study Coping with psychological trauma plays a central role in victims of acts of violence. The Victim Reparation Law (in German: Opferentschadigungsgesetz, OEG) provides services for the reparation of participation. To further improve the care of the affected persons, the characteristics of the victims were determined in relation to the acts and the perpetrators. Method 312 OEG-files of adult victims were sighted. On the basis of a system of categories, demographic data, trauma specific and health-related characteristics of the victims, relevant characteristics of the perpetrators and the violence, as well as predictors for the time difference between the violent act and the application were determined. Results Mostly women (71%) were affected by acts of violence. 49% of the affected persons suffered from physical and 87% from psychological impairment (64% of these from a posttraumatic stress disorder). 55% of the acts of violence were sex crimes, 47% were bodily injuries and in 44% additional psychological abuse existed. The perpetrators were often connected to the victims. 42% of the offenses concerned domestic violence. Time difference between the act of violence and the request for help which is provided by law variated between 0 and 52 years. 50% made use of help within one year, further 15.2% within five years. In 34.3% of cases, time difference was longer than 5 years. In case of serial and severe traumatizations, depression and close connections between victim and perpetrator help provided by law was called for only late. Conclusion Many victims of acts of violence acts are psychologically traumatized. In order to prevent a traumatization from becoming a permanent health disorder, the offering of early evidence-based specialist or psychological acute therapy (e. g., in trauma outpatient clinics) ought to be optimized.

Zusammenfassung. Fragestellung: Die deskriptive Untersuchung von Anträgen auf Opferentschädigung für Kinder und Jugendliche sowie der soziodemografischen und traumaspezifischen Daten der Betroffenen. Methodik: Eine Analyse von 100 Akten der Opferentschädigung von Kindern und Jugendlichen mittels eines selbstentwickelten Kategoriensystems. Ergebnisse: Die Akten beinhalten ausschließlich interpersonelle Traumata, wovon 59 % Typ-II-Traumata sind. Die häufigste Gewaltform ist sexuelle Gewalt. Die Täter sind überwiegend Personen aus dem häuslichen Umfeld. Bei 79 % der Opfer wurden psychische Störungen diagnostiziert, am häufigsten die Posttraumatische Belastungsstörung. Schlussfolgerungen: Sexuell missbrauchte Kinder und Jugendliche machen einen Großteil der Zielgruppe von kinder- und jugendpsychotherapeutischen Traumaambulanzen nach dem Opferentschädigungsgesetz (OEG) aus. Solche Traumaambulanzen sollten demnach eine spezifische Expertise in der psychotherapeutischen Behandlung dieser Kinder und Jugendlichen anbieten.

Zusammenfassung Hintergrund Schätzungen zum E-Zigarettenkonsum in Deutschland variieren erheblich. Die Nutzung von E-Zigaretten zur Tabakentwöhnung wird kritisch diskutiert. Anhand aktueller Daten soll die Verbreitung des Konsums von E-Zigaretten und deren Nutzung in der erwachsenen Allgemeinbevölkerung Deutschlands dargestellt werden. Methoden Als Datenbasis diente der Epidemiologische Suchtsurvey 2015, eine bundesweite Befragung von 18- bis 64-jährigen in Deutschland (n=9,204; Rücklaufquote: 52,2%). Ergebnisse E-Zigaretten waren einem Großteil der Befragten bekannt (85,3%, 43,5 Mio.), wohingegen lediglich 2,9% (1,5 Mio.) E-Zigaretten in den letzten 30 Tagen genutzt hatten. Jüngere Personen (OR=0,95, 95%-KI=(0,93; 0,97)), Männer (OR=1,45, 95%-KI=(1,02; 2,07)) und Raucher (OR=12,53, 95%-KI=(8,71; 18,03)) zeigten ein signifikant höheres Risiko, E-Zigaretten zu konsumieren. Etwa ein Drittel (36,6%) der Raucher und Ex-Raucher konventioneller Zigaretten nutzte E-Zigaretten zur Tabakentwöhnung, wovon ein Fünftel (21,3%) mit dem Rauchen aufhören konnte. Schlussfolgerungen E-Zigarettennutzer scheinen häufiger männlich, jünger und Raucher konventioneller Zigaretten zu sein. Neben Neugierde ist die Veränderung des Rauchverhaltens ein bedeutendes Konsummotiv. Die Ergebnisse deuten darauf hin, dass der Gebrauch von E-Zigaretten vereinzelt zur Tabakentwöhnung beitragen kann, die Mehrheit der Nutzer aber weiterhin konventionelle und/oder E-Zigaretten konsumiert.

Thought-shape fusion (TSF) is a cognitive distortion associated with eating disorders (ED). A similar distortion, thought-abandonment fusion (TAbF), is assumed to occur in borderline personality disorder (BPD). In this study the specificity of TSF in participants with anorexia nervosa (AN) and TAbF in participants with BPD was examined. 63 patients completed questionnaires assessing the manifestation of trait-TAbF and trait-TSF, as well as relevant psychopathology. Nonparametric conditional inference trees were used to test for cognitive disorder-specificity. Participants with AN exhibited higher trait-TSF-scores than those with BPD, when participants with BPD and a co-occurring AN were removed. Trait-TSF in participants with AN seemed to be disorder-specific. Participants with BPD and a co-occurring AN had the highest TAbF-scores. The specificity hypothesis could only be partially confirmed for trait-TAbF: while participants with BPD and a co-occurring AN tended to have the highest trait-TAbF scores, high mean values could also be found in participants with AN. The results indicate that TAbF is not specific to BPD, but may also play a role in AN. Both distortions seem to play a role in the maintenance of the respective disorders.
