Abstract

Background: Hand eczema is a common inflammatory skin disorder. Health care providers need continuously updated information about the management of hand eczema to ensure best treatment for their patients. Objectives: To update the European Society of Contact Dermatitis guideline on the diagnosis, prevention, and treatment on of hand eczema. Method: The Guideline Development Group (GDG) was established on behalf of the ESCD. A call for interest was launched via the ESCD website and via the ESCD members' mailing list. Appraisal of the evidence for therapeutic and preventive interventions was applied and a structured method of developing consensus was used and moderated by an external methodologist. The final guideline was approved by the ESCD executive committee and was in external review on the ESCD webpage for 1 month. Results: Consensus was achieved for several statements and management strategies. Conclusion: The updated guideline should improve management of hand eczema.

Abstract

A 10-year old girl and her mother visited four dermatologists in several months because of erosions in her face, suspected for dermatitis artefacta. However, when previously confronted the patient denied this diagnosis. Immunofluorescence investigations were negative. After building a trusted physician-patient relationship and empathically confronting the mother,the mother herself was able to provide her daughter a narrow escape, which resulted in complete remission of her condition. Dermatitis artefacta is a spot diagnosis, however the treatment is challenging for both the physician and the patient. A trusting physician-patient relation is crucial and providing verbal and non-verbal support is essential. One way of doing this is asking the patient to keep a diary. In addition every patient deserves a thorough but compact investigation. Once internal causes have been excluded and a trustful relationship has been established one can precede to a narrow escape or confrontation. In this article we provide practical tricks on how to approach a patient with dermatitis artefacta.

12. Developing an allergy management support system (AMSS) for primary care: Agreement between the AMSS and the allergy specialist

Abstract

Background: Diagnosis and management of allergic patients is often initially and exclusively performed by GPs. Since the prevalence of allergy is increasing and many GPs report having difficulties in diagnosing and managing allergic patients, an Allergy Management Support System (AMSS) for primary care is under development. The aim of this study was to determine the agreement on allergy management between the AMSS and allergy specialists. Methods: The questionnaire of the AMSS was administered to patients suspected of being allergic who were referred by GPs to (pediatric) allergists or dermatologists. The responses were analyzed by 2 researchers who allocated the patients into predefined management categories using the algorithms of the AMSS. Based on the clinical correspondence, 2 allergists and 2 dermatologist allocated the same patients in the same way using their clinical knowledge, which was considered to be the gold standard. Agreement between the gold standard and the AMSS was investigated by ascertaining concordance of management categories. The inter-rater reliability between the specialists and researchers was calculated using Cohen's kappa. Results: 42 patients (38% male, mean age 26.2 years) completed the questionnaire. Mean time between completing the questionnaire and the consult with allergy specialist was 8 days (SD 11 days). Abstract

Background: Hand eczema is a common condition; it is often chronic and can be difficult to treat. Cyclosporine is used off-label to treat severe hand eczema; however, the evidence for this treatment is scarce., Objective: To examine the drug survival of cyclosporine in a daily practice cohort of patients with chronic hand eczema., Methods: This retrospective daily use study included hand eczema patients who were treated with cyclosporine between 01-06-1999 and 01-06-2014 in two Dutch university hospitals. Patient and treatment characteristics were retrospectively collected from medical charts. First treatment episodes were analysed by means of Kaplan-Meier drug survival curves. Possible determinants of drug survival were analysed by Cox regression models. Treatment effectiveness was analysed with a retrospective physician's global assessment., Results: A total of 102 patients were treated with cyclosporine. The median drug survival rate was 0.86 years (10.3 months). The overall drug survival rate after 6 months, 1, 2 and 3 years were 61.7%, 45.2%, 18.6% and 13.9% respectively. Main reasons for discontinuation were adverse events, especially early in treatment, and ineffectiveness. After 3 months, a good response to treatment was recorded in 62.9% of the patients., Conclusion: Cyclosporine had a median drug survival of 10.3 months. Especially patients with recurrent vesicular hand eczema showed a good treatment response., (© 2015 European Academy of Dermatology and Venereology.)
